| 1 | Quality of ulcer healing in gastrointestinal tract:Its pathophysiology and clinical relevance显示文摘In this paper,we review the concept of quality of ulcer healing(QOUH) in the gastrointestinal tract and its role in the ulcer recurrence.In the past,peptic ulcer disease(PUD) has been a chronic disease with a cycle of repeated healing/remission and recurrence.The main etiological factor of PUD is Helicobacter pylori(H.pylori),which is also the cause of ulcer recurrence.However,H.pylori-negative ulcers are present in 12%-20% of patients;they also recur and are on occasion intractable.QOUH focuses on the fact that mucosal and submucosal structures within ulcer scars are incompletely regenerated.Within the scars of healed ulcers,regenerated tissue is immature and with distorted architecture,suggesting poor QOUH.The abnormalities in mucosal regeneration can be the basis for ulcer recurrence.Our studies have shown that persistence of macrophages in the regenerated area plays a key role in ulcer recurrence.Our studies in a rat model of ulcer recurrence have indicated that proinflammatory cytokines trigger activation of macrophages,which in turn produce increased amounts of cytokines and chemokines,which attract neutrophils to the regenerated area.Neutrophils release proteolytic enzymes that destroy the tissue,resulting in ulcer recurrence.Another important factor in poor QOUH can be deficiency of endogenous prostaglandins and a deficiency and/or an imbalance of endogenous growth factors.Topically active mucosal protective and antiulcer drugs promote high QOUH and reduce inflammatory cell infiltration in the ulcer scar.In addition to PUD,the concept of QOUH is likely applicable to inflammatory bowel diseases including Crohn's disease and ulcerative colitis. | Tetsuo Arakawa Toshio Watanabe Tetsuya Tanigawa Kazunari Tominaga Yasuhiro Fujiwara Kenichi Morimoto | 2012 | World Journal of Gastroenterology2012，18，35： | 16 |
| 2 | Primary hepatic benign schwannoma显示文摘Schwannoma is predominantly a benign neoplasm of the Schwann cells in the neural sheath of the peripheral nerves.Occurrence of schwannoma in parenchymatous organs,such as liver,is extremely rare.A 64-year-old man without neurofibromatosis was observed to have a space-occupying lesion of 23mm diameter in the liver during follow-up examination for a previously resected gastrointestinal stromal tumor(GIST)in the small intestine.He underwent lateral segmentectomy of the liver under a provisional diagnosis of hepatic metastatic recurrence of the GIST.Histological examination confirmed the diagnosis of a benign schwannoma,confirmed by characteristic pathological findings and positive immunoreactions with the neurogenic marker S-100 protein,but negative for c-kit,or CD34.The tumor was the smallest among the reported cases.When the primary hepatic schwannoma is small in size,preopera- tive clinical diagnosis is difficult.Therefore,this disease should be listed as differential diagnosis for liver tumor with clinically benign characteristics. | Michihiro Hayashi Atsushi Takeshita Kazuhiro Yamamoto Nobuhiko Tanigawa | 2012 | World Journal of Gastrointestinal Surgery2012，4，3： | 7 |

| 4 | NOTES:The question for minimal resection and sentinel node in early gastric cancer显示文摘Surgical therapy for gastric cancer involves both removal of the cancer lesion and complete lymph node dissection.Natural orifice transluminal endoscopic surgery(NOTES) is considered to represent the next revolution in surgery.Many surgeons and endoscopists believe that NOTES may be a superior alternative for early gastric cancer treatment.Sentinel node(SN) navigation surgery for gastric cancer:Single institution results of SN mapping for early gastric cancer are increasingly being considered acceptable.Furthermore,a major large-scale clinical trial of SN mapping for gastric cancer has recently been completed by The Japan Society of SN Navigation Surgery study group.They reported false negative rate of 7.0 while the sensitivity of metastasis detection based on SN status was 93.Combination of SN biopsy and NOTES:This concept was first described by Cahill et al who proved the feasibility of lymphatic mapping and SN biopsy by NOTES.Lymphatic channel filling was immediately observable via the intraperitoneal optics.Partial resection of the stomach by hybrid NOTES:Several centers have already reported gastrectomy assisted by NOTES using the transvaginal route.However,the main problem of full-thickness resection of gastric wall remains endoscopic gastric closure.Establishing an endoscopic suturing method would be an important step toward expanding potential indications.NOTES is met with both enthusiasm and skepticism but will gain its own place as human creativity eventually provides solutions to its technical limitations.In the near future,NOTES can evolve the capacity to complement the existing armamentarium for gastric cancer surgery. | Mitsuhiro Asakuma Ronan A Cahill Sang-Woong Lee Eiji Nomura Nobuhiko Tanigawa | 2010 | World Journal of Gastrointestinal Surgery2010，2，6： | 3 |
| 5 | Peritoneovenous shunt for intractable ascites due to hepatic lymphorrhea after hepatectomy显示文摘A peritoneovenous shunt has become one of the most effi cient procedures for intractable ascites due to liver cirrhosis.A case of intractable ascites due to hepatic lymphorrhea after hepatectomy for hepatocellular carcinoma that was successfully treated by the placement of a peritoneovenous shunt is presented.A 72-year-old Japanese man underwent partial resection of the liver for hepatocellular carcinoma associated with hepatitis C viral infection.After hepatectomy,a considerable amount of ascites ranging from 800-4600 mL per day persisted despite conservative therapy,including numerous infusions of albumin and plasma protein fraction and administration of diuretics.Since the patient's general condition deteriorated,based on the diagnosis of intractable hepatic lymphorrhea,a subcutaneous peritoneovenous shunt was inserted.The patient's postoperative course was uneventful and the ascites decreased rapidly,with serum total protein and albumin levels and hepatic function improving accordingly.For intractable ascites due to hepatic lymphorrhea after hepatectomy,we recommend the placement of a peritoneovenous shunt as a procedure that can provide immediate effectiveness without increased surgical risk. | Yoshihiro Inoue Michihiro Hayashi Fumitoshi Hirokawa Atsushi Takeshita Nobuhiko Tanigawa | 2011 | World Journal of Gastrointestinal Surgery2011，3，1： | 3 |

| 13 | Diffusion-weighted imaging might be useful for reactive lymphoid hyperplasia diagnosis of the liver: A case report显示文摘BACKGROUND Reactive lymphoid hyperplasia(RLH)of the liver is a rare liver lesion.It is considered difficult to differentiate radiologically from hepatocellular carcinoma,metastatic liver tumor and other pathologies.CASE SUMMARY A 54-year-old woman presented to our hospital with RLH of the liver.The patient had a diagnosis of metastatic carcinoma of the liver from an unknown origin and subsequently underwent partial hepatectomy.However,histopathological analysis revealed RLH.The lesion showed perinodular enhancement in the arterial phase on contrast-enhanced computed tomography and magnetic resonance imaging.On diffusion-weighted imaging(DWI),we encountered linear hyperintensity along the portal tract consecutive to the liver lesion,which is a new characteristic radiologic finding.This finding corresponded to the lymphoid cell infiltration of the portal tract.Furthermore,there was strongly restricted diffusion on the apparent diffusion coefficient map.We used these characteristic radiologic findings to diagnose the lesion as a lymphoproliferative disease.CONCLUSION The linear hyperintensity consecutive to the liver lesion on DWI provided additional valuable diagnostic information. | Taro Tanaka Kazuhiro Saito Daisuke Yunaiyama Jun Matsubayashi Yuichi Nagakawa Maki Tanigawa Toshitaka Nagao | 2020 | World Journal of Clinical Cases2020，8，21： | 2 |
