1重度颌面颈部间隙感染的治疗--32例患者回顾性分析显示文摘目的:回顾总结重症颌面部间隙感染的治疗经验。方法:分析32例重症颌面颈部间隙感染患者的基本信息、治疗过程及转归,总结治疗经验。结果:2007-01~2013-01收治32位患者,病例数逐年增加,平均年龄57.71岁。11例(34.37%)患者出现呼吸道危象;19例(59.3%)伴发全身系统性疾病。8例患者转入胸外科及呼吸科,1例患者死亡,23例好转或治愈。脓送检率96.87%,检出率37.50%。培养结果以链球菌为主,其它可见葡萄球菌、铜绿假单胞菌、粪肠球菌、产黑素普雷沃菌、干燥奈瑟球菌、热带假丝酵母菌。结论:重症间隙感染患者近年明显增加。建议治疗措施:①保证呼吸道通畅及生命安全;②尽早手术切开引流,去除病灶;③降糖、降压、降温、镇痛、营养支持补液等多学科协同治疗;④全身抗感染治疗。丁明超 Ujjwal Koirala 王维戚 石晋 蔡宏伟 薄斌 赵晋龙 马秦 2014实用口腔医学杂志2014,30,5:20
2正颌手术不同术式治疗重度阻塞性睡眠呼吸暂停低通气综合征疗效回顾分析显示文摘目的 分析正颌手术不同术式治疗重度阻塞性睡眠呼吸暂停低通气综合征(obstructive sleep apneahypopnea syndrome,OSAHS)的疗效.方法 对2011年1月至2014年1月就诊于第四军医大学口腔医学院口腔颌面外科重度OSAHS患者24例,采用3种手术术式:①双颌水平前徙术+颏成形术(5例);②双颌逆时针前旋转+颏成形术(13例);③双颌水平前徙术+根尖下截骨后退术(6例).手术前、后对患者进行多导睡眠监测(polysomnography,PSG)和头影测量分析,比较术前、术后患者的呼吸紊乱指数(apnea and hypopea index,AHI)、体质量指数(body mass index,BMI)、平均血氧饱和度(average blood oxyen saturation,AOS)、后气道间隙(posterior airway space,PAS)、反映上颌突度的SNA角及反映下颌突度的SNB角等参数指标,采用SAS 8.02软件进行术前、术后配对t检验,评估手术效果.结果 依据斯坦福标准,手术治疗成功率达100%.双颌水平前徙术+颏成形术术前、术后AHI、SNA、SNB、SNPg(反映颏突度)、PAS比较,差异均有统计学意义(P<0.000 1);双颌逆时针前旋转+颏成形术术前、术后患者AHI、SNB、SNPg、PAS比较,差异均有统计学意义(P<0.01),SNA术前(82.25±2.71)°、术后(86.54±3.65)°(P=0.000 2),SNB术前(73.65±3.80)°,术后(81.37±2.96)°(P<0.000 1);双颌水平前徙术+根尖截骨后退术术前、术后患者AHI、SNPg、PAS比较,差异均有统计学意义(P<0.0001),SNA术前(82.18±4.27)°、术后(84.19±2.70)°(P=0.201 5),SNB术前(73.28±3.04)°、术后(75.35±2.56)°(P=0.2640),SNPg术前(75.91±4.06)°、术后(85.42±3.05)°(P<0.000 1).术后患者平均正畸时间8.3个月.结论 ①双颌前徙术是治疗重度OSAHS的有效术式,术前需行头影测量、计算机辅助设计,选择个性化手术术式;②双颌水平前徙术+颏前徙术术后面型前突畸形明显;③逆时针旋转双颌前徙术+颏前徙术达到治疗目的同时尽量减小对面型的影响;④双颌水平前徙术+根尖下截骨后退术对面型影响小,但手术时间长、创伤大,术后需较长时间正畸治疗.于擘 丁明超 石晋 Ujjwal Koirala 王维戚 马秦 2015中华口腔医学杂志2015,50,4:12

4Imaging in pulmonary hydatid cysts显示文摘Hydatid disease is a zoonosis that can involve almost any organ in the human body. After the liver, the lungs are the most common site for hydatid disease in adults. Imaging plays a pivotal role in the diagnosis of the disease, as clinical features are often nonspecific. Classical radiological signs of pulmonary hydatid cysts have been described in the literature, aiding in the diagnosis of the disease. However, complicated hydatid cysts can prove to be a diagnostic challenge at times due to their atypical imaging features. Radiography is the initial imaging modality. Computed tomography can provide a specific diagnosis in complicated cases. Ultrasound is particularly useful in peripheral lung lesions. 6Effect of forest fire on soil microbial biomass and enzymatic activity in oak and pine forests of Uttarakhand Himalaya, India显示文摘Background:Forest fire incidences in the Himalayan region of Uttarakhand,India are very common in summers.Pine and oak are the principal and dominant species of Himalayan subtropical forest and Himalayan temperate forest,respectively.Forest vegetation influences the physicochemical and biological properties of soil and forest fire in pine and oak forests may have a different effect on the physicochemical and biological properties of soil.Therefore,the present study was carried out to assess the impact of forest fire on soil microbial properties,enzymatic activity,and their relationship with soil physicochemical properties in the advent of forest fire in the pine and oak forests of the Garhwal region of Uttarakhand Himalaya,India.Results:The soil microbial biomass carbon and nitrogen,soil basal respiration,and acid phosphatase activity decreased,whereas dehydrogenase activity increased at burnt sites of both forest types.The overall change in soil microbial biomass carbon was 63 and 40%at the burnt oak forest and burnt pine forest,respectively.Dehydrogenase activity and acid phosphatase activity showed a strong positive correlation with soil organic matter(r=0.8)and microbial indices,respectively.The ratio of soil microbial biomass carbon/nitrogen was reduced at burnt sites of both forest types.Factor analysis results showed that fire had a significant impact on soil characteristics.The soil basal respiration was linked with macro-and micronutrients at burnt sites,whereas at control sites,it was linked with physicochemical properties of soil along with nutrients.Conclusion:Forest fire had a significant impact on soil properties of both forest types.The impact of forest fire on soil microbial biomass carbon was stronger in the oak forest than in the pine forest.Forest type influenced soil enzymatic activity at burnt sites.The bacterial community was dominated over fungi in burnt sites of both forests.Soil microbial indices can be used as a selective measure to assess the impact of fire.Furthermore,forest type plays an important role in regulating the impact of forest fire on soil properties.Devanshi Singh Priyanka Sharma Ujjwal Kumar Achlesh Daverey Kusum Arunachalam 2021Ecological Processes2021,10,1:2

9Generation of ultra-wide and flat optical frequency comb based on electro absorption modulator显示文摘A novel technique is proposed for the generation of ultra-wide and flat optical frequency comb(OFC)based on serially cascading three stages of electro absorption modulators(EAMs)through sinusoidal radio frequency(RF)signals by setting frequencies at f GHz,f/2 GHz and f/4 GHz.Here,the first stage acts as subcarrier generator,the second stage acts as subcarrier doubler,and the third stage acts as subcarrier quadrupler.In addition,a higher number of subcarriers can easily be generated by adjusting the driving sinusoidal RF signal.In this paper,cascading three stages of EAMs driven by50 GHz,25 GHz and 12.5 GHz clock sources,we obtain 272 subcarriers with spacing of 2.5 GHz and power deviation within 1 d B.Theoretical analysis of serially cascaded EAMs for subcarrier generation is also investigated.Principal analysis and simulation of this technique are demonstrated.Ujjwal Jaisingh Thangaraj 2018Optoelectronics Letters2018,14,3:2

16The future of mountain agriculture amidst continual farm-exit, livelihood diversification and outmigration in the Central Himalayan villages显示文摘Agrarian patterns in mountain ecosystems have seen a considerable change in the past decades with a vast proportion of the population abandoning traditional agriculture and diversifying their livelihood options, primarily through out-migration. This trend is especially common amongst the subsistence based family-farming communities in developing countries. This study aims at assessing methods of livelihood diversification and factors influencing farm-exit in Central Himalayan villages of Uttarakhand, India, while trying to understand local perspectives on challenges in pursuing agriculture as a viable livelihood option. We collected qualitative and quantitative data from 951 households across 60 villages evenly distributed across six hill districts of Uttarakhand, using key-person interviews and household surveys. The results of the study highlight that farm-exit is significantly influenced by livelihood diversification, number of migrants, number of female family members, and availability of irrigation facilities. In general, perception of the respondents towards agriculture as a viable livelihood option is rather unenthusiastic, with 87% of the respondents citing human wildlife conflict as the main challenge in pursuing farming, amongst several other challenges. Diversification is an integral component of present rural economy with 80.13% of the total population dependent on more than one source of income, to maintain their livelihoods. However, there is no statistically significant influence of livelihood diversification on annual income of the household. If agro-based entrepreneurial ventures are to be promoted in the region there is an urgent need for timely introduction of radical policy, institutional, and land-reforms.
