| 2 | Endoscopic diagnosis of extrahepatic bile duct carcinoma:Advances and current limitations显示文摘The accurate diagnosis of extrahepatic bile duct carcinoma is difficult,even now.When ultrasonography(US)shows dilatation of the bile duct,magnetic resonance cholangiopancreatography followed by endoscopic US(EUS)is the next step.When US or EUS shows localized bile duct wall thickening,endoscopic retrograde cholangiopancreatography should be conducted with intraductal US(IDUS)and forceps biopsy.Fluorescence in situ hybridization increases the sensitivity of brush cytology with similar specificity.In patients with papillary type bile duct carcinoma,three biopsies are sufficient.In patients with nodular or infiltrating-type bile duct carcinoma,multiple biopsies are warranted,and IDUS can compensate for the limitations of biopsies.In preoperative staging,the combination of dynamic multidetector low computed tomography(MDCT)and IDUS is useful for evaluating vascular invasion and cancer depth infiltration.However,assessment of lymph nodes metastases is difficult.In resectable cases,assessment of longitudinal cancer spread is important.The combination of IDUS and MDCT is useful for revealing submucosal cancer extension,which is common in hilar cholangiocarcinoma.To estimate the mucosal extenextension,which is common in extrahepatic bile duct carcinoma,the combination of IDUS and cholangioscopy is required.The utility of current peroral cholangioscopy is limited by the maneuverability of the“baby scope”.A new baby scope(10 Fr),called“SpyGlass”has potential,if the image quality can be improved.Since extrahepatic bile duct carcinoma is common in the Far East,many researchers in Japan and Korea contributed these studies,especially,in the evaluation of longitudinal cancer extension. | Kiichi Tamada Jun Ushio Kentaro Sugano | 2011 | World Journal of Clinical Oncology2011,2,5: | 26 |
| 3 | Transcatheter arterial chemoembolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma显示文摘AIM:To evaluate the efficacy of transcatheter arterial chemoembolization(TACE) using a suspension of a fine-powder formulation of cisplatin(DDPH) for hepatocellular carcinoma(HCC).METHODS:The study population was comprised of 164 patients who were treated by TACE alone.Of these patients,76 underwent TACE using a suspension of DDPH in lipiodol(LPD)(DDPH group),and the remaining 88 underwent TACE with an emulsion of doxorubicin(ADM) with LPD(ADM group).We compared the DDPH group with the ADM group in terms of the objective early response rate,progression free survival(PFS) and overall survival(OS).RESULTS:The objective early response rate in the DDPH group was significantly higher than that in the ADM group(54% vs 24%,P < 0.001).The PFS rate in the DDPH group was also significantly higher than that in the ADM group(P < 0.001).Moreover,the OS in the DDPH group was significantly longer than that in the ADM group(P = 0.002).Although the incidence rate of nausea or vomiting in the DDPH group was higher than that in the ADM group,the ADM group showed a higher incidence rate of the adverse events of hepatic arterial damage and leucopenia.No other serious complications were observed in either group.CONCLUSION:We conclude that TACE using a suspension of DDPH in LPD could be a useful treatment for HCC. | Kazuhiro Kasai Akira Ushio Kei Sawara Yasuhiro Miyamoto Yukiho Kasai Kanta Oikawa Hidekatsu Kuroda Yasuhiro Takikawa Kazuyuki Suzuki | 2010 | World Journal of Gastroenterology2010,16,27: | 6 |
| 4 | Therapeutic efficacy of transarterial chemo-embolization with a fine-powder formulation of cisplatin for hepatocellular carcinoma显示文摘AIM:To evaluate the efficacy of transarterial chemoembolization(TACE) using a suspension of a finepowder formulation of cisplatin(DDPH) in lipiodol(LPD) in the treatment of hepatocellular carcinoma(HCC).METHODS:The subjects were 262 HCC patients treated with TACE using a DDPH-LPD suspension.The DDPH-LPD suspension was prepared by mixing 50 mg of DDPH into 10 mL of LPD.TACE was repeated when treated lesions relapsed and/or new hepatic lesions were detected.These patients received additional TACE using the same agent.TACE was repeated until complete regression of the tumor was obtained.The primary efficacy endpoint of the current study was the objective early response rate.Secondary efficacy endpoints were progression-free survival(PFS) and overall survival.RESULTS:The objective early response rate was 43.6%.Cumulative PFS rates were 56.7% at 6 mo,23.1% at 12 mo,13.4% at 18 mo,and 10.5% at 24 mo.The median PFS was 6.6 mo.Cumulative survival rates were 90.6% at 6 mo,81.9% at 12 mo,70.5% at 24 mo,and 58.8% at 36 mo.Median survival time was 46.6 mo.All adverse reactions were controllable by temporary suspension of treatment.No serious complications or treatment-related deaths were observed.CONCLUSION:TACE using a suspension of DDPH in LPD may be a useful treatment for HCC. | Kazuhiro Kasai Akira Ushio Yukiho Kasai Kei Sawara Yasuhiro Miyamoto Kanta Oikawa Yasuhiro Takikawa Kazuyuki Suzuki | 2013 | World Journal of Gastroenterology2013,19,14: | 3 |

| 8 | Changes in physicochemical properties of proteins in Kayserian Pastirma made from the M.semimembranosus muscle of cows during traditional processing显示文摘In the current study,we examined the effects of beef processing to produce pastirma on the physicochemical properties of proteins in M.semimembranosus(SEM)muscle.Protein concentrations significantly increased in pastirma muscles(P<0.01),as a result of the salting and curing process.The surface hydrophobicity values of processed samples were higher than those without processing at all guanidine-HCl concentrations,suggesting hydrophobicity increased,which may attribute to the new generated peptides during the traditional pastirma-making process.The metmyoglobin content greatly increased(by as much as 89%)in pastirma samples compared with the unprocessed samples.The images of histology also demonstrate that the pastirma processing had no negative impact on the structure of the muscle.The results from this study suggest that the traditional pastirma-making process catalyzed the enzymatic digestion of muscle proteins,and the differences in some physicochemical parameters between the control and pastirma samples were thus likely to be contributable to protein digestion.Thus,the traditional pastirma-making process results in the degradation of many proteins into peptides,which might then be obtainable as functional components to treat human dietand lifestyle-related diseases such as hypertension,hyperglycemia syndromes or to be used as nutraceuticals.©2013 Beijing Academy of Food Sciences.Production and hosting by Elsevier B.V.All rights reserved. | Abdulatef Mrghni Ahhmed Gen Kaneko Hideki Ushio Tomo Inomata Hasan Yetim Safa Karaman Michio Muguruma Ryoichi Sakata | 2013 | Food Science and Human Wellness2013,2,1: | 2 |
