3依托泊苷联合洛铂或顺铂治疗广泛期小细胞肺癌的对照研究显示文摘目的观察洛铂(lobaplatin,LBP)或顺铂(cisplatin,DDP)分别联合依托泊苷(etoposide,VP-16)治疗广泛期小细胞肺癌(small cell lung cancer,SCLC)的近期疗效、不良反应和住院情况。方法 46例广泛期SCLC分为DDP联合VP-16的EP组23例,LBP联合VP-16的EL组23例。21d为1周期,连用2个周期评价近期疗效、毒性反应及住院情况。结果 EL组和EP组比较,客观有效率、住院时间、白细胞下降率及血小板下降率,均无显著性差异。两组化疗后肺癌肿瘤标志物中神经元特异性烯醇化酶(NSE)均明显减低,但癌胚抗原(CEA)和细胞角蛋白19片段(CYFRA21-1)无明显减低,两组之间无显著性差异。EL组消化道反应率明显减低,但诊疗费用明显增加。结论 EL和EP同样为广泛期SCLC安全有效的化疗方案,EL组消化道反应明显减少,但医疗费用明显增加。陈天君 高飞 孙忠民 朱柏 THAKUR Asmitanand 任徽 阳甜 陈明伟 2013西安交通大学学报(医学版)2013,34,2:23
4改良前外侧入路治疗老年复杂胫骨平台骨折的临床效果显示文摘目的探讨采用改良前外侧入路治疗老年Schatzker V型及VI型骨折的优势。方法对我院2016~2018年收治的35例老年复杂性胫骨平台骨折患者(Schatzker V型及VI型)的临床资料进行分析。均采用改良前外侧入路进行手术治疗。术后即刻及1、2、3、6、12个月对患者进行X线片检查,测量胫骨平台内翻角(tibial plateau angle,TPA)及后倾角(posterior slope angle,PA),采用Rasmussen放射学评分评价手术复位及骨折愈合情况,采用Rasmussen膝关节功能评分及美国纽约特种外科医院(HSS)评分系统,对患者膝关节功能康复进行评价。结果本组共35例,其中男15例,女20例,年龄61~81岁,平均(69.46±6.15)岁。Schatzker分型V型20例,VI型15例。术后即刻X线片显示共30例获解剖复位,5例关节面遗留<2 mm台阶。Rasmussen放射学评分12~18分,平均(16.26±1.93)分。各时间点TPA值相比,差异无统计学意义(F=0.05,P=0.99)。各时间点内侧PA值相比,差异无统计学意义(F=0.08,P=0.99)。各时间点外侧PA值相比,差异无统计学意义(F=0.07,P=0.98)。术后HSS评分为79~95分,平均(86.51±4.29)分。Rasmussen膝关节功能评分为18~29分,平均(25.54±2.96)分。膝关节活动范围为90°~135°,平均(120.37±10.99)°。所有患者术后1年随访均显示骨折愈合。结论采用改良前外侧入路治疗老年Schatzker V型及VI型胫骨平台骨折具有术中软组织损伤小的特点,且Rasmussen评分及HSS评分显示术后膝关节功能恢复满意。刘燊 Shyam Sundar Thakur 程鑫 郝岩 张晓雷 郝丁宇 张磊 周先虎 冯世庆 2019中国骨与关节杂志2019,8,7:19
5低分子肝素、N-乙酰半胱氨酸干预对COPD大鼠气道重塑的影响显示文摘目的探讨低分子肝素(LMWH)、N-乙酰半胱氨酸(NAC)对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)大鼠气道重塑的影响。方法将40只6~8周鼠龄按雌雄各半的SD大鼠分为5组,即健康对照组(C组),熏香烟及脂多糖(LPS)方法制备的COPD模型组(M组),以及低分子肝素(LMWH)注射组(L组)和N-乙酰半胱氨酸(NAC)灌胃组(N组)及前二者同时干预组(LN组);光镜下观察大鼠病理学改变;以图像分析系统测量其支气管壁厚度;偏振光显微镜下观察各组大鼠支气管黏膜下胶原天狼猩红染色的气道胶原沉积。结果①模型制备:本研究制备的COPD模型大鼠支气管、肺病理改变与人类COPD变化基本一致。②支气管壁厚度:与对照组比较,M组、L组、N组和LN组管壁厚度明显增加(P<0.01);而与M组比较,L组、LN组管壁厚度均减少,且以LN组减少最为明显(P<0.01),N组亦有减少但差异无统计学意义(P>0.05)。③胶原沉积:与对照组比较,各组主要呈现Ⅰ型胶原沉积,L组和N组胶原减少,LN组胶原减少更趋明显。结论①较短时间的熏香烟+灌注LPS是建立COPD大鼠模型的简单易行的方法;②LMWH、NAC干预尤其是联合应用可通过不同作用机制减轻COPD发病过程中气道壁厚度及气道胶原沉积,进而减轻COPD气道重塑。卫小红 王军辉 Asmitanand Thakur 马爱群 张增铁 2014西安交通大学学报(医学版)2014,35,2:16
6齐墩果酸诱导人肺腺癌细胞A549凋亡及其与细胞内钙离子的关系显示文摘目的通过检测齐墩果酸(oleanolic acid,OA)干预后的A549细胞的凋亡和其内钙离子浓度,探索OA对A549细胞的作用及其可能的机制。方法体外培养人肺腺癌细胞系A549。设0、10、20、40μg/ml的OA浓度干预组。选择对数生长期的细胞,在不同浓度OA干预24 h,采用Annexin V/PI流式细胞术(flowcytometry,FCM)检测各实验组细胞的凋亡情况,测定细胞的荧光强度值并计算出细胞内钙离子浓度;以曲线拟合描述细胞凋亡率与钙离子荧光强度之间的相关性。结果FCM检测显示10、20、40μg/ml OA可诱导A549细胞凋亡,凋亡率呈现浓度-效应关系;20、40μg/ml OA干预24 h,A549细胞凋亡率明显增加,与0μg/ml组比较,差异有显著性(P<0.01);10、20、40μg/mlOA组干预24 h,各药物干预组A549细胞内钙离子荧光强度均明显高于0μg/ml组,荧光强度随OA浓度的提高呈现增加趋势,差异有显著统计学意义(P<0.01);曲线拟合显示细胞凋亡率和细胞内钙离子浓度之间有明显相关性(R=0.981,P<0.01)。结论OA具有浓度依赖地诱导人肺腺癌细胞凋亡的作用;OA诱导细胞凋亡可能与其导致细胞内钙超载有关。卫小红 邵杰 王军辉 Asmitanand Thakur 2009同济大学学报(医学版)2009,30,5:13

8淋巴细胞和Ⅲ型前胶原氨基端肽与肺气肿合并肺纤维化的关系显示文摘目的:观察肺气肿合并肺纤维化(CPFE)淋巴细胞亚型的分布和Ⅲ型前胶原氨基端肽(PⅢNP)水平及纤维化程度,探讨其炎症反应、免疫状态与肺纤维化(PF)的关系。方法:对2005—03/2007—03西安交通大学第一附属医院收治的21例CPFE、25例特发性肺纤维化(IPF)19例对照进行如下检测:光镜下观察肺活检标本的病理改变;对支气管肺泡灌洗液(BALF)进行炎细胞计数和分类;流式细胞术(FCM)检测BALF中T淋巴细胞亚型;放射免疫法检测BALF和血清PⅢNP水平。结果:活检病理结果发现,IPF组纤维化率高于CPFE组(P〈0.01),但两组纤维化率与是否吸烟均不相关(P〉0.05);BALF炎细胞分类结果显示CPFE组细胞总数和淋巴细胞比例均高于IPF组和对照组(P〈0.05,P〈0.01);FCM检测BALF发现CPFE组CD8+T淋巴细胞百分比高于IPF组和对照组(P〈0.05);CPFE组和IPF组CD4+/CD8+比值均明显低于对照组(P〈0.01);三组之间CD4+的百分率无统计学差异;CPFE组与IPF组血清PⅢNP检测值均高于对照组(P〈0.01),其中CPFE组血清PⅢNP检测值显著低于IPF组(P〈0.01)。BALF与血清PⅢNP水平呈正相关(γ=0.82)。结论:CPFE的PF病变具有自身特点和发展规律,香烟烟雾刺激可能并非其直接或主要致纤维化因素;CPFE肺组织局部具有较IPF和对照明显且以CD8+T淋巴细胞增高为主的炎性反应,抗淋巴细胞炎症和免疫调节治疗可能有助于改善病情;血清PⅢNP可用作早期发现CPFE和监测PF治疗的指标。卫小红 吴小燕 王军辉 Asmitanand Thakur 马爱群 2010细胞与分子免疫学杂志2010,26,7:10
9CT多平面重建联合支气管镜诊断原发性气管-支气管淀粉样变性显示文摘目的:总结原发性气管-支气管淀粉样变性(primary tracheobronchial amyloidosis,PTBA)的特征性CT征象,并结合支气管镜下表现,以提高对此病的诊断正确率。方法:回顾性分析6例PTBA患者CT多平面重建图像的影像学特点,并结合支气管镜下所见和病理学资料,归纳PTBA的特异性CT征象、内镜表现和病理特点。结果:6例患者均经支气管镜检查,再通过刚果红染色阳性确诊。所有患者CT和镜下均表现为弥漫性气道管壁不规则增厚,管腔不同程度狭窄,以累及气管下段、主支气管及叶支气管为著,黏膜触之易出血。5例患者大气道出现大小不等的壁结节,局部气道表面呈波浪样改变,典型者呈'铺路石征'。4例患者表现为气道壁广泛性轨道样及结节样钙化,累及大气道环周,其中轨道样钙化在诊断PTBA时最具特征性,在双侧主支气管表现尤为明显。2例患者出现阻塞性肺不张,3例患者存在肺门/纵隔淋巴结钙化,但均不具有特异性。CT初诊时误诊为支气管内膜结核等疾病,通过分析发现忽视大气道管壁广泛性增厚、'铺路石征'及轨道样钙化是导致误诊的主要原因。结论:加强CT的多方位观察,重视PTBA的特异性CT征象和支气管镜下所见是避免误诊的关键。李丹 王蕾 Thakur Asmit 王春宝 王渊 2015中南大学学报(医学版)2015,40,10:8
10Exosomes:Small vesicles with big roles in cancer,vaccine development,and therapeutics显示文摘Cancer is a deadly disease that is globally and consistently one of the leading causes of mortality every year.Despite the availability of chemotherapy,radiotherapy,immunotherapy,and surgery,a cure for cancer has not been attained.Recently,exosomes have gained significant attention due to the therapeutic potential of their various components including proteins,lipids,nucleic acids,miRNAs,and lncRNAs.Exosomes constitute a set of tiny extracellular vesicles with an approximate diameter of 30-100 nm.They are released from different cells and are present in biofluids including blood,cerebrospinal fluid(CSF),and urine.They perform crucial multifaceted functions in the malignant progression of cancer via autocrine,paracrine,and endocrine communications.The ability of exosomes to carry different cargoes including drug and molecular information to recipient cells make them a novel tool for cancer therapeutics.In this review,we discuss the major components of exosomes and their role in cancer progression.We also review important literature about the potential role of exosomes as vaccines and delivery carriers in the context of cancer therapeutics.Abhimanyu Thakur Diana Carolina Parra Pedram Motallebnejad Marcelo Brocchi Huanhuan Joyce Chen 2022Bioactive Materials2022,7,4:8
11Anti-HBc screening in Indian blood donors:Still an unresolved issue显示文摘AIM:To study the seroprevalence of antibody to hepatitis B core antigen (anti-HBc) in healthy blood donors negative for HBsAg and to evaluate whether anti-HBc detection could be adopted in India as a screening assay for HBV in addition to HBsAg. METHODS: A total of 1700 serum samples collected from HBsAg-negative healthy blood donors were tested for the presence of anti-HBc antibody (IgM + IgG). All samples reactive for anti-HBc antibody were then investigated for presence of anti-HBs and for liver function tests (LFTs). One hundred serum samples reactive for anti-HBc were tested for HBV DNA by PCR method. RESULTS: Out of 1700 samples tested, 142 (8.4%) blood samples were found to be reactive for anti-HBc. It was signif icantly lower in voluntary (6.9%) as compared to replacement donors (10.4%, P = 0.011). Seventy- two (50.7%) anti-HBc reactive samples were also reactive for anti-HBs with levels > 10 mIU/mL and 70 (49.3%) samples were non-reactive for anti-HBs, these units were labeled as anti-HBc-only. These 142 anti-HBc reactive units were also tested for liver function test. HBV DNA was detected in only 1 of 100 samples tested. CONCLUSION: Keeping in view that 8%-18% of donor population in India is anti-HBc reactive, inclusion of anti- HBc testing will lead to high discard rate. Anti-HBs as proposed previously does not seem to predict clearance of the virus. 12肠内营养联合吉西他滨+顺铂方案治疗Ⅳ期肺腺癌的临床疗效研究显示文摘目的探讨肠内营养(EN)联合吉西他滨+顺铂(GP)方案化疗对Ⅳ期肺腺癌的临床疗效。方法将52例经病理组织细胞学检查确诊的Ⅳ期肺腺癌且采用GP方案化疗的患者随机分为研究组和对照组,每组26例。对照组依日常习惯进食,每日外周静脉用葡萄糖、20%脂肪乳、氨基酸等营养支持治疗。研究组在对照组治疗基础上,加用肠内营养素佳膳或肠内营养粉剂(TP)。分别观察两组患者的近期疗效、营养指标(血红蛋白、血清清蛋白)和临床指标。结果治疗后对照组患者部分缓解8例、稳定12例、恶化6例;研究组部分缓解9例,稳定13例、恶化4例,两组患者的近期疗效间差异无统计学意义(u=-0.512,P>0.05)。两组患者治疗前血红蛋白、血清清蛋白水平比较,差异均无统计学意义〔血红蛋白:(96±8)g/L和(98±7)g/L;血清清蛋白:(34±4)g/L和(35±4)g/L,P>0.05〕,而治疗后比较,差异均有统计学意义〔血红蛋白:(104±8)g/L和(109±7)g/L;血清清蛋白:(37±4)g/L和(38±3)g/L,P<0.05〕。两组患者电解质紊乱发生率、化疗依从率及住院时间比较,差异均有统计学意义(P<0.05),而消化道不良反应发生率比较,差异无统计学意义(P>0.05)。结论肠内营养联合GP方案化疗是治疗Ⅳ期肺腺癌安全有效的方法,可改善肺腺癌化疗患者的营养状况,提高其化疗依从性,缩短住院时间。陈天君 孙忠民 Asmitanand Thakur 阳甜 李洋 陈明伟 2013中国全科医学2013,16,2:7

15支气管镜氩等离子体凝固对管腔内肺癌患者呼吸功能的影响显示文摘目的探讨支气管镜氩等离子体凝固(argon plasma coagulation,APC)对管腔内肺癌患者呼吸功能的影响和治疗安全性。方法对35例经支气管镜检查病理确诊的管腔内肺癌患者行APC治疗,对治疗前后呼吸功能(肺功能、动脉血氧分压)、体力状况评分、气道阻塞再通进行分析。结果经支气管镜APC治疗后,呼吸功能和体力状况评分有显著改善,气道再通有效率高(总有效率94.29%),治疗过程中没有发生严重并发症。结论支气管镜APC治疗能改善管腔内肺癌患者呼吸功能和生活质量,是治疗管腔内肺癌安全有效的方法。陈天君 Asmitanand Thakur 阳甜 朱柏 陈明伟 2011中国全科医学2011,14,33:6
16Ginsenoside Rg1 ameliorates bloodebrain barrier disruption and traumatic brain injury via attenuating macrophages derived exosomes miR-21 release显示文摘During the traumatic brain injury(TBI),improved expression of circulatory miR-21 serves as a diagnostic feature.Low levels of exosome-miR-21 in the brain can effectively improve neuroinflammation and bloodebrain barrier(BBB)permeability,reduce nerve apoptosis,restore neural function and ameliorate TBI.We evaluated the role of macrophage derived exosomes-miR-21(M-Exos-miR-21)in disrupting BBB,deteriorating TBI,and Rg1 interventions.IL-1β-induced macrophages(ⅡA)-Exos-miR-21 can activate NF-kB signaling pathway and induce the expressions of MMP-1,-3 and-9 and downregulate the levels of tight junction proteins(TJPs)deteriorating the BBB.Rg1 reduced miR-21-5 p content in ⅡA-Exos(RⅡA-Exos).The interaction of NMIIAe HSP90 controlled the release of Exos-miR-21,this interaction was restricted by Rg1.Rg1 could inhibit the Exos-miR-21 release in peripheral blood flow to brain,enhancing TIMP3 protein expression,MMPs proteolysis,and restricting TJPs degradation thus protected the BBB integrity.Conclusively,Rg1 can improve the cerebrovascular endothelial injury and hold the therapeutic potential against TBI disease.Kefeng Zhai Hong Duan Wei Wang Siyu Zhao Ghulam Jilany Khan Mengting Wang Yuhan Zhang Kiran Thakur Xuemei Fang Chao Wu Jianbo Xiao Zhaojun Wei 2021Acta Pharmaceutica Sinica B2021,11,11:6

19Magnesium based implants for functional bone tissue regeneration–A review显示文摘Magnesium(Mg)has emerged as one of the third-generation biomaterials for regeneration and support of functional bone tissue.Mg is a better choice over permanent implants such as titanium,stainless steel,cobalt-chrome as magnesium is biodegradable and does not require a second surgery for its removal after bone tissue recovery.It also reduces the risk of stress shielding as its elastic modulus is closer to human bone in comparison to permanent implants and other biodegradable metallic implants based on Iron and Zinc.Most importantly,Mg is osteoconductive thus stimulates new bone formation and possess anti-bacterial properties hence reducing the risk of failure due to infection.Despite its advantages,a major concern with pure Mg is its rapid bio-corrosion in presence of body fluids due to which the mechanical integrity of the implant deteriorates before healing of the tissue is complete.Mechanical properties of Mg-based implants can be enhanced by mechanical processing,alloying,and topology optimization.To reduce the corrosion/degradation rate,Mg has been alloyed with metals,reinforced with ceramics,and surface coatings have been applied so that the degradation rate of Mg-based implant matches with that of healing rate of bone tissue.The present review discusses the effect of alloying elements and reinforcing ceramics on microstructure,mechanical,and corrosion properties of Mg-based orthopedic implants.In addition,the biocompatibility of Mg-based alloys,composites,and coatings applied on Mg implants has been highlighted.Further,different methods of fabricating porous implants have been highlighted as making the implant porous facilitates the growth of new bone tissue through the pores.Gavish Uppal Amit Thakur Amit Chauhan Saroj Bala 2022Journal of Magnesium and Alloys2022,10,2:4
20Assessment of biomass and net primary productivity of a dry tropical forest using geospatial technology显示文摘This study quantifies biomass, aboveground and belowground net productivity, along with additional environmental factors over a 2-3 year period in Barnawapara Sanctuary of Chhattisgarh, India through satellite remotesensing and GIS techniques. Ten sampling quadrates20×20, 5×5 and 1×1 m were randomly laid for overstorey (OS), understorey (US) and ground vegetation(GS), respectively. Girth of trees was measured at breast height and collar diameters of shrubs and herbs at 0.1 m height. Biomass was estimated using allometric regression equations and herb biomass by harvesting. Net primary productivity (NPP) was determined by Ssumming biomass increment and litter crop values. Aspect and slope influenced the vegetation types, biomass and NPP in different forests. Standing biomass and NPP varied from 18.6 to101.5 Mg ha^(-1) and 5.3 to 12.7 Mg ha^(-1) a^(-1), respectively,in different forest types. The highest biomass was found in dense mixed forest, while net production recoded in Teak forests. Both were lowest in degraded mixed forests of different forest types. OS, US and GS contributed 90.4, 8.7and 0.7%, respectively, for the total mean standing biomass in different forests.
