| 5 | Transradial approach for acute stroke intervention: technical procedure and clinical outcomes显示文摘background and purpose Radial artery catheterisation is an alternate route of access that has recently started to gain more widespread use for neuroendovascular procedures,including acute stroke intervention.In this small case series,we present our institution’s outcomes in patients undergoing acute stroke interventions via transradial access.Materials and methods We present a retrospective study of 15 patients who underwent acute stroke intervention via radial artery access.We analyse these patients’periprocedural and clinical outcomes after undergoing mechanical thrombectomy.results A total of 15 consecutive patients were included in the study(9 males and 6 females),and all patients were able to successfully undergo mechanical thrombectomy via radial artery access.The mean time of arterial puncture to reperfusion was 50±28 min(range:15-104).A TICI 2b/3 revascularisation was achieved in 13/15 patients(87%);a TICI 1 and TICI 2a outcome was achieved on the other two patients.One patient incurred an iatrogenic vessel dissection during the procedure.Eight of out 15 patients(53%)had favourable mRS(0-3)at the time of discharge from the hospital.Conclusion Radial artery catheterisation is technically feasible for performing acute stroke interventions with favourable time to revascularisation and good overall clinical outcomes. | Omaditya Khanna Nikolaos Mouchtouris Ahmad Sweid Nohra Chalouhi Ritam Ghosh Fadi Al Saiegh Michael R Gooch Stavropoula Tjoumakaris Robert H Rosenwasser Victor Romo Pascal Jabbour | 2020 | Stroke & Vascular Neurology2020，5，1： | 4 |
| 6 | Season of the year influences infection rates following total hip arthroplasty显示文摘AIM To research the influence of season of the year on periprosthetic joint infections.METHODS We conducted a retrospective review of the entire Medicare files from 2005 to 2014. Seasons were classified as spring, summer, fall or winter. Regional variations were accounted for by dividing patients into four geographic regions as per the United States Census Bureau(Northeast, Midwest, West and South). Acute postoperative infection and deep periprosthetic infections within 90 d after surgery were tracked. RESULTS In all regions, winter had the highest incidence of periprosthetic infections(mean 0.98%, SD 0.1%) and was significantly higher than other seasons in the Midwest, South and West(P < 0.05 for all) but not the Northeast(P = 0.358). Acute postoperative infection rates were more frequent in the summer and were significantly affected by season of the year in the West.CONCLUSION Season of the year is a risk factor for periprosthetic joint infection following total hip arthroplasty(THA). | 7 | Interferon-β induced in female genital epithelium by HIV-1 glycoprotein 120 via Toll-like-receptor 2 pathway acts to protect the mucosal barrier显示文摘More than 40%of HIV infections occur via female reproductive tract(FRT)through heterosexual transmission.Epithelial cells that line the female genital mucosa are the first line of defense against HIV-1 and other sexually transmitted pathogens.These sentient cells recognize and respond to external stimuli by induction of a range of carefully balanced innate immune responses.Previously,we have shown that in response to HIV-1 gp120,the genital epithelial cells(GECs)from upper reproductive tract induce an inflammatory response that may facilitate HIV-1 translocation and infection.In this study,we report that the endometrial and endocervical GECs simultaneously induce biologically active interferon-β(IFNβ)antiviral responses following exposure to HIV-1 that act to protect the epithelial tight junction barrier.The innate antiviral response was directly induced by HIV-1 envelope glycoprotein gp120 and addition of gp120 neutralizing antibody inhibited IFNβproduction.Interferon-βwas induced by gp120 in upper GECs through Toll-like receptor 2 signaling and required presence of heparan sulfate on epithelial cell surface.The induction of IFNβwas dependent upon activation of transcription factor IRF3(interferon regulatory factor 3).The IFNβwas biologically active,had a protective effect on epithelial tight junction barrier and was able to inhibit HIV-1 infection in TZM-bl indicator cells and HIV-1 replication in T cells.This is the first report that recognition of HIV-1 by upper GECs leads to induction of innate antiviral pathways.This could explain the overall low infectivity of HIV-1 in the FRT and could be exploited for HIV-1 prophylaxis. | Aisha Nazli Sara Dizzell Muhammad Atif Zahoor Victor H Ferreira Jessica Kafka Matthew William Woods Michel Ouellet Ali A Ashkar Michel J Tremblay Dawn ME Bowdish Charu Kaushic | 2019 | Cellular & Molecular Immunology2019，16，2： | 2 |
