| 1 | Laparoscopic vs open partial colectomy in elderly patients:Insights from the American College of Surgeons- National Surgical Quality Improvement Program database显示文摘AIM: To compare the outcomes between the laparoscopic and open approaches for partial colectomy in elderly patients aged 65 years and over using the American College of Surgeons- National Surgical Quality Improvement Program(ACS NSQIP) database. METHODS: The ACS NSQIP database for the years 2005-2011 was queried for all patients 65 years and above who underwent partial colectomy. 1:1 propensity score matching using the nearest- neighbor method was performed to ensure both groups had similar preoperative comorbidities. Outcomes including postoperative complications, length of stay and mortality were compared between the laparoscopic and open groups. χ2 and Fisher's exact test were used for discrete variables and Student's t-test for continuous variables. P < 0.05 was considered significant and odds ratios with 95%CI were reported when applicable. RESULTS: The total number of patients in the ACS NSQIP database of the years 2005-2011 was 1777035. We identified 27604 elderly patients who underwent partial colectomy with complete data sets. 12009(43%) of the cases were done laparoscopically and15595(57%) were done with open. After propensity score matching, there were 11008 patients each in the laparoscopic(LC) and open colectomy(OC) cohorts. The laparoscopic approach had lower post-operative complications(LC 15.2%, OC 23.8%, P < 0.001), shorter length of stay(LC 6.61 d, OC 9.62 d, P < 0.001) and lower mortality(LC 1.6%, OC 2.9%, P < 0.001). CONCLUSION: Even after propensity score matching, elderly patients in the ACS NSQIP database having a laparoscopic partial colectomy had better outcomes than those having open colectomies. | 2 | A study of the correlation between dengue and weather in Kandy City, Sri Lanka (2003 -2012) and lessons learned显示文摘Background:Weather variables affect dengue transmission.This study aimed to identify a dengue weather correlation pattern in Kandy,Sri Lanka,compare the results with results of similar studies,and establish ways for better control and prevention of dengue.Method:We collected data on reported dengue cases in Kandy and mid-year population data from 2003 to 2012,and calculated weekly incidences.We obtained daily weather data from two weather stations and converted it into weekly data.We studied correlation patterns between dengue incidence and weather variables using the wavelet time series analysis,and then calculated cross-correlation coefficients to find magnitudes of correlations.Results:We found a positive correlation between dengue incidence and rainfall in millimeters,the number of rainy and wet days,the minimum temperature,and the night and daytime,as well as average,humidity,mostly with a five-to seven-week lag.Additionally,we found correlations between dengue incidence and maximum and average temperatures,hours of sunshine,and wind,with longer lag periods.Dengue incidences showed a negative correlation with wind run.Conclusion:Our results showed that rainfall,temperature,humidity,hours of sunshine,and wind are correlated with local dengue incidence.We have suggested ways to improve dengue management routines and to control it in these times of global warming.We also noticed that the results of dengue weather correlation studies can vary depending on the data analysis. | N D B Ehelepola Kusalika Ariyaratne W M N P Buddhadasa Sunil Ratnayake Malani Wickramasinghe | 2015 | Infectious Diseases of Poverty2015，4，1： | 2 |
| 3 | Structural and luminescence enhancement properties of Eu^(3+)/Ag nanocrystallites doped SiO_2-TiO_2 matrices显示文摘We reported here the structural and optical characterisation of silver nanocrystallites/Eu3+ :SiO2-TiO2 matrices synthesised through sol-gel route. Structural characterisations were done by using energy dispersive spectroscopy (EDX), X-ray diffraction (XRD), Fourier transform infrared spectroscopy (FTIR), atomic force microscope (AFM) and transmission electron microscopy (TEM) measurements and optical characterisations were performed by absorption and emission spectroscopy. The TEM and XRD measurements confirmed the presence of nanocrystals. A broad absorption band was observed due to surface plasmon resonance of silver nanocrystals. The effect of silver nanocrystals on the emission spectrum of Eu3+ doped SiO2-TiO2 matrices was discussed. An attempt was made to explain this fluorescence enhancement by invoking phenomena such as energy transfer, asymmetry ratio, surface plasmon, surface roughness, crystallinity and grain boundary. Our analysis, based on the experimental results, suggested that all the phenomena except crystallinity and grain boundary had favourable effects on fluorescence enhancement. We also estimated the relevant parameters associated with the phenomena that affected the fluorescence emission from the Eu3+ ions in the matrix.
