1Coronary heart disease:Significance of liver X receptor α genomics显示文摘Crosstalk between lipid peroxidation and inflammation is known to be a pathognomonic feature for the development of coronary heart disease(CHD).In this regard ligand activated liver X receptor(LXR)-α has emerged as a key molecular switch by its inherent ability to modulate an array of genes involved in these two fundamental cellular processes.In addition,LXR-α has also been found to play a role in hepatic lipogenesis and innate immunity.Although several lines of evidence in experimental model systems have established the atheroprotective nature of LXR-α,human subjects have been reported to possess a paradoxical situation in which increased blood cellular LXR-α gene expression is always accompanied by increased coronary occlusion.This apparent paradox was resolved recently by the finding that CHD patients possess a deregulated LXR-α transcriptome due to impaired ligand-receptor interaction.This blood cellular mutated LXR-α gene ex- pression correlated specifically with the extent of coro- nary occlusion and hence need is felt to devise new synthetic ligands that could restore the function of this mutated LXR-αprotein in order to modulate genes involved in reverse cholesterol transport and suppression of the inflammatory response leading to the effective treatment of CHD.Vivek Priy Dave Deepak Kaul 2010World Journal of Cardiology2010,2,6:3

3Lipid-polymer hybrid nanoparticles: Development & statistical optimization of norfloxacin for topical drug delivery system显示文摘Poly lactic acid is a biodegradable, biocompatible, and non-toxic polymer, widely used in many pharmaceuticalpreparations such as controlled release formulations, parenteral preparations, surgicaltreatment applications, and tissue engineering. In this study, we prepared lipid-polymer hybrid nanoparticlesfor topical and site targeting delivery of Norfloxacin by emulsification solvent evaporationmethod (ESE). The design of experiment (DOE) was done by using software to optimize the result, andthen a surface plot was generated to compare with the practical results. The surface morphology, particlesize, zeta potential and composition of the lipid-polymer hybrid nanoparticles were characterized bySEM, TEM, AFM, and FTIR. The thermal behavior of the lipid-polymer hybrid nanoparticles was characterizedby DSC and TGA. The prepared lipid-polymer hybrid nanoparticles of Norfloxacin exhibited anaverage particle size from 178.6 ± 3.7 nm to 220.8 ± 2.3 nm, and showed very narrow distribution withpolydispersity index ranging from 0.206 ± 0.36 to 0.383 ± 0.66. The surface charge on the lipid-polymerhybrid nanoparticles were confirmed by zeta potential, showed the value from t23.4 ± 1.5 mVto t41.5 ± 3.4 mV. An Antimicrobial study was done against Staphylococcus aureus and Pseudomonasaeruginosa, and the lipid-polymer hybrid nanoparticles showed potential activity against these two.Lipid-polymer hybrid nanoparticles of Norfloxacin showed the %cumulative drug release of 89.72% in24 h. A stability study of the optimized formulation showed the suitable condition for the storage oflipid-polymer hybrid nanoparticles was at 4 ± 2 C/60 ± 5% RH. 5Clinical benefit and tolerability of adjuvant intraperitoneal chemotherapy in patients who have or have not received neoadjuvant chemotherapy for advanced ovarian cancer显示文摘BACKGROUND Adjuvant chemotherapy using intraperitoneal(IP)treatment has demonstrated survival benefit over intravenous(IV)therapy alone in patients treated with upfront debulking surgery for advanced stage ovarian cancer.Neoadjuvant chemotherapy followed by interim surgery and adjuvant chemotherapy has similar outcome in survival as compared to upfront surgery followed by adjuvant IV chemotherapy.IP chemotherapy has not been widely adopted in clinical practice for a number of reasons.Whether IP chemotherapy delivered in the patients who received neoadjuvant chemotherapy can be well tolerated or confers any clinical benefit has not been well studied.AIM To evaluate the experience of adjuvant IP chemotherapy in the community cancer clinic setting,and the clinical benefit and tolerability of incorporating IP chemotherapy in patients who received neoadjuvant treatment.METHODS We retrospectively evaluated toxicities and outcomes of patients with stage III and IV ovarian cancer diagnosed at our institution between 07/2007 and 07/2015 who received intraperitoneal chemotherapy after cytoreductive surgery(group 1)or after neoadjuvant chemotherapy followed by interim surgery(group 2).RESULTS Thirty eight patients were treated with IP chemotherapy,median age was 54 years old(range 38.6 to 71 years).In group 1(n=25),12(48%)of the patients completed 4 or more cycle of IP treatment after upfront debulking surgery;while in group 2(n=13),8(61.5%)of the patients completed all 3 cycles of the assigned IP chemotherapy after receiving neoadjuvant IV chemotherapy followed by surgery,and 2(15.4%)more patients tolerated more than 3 cycles.In those patients who did not get planned IP chemotherapy,most of them were treated with substitutional IV chemotherapy,and the completion rate for 6 cycles of IV+IP was 92%.Abdominal pain,(64%in group 1 and 38%in group 2),vomiting,(36%in group 1 and 30.8%in group 2),dehydration(16%in group 1 and 15.4%in group 2),and hypomagnesemia(12%in group 1 and 15.4%in group 2)were the most common adverse effects in all patients,while patients who have received neoadjuvant chemotherapy were more likely to get hypokalemia,fatigue and renal insufficiency.Progression free survival(PFS)was 26.5 mo(95%CI 14.9,38.0)in group 1 and 27.6 mo(95%CI 13.1,42.1)in group 2.The overall survival was 100.2 mo(95%CI 67.9,132.5)for group 1 and 68.2 mo(95%CI 32.2,104.0)for group 2.For the entire cohort,PFS was 26.5 mo(95%CI 15.9,37.0)and OS was 78.8 mo(95%CI 52.3,105.4).CONCLUSION The use of IP/IV chemotherapy can be safely administrated in the community cancer clinic setting.The use of IP/IV chemotherapy in patients who have received neoadjuvant chemotherapy followed by surgery is feasible and tolerable.Despite various modification of the IP regimen,incorporation of IP chemotherapy in the adjuvant setting appears to be associated with improved PFS and overall survival.Trishala Meghal Vishangi Dave Horace Tang Vivek Kumar Yiqing Xu 2019World Journal of Clinical Oncology2019,10,5:1

7Design and evaluation of aceclofenac ocular inserts with special reference to cataracts and conjunctivitis显示文摘The eyes present unique opportunities and challenges when it comes to the delivery of pharmaceuticals.While absorption by this route is bungling,there are a few side effects with conventional dosage forms.Ocular inserts were prepared with prolonged release of drug and minimum swelling within cul-de-sac using aceclofenac.The work focused on treatment of conjunctivitis and cataracts by formulating ocular inserts of different polymeric combination of aceclofenac using hydroxypropyl methyl cellulose(HPMC,3% to 5%),chitosan(3% to 5%),poly vinyle alcohol(PVA,3% to 5%),methyl cellulose(MC,3% to 5%) as drug reservoir and ethyl cellulose(EC) polymer as rate controlling membrane by solvent casting technique with the objective of increasing contact time,achieving controlled release and greater therapeutic efficiency.The prepared ocular insert were then evaluated for physical appearances tensile strength,elongation at break(%),weight variation,uniformity of thickness,moisture absorption(%),pH,folding endurance,Fourier Transform Infrared spectroscopy,differential scanning calorimetry.Physicochemical characterization and in vitro transcorneal permeation studies reveals that,the prepared ocular insert formulations F2 and F8 containing HPMC and PVA had released their drug content,98.54% and 96.24%,respectively,over an extended period of 24 h.Hence these formulations were selected as best optimized formulations.It can be concluded that hydroxy propyl methyl cellulose is a good film forming hydrophilic polymer which shows potential agent for ocular drug delivery system.Incorporation of polyethylene glycol enhances the permeability of aceclofenac ocular insert and has perfect zero order release,proving a promising controlled release delivery system.Vivek Dave Sachdev Yadav Sarvesh Paliwal Swapnil Sharma Dhirendra Kumar 2013Journal of Chinese Pharmaceutical Sciences2013,22,5:1
8Heart and Brain:A neutro-genomic link显示文摘The philosophy of heart and brain are very ancient in our literature where the things good for the heart are not suggested good for the brain and vice-versa.Modern medicine is characterized by a high degree of specialization and the heart-brain connection that could be targeted to treat these complex cardiovascular/brain disorders.The idea that adverse diet/genome interactions can cause disease is not new.In the recent era the science of nutritional genomics have increased our understanding of diet-health-gene interactions and have provided a number of benefits for individuals,groups and societies.Since dietary chemicals are regularly ingested and participate indirectly and directly in regulating gene expression,it follows that a subset of genes regulated by diet must be involved in disease initiation,progression,and severity.In this regards Liver X Receptor(LXR)-a,a key transcription factors,associated with the several chronic pathological situation including coronary heart disease and neurodegenerative diseases have recently been found to be regulated by the dietary components.The crucial findings at molecular biology unit,Post Graduate Institute of Medical Education and Research(PGIMER),Chandigarh,INDIA have not only forced us to explore nutritional genomics as a holistic systems approach to understand the relationship between diet and health,but also to look into the disease preventing and health promoting foods that match our lifestyles,cultures and genetics.After all,we are what we eat.Vivek Priy Dave Deepak Kaul 2013World Journal of Biological Chemistry2013,4,2:0
