| 1 | Reconstruction of precipitation variation from tree rings in recent 1000 years in Delingha, Qinghai显示文摘Using seven well-replicated Qilian juniper (Sabina przewalskii Kom.) ring-width chronologies developed at Zongwulong and Shalike Mts. in the northeastern part of the Qaidam Basin annual precipitation from previous July to current June in the recent 1000 years was re- constructed for Delingha. The reconstruction can capture 63.1% of precipitation variance and the equation was stable over time. For the reconstructed precipitation, wet periods occurred in AD1520—1633 and 1933—2001, whereas dry intervals in 1429—1519 and 1634—1741. In ad- dition, the magnitude in precipitation variation was lower before 1430 with about 15 mm, but it increased to 30 mm during the period of 1430 to 1850. After 1850, the precipitation variance de- creased again. In contrast to the increase in temperature, a decrease in annual precipitation was evident since the 1990s. The agreement in low-frequency variation between the reconstruction and the glacier accumulation and particulate content in Dunde ice cores during the recent several hundred years suggested that the precipitation reconstructed in this study was rather reliable, and represented a regional signal. | 2 | Roux-en-Y versus BillrothⅠreconstruction after distal gastrectomy for gastric cancer:A meta-analysis显示文摘AIM:To conduct a meta-analysis to compare Rouxen-Y(R-Y) gastrojejunostomy with gastroduodenal Billroth Ⅰ(B-Ⅰ) anastomosis after distal gastrectomy(DG) for gastric cancer.METHODS:A literature search was performed to identify studies comparing R-Y with B-Ⅰ after DG for gastric cancer from January 1990 to November 2012 in Medline,Embase,Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials in The Cochrane Library.Pooled odds ratios(OR) or weighted mean differences(WMD) with 95%CI were calculated using either fixed or random effects model.Operative outcomes such as operation time,intraoperative blood loss and postoperative outcomes such as anastomotic leakage and stricture,bile reflux,remnant gastritis,reflux esophagitis,dumping symptoms,delayed gastric emptying and hospital stay were the main outcomes assessed.Meta-analyses were performed using RevMan 5.0 software(Cochrane library).RESULTS:Four randomized controlled trials(RCTs) and 9 non-randomized observational clinical studies(OCS) involving 478 and 1402 patients respectively were included.Meta-analysis of RCTs revealed that R-Y reconstruction was associated with a reduced bile reflux(OR 0.04,95%CI:0.01,0.14;P < 0.00 001) and remnant gastritis(OR 0.43,95%CI:0.28,0.66;P = 0.0001),however needing a longer operation time(WMD 40.02,95%CI:13.93,66.11;P = 0.003).Metaanalysis of OCS also revealed R-Y reconstruction had a lower incidence of bile reflux(OR 0.21,95%CI:0.08,0.54;P = 0.001),remnant gastritis(OR 0.18,95%CI:0.11,0.29;P < 0.00 001) and reflux esophagitis(OR 0.48,95%CI:0.26,0.89;P = 0.02).However,this reconstruction method was found to be associated with a longer operation time(WMD 31.30,95%CI:12.99,49.60;P = 0.0008).CONCLUSION:This systematic review point towards some clinical advantages that are rendered by R-Y compared to B-Ⅰ reconstruction post DG.However there is a need for further adequately powered,welldesigned RCTs comparing the same. | Jun-Jie Xiong Kiran Altaf Muhammad A Javed Quentin M Nunes Wei Huang Gang Mai Chun-Lu Tan Rajarshi Mukherjee Robert Sutton Wei-Ming Hu Xu-Bao Liu | 2013 | World Journal of Gastroenterology2013,19,7: | 33 |

| 4 | Current concept in dysplastic hip arthroplasty: Techniques for acetabular and femoral reconstruction显示文摘Adult patients with developmental dysplasia of the hip develop secondary osteoarthritis and eventually end up with total hip arthroplasty(THA) at younger age. Because of altered anatomy of dysplastic hips, THA in these patients represents technically demanding procedure. Distorted anatomy of the acetabulum and proximal femur together with conjoined leg length discrepancy present major challenges during performing THA in patients with developmental dysplasia of the hip. In addition, most patients are at younger age, therefore, soft tissue balance is of great importance(especially the need to preserve the continuity of abductors) to maximise postoperative functional result. In this paper we present a variety of surgical techniques availablefor THA in dysplastic hips, their advantages and disadvantages. For femoral side first we peel of all rotators and posterior part of gluteus medius and vastus lateralis from greater trochanter on the very thin flake of bone. This method allows us to adequately shorten proximal femoral stump, with possibility of additional resection of proximal femur. The diversity of fracture patterns encountered renders objective classification of prognostic value challenging. Non-operative management has been associated with good functional outcomes in stable, minimally displaced and certain types of displaced fractures.Absolute indications for surgery are infrequent and comprise compound, pathological, multi-fragmentary head-splitting fractures and fracture dislocations, as well as those associated with neurovascular injury. A constantly expanding range of reconstructive and replacement options however has been extending the indications for surgical management of complex proximal humerus fractures. As a result, management decisions are becoming increasingly complicated, in an attempt to provide the best possible treatment for each indi-vidual patient, that will successfully address their specific fracture configuration, comorbidities and functional expectations. | 8 | A statistical model for porous structure of rocks显示文摘The geometric features and the distribution properties of pores in rocks were in- vestigated by means of CT scanning tests of sandstones. The centroidal coordi- nates of pores, the statistic characterristics of pore distance, quantity, size and their probability density functions were formulated in this paper. The Monte Carlo method and the random number generating algorithm were employed to generate two series of random numbers with the desired statistic characteristics and prob- ability density functions upon which the random distribution of pore position, dis- tance and quantity were determined. A three-dimensional porous structural model of sandstone was constructed based on the FLAC3D program and the information of the pore position and distribution that the series of random numbers defined. On the basis of modelling, the Brazil split tests of rock discs were carried out to ex- amine the stress distribution, the pattern of element failure and the inosculation of failed elements. The simulation indicated that the proposed model was consistent with the realistic porous structure of rock in terms of their statistic properties of pores and geometric similarity. | 9 | Application of a medical image processing system in liver transplantation显示文摘BACKGROUND: At present, imaging is used not only to show the form of images, but also to make three-dimensional (3D) reconstructions and visual simulations based on original data to guide clinical surgery. This study aimed to assess the use of a medical image-processing system in liver transplantation surgery. METHODS: The data of abdominal 64-slice spiral CT scan were collected from 200 healthy volunteers and 37 liver cancer patients in terms of hepatic arterial phase, portal phase, and hepatic venous phase. A 3D model of abdominal blood vessels including the abdominal aorta system, portal vein system, and inferior vena cava system was reconstructed by an abdominal image processing system to identify vascular variations. Then, a 3D model of the liver was reconstructed in terms of hepatic segmentation and liver volume was calculated. The FreeForm modeling system with a PHANTOM force feedback device was used to simulate the real liver transplantation environment, in which the total process of liver transplantation was completed. RESULTS: The reconstructed model of the abdominal blood vessels and the liver was clearly demonstrated to be three- dimensionally consistent with the anatomy of the liver, in which the variations of abdominal blood vessels were identified and liver segmentation was performed digitally. In the model, liver transplantation was simulated subsequently, and different modus operandi were selected successfully. | 10 | Anus-preserving rectectomy via telescopic colorectal mucosal anastomosis for low rectal cancer: Experience from a Chinese cohort显示文摘AIM: To investigate the safety and efficacy of anus-preserving rectectomy via telescopic colorectal mucosal anastomosis (TCMA) for low rectal cancer. METHODS: From August 1993 to October 2012, 420 patients including 253 males and 167 females with low rectal cancer underwent transabdominal and transanal anterior resection, followed by TCMA. The distance be-tween the anus and inferior margin of the tumor ranged from 5 to 7 cm, and was 5 cm in 6 patients, 6 cm in 127, and 7 cm in 287 patients. Tumor-node-metastasis staging showed that 136 patients had stage Ⅰ, 252 had stage Ⅱ and 32 had stage Ⅲ. Fifty-six patients with T3 or over received preoperative neoadjuvant chemoradio-therapy. RESULTS: The postoperative follow-up rate was 91.9% (386/420) with a median time of 6.4 years. All 420 pa-tients underwent radical resection. No postoperativedeath occurred. Postoperative complications included anastomotic leakage in 13 (3.1%) patients and anas-tomotic stenosis in 7 (1.6%). The local recurrence rate after surgery was 6.2%, the hepatic metastasis rate was 13.2% and the pulmonary metastasis rate was 2.3%. The 5-year survival rate was 74.0% and the disease-free survival rate was 71.0%. Kirwan classification showed that continence was good in 94.4% of patients with stage I when scored 12 mo after resection. | 11 | Reconstruction of paleocoastlines for the northwestern South China Sea since the Last Glacial Maximum显示文摘The range of relative sea level rise in the northwestern South China Sea since the Last Glacial Maximum was over 100 m. As a result, lowland regions including the Northeast Vietnam coast, Beibu Gulf, and South China coast experienced an evolution from land to sea. Based on the principle of reconstructing paleogeography and using recent digital elevation model, relative sea level curves, and sediment accumulation data, this paper presents a series of paleogeographic scenarios back to 20 cal. ka BP for the northwestern South China Sea. The scenarios demonstrate the entire process of coastline changes for the area of interest. During the late glacial period from 20 to 15 cal. ka BP, coastline slowly retreated, causing a land loss of only 1×104 km2, and thus the land-sea distribution remained nearly unchanged. Later in 15-10 cal. ka BP coastline rapidly retreated and area of land loss was up to 24×104 km2, causing lowlands around Northeast Vietnam and South China soon to be underwater. Coastline retreat continued quite rapidly during the early Holocene. From 10 to 6 cal. ka BP land area had decreased by 9×104 km2, and during that process the Qiongzhou Strait completely opened up. Since the mid Holocene, main controls on coastline change are from vertical crustal movements and sedimentation. | 12 | Comparison between uncut Roux-en-Y and Roux-en-Y reconstruction after distal gastrectomy for gastric cancer: A meta-analysis显示文摘AIM To compare uncut Roux-en-Y(U-RY) gastrojejunostomy with Roux-en-Y(RY) gastrojejunostomy after distal gastrectomy(DG) for gastric cancer.METHODS A literature search was conducted in Pubmed, Embase, Web of Science, Cochrane Library, Science Direct, Chinese National Knowledge Infrastructure, Wanfang, and China Science and Technology Journal Database to identify studies comparing U-RY with RY after DG for gastric cancer until the end of December 2017. Pooled odds ratio or weighted mean difference with 95% confidence interval was calculated using either fixed-or random-effects models. Perioperative outcomes such as operative time, intraoperative blood loss, and hospital stay; postoperative complications such as anastomotic bleeding, stricture and ulcer, reflux gastritis/esophagitis, delayed gastric emptying, and Roux stasis syndrome; and postoperative nutritional status(serum hemoglobin, total protein, and albumin levels) were the main outcomes assessed. Metaanalyses were performed using RevM an 5.3 software.RESULTS Two randomized controlled trials and four nonrandomized observational clinical studies involving 403 and 488 patients, respectively, were included. The results of the meta-analysis showed that operative time [weighted mean difference(WMD):-12.95; 95%CI:-22.29 to-3.61; P = 0.007] and incidence of reflux gastritis/esophagitis(OR: 0.40; 95%CI: 0.20-0.80; P = 0.009), delayed gastric emptying(OR: 0.29; 95%CI: 0.14-0.61; P = 0.001), and Roux stasis syndrome(OR: 0.14; 95%CI: 0.04-0.50; P = 0.002) were reduced; and the level of serum albumin(WMD: 0.71; 95%CI: 0.24-1.19; P = 0.003) was increased in patients undergoing U-RY reconstruction compared with those undergoing RY reconstruction. No differences were found with respect to intraoperative blood loss, hospital stay, anastomotic bleeding, anastomotic stricture, anastomotic ulcer, the levels of serum hemoglobin, and serum total protein. | 13 | Immediate nipple-areola-sparing mastectomy reconstruction: An update on oncological and reconstruction techniques显示文摘Nipple-sparing mastectomy(NSM) is a safe technique in patients who are candidates for conservation breast surgery. However, there is worry concerning its oncological safety and surgical outcome in terms of postoperative complications. The authors reviewed the literature to evaluate the oncological safety, patient selection, surgical techniques, and also to identify the factors influencing postoperative outcome and complication rates. Patient selection and safety related to NSM are based on oncological and anatomical parameters. Among the main criteria, the oncological aspects include the clinical stage of breast cancer, tumor characteristics and location including small, peripherally located tumors, without multicentricity, or for prophylactic mastectomy. Surgical success depends on coordinated planning with the oncological surgeon andcareful preoperative and intraoperative management. In general, the NSM reconstruction is related to autologous and alloplastic techniques and sometimes include contra-lateral breast surgery. Choice of reconstructive technique following NSM requires accurate consideration of various patient related factors, including: breast volume, degree of ptosis, areola size, clinical factors, and surgeon's experience. In addition, tumor related factors include dimension, location and proximity to the nipple-areola complex. Regardless of the fact that there is no unanimity concerning the appropriate technique, the criteria are determined by the surgeon's experience and the anatomical aspects of the breast. The positive aspects of the technique utilized should include low interference with the oncological treatment, reproducibility, and long-term results. Selected patients can have safe outcomes and therefore this may be a feasible option for early breast cancer management. However, available data demonstrates that NSM can be safely performed for breast cancer treatment in selected cases. For patients with chronic lateral patellar instability, the standard surgical approach is to stabilize the patella through a medial patellofemoral ligament(MPFL) reconstruction. Foreseeably, an increasing number of revision surgeries of the reconstructed MPFL will be seen in upcoming years. In this paper, the causes of failed MPFL reconstruction are analyzed:(1) incorrect surgical indication or inappropriate surgical technique/patient selection;(2) a technical error; and(3) an incorrect assessment of the concomitant risk factors for instability. An understanding of the anatomy and biomechanics of the MPFL and cautiousness with the imaging techniques while favoring clinical over radiological findings and the use of common sense to determine the adequate surgical technique for each particular case, are critical to minimizing MPFL surgery failure. Firstly, the severity of the cold weath-er events such as snowing and severe frost, which were frequently described in gazetteers,was estimated by using modern instrumental temperature observations. Secondly, the assort-ed cold events were classified into three classes according to the severity. the severity indexof them being --0.5, -- 1.0 and --2.0 respectively. The severity index was found fot eachseason since 1380 AD while there were somc descriptions of cold events. Finally, the re-gression coefficients between the decade sum of severity index and decade mean seasonal tem-perature anomalies was calculated. It shows that the severity index of--1 is equal to --0.2℃of the temperature anomaly. | 16 | Anterior cruciate ligament reconstruction best practice: A review of graft choice显示文摘There is much literature about differing grafts used in anterior cruciate ligament(ACL) reconstruction. Much of this is of poor quality and of a low evidence base. We review and summarise the literature looking at the four main classes of grafts used in ACL reconstruction; bone-patella tendon-bone, hamstrings, allograft and synthetic grafts. Each graft has the evidence for its use reviewed and then compared, where possible, to the others. We conclude that although there is no clear 'best' graft, there are clear differences between the differing graft choices. | 17 | Comparatively lower postoperative hepatolithiasis risk with hepaticocholedochostomy versus hepaticojejunostomy显示文摘BACKGROUND:Optimal surgical technique plays a key role in preventing the postoperative recurrence of hepatolithiasis.Tian et al developed the subcutaneous tunnel and hepatocholangioplasty using the gallbladder (STHG) technique and applied it in hepatolithiasis patients who had an approximately normal gallbladder and sphincter of Oddi.However,the technique is controversial.In the present study,a canine model was established for hepatocholangioplasty (HC) and hepaticojejunostomy (HJ) to simulate STHG and Roux-en-Y cholangiojejunostomy in the clinic,respectively.Then,the alterations of bile components in the vicinity of the anastomosis were compared.This may provide an experimental guide for choosing an optimal technique for the treatment of hepatolithiasis in the clinic.METHODS:The animals were randomly separated into a control group (5 dogs) and a model group (stenosis of the common bile duct;24 dogs).The 24 dogs in the model group were randomly divided into an HC group and an HJ group (12/group).Bile was collected from the bile duct at 1 and 5 months after the operation,and the bile components were determined.RESULTS:The levels of total bile acid,cholesterol,total bilirubin,and phospholipid in the HC group were higher than those in the HJ group (P<0.05).However,no statistical difference was seen in unconjugated bilirubin and calcium ions.The mucin level in bile in the HC group was lower than that in the HJ group at 5 months after the operation (P<0.05).The postoperative lipid peroxidation level was remarkably lower than that in the HJ group (P<0.05).However,the superoxide dismutase level was remarkably higher than that in the HJ group (P<0.05).Finally,a significant difference was found in the positive bacterial culture rate in bile between the groups.CONCLUSION:Changes of bile components near the anastomosis after HC might be more preferable for preventing hepatolithiasis formation than HJ. | Zhang, Xiao-Jin Jiang, Yi Wang, Xu Tian, Fu-Zhou Lv, Li-Zhi | 2010 | Hepatobiliary & Pancreatic Diseases International2010,9,1: | 12 |
| 18 | Large field of view computed laminography with the asymmetric rotational scanning geometry显示文摘X-ray computed laminography(CL)is used in the fields of industrial inspection and medical imaging.It can provide the internal structure three-dimensional(3D)information of a region of the objects nondestructively.It is important to the clinical diagnosis and the quality control of flat objects like printed circuit boards,aircraft wings and satellite solar panels.With the restriction that the imaging region must be within the X-ray beam formed by the X-ray source and the detector,the imaging field of view of CL is limited by the size of detector.A new CL method with an asymmetric rotational cone-beam scanning geometry,called large field of view CL,is presented to overcome the existing disadvantage.It can extend the imaging region when the imaging spatial resolution keeps the same as that of the conventional CL.It can also improve the imaging spatial resolution when the imaging region keeps the same as that of the conventional CL.The asymmetric configuration can be achieved by offsetting the detector from the conventional symmetric configuration.It does not,however,require new detectors and X-ray source nor alter the scanning mechanical system.The filtered back-projection(FBP)reconstruction algorithm and the data truncation smoothing functions have been deduced to reconstruct the images directly from the data acquired with this asymmetric configuration.We performed numerical studies and experiments to demonstrate and validate the proposed approach. | 19 | New insights in the treatment of acromioclavicular separation显示文摘A direct force on the superior aspect of the shoulder may cause acromioclavicular(AC) dislocation or separation. Severe dislocations can lead to chronic impairment, especially in the athlete and high-demand manual laborer. The dislocation is classified according to Rockwood. Types Ⅰ?and Ⅱ are treated nonoperatively, while types Ⅳ, Ⅴ and Ⅵ are generally treated operatively. Controversy exists regarding the optimal treatment of type Ⅲ dislocations in the high-demand patient. Recent evidence suggests that these should be treated nonoperatively initially. Classic surgical techniques were associated with high complication rates, including recurrent dislocations and hardware breakage. In recent years, many new techniques have been introduced in order to improve the outcomes. Arthroscopic reconstruction or repair techniques have promising short-term results. | 20 | Review of diagnostic and therapeutic endoscopic retrograde cholangiopancreatography using several endoscopic methods in patients with surgically altered gastrointestinal anatomy显示文摘The endoscopic approach for biliary diseases in patients with surgically altered gastrointestinal anatomy(SAGA) had been generally deemed impractical. However, it was radically made feasible by the introduction of double balloon endoscopy(DBE) that was originally developed for diagnosis and treatments for small-bowel diseases. Followed by the subsequent development of singleballoon endoscopy(SBE) and spiral endoscopy(SE), interventions using several endoscopes for biliary disease in patients with SAGA widely gained an acceptance as a new modality. Many studies have been made on this new technique. Yet, some problems are to be solved. For instance, the mutual unavailability among devices due to different working lengths and channels, and unestablished standardization of procedural techniques can be raised. Additionally, in an attempt to standardize endoscopic procedures, it is important to evaluate biliary cannulating methods by case with existence of papilla or not. A full comprehension of the features of respective scope types is also required. However there are not many papers written as a review.
