1Fast-track surgery could improve postoperative recovery in radical total gastrectomy patients显示文摘AIM: To assess the impact of fast-track surgery (FTS) on hospital stay, cost of hospitalization and complications after radical total gastrectomy. METHODS: A randomized, controlled clinical trial was conducted from November 2011 to August 2012 in the Department of Digestive Surgery, Xijing Hospital of Digestive Diseases, the Fourth Military Medical University. A total of 122 gastric cancer patients who met the selection criteria were randomized into FTS and conventional care groups on the first day of hospitalization. All patients received elective standard D2 total gastrectomy. Clinical outcomes, including duration of flatus and defecation, white blood cell count, postoperative pain, duration of postoperative stay, cost of hospitalization and complications were recorded and evaluated.Two specially trained doctors who were blinded to the treatment were in charge of evaluating postoperative outcomes, discharge and follow-up. RESULTS: A total of 119 patients finished the study, including 60 patients in the conventional care group and 59 patients in the FTS group. Two patients were excluded from the FTS group due to withdrawal of consent. One patient was excluded from the conventional care group because of a non-resectable tumor. Compared with the conventional group, FTS shortened the duration of flatus (79.03 ± 20.26 hvs 60.97 ± 24.40 h, P = 0.000) and duration of defecation (93.03 ± 27.95 h vs 68.00 ± 25.42 h, P = 0.000), accelerated the decrease in white blood cell count [P < 0.05 on postoperative day (POD) 3 and 4], alleviated pain in patients after surgery (P < 0.05 on POD 1, 2 and 3), reduced complications (P < 0.05), shortened the duration of postoperative stay (7.10 ± 2.13 dvs 5.68 ± 1.22 d,P = 0.000), reduced the cost of hospitalization (43783.25 ± 8102.36 RMBvs 39597.62 ± 7529.98 RMB,P = 0.005), and promoted recovery of patients. 3Hepatoprotective and antioxidant activity of a mangrove plant Lumnitzera racemosa显示文摘Objective:To identify the hepatoprotective and in vitro antioxidant activity of Lumnitzera racemosa(L.racemosa)leaf extract.Methods:Animals in Group 1 served as vehicle control,Group 2 served as liepatoloxin(CCL_4 treated)group,Group 3 served as positive control(Silyinarin)group,and Group 4,5 and 6 served as(75,150 and 300 mg/kg bw p.o.)L.raremosa leaf extract treated groups.Moreover,in vitro antioxidant DPPH,hydroxyl radical scavenging activity(HRSA),NO,ferric reducing antioxidant power(FRAP),lipid hydroperoxide(LPO)and super oxide dismutase(SOD)were also analyzed for the leaf extract.Results:The levels of the serum parameters such as serum glutamic oxaloacetic transaminase(SCOT),serum glutamic pyruvic transaminase(SGPT),alkaline phosphatase(ALP),bilirubin,cholesterol(CHL),sugar and lactate dehydrogenase(LDU)were significantly increased in CCL_4 treated rats when compared with the control group(P<0.05).But the L.racemosa leaf extract treated rats showed maximum reduction of SGOT[(210.36±19.63)IU/L],SGPT[(82.37±13.87)IU/L],ALP[(197.63±23.43)IU/L],bilurubin[(2.15±0.84)mg/dL],cholesterol[(163.83±15.63)mg/dL],sugar[(93.00±7.65)mg/dL]and LDH[(1134.00±285.00)IU/L]were observed with the high dose(300 mg/kg bw)of leaf extract treated rats.Histopathological scores showed that,no visible changes were observed with high dose(300 mg/kg bw)of leaf extract treated rats except few mild necrosis.The IC_(50)values were observed as(56.37±4.87)μg/mL,(57.68±1.98)μg/mL,(64.I5±2.90)μg/mL,(61.94±3.98)μg/mL,(94.53±1.68)μg/mL and(69.7±2.65)μg/mL for DPPH,HRSA,NO,FRAP,LPO and SOD radical scavenging activities,respectively.Conclusions:In conclusion,the hepatoprotective effect of the L.racemosa leaf extract might be due to the presence of phenolic groups,terpenoids and alkaloids and in vitro antioxidant properties.Sundaram Ravikumar Murugesan Gnanadesigan 2011Asian Pacific Journal of Tropical Biomedicine2011,1,5:21
4Therapeutic efficacy of the Qing Dai in patients with intractable ulcerative colitis显示文摘Ulcerative colitis (UC) is a chronic inflammatory bowel disease that may become intractable when treated with conventional medications such as aminosalicylates, corticosteroids, and azathioprine. The herbal medicine Qing Dai has traditionally been used in Chinese medicine to treat UC patients, but there is a lack of published data on the efficacy of Qing Dai in UC treatment. We report several cases of patients with intractable UC who take Qing Dai in a retrospective observational study. Furthermore, we explore the mechanisms of action of Qing Dai. Nine patients with active UC who received conventional medications but wished to receive Qing Dai as an alternative medication were included in our analysis. The UC severity level was determined based on the clinical activity index (CAI). Additionally, 5 of the 9 patients were endoscopically evaluated according to the Matts grading system. Each patient received 2 g/d of Qing Dai orally and continued taking other medications for UC as prescribed. Electron spin resonance was applied to explore the mechanisms of action of Qing Dai. After 4 mo of treatment with Qing Dai, the CAI score decreased from 8.3 ± 2.4 to 2.4 ± 3.4 (mean ± SD; P < 0.001). Similarly, the endoscopic Matts grade decreased from 3.4 ± 0.5 to 2.2 ± 0.8 (P = 0.02). Six of 7 patients who were on prednisolone upon enrollment in the study were able to discontinue this corticosteroid. Electron spin resonance revealed that Qing Dai possesses strong hydroxyl radical scavenging activity. Qing Dai showed significant clinical and endoscopic efficacy in patients who failed to respond to conventional medications. 5Antioxidant activities of Indigofera cassioides Rottl.Ex.DC.using various in vitro assay models显示文摘Objective:To evaluate the antioxidant potential of methanolic leaf extract of Indigofera cassioides(MEIC) using various in vilro antioxidant assay systems.Methods:Antioxidant and free radical scavenging activity of MEIC was assayed by using different in vitro models like ABTS, DPPH,nitric oxide,superoxide,hydrogen peroxide and hydroxyl radical.Reductive ability of the extract was tested by the complex formation with potassium ferricyanide.Further total phenol and flavonoid contents of the crude extract were also determined.Rutin and ascorbic acid were used as standards.Results:MEIC exhibited potent and concentration dependent free radical scavenging activity in all the tested models.Reductive ability was also found to increase with increase in MEIC concentration.Total phenol and flavonoid content determination showed that the extract is rich in phenols and flavonoids.Conclusions:All the results of the in vitro antioxidant assays reveal potent antioxidant and free radical scavenging activity of the leaves of Indigofera cassioides,equivalent to that of standard ascorbic acid and rutin.This potent antioxidant activity may be attributed to its high phenolic and flavonoid contents.R Senthil Kumar B Rajkapoor P Perumal 2012Asian Pacific Journal of Tropical Biomedicine2012,2,4:15

8Prognostic role of neutrophil-lymphocyte ratio in operable esophageal squamous cell carcinoma显示文摘AIM: To determine the prognostic significance of preoperative serum neutrophil-lymphocyte ratio(NLR) in esophageal squamous cell carcinoma(ESCC).METHODS: Data from 371 eligible patients with ESCC who had undergone surgery with curative intent at our institution between October 2000 and May 2007 were retrospectively recruited for analysis. The cutoff value of NLR was 3.0 as determined by the receiver operating characteristic curve, which discriminated between survival and death; the area under the curve was 0.709, and the sensitivity and specificity were 66.1% and 69.1%, respectively, at the cutoff point. The correlation between the NLR and clinicopathological characteristics was analyzed using a χ2 test. The prognostic influence of the NLR and other clinicopathological factors on cancer-specific survival(CSS) and recurrence-free survival(RFS) was studied using the Kaplan-Meier method. To evaluate the independent prognostic value of NLR, multivariate Cox regression models were applied.RESULTS:The median age of the patients was 57.0years,and 276/371(74.4%)patients were male.The NLR was≤3.0 in 80.1%(297/371)of the patients,and the remaining 19.9%(74/371)had an NLR>3.0.Median postoperative follow-up was 66.0 mo[interquartile range(IQR):49.0-76.0 mo],with a follow-up rate of 94%.Follow-up was not significantly different between patients with an NLR≤and>3.0(63.13±1.64 vs 61.52±3.66,P=0.711).However,higher preoperative serum NLR was associated with significantly increased risks of higher pathological tumor status(P=0.007).A significant,independent association between high preoperative serum NLR and poor clinical outcome was identified in a multivariate analysis for CSS(HR=1.591;P=0.007)and RFS(HR=1.525;P=0.013).Moreover,when patients were stratified by pathological tumor-node-metastasis(TNM)staging,the adverse effects of preoperative serum NLR on CSS(HR=2.294;P=0.008)and RFS(HR=2.273;P=0.008)were greatest in those patients with stageⅢA disease.CONCLUSION:Preoperative serum NLR is a useful prognostic marker to complement TNM staging for operable ESCC patients,particularly in patients with stageⅢA disease.Hao Duan Xu Zhang Fei-Xiang Wang Mu-Yan Cai Guo-Wei Ma Hong Yang Jian-Hua Fu Zi-Hui Tan Yu-Qi Meng Xia-Yu Fu Qi-Long Ma Peng Lin 2015World Journal of Gastroenterology2015,21,18:13
9Preparation and properties of RE^(3+) doped luminescent co-polymer by solution copolymerization显示文摘Bonded type RE3+ doped luminescent co-polymer was synthesized by solution free radical copolymerization. The influence of charge sequence, monomers and co-polymerized method on properties and structures of the co-polymers was studied. The emission intensity of the co-polymers at different RE3+ concentrations was tested. The results showed that the co-polymers of Eu-PSt and Eu-PMMA both had wide absorption peak at 200-400 nm and the strongest peak appeared at 235 nm. The fluorescent intensity of Eu3+ doped polystyrene co-polymer was stronger than that of Eu3+ doped PMMA copolymer. The characteristic emission of europium ions was observed in the co-polymers. The copolymer doped with rare earth elements showed the 'sensitization effect' for the central ions. 10Clinical characteristics and prognosis of young patients with colorectal cancer in Eastern China显示文摘AIM:To explore the clinical characteristics and prognosis of young patients with colorectal cancer patients in Eastern China.METHODS:A total of 1335 patients with colorectal cancer treated from December 1985 to December 2005at the Second Affiliated Hospital of Zhejiang University School of Medicine were studied retrospectively.The patients were divided into two groups,a younger group(aged≤30 years)and an older group(aged>30 years),and comparison was made in the clinical characteristics and prognosis between the two groups.Chisquare test was used for data analysis of all categorical variables,and overall survival(OS)was calculated by the Kaplan-Meier method.A multivariate analysis was performed using the Cox model.RESULTS:There were 42(3.1%)and 1293(96.9%)cases in the younger group and older group,respectively.Univariate analysis showed that the 5-and10-year OS in the younger group were 33.9%and26.1%,respectively,and those in the older group were60.1%and 52.2%,respectively.Younger group had poor survival(χ2=14.146,P=0.000).Multivariate analysis revealed that age was not a dependent factor for prognosis(OR=0.866,95%CI:0.592-1.269,P=0.461).Stratified analysis indicated that in stageⅢandⅣdisease,the 5-and 10-year OS were 24.6%and14.8%in the younger group,and 40.4%and 33.3%in the older group,respectively,with a significant difference between the two groups(χ2=5.101,P=0.024).In the subgroup of radical surgery,the 5-and 10-year OS were 44.3%and 34.2%in the younger group,and69.6%and 60.5%in the older group,with a difference being significant between the two groups(χ2=7.830,P=0.005).CONCLUSION:Compared with older patients,the younger patients have lower survival,especially in the subgroups of stageⅢandⅣdisease and radical surgery.Jian-Fei Fu Yan-Qin Huang Jiao Yang Cheng-Hao Yi Hai-Long Chen Shu Zheng 2013World Journal of Gastroenterology2013,19,44:11

13Regulatory role of mitochondria in oxidative stress and atherosclerosis显示文摘Mitochondrial physiology and biogenesis play a crucial role in the initiation and progression of cardiovascular disease following oxidative stress-induced damage such as atherosclerosis(AST).Dysfunctional mitochondria caused by an increase in mitochondrial reactive oxygen species(ROS)production,accumulation of mitochondrial DNA damage,and respiratory chain deficiency induces death of endothelial/smooth muscle cells and favors plaque formation/rupture via the regulation of mitochondrial biogenesis-related genes such as peroxisome proliferator-activated receptorγcoactivator(PGC-1),although more detailed mechanisms still need further study.Based on the effect of healthy mitochondria produced by mitochondrial biogenesis on decreasing ROS-mediated cell death and the recent finding that the regulation of PGC-1 involves mitochon- drial fusion-related protein(mitofusin),we thus infer the regulatory role of mitochondrial fusion/fission balance in AST pathophysiology.In this review,the first section discusses the possible association between AST-inducing factors and the molecular regulatory mechanisms of mitochondrial biogenesis and dynamics,and explains the role of mitochondria-dependent regulation in cell apoptosis during AST development. Furthermore,nitric oxide has the Janus-faced effect by protecting vascular damage caused by AST while being a reactive nitrogen species(RNS)which act together with ROS to damage cells.Therefore,in the second section we discuss mitochondrial ATP-sensitive K+ channels,which regulate mitochondrial ion transport to maintain mitochondrial physiology,involved in the regulation of ROS/RNS production and their influence on AST/cardiovascular diseases(CVD).Through this review,we can further appreciate the multi-regulatory functions of the mitochondria involved in AST development.The understanding of these related mechanisms will benefit drug development in treating AST/CVD through targeted biofunctions of mitochondria.Jui-Chih Chang Shou-Jen Kou Chin-San Liu 2010World Journal of Cardiology2010,2,6:10

15Anisodamine protects against neuronal death following cerebral ischemia in gerbils显示文摘To study the effect of anisodamine on neuronal death and hydroxyl radical (OH·) production during forebrain ischemia reperfusion in gerbils Methods The tested gerbils were divided into 3 groups, including sham operated, control and anisodamine groups. In each group, there were 8 animals for biochemical examination and 6 animals for histologic study. Forebrain ischemia was induced by occlusion the bilateral common carotid arteries for 10 min in gerbils. 2,3 and 2,5 DHBA outputs were determined by high performance hiquid chromatography coupled with electrochemical detection. Behavioral change was tested by open field test and neuronal death was assessed by histological examination.Results The exploratory activities of gerbils in the control group were significantly higher than those in the anisodamine group on all test days The amount of viable looking neurons in the medial, middle and lateral CA1 sectors in anisodamine group were 41%±12%, 50%±21% and 67%±15% of the sham operated gerbils, respectively, being significantly higher than those in the control group (3%±2%, 4%±3% and 7%±4% of sham, P <0 01) The 2,3 DHBA outputs in the control group increased by 5 fold of the sham operated gerbils after reperfusion for 60 min, but the 2,3 DHBA outputs in the anisodamine group were only 2 4 fold of sham operated gerbils, being significantly lower than that in the control group ( P <0 01) The 2,5 DHBA outputs in the control group were significantly higher than those in the sham operated group ( P <0 05) Conclusion Anisodamine has inhibitory effects on neuronal death and OH·production during cerebral ischemia reperfusion in陈群 曾因明 2000Chinese Medical Journal2000,,7:10
16Comparative analysis of robotic vs laparoscopic radical hysterectomy for cervical cancer显示文摘BACKGROUND Cervical cancer is the most common gynecological malignancy,ranking first in female reproductive malignancies with more than 500000 new cases and 275000 deaths each year.Traditionally,open radical hysterectomy is considered the standard surgical procedure for the treatment of resectable cervical cancer.The latest guidelines from the National Comprehensive Cancer Network and the European Society of Gynecological Oncology suggest that open surgery and laparoscopic surgery(using traditional laparoscopic or robotic techniques)are the main surgical approaches for radical hysterectomy for patients with stage IA2-IIA cervical cancer.Robotic surgery has been increasingly used in abdominal surgery and has shown more beneficial effects.AIM To analyse the perioperative conditions,complications,and short-term and longterm effects in patients undergoing robotic radical hysterectomy(RRH)and laparoscopic radical hysterectomy(LRH)to compare their clinical efficacy,safety,and feasibility.METHODS The perioperative data of patients undergoing RRH and LRH were extracted and collected from the database of surgical treatments for cervical cancer for statistical analysis.RESULTS Of the patients,342 underwent LRH for cervical cancer,and 216 underwent RRH.The total complication rate was 9.65%(20 patients)in the RRH group and 17.59%(60 patients)in the LRH group.The complication rate was significantly lower in the RRH group than in the LRH group.There was no significant difference in the follow-up period(P=0.658).The total recurrence rates were 15.7%and 12%in the RRH and LRH groups,respectively.The progression-free survival time was 28.91±15.68 mo and 28.34±15.13 mo in the RRH and LRH groups,respectively(P=0.669).The overall survival(OS)rates were 92.13%and 94.45%in the RRH and LRH groups,respectively(P=0.292).The OS time was 29.87±15.92 mo and 29.41±15.14 mo in the RRH and LRH groups,respectively(P=0.732).The survival curves and the progression-free survival curves were not statistically significantly different between the two groups(P=0.407 and 0.28,respectively).CONCLUSION RRH is associated with significantly less operative time and blood loss than LRH.The two procedures have similar complication rates,OS,and progression-free survival time.Li Chen Li-Ping Liu Na Wen Xiao Qiao Yuan-Guang Meng 2019World Journal of Clinical Cases2019,7,20:9
17Superior mesenteric venous thrombosis after laparoscopic radical resection of rectal cancer: a report of a rare case and literature review显示文摘Mesenteric venous thrombosis(MVT) is rare, but life-threatening. MVT is often characterized by occult and nonspecific signs and symptoms. Diagnosis requires a high index of clinical suspicion, and emergency surgery is necessary to optimize patient survival, especially in people aged more than 70 years. MVT is a rare but fatal complication after laparoscopic radical surgery for rectal cancer. 18Inhibitory effects of chitosan on superoxide anion radicals and lipid free radicals显示文摘With the electron spin resonance (ESR) technique, the inhibitory effects of chitosan on superoxide anion radicals and linoleic acid lipid radicals were found. The inhibitory ratio E for these two kinds of radicals is in proportion to the concentration of chitosan. 19Duct-to-duct biliary reconstruction after radical resection of Bismuth Ⅲ a hilar cholangiocarcinoma显示文摘At present, radical resection remains the only effective treatment for patients with hilar cholangiocarcinoma. The surgical approach for R0 resection of hilar cholangiocarcinoma is complex and diverse, but for the biliary reconstruction after resection, almost all surgeons use Roux-en-Y hepaticojejunostomy. A viable alternative to Roux-en-Y reconstruction after radical resection of hilar cholangiocarcinoma has not yet been proposed. We report a case of performing duct-to-duct biliary reconstruction after radical resection of Bismuth Ⅲa hilar cholangiocarcinoma. End-to-end anastomosis between the left hepatic duct and the distal common bile duct was used for the biliary reconstruction, and a singlelayer continuous suture was performed along the bile duct using 5-0 prolene. The patient was discharged favorably without biliary fistula 2 wk later. Evidence for tumor recurrence was not found after an 18 mo follow- up.
