3Would integrated Western and traditional Chinese medicine have more benefits for stroke rehabilitation? A systematic review and meta-analysis显示文摘Background Stroke is a major cause of death or long-term disability worldwide.Many patients with stroke receive integrative therapy consisting of Western medicine(WM)and routine rehabilitation in conjunction with Chinese medicine(CM),such as acupuncture and Chinese herbal medicine.However,there is no available evidence on the effectiveness of the combined use of WM and CM interventions in stroke rehabilitation.Aims The purpose of this meta-analysis is to evaluate the results of all individual studies to assess the combined use of CM and WM in stroke rehabilitation compared with WM only.Methods The Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines were followed.MEDLINE,EMBASE,Cochrane and China National Knowledge Infrastructure(CNKI)were searched.The included outcomes were dependency,motor function,depression and swallowing function.Subgroup analysis was performed,and publication bias was assessed using funnel plots.Summary of review 58 studies and 6339 patients were included in the meta-analysis.Subgroup analysis revealed that combined therapy comprising both acupuncture and WM had a superior effect on improving dependency and swallowing function compared with standard WM therapy alone.Potential superiority of combined therapy comprising CM and WM in improving depression compared with standard WM therapy was also found.Conclusions Our results indicate that the combined use of CM and WM could be more efficacious in stroke rehabilitation compared with the use of WM therapy alone.However,most studies were short in duration(2 to 4 weeks)and prone to different types of biases,which prevents making any conclusion regarding the long-term effects and raises concerns regarding true efficacy in context of high likelihood of Hawthorn bias.So,more randomised controlled trials with more rigorous design and longer duration of treatment and follow-up need to be conducted to compare WM alone versus WM and CM combined.Linda LD Zhong Ya Zheng Alexander Y Lau Norman Wong Liang Yao Xingyao Wu Tengteng Shao Zhenxing Lu Huijuan Li Chun Sum Yuen Jianwen Guo Suzanne Lo Janita Chau Kam Wa Chan Bacon Fung Leung Ng Zhaoxiang Bian Edwin Chau-leung Yu 2022Stroke & Vascular Neurology2022,7,1:7
4Role of stem cell therapies in treating chronic wounds:A systematic review显示文摘BACKGROUND The impairment of cutaneous wound healing results in chronic,non-healing wounds that are caused by altered wound environment oxygenation,tissue injury,and permissive microbial growth.Current modalities for the treatment of these wounds inadequately address the complex changes involved in chronic wound pathogenesis.Consequently,stem cell therapies have emerged as a potential therapeutic modality to promote cutaneous regeneration through trophic and paracrine activity.AIM To investigate current literature regarding use of stem cell therapies for the clinical treatment of chronic,non-healing wounds.METHODS PubMed,EMBASE,Cochrane Library,Web of Science,and Scopus were queried with combinations of the search terms“mesenchymal stem cells,”“adult stem cells,”“embryonic stem cells,”“erythroid precursor cells,”“stem cell therapies,”and“chronic wounds”in order to find relevant articles published between the years of 2000 and 2019 to review a 20-year experience.Reference lists from the articles were reviewed to identify additional pertinent articles.Retrieved manuscripts(reviews,case reports/series,retrospective/prospective studies,and clinical trials)were evaluated by the authors for their depiction of clinical stem cell therapy use.Data were extracted from the articles using a standardized collection tool.RESULTS A total of 43 articles describing the use of stem cell therapies for the treatment of chronic wounds were included in this review.While stem cell therapies have been explored in in vitro and in vivo applications in the past,recent efforts are geared towards assessing their clinical role.A review of the literature revealed that adipose-derived stem cells,bone marrow-derived stem cells,bone marrowderived mononuclear cells,epidermally-derived mesenchymal stem cells,fibroblast stem cells,keratinocyte stem cells,placental mesenchymal stem cells,and umbilical cord mesenchymal stem cells have all been employed in the treatment of chronic wounds of various etiologies.Most recently,embryonic stem cells have emerged as a novel stem cell therapy with the capacity for multifaceted germ cell layer differentiation.With the capacity for self-renewal and differentiation,stem cells can enrich existing cell populations in chronic wounds in order to overcome barriers impeding the progression of wound healing.Further,stem cell therapies can be utilized to augment cell engraftment,signaling and activity,and resultant patient outcomes.CONCLUSION Assessing observed clinical outcomes,potential for stem cell use,and relevant therapeutic challenges allows wound care stakeholders to make informed decisions regarding optimal treatment approaches for their patients’chronic wounds.Anjali C Raghuram Roy P Yu Andrea Y Lo Cynthia J Sung Melissa Bircan Holly J Thompson Alex K Wong 2020World Journal of Stem Cells2020,12,7:5

7机器人在头颈外科中的应用--香港中文大学经验显示文摘目的:机器人手术目前已在头颈部多个区域得到了应用,本文探讨香港中文大学头颈外科机器人手术的经验。方法:回顾性分析2015年1月以来在香港中文大学耳鼻咽喉头颈外科开展的所有机器人头颈外科手术病例资料。结果:24例患者采用达芬奇S或达芬奇Xi系统进行了手术,21例患者使用新型柔性机器人达芬奇SP系统行了手术。均无二次手术等严重不良事件。已经发表的达芬奇SP系统手术的7例患者早期结果表明,通过达芬奇SP系统可以到达鼻咽、口咽部、下咽部、喉部和咽后淋巴结完成手术,不需中途改变手术方式,没有发生与使用该系统有关的严重不良事件或不良事件。结论:通过我们在香港中文大学的经验,我们认为采用达芬奇机器人系统进行经口手术或经耳后入路颈部手术是安全有效的。陈英权 王维杨 梁发雅 黄晓明 Jason Y K Chan Eddy W Y Wong 2018临床耳鼻咽喉头颈外科杂志2018,32,14:4
8北京市城乡40岁以上限定人群视网膜血管异常的患病率显示文摘目的描述北京市城乡限定人群视网膜血管异常(RMA)患病率及其分布特征。设计横断面研究。研究对象'北京眼病研究'城乡40岁以上居民4439人。方法对每位受检者进行问卷调查和全面眼科检查,其中包括散瞳后彩色眼底照相检查。随机选每位受检者一眼进行眼底照片中视网膜血管异常的评价。局限性视网膜血管异常(局限性视网膜动脉缩窄、动静脉交叉压迫征和视网膜病变)采用定性方法评价;弥漫性视网膜动脉缩窄采用计算机辅助的定量方法评价。计算以上各种视网膜血管异常的患病率,并分析其分布特征。主要指标视网膜血管异常的患病率。结果局限性视网膜动脉缩窄、动静脉交叉压迫征和视网膜病变的患病率分别为7.5%、7.2%和7.5%,三者均随年龄的增加而逐渐增加(P<0.001);男性的视网膜血管异常患病率(24.4%)明显比女性(17.6%)高(P<0.001)。各种局限性视网膜血管异常和弥漫性视网膜动脉缩窄的患病率在农村人群均明显高于城市人群(P均<0.001)。结论北京市限定地区城乡40岁以上人群中视网膜血管异常并不少见,其患病率随年龄增加逐渐增加;农村居民比城市居民患病率高。王爽 徐亮 Jonas Jost B Wong Tien Y 王云松 王亚星 杨桦 李静 Sun Cong 2010眼科2010,19,4:4
9新加坡成人慢性呼吸系统疾病患者6分钟步行距离的最小临床重要差异值研究(英文)显示文摘目的:6 min步行距离(6MWD)是临床上常见的功能性运动能力测试,特别是对于慢性呼吸系统疾病患者。一些文献记载了关于白种人的6MWD的最小临床重要差异(MCID),但对亚洲人群的相关信息未见记载。本研究旨在建立亚洲成人慢性呼吸道疾病患者6MWD的最小临床重要差异。方法:采用'锚定'法和'分布'法对新加坡中央医院肺康复计划登记处的资料进行了分析。'锚定'法收集确诊为慢性呼吸道疾病的患者50例,其中男38例,女12例;平均年龄(64.2±12.3)岁,通过电话采访及使用整体变化评估量表(GROC)评价患者肺康复计划后的自觉改善程度,再将患者在肺康复计划中6MWD的改变数值与相对应的患者自觉改善程度进行比较,这种关系用操作特征曲线(ROC)统计表示。通过'分布'法确定6MWD的MCID,从门诊和住院的肺康复计划(PRP)参与者中检索医疗记录136例,其中男103例,女33例;平均年龄(60.1±14.1)岁。结果 :ROC曲线表明6MWD出现临床显著性变化的最佳临界值为17.4 m,其对应的敏感性为79.4%,特异性为81.3%,阳性似然比为4.23。由'分布'法确定6MWD的最小临床重要差异值为18.6 m。结论:18.6 m为新加坡华人6MWD的最小临床重要差异的估计值。Meredith T L YEUNG Alan W P WONG Katherine S L HUANG Alice Y M JONES 2015康复学报2015,25,1:3

16Risk stratification in symptomatic intracranial atherosclerotic disease with conventional vascular risk factors and cerebral haemodynamics显示文摘Background and purpose Symptomatic intracranial atherosclerotic stenosis(sICAS)is associated with a considerable risk of recurrent stroke despite contemporarily optimal medical treatment.Severity of luminal stenosis in sICAS and its haemodynamic significance quantified with computational fluid dynamics(CFD)models were associated with the risk of stroke recurrence.We aimed to develop and compare stroke risk prediction nomograms in sICAS,based on vascular risk factors and these metrics.Methods Patients with 50%-99%sICAS confirmed in CT angiography(CTA)were enrolled.Conventional vascular risk factors were collected.Severity of luminal stenosis in sICAS was dichotomised as moderate(50%-69%)and severe(70%-99%).Translesional pressure ratio(PR)and wall shear stress ratio(WSSR)were quantified via CTA-based CFD modelling;the haemodynamic status of sICAS was classified as normal(normal PR&WSSR),intermediate(otherwise)and abnormal(abnormal PR&WSSR).All patients received guideline-recommended medical treatment.We developed and compared performance of nomograms composed of these variables and independent predictors identified in multivariate logistic regression,in predicting the primary outcome,recurrent ischaemic stroke in the same territory(SIT)within 1 year.Results Among 245 sICAS patients,20(8.2%)had SIT.The D2H2A nomogram,incorporating diabetes,dyslipidaemia,haemodynamic status of sICAS,hypertension and age≥50 years,showed good calibration(P for Hosmer-Lemeshow test=0.560)and discrimination(C-statistic 0.73,95%CI 0.60 to 0.85).It also had better performance in risk reclassification and provided larger net benefits in decision curve analysis,compared with nomograms composed of conventional vascular risk factors only,and plus the severity of luminal stenosis in sICAS.Sensitivity analysis in patients with anterior-circulation sICAS showed similar results.Conclusions The D2H2A nomogram,incorporating conventional vascular risk factors and the haemodynamic significance of sICAS as assessed in CFD models,could be a useful tool to stratify sICAS patients for the risk of recurrent stroke under contemporarily optimal medical treatment.Xuan Tian Hui Fang Linfang Lan Hing Lung Ip Jill Abrigo Haipeng Liu Lina Zheng Florence S Y Fan Sze Ho Ma Bonaventure Ip Bo Song Yuming Xu Jingwei Li Bing Zhang Yun Xu Yannie O Y Soo Vincent Mok Ka Sing Wong Thomas W Leung Xinyi Leng 2023Stroke & Vascular Neurology2023,8,1:2
