| 1 | Surgical intervention for advanced valvular heart disease in 227 cases显示文摘Background Although the results of surgical treatment in cardiac valve disease continue to improve, the postoperative mortality rate and the rate of complications in patients with advanced valvular heart disease (AVHD) are still very high. We did this retrospective study to summarize the surgical experience of heart valve replacement for patients with AVHD and discuss effective ways to improve the surgical outcome.Methods From January 1994 to October 2003, surgical procedures of heart valve replacement were performed on 227 (136 men and 91 women) patients with AVHD in our Department of Cardiothoracic Surgery. The clinical data of all patients were collected and analysed. Patients’ age ranged from 10 years to 77 years. In preoperative cardiac function grading, 157 cases were NYHA III and 70 cases NYHA IV. Fifty-one patients had had cardiac operations. The ultrasonic cardiac graphs showed that 145 patients suffered from moderate or severe pulmonary hypertension and 73 had combined giant left ventricle. Mitral valve replacement was performed in 32 cases, aortic valve replacement in 90, tricuspid valve replacement in 1, combined mitral and aortic replacement in 103 and combined mitral and tricuspid replacement in 1. Nineteen patients also received surgical corrections for other minor abnormalities during the operations. A logistic model was established to evaluate the influence of perioperative factors on the mortality rate. Results The operative mortality rate was 13.2% (30/227). The main causes of death included multiple organ dysfunction syndrome (MODS), low cardiac output syndrome and ventricular fibrillation. From the results of the binary noncounterpart multivariate logistic regression, the following statistically significant factors were found to influence the operative mortality rate: redo operation, age ≥55 years, preoperative NYHA cardiac function grading, extracorporeal circulation time ≥120 minutes and postoperative usage of GIK (glucose, insulin and potassium) solution. All factors were risk ones except postoperative application of GIK. The Hosmer-Lemeshow goodness of fit coefficient of this model was 0.976. Conclusions The risk factors associated with postoperative mortality rate in the patients with AVHD were redo operation, age ≥55 years, preoperative NYHA cardiac function grading and extracorporeal circulation time ≥120 minutes. Postoperative usage of GIK acted as a kind of metabolic therapy and will improve the recovery for patients with AVHD. | 2 | The tribological behavior of a surface-nanocrystallized magnesium alloy AZ31 sheet after ultrasonic shot peening treatment显示文摘A magnesium alloy AZ31 sheet was processed by ultrasonic shot peening treatment to fabricate a surface nanocrystalline,and a ball-on-disk dry sliding wear test was performed to evaluate the tribological behavior after treatment.The microstructure observation indicated a gradient nanocrystalline structure was formed after USSP treatment.The microhardness at the top surface was improved from 60 HV to 145 HV after treatment.The formed nanocrystalline resulted in an easy formation of MgO patches on the surface and reduced the coefficient of friction.Moreover,the formed nanocrystalline leaded to a retard of delamination with increasing the sliding speed and applied load,which was due to its stronger sub-surface.Under high sliding speed(0.5 m/s)and high applied load(50 N),it was firstly found that the formed nanocrystalline prevented the happening of thermal softening and melting.The possible reasons accounting for the prevention of thennal softening and melting were discussed accordingly. | Jianyue Zhang Yongxin Jian Xuzhe Zhao Dean Meng Fusheng Pan Qingyou Han | 2021 | Journal of Magnesium and Alloys2021,9,4: | 3 |

| 18 | Synchronous measurements of prefrontal activity and pulse rate variability during online video game playing with functional near-infrared spectroscopy显示文摘Interactions between the central nervous system(CNS)and autonomic nervous system(ANS)play a crucial role in modulating perception,cognition,and emotion production.Previous studies on CNS–ANS interactions,or heart–brain coupling,have often used heart rate variability(HRV)metrics derived from electrocardiography(ECG)recordings as empirical measurements of sympathetic and parasympathetic activities.Functional near-infrared spectroscopy(fNIRS)is a functional brain imaging modality that is increasingly used in brain and cognition studies.The fNIRS signals contain frequency bands representing both neural activity oscillations and heartbeat rhythms.Therefore,fNIRS data acquired in neuroimaging studies can potentially provide a single-modality approach to measure task-induced responses in the brain and ANS synchronously,allowing analysis of CNS–ANS interactions.In this proof-of-concept study,fNIRS was used to record hemodynamic changes from the foreheads of 20 university students as they each played a round of multiplayer online battle arena(MOBA)game.From the fNIRS recordings,neural and heartbeat frequency bands were extracted to assess prefrontal activities and shortterm pulse rate variability(PRV),an approximation for short-term HRV,respectively.Under the experimental conditions used,fNIRS-derived PRV metrics showed good correlations with ECG-derived HRV golden standards,in terms of absolute measurements and video game playing(VGP)-related changes.It was also observed that,similar to previous studies on physical activity and exercise,the PRV metrics closely related to parasympathetic activities recovered slower than the PRV indicators of sympathetic activities after VGP.It is concluded that it is feasible to use fNIRS to monitor concurrent brain and ANS activations during online VGP,facilitating the understanding of VGP-related heart–brain coupling. | Xuzhe Zhang Kehong Long Ningxin Wang Jin Zhang Hao Lei | 2023 | Journal of Innovative Optical Health Sciences2023,16,6: | 1 |
