Hepatic abscess associated with Salmonella serotype B in a chronic alcoholic patient显示文摘BACKGROUND: Hepatic abscess secondary to Salmonella is extremely rare in the United States and other Western countries. METHODS: A 43-year-old Caucasian man, with a history of chronic alcohol abuse, was admitted to the hospital for intermittent abdominal pain, fever and diarrhea. Clinical radiological and laboratory results were analyzed. Medical literature in PubMed pertaining to similar cases was reviewed. RESULTS: Stool culture was positive for Salmonella serotype B and a CT scan of the abdomen with contrast was consistent with a solitary hepatic abscess. Appropriate intravenous antibiotics followed by oral maintenance therapy for six weeks resulted in a complete clinical recovery and radiographic resolution. CONCLUSIONS: The cause of Salmonella hepatic abscess in our patient was most likely associated with decreased mucosal resistance to the bacteria, seeding of infection via transient portal bacteremia and loss of host immunity. Our case highlights the fact that appropriate antibiotic alone is sufficient in the management of a solitary hepatic abscess less than 3-5 cm.Sidhartha S Tulachan Charles E Wilkins Anthony F Cutrona David Gemmel Thomas P Marnejon 2013Hepatobiliary & Pancreatic Diseases International2013,12,4:0
Does Rationality Really Work in Bureaucratic Organizations?A Critical Juncture Perspective显示文摘A pool of literatures in line with the notion of rationality projects that the rational people in bureaucratic organizations perform well. The other line of literatures on rationality cites different stories with limitations on rationality. In line with this, the objective of this paper is to investigate whether rationality really works in bureaucratic organizations in the neo-liberal economic terrain of Nepal. In doing so,the paper looks from critical juncture perspective of political paradigm shift and its impact on bureaucrats and bureaucratic organizations. The analysis reveals increased dependence of bureaucrats on domestic or foreign donors.Seemingly, the growing trend to be loyal towards them has replaced rationality, especially after the restoration of democracy in Nepal. The conclusion asks for right men in right place for timely delivery of goods and services to the public.Bojindra Prasad Tulachan Gi-Cheon Park 2022Journal of US-China Public Administration2022,19,1:0
He Is Absolutely a Good Player. Why Don’t You Keep (Him) in Your Team? Industrial Relations From Historical Critical Juncture Perspective显示文摘Much of the industrial relations scholarships is in the strand for system approach in explaining industrial relations, following WW II.However,1980s and 1990s phenomenon heralded towards deregulations and institutional convergence, massively in advanced, and later in emerging economies. This has led to the weakest prospect of industrial relations in mainstream literatures in that the frontline literatures are on a call for revitalization or reshaping of industrial relations. Thus, the paper has a check(within Dunlopian framework)whether the Nepalese industrial relations has held similar trajectory of industrial relations as in the other parts of the world. In doing so,the paper considers historical critical juncture perspective of political landscape, following WW II that struck South Asia with wave of democracy in 1980s and 1990s and how it influenced the institutionalization of industrial and labor relations in the early 1990s,with the enactment of labor act of 1992 for the first time in Nepal. The conclusion marks highly politicized industrial and labor relations with the frailest position of the employers within the system framework of industrial and labor relations.Following that, the major theoretical contributions of the paper are described for the advancement of the existing industrial and labor relations literature.Bojindra Prasad Tulachan Balabhadra Rai 2021Journal of US-China Public Administration2021,18,6:0
The Positive-Activity Model Revisited: A Review From Critical Juncture Perspective of COVID-19显示文摘The positive-activity model deals with the role of the positive interventions in increasing wellbeing in normal situation. In that positive responses(positive emotions, positive cognitions, positive behaviors and need satisfaction)mediate the relationship between performance of interventions and wellbeing, whereas intervention traits, person traits and individual-intervention fit moderate the very relationship. In absence of the external environment of traumatic events or situations in the very model,COVID-19 is critical factor to further explaining the mechanism. Therefore, the objective of this frontline paper is to unveil the unchartered individual-in-situation and the individual-in-situation-intervention fit as moderators in explaining the relationship between the performance of the intervention and wellbeing in the backdrop of traumatic events or situations. In doing so,the positive-activity model is revisited and the COVID-19 phenomenon as reference is put into consideration. To that end, propositions are made for larger development of the positive-intervention framework.Further, future research is asked to make empirical investigations on the onset of/post-traumatic events or situations such as COVID-19 pandemic.Bojindra Prasad Tulachan Krishna Prasad Paudyal 2020Journal of US-China Public Administration2020,17,6:0
Is Employment Relations Towards Deregulation and Institutional Convergence Across the Globe?显示文摘Massive academics are at say of deregulations and institutional convergence in employment relations across the globe,be them in advanced or emerging economies. To benefit of doubt:Is it really in all countries in the world?Or are they the representative stories? Thus, the objective of this paper is to explore the recent context of industrial and labor relations in Nepal. To this end,the authors consider the recent Labor Act of 2017 as the fundamental basis for the examination of employment relations against the growing deregulations and institutional convergence in the neo-liberal contexts of post-globalization,highly competitive markets,export-oriented business, super digital technological advancement and increased labor migration abroad. The Labor Act of 2017 shows that the employment relations is around the system approach with collective bargaining, collective dispute settlement and the provision of arbitration, mediation or third-party system,to name a few. This indicates that the employment relations of deregulations and institutional convergence in the advanced and emerging economics, including Asia are some of the representative ones.In that the factors to influence such employment relations vary across the countries even though there has been increased competition, digitalization, labor migration and knowledge-intensive work around. In contrary, within the local, national context or institutional labor framework, the actors and the path dependence of overall employment relations do matter in the study of changing employment relations. Thus, the Labor Act of 2017 in Nepal is put into consideration as a case study to acknowledge the employment relations against the changing, global one.Further, the story would contribute to revitalization of industrial and labor relations and divergence of employment relations in the pool of literatures.Bojindra Prasad Tulachan Balabhadra Rai 2020Journal of US-China Public Administration2020,17,5:0
