2Focus on peripherally inserted central catheters in critically ill patients显示文摘Venous access devices are of pivotal importance for an increasing number of critically ill patients in a variety of disease states and in a variety of clinical settings(emergency, intensive care, surgery) and for different purposes(fluids or drugs infusions, parenteral nutrition, antibiotic therapy, hemodynamic monitoring, procedures of dialysis/apheresis). However, healthcare professionals are commonly worried about the possible consequences that may result using a central venous access device(CVAD)(mainly, bloodstream infections and thrombosis), both peripherally inserted central catheters(PICCs) and centrally inserted central catheters(CICCs). This review aims to discuss indications, insertion techniques, and care of PICCs in critically ill patients. PICCs have many advantages over standard CICCs. First of all, their insertion is easy and safe-due to their placement into peripheral veins of the armand the advantage of a central location of catheter tip suitable for all osmolarity and p H solutions. Using the ultrasound-guidance for the PICC insertion, the risk of hemothorax and pneumothorax can be avoided, as wellas the possibility of primary malposition is very low. PICC placement is also appropriate to avoid post-procedural hemorrhage in patients with an abnormal coagulative state who need a CVAD. Some limits previously ascribed to PICCs(i.e., low flow rates, difficult central venous pressure monitoring, lack of safety for radio-diagnostic procedures, single-lumen) have delayed their start up in the intensive care units as common practice. Though, the recent development of power-injectable PICCs overcomes these technical limitations and PICCs have started to spread in critical care settings. Two important take-home messages may be drawn from this review. First, the incidence of complications varies depending on venous accesses and healthcare professionals should be aware of the different clinical performance as well as of the different risks associated with each type of CVAD(CICCs or PICCs). Second, an inappropriate CVAD choice and, particularly, an inadequate insertion technique are relevant-and often not recognized-potential risk factors for complications in critically ill patients. We strongly believe that all healthcare professionals involved in the choice, insertion or management of CVADs in critically ill patients should know all potential risk factors of complications. This knowledge may minimize complications and guarantee longevity to the CVAD optimizing the risk/benefit ratio of CVAD insertion and use. Proper management of CVADs in critical care saves lines and lives. ESD starts with evaluation of the lesion, as accurate assessment of the depth and margin of the lesion is essential. The devices and strategies used in ESD vary, depending on the nature of the lesion. Prior to the procedure, the operator must be knowledgeable about the treatment strategy(ies), the device(s) to use, the electrocautery machine settings, the substances to inject, and other aspects. In addition, the operator must be able to manage complications, should they arise, including immediate recognition of the complication(s) and its treatment. Finally, in case the ESD treatment is not successful, the operator should be prepared to apply alternative treatments. 6Prevention of stress-related ulcer bleeding at the intensive care unit: Risks and benefits of stress ulcer prophylaxis显示文摘Stress-related mucosal disease is a typical complication of critically ill patients in the intensive care unit(ICU). It poses a risk of clinically relevant upper gastrointestinal(GI) bleeding. Therefore, stress ulcer prophylaxis(SUP)is recommended in high-risk patients, especially those mechanically ventilated > 48 h and those with a manifest coagulopathy. Proton pump inhibitors(PPI) and, less effectively, histamine 2 receptor antagonists(H2RA) prevent GI bleeding in critically ill patients in the ICU. However, the routine use of pharmacological SUP does not reduce overall mortality in ICU patients. Moreover, recent studies revealed that SUP in the ICU might be associated with potential harm such as an increased risk of infectious complications, especially nosocomial pneumonia and Clostridium difficile-associated diarrhea. Additionally, special populations such as patients with liver cirrhosis may even have an increased mortality rate if treated with PPI. Likewise, PPI can be toxic for both the liver and the bone marrow, and some PPI show clinically relevant interactions with important other drugs like clopidogrel. Therefore, the agent of choice, the specific balance of risks and benefits for individual patients as well as the possible dose of PPI has to be chosen carefully. Alternatives to PPI prophylaxis include H2 RA and/or sucralfate. 8Constraint-induced movement therapy enhances angiogenesis and neurogenesis after cerebral ischemia/reperfusion显示文摘Constraint-induced movement therapy after cerebral ischemia stimulates axonal growth by decreasing expression levels of Nogo-A,RhoA,and Rho-associated kinase(ROCK)in the ischemic boundary zone.However,it remains unclear if there are any associations between the Nogo-A/RhoA/ROCK pathway and angiogenesis in adult rat brains in pathological processes such as ischemic stroke.In addition,it has not yet been reported whether constraint-induced movement therapy can promote angiogenesis in stroke in adult rats by overcoming Nogo-A/RhoA/ROCK signaling.Here,a stroke model was established by middle cerebral artery occlusion and reperfusion.Seven days after stroke,the following treatments were initiated and continued for 3 weeks:forced limb use in constraint-induced movement therapy rats(constraint-induced movement therapy group),intraperitoneal infusion of fasudil(a ROCK inhibitor)in fasudil rats(fasudil group),or lateral ventricular injection of NEP1-40(a specific antagonist of the Nogo-66 receptor)in NEP1-40 rats(NEP1-40 group).Immunohistochemistry and western blot assay results showed that,at 2 weeks after middle cerebral artery occlusion,expression levels of RhoA and ROCK were lower in the ischemic boundary zone in rats treated with NEP1-40 compared with rats treated with ischemia/reperfusion or constraint-induced movement therapy alone.However,at 4 weeks after middle cerebral artery occlusion,expression levels of RhoA and ROCK in the ischemic boundary zone were markedly decreased in the NEP1-40 and constraint-induced movement therapy groups,but there was no difference between these two groups.Compared with the ischemia/reperfusion group,modified neurological severity scores and foot fault scores were lower and time taken to locate the platform was shorter in the constraint-induced movement therapy and fasudil groups at 4 weeks after middle cerebral artery occlusion,especially in the constraint-induced movement therapy group.Immunofluorescent staining demonstrated that fasudil promoted an immune response of nerve-regeneration-related markers(BrdU in combination with CD31(platelet endothelial cell adhesion molecule),Nestin,doublecortin,NeuN,and glial fibrillary acidic protein)in the subventricular zone and ischemic boundary zone ipsilateral to the infarct.After 3 weeks of constraint-induced movement therapy,the number of regenerated nerve cells was noticeably increased,and was accompanied by an increased immune response of tight junctions(claudin-5),a pericyte marker(a-smooth muscle actin),and vascular endothelial growth factor receptor 2.Taken together,the results demonstrate that,compared with fasudil,constraint-induced movement therapy led to stronger angiogenesis and nerve regeneration ability and better nerve functional recovery at 4 weeks after cerebral ischemia/reperfusion.In addition,constraint-induced movement therapy has the same degree of inhibition of RhoA and ROCK as NEP1-40.Therefore,constraint-induced movement therapy promotes angiogenesis and neurogenesis after cerebral ischemia/reperfusion injury,at least in part by overcoming the Nogo-A/RhoA/ROCK signaling pathway.All protocols were approved by the Institutional Animal Care and Use Committee of China Medical University,China on December 9,2015(approval No.2015 PS326 K).Zhi-Yong Zhai Juan Feng 2019Neural Regeneration Research2019,14,10:22
9Structural and functional damage to the hippocampal neurovascular unit in diabetes-related depression显示文摘Previous studies have shown that models of depression exhibit structural and functional changes to the neurovascular unit. Thus, we hypothesized that diabetes-related depression might be associated with damage to the hippocampal neurovascular unit. To test this hypothesis, neurons, astrocytes and endothelial cells were isolated from the brain tissues of rat embryos and newborn rats. Hippocampal neurovascular unit co-cultures were produced using the Transwell chamber co-culture system. A model of diabetes-related depression was generated by adding 150 mM glucose and 200 μM corticosterone to the culture system and compared with the neuron + astrocyte and astrocyte + endothelial cell co-culture systems. Western blot assay was used to measure levels of structural proteins in the hippocampal neurovascular unit co-culture system. Levels of basic fibroblast growth factor, angiogenic factor 1, glial cell line–derived neurotrophic factor, transforming growth factor β1, leukemia inhibitory factor and 5-hydroxytryptamine in the hippocampal neurovascular unit co-culture system were measured by enzyme-linked immunosorbent assay. Flow cytometry and terminal deoxynucleotidyl transferase(TdT)-mediated dUTP nick end labeling staining was used to assess neuronal apoptosis in the hippocampal neurovascular unit. The neurovascular unit triple cell co-culture system had better barrier function and higher levels of structural and secretory proteins than the double cell co-culture systems. In comparison, in the model of diabetes-related depression, the neurovascular unit was damaged with decreased barrier function, poor structural integrity and impaired secretory function. Moreover, neuronal apoptosis was markedly increased, and 5-hydroxytryptamine levels were reduced. These results suggest that diabetes-related depression is associated with structural and functional damage to the neurovascular unit. 10家长微信视频探视模式在NICU无陪护患儿中的应用显示文摘目的:探索微信视频在新生儿重症监护室( NICU)无陪护患儿家长探视中的应用,寻求降低无陪护患儿家长焦虑情绪的方法。方法选取2015年12月—2016年4月入住江苏省某三级甲等NICU的患儿家长100例,按随机数字表法分为观察组(微信视频组)50例及对照组(限时探视组)50例。对照组采用限时探视,观察组在对照组的基础上采用微信视频探视,于患儿入院时、第一次探视及出院时采用焦虑自评量表( SAS)及自制的满意度调查表进行评估和调查,比较两组探视家长焦虑状况及患儿家长对护理工作满意度。结果两组患儿家长入院时,比较差异无统计学意义( t=1.72,P〉0.05);第一次探视后及出院时观察组患儿家长焦虑心理明显减轻,两组焦虑评分:第一次探视时及出院时观察组得分分别为(51.35±7.38)分和(23.55±3.43)分,对照组分别为(59.68±8.42)分和(26.36±3.97)分,两组比较差异均有统计学意义( t值分别为-2.86,2.22;〈0.05);观察组患儿家长满意率为100%,高于对照组患儿家长满意率的88%,两组比较差异有统计学意义(t=6.383,〈0.05)。结论微信视频探视能够有效缓解无陪护患儿家长的焦虑情绪,有利于患儿康复,可以化解许多潜在的矛盾与纠纷,提高患儿家长对工作满意度,家长被动探视转变为主动探视的模式,减少了探视管理成本,为无陪护患儿探视的管理提供了一种新的有效方法。陈惠娟 范从海 曹军华 蔡盈 杨雪梅 许燕 魏雪 2016中华现代护理杂志2016,22,31:19
11Risk factors for ventilator-associated pneumonia in trauma patients:A descriptive analysis显示文摘BACKGROUND:We sought to evaluate the risk factors for developing ventilator-associated pneumonia(VAP)and whether the location of intubation posed a risk in trauma patients.METHODS:Data were retrospectively reviewed for adult trauma patients requiring intubation for>48 hours,admitted between 2010 and 2013.Patients’demographics,clinical presentations and outcomes were compared according to intubation location(prehospital intubation[PHI]vs.trauma room[TRI])and presence vs.absence of VAP.Multivariate regression analysis was performed to identify predictors of VAP.RESULTS:Of 471 intubated patients,332 patients met the inclusion criteria(124 had PHI and208 had TRI)with a mean age of 30.7±14.8 years.PHI group had lower GCS(P=0.001),respiratory rate(P=0.001),and higher frequency of head(P=0.02)and chest injuries(P=0.04).The rate of VAP in PHI group was comparable to the TRI group(P=0.60).Patients who developed VAP were 6 years older,had significantly lower GCS and higher ISS,head AIS,and higher rates of polytrauma.The overall mortality was 7.5%,and was not associated with intubation location or pneumonia rates.In the early-VAP group,gram-positive pathogens were more common,while gram-negative microorganisms were more frequently encountered in the late VAP group.Logistic regression analysis and modeling showed that the impact of the location of intubation in predicting the risk of VAP appeared only when chest injury was included in the models.CONCLUSION:In trauma,the risk of developing VAP is multifactorial.However,the location of intubation and presence of chest injury could play an important role.Suresh Kumar Arumugam Insolvisagan Mudali Gustav Strandvik Ayman El-Menyar Ammar Al-Hassani Hassan Al-Thani1 2018World Journal of Emergency Medicine2018,9,3:17
12Characteristics of paleoflood deposits archived in unit T0403 of Yuxi Site in the Three Gorges reservoir areas,China显示文摘Based on the detailed analyses,multi-proxies such as AMS 14 C dating,grain size,component and morphology of heavy minerals,micromorphology of zircon,magnetic susceptibility,Rb/Sr and Hg content are used to research the paleoflood sediments archived in the Yuxi Site.The research indicates that, since 7.6 kaBP,at least 16 times of paleoflood with water level above Wusong Elevation 147.024 m(a.s.l.) left deposits in the Neolithic layers of unit T0403 from the Yuxi Site.The results are induced from the following aspects:(1)The plaeoflood sediments take on great similarities with modern flood sediments in the Yuxi Site and Zhongba Site in probability cumulative curves which mainly show a pattern of 3 segments in fluvial pattern.(2)There is some resemblance between the heavy mineral components and the zircon shape characteristics of paleoflood deposits and those of modern flood deposits.(3) Magnetic susceptibility values(40.44―70.10SI)are lower than those of sediments from cultural layers (59.59―188.68SI).(4)Hg values(290.71―742.51 ng/g)are lower than those of sediments from cultural layers(344.16―10518.17 ng/g).(5)Rb/Sr values are higher than those of sediments from cultural layers, while those of the 4th,5th,6th,7th,8th cultural layers are high,which shows that they are inundated by paleoflood.The reason for many flood deposits existing is related to the site situated on the first terrace where the Yuxi River joins with the Yangtze River.As there are some similarities between the plaeoflood sediments and those of the Zhongba Site,it is feasible to confirm the existence of paleoflood sediments based on the above points.ZHU Cheng MA ChunMei XU WeiFeng BAI JiuJiang ZHENG ChaoGui ZHU GuangYao WANG HuiLin CHEN Ye LU XueFeng 2008Chinese Science Bulletin2008,53,S1:16
13ERA modal identification method for hydraulic structures based on order determination and noise reduction of singular entropy显示文摘Modal parameter has been considered as one of the indices in assessing the working state of hydraulic structures. The method for obtaining the modal parameter indices is crucial for health monitoring of hydraulic structures. Traditionally, model parameters are measured from the prototype dynamic test under the excitation of special equipment. The preparation is time-consuming and the method is difficult to carry out. We propose a method in this paper to denoise, reconstruct, determine the order and identify modal parameters based on singular entropy (SE) of eigenmatrix. It can be used to solve the problem of system eigenmatrix order determination, and to identify the modal order and the characteristics of structures under the working state. Finally, this method is applied to identify the operational modal parameters of Ertan arch dam under different flood discharging states and vibration frequencies of the powerhouse with a two-row placed unit in Lijiaxia hydropower station during the unit halting process. 15Tectonic evolution of the West Kunlun Orogenic Belt along the northern margin of the Tibetan Plateau:Implications for the assembly of the Tarim terrane to Gondwana显示文摘The West Kunlun orogenic belt(WKOB) along the northern margin of the Tibetan Plateau is important for understanding the evolution of the Proto-and Paleo-Tethys oceans. Previous investigations have focused on the igneous rocks and ophiolites distributed mostly along the Xinjiang-Tibet road and the China-Pakistan road, and have constructed a preliminary tectonic model for this orogenic belt. However, few studies have focused on the so-called Precambrian basement in this area. As a result, the tectonic affinity of the individual terranes of the WKOB and their detailed evolution process are uncertain. Here we report new field observations, zircon and monazite U-Pb ages of the 'Precambrian basement' of the South Kunlun terrane(SKT) and the Tianshuihai terrane(TSHT), two major terranes in the WKOB. Based on new zircon U-Pb age data, the amphibolite-facies metamorphosed volcanosedimentary sequence within SKT was deposited during the late Neoproterozoic to Cambrian(600-500 Ma), and the flysch-affinity Tianshuihai Group, as the basement of the TSHT, was deposited during the late Neoproterozoic rather than Mesoproterozoic. The rock association of the volcano-sedimentary sequence within SKT suggests a large early Paleozoic accretionary wedge formed by the long-term lowangle southward subduction of the Proto-Tethys Ocean between Tarim and TSHT. The amphibolitefacies metamorphism in SKT occurred at ca. 440 Ma. This ca. 440 Ma metamorphism is genetically related to the closure of the Proto-Tethys Ocean between Tarim and the Tianshuihai terrane, which led to the assembly of Tarim to Eastern Gondwana and the final formation of the Gondwana. Since the late Paleozoic to early Mesozoic, the northward subduction of the Paleo-Tethys Ocean along the HongshihuQiaoertianshan belt produced the voluminous early Mesozoic arc-signature granites along the southern part of NKT-TSHT. The Paleo-Tethys ocean between TSHT and Karakorum closed at ca. 200 Ma, as demonstrated by the monazite age of the paragneiss in the Kangxiwa Group. 16Emphasizing Essential Words for Sentiment Classification Based onRecurrent Neural Networks显示文摘With the explosion of online communication and publication, texts become obtainable via forums, chat mes-sages, blogs, book reviews and movie reviews. Usually, these texts are much short and noisy without sufficient statisticalsignals and enough information for a good semantic analysis. Traditional natural language processing methods such asBow-of-Word (BOW) based probabilistic latent semantic models fail to achieve high performance due to the short textenvironment. Recent researches have focused on the correlations between words, i.e., term dependencies, which could behelpful for mining latent semantics hidden in short texts and help people to understand them. Long short-term memory(LSTM) network can capture term dependencies and is able to remember the information for long periods of time. LSTMhas been widely used and has obtained promising results in variants of problems of understanding latent semantics of texts.At the same time, by analyzing the texts, we find that a number of keywords contribute greatly to the semantics of thetexts. In this paper, we establish a keyword vocabulary and propose an LSTM-based model that is sensitive to the wordsin the vocabulary; hence, the keywords leverage the semantics of the full document. The proposed model is evaluated ina short-text sentiment analysis task on two datasets: IMDB and SemEval-2016, respectively. Experimental results demon-strate that our model outperforms the baseline LSTM by 1%-2% in terms of accuracy and is effective with significantperformance enhancement over several non-recurrent neural network latent semantic models (especially in dealing withshort texts). 17Hyodeoxycholic acid protects the neurovascular unit against oxygen-glucose deprivation and reoxygenation-induced injury in vitro显示文摘Calculus bovis is commonly used for the treatment of stroke in traditional Chinese medicine. Hyodeoxycholic acid(HDCA) is a bioactive compound extracted from calculus bovis. When combined with cholic acid, baicalin and jas-minoidin, HDCA prevents hypoxia-reoxygenation-induced brain injury by suppressing endoplasmic reticulum stress-mediated apoptotic signaling. However, the effects of HDCA in ischemic stroke injury have not yet been studied. Neurovascular unit(NVU) dysfunction occurs in ischemic stroke. Therefore, in this study, we investigated the effects of HDCA on the NVU under ischemic conditions in vitro. We co-cultured primary brain microvascular endothelial cells, neurons and astrocytes using a transwell chamber co-culture system. The NVU was pre-treated with 10.16 or 2.54 μg/mL HDCA for 24 hours before exposure to oxygen-glucose deprivation for 1 hour. The cell counting kit-8 assay was used to detect cell activity. Flow cytometry and terminal deoxynucleotidyl transferase-mediated dUTP nick end labeling were used to assess apoptosis. Enzyme-linked immunosorbent assay was used to measure the expression levels of inflammatory cytokines, including interleukin-1β, interleukin-6 and tumor necrosis factor-α, and neurotrophic factors, including brain-derived neurotrophic factor and glial cell line-derived neurotrophic factor. Oxidative stress-related factors, such as superoxide dismutase, nitric oxide, malondialdehyde and γ-glutamyltransferase, were measured using kits. Pretreatment with HDCA significantly decreased blood-brain barrier permeability and neuronal apoptosis, significantly increased transendothelial electrical resistance and γ-glutamyltransferase activity, attenuated oxidative stress damage and the release of inflammatory cytokines, and increased brain-derived neurotrophic factor and glial cell line-derived neurotrophic factor expression. Our findings suggest that HDCA maintains NVU morphological integrity and function by modulating inflammation, oxidation stress, apoptosis, and the expression of neurotrophic factors. Therefore, HDCA may have therapeutic potential in the clinical management of ischemic stroke. 18环节管理在NICU医疗器械性压疮预防中的应用效果显示文摘目的:探讨NICU医疗器械性压疮环节管理,通过各环节管理,达到降低NICU医疗器械性压疮的发生率。方法选取2012年6月—2014年5月726例患者作为对照组,采用常用的预防措施、常规的压疮分期进行护理。另收集2014年6月—2016年5月869例患者作为观察组,观察组在对照组护理的基础上,进行各环节干预:通过成立科室压疮督导小组进行干预、制定常用仪器设备相关压疮的护理措施、建立预防和管理医疗器械相关压疮的管理总则、加强人员培训考核及制定各级职责,观察医疗器械性压疮的发生率。结果对照组发生医疗器械相关性压疮28例,发生率为3.9%,观察组发生医疗器械相关性压疮7例,发生率为0.8%;对照组压疮发生的严重程度高于观察组,创面的治愈率低于观察组,差异有统计学意义( U=-2.001,〈0.05);培训后护理人员对医疗器械相关压疮知识掌握情况总分为(26.2±1.7)分,高于培训前(17.6±0.8)分,差异有统计学意义(t=-6.04,〈0.01)。结论通过对医疗器械相关压疮的环节管理,降低了医疗器械性压疮的发生率,提高了护理质量,规范了医疗器械的使用,更新了护士对医疗器械压疮护理的新理念。郑翠霞 王秀萍 纪进华 徐娟 牛双 2016中华现代护理杂志2016,22,31:11

20Maize response to elevated plant density combined with lowered N-fertilizer rate is genotype-dependent显示文摘Increasing plant density and improving N fertilizer rate along with the use of high density-tolerant genotypes would lead to maximizing maize(Zea mays L.) grain productivity per unit land area. The objective of this investigation was to match the functions of optimum plant density and adequate nitrogen fertilizer application to produce the highest possible yields per unit area with the greatest maize genotype efficiency. Six maize inbred lines differing in tolerance to low N and high density(D) [three tolerant(T); L-17, L-18, L-53,and three sensitive(S); L-29, L-54, L-55] were chosen for diallel crosses. Parents and crosses were evaluated in the 2012 and 2013 seasons under three plant densities: low(47,600),medium(71,400), and high(95,200) plants ha-1and three N fertilization rates: low(no N addition), medium(285 kg N ha-1) and high(570 kg N ha-1). The T × T crosses were superior to the S × S and T × S crosses under the low N–high D environment in most studied traits across seasons. The relationships between the nine environments and grain yield per hectare(GYPH) showed near-linear regression functions for inbreds L54, L29, and L55 and hybrids L18 × L53 and L18 × L55 with the highest GYPH at a density of47,600 plants ha-1and N rate of 570 kg N ha-1and a curvilinear relationship for inbreds L17, L18, and L53 and the rest of the hybrids with the highest GYPH at a density of95,200 plants ha-1combined with an N rate of 570 kg N ha-1.
