| 2 | The APR triangle:A practical zone in the Glissonean approach to laparoscopic anatomical right hepatectomy显示文摘Background:An understanding of vascular anatomy is crucial for the safe performance of laparoscopic anatomical liver excision.We discovered a triangular zone during the laparoscopic right liver surgery and termed this zone the APR triangle.The purpose of this study was to determine the probability of the existence of the APR triangle and elucidate its various forms.Methods:Analyzed three-dimensional image reconstructions of 66 individuals who underwent liver surgery and calculated the statistics for various types of APR triangles under various grouping settings.Results:The APR triangle was present in the majority of cases,with right hepatic vein trunk type in 68%and right hepatic vein branch type in 21%,respectively.The angle between the right anterior and right posterior hepatic pedicles(AP&PP)was at most between 45 and 90°(74%).There was a 35%chance that at least one of the AP&PP was longer than 2 cm,and a 39%chance that both were.The right posterior pedicle first branch would appear at the bifurcation of AP&PP in 13%only.Conclusions:The APR triangle is objectively present and may represent a practical zone for performing laparoscopic right hepatic anatomical resection more simply and safely. | Jie Chen Ziyu Zhang Rui Zhou Anzhi Wang Jun Cao Yajin Chen | 2022 | iLIVER2022,1,3: | 2 |
| 3 | Free fatty acid-induced miR-22 inhibits gluconeogenesis via SIRT-1-mediated PGC-1α expression in nonalcoholic fatty liver disease显示文摘Background and aims:Free fatty acids(FFAs)are one of the important regulators of the progression of nonalcoholic fatty liver disease.The FFAs are shown to modulate the metabolic status of the liver by modulating several cellular pathways in hepatocytes.Here,we elucidated the role of miR-22 in modulating FFAs-mediated gluconeogenesis.Methods:Huh7 and WRL68 cells were transfected with nonspecific miRNA,miR-22 premiRs or anti-miR-22 oligos followed by incubation with palmitic acid,oleic acid,and linoleic acid(300μM each)for 48 and 72 h after transfection.The expression of miR-22 was performed using real-time polymerase chain reaction and Western blots were performed for SIRT-1,PGC-1α,PEPCK,and glucose-6-phosphatase.Three groups of C57BL/6 mice(6 mice per group)were fed with standard diet,choline sufficient L-amino acid defined diet or choline-deficient L-amino acid defined(CDAA)diet for 6,18,32,or 54 weeks.Triglycerides content was measured in the serum.Expression of miR-22 and the protein expression of gluconeogenic enzymes were analyzed in the tissue samples.Results:Incubation of miR-22-transfected cells with FFAs inhibited the expression of SIRT-1,PGC-1α,PEPCK,and glucose-6-phosphatase,while miR-22 expression was increased.These changes were reversed when the cells were transfected with anti-miR-22 oligos.CDAA-fed mice showed the significant increase in triglycerides content and miR-22 expression,while there was an inhibition of SIRT-1,PGC-1α,PEPCK,and glucose-6-phosphatase expression in CDAA-fed mice. | Ajay K.Yadav Teja Naveen Sata Daksh Verma Amrendra K.Sah Amit K.Mishra Mrinalini Md.Musa Hossain Kishor Pant Senthil K.Venugopal | 2023 | iLIVER2023,2,1: | 1 |
| 4 | Tenofovir disoproxil fumarate therapy in patients with chronic hepatitis B and advanced fibrosis or compensated cirrhosis显示文摘Background and aims:Tenofovir disoproxil fumarate(TDF)is the first-line therapy for chronic hepatitis B.This interim analysis presents the efficacy and safety data for TDF at Week 144 in patients with chronic hepatitis B and advanced fibrosis or compensated cirrhosis from China.Methods:Patients were assessed for incidence of newly diagnosed hepatocellular carcinoma(HCC)and disease progression,liver stiffness measurement(LSM),virological suppression(serum hepatitis B virus DNA<20 IU/mL),alanine aminotransferase normalization,hepatitis B e antigen(HBeAg)loss and seroconversion,histological liver fibrosis score,and safety at Week 144.Results:Overall,197 patients were enrolled.At Week 144,the incidence of newly diagnosed HCC was observed in 2.1%patients,and the incidence of disease progression was observed in 3.6%patients.The mean(standard deviation)change in LSM from baseline was5.1(5.85)kPa.Serum hepatitis B virus DNA<20 IU/mL was observed in 94.1%patients,alanine aminotransferase normalization in 33.5%patients,HBeAg loss in 35.6%patients,and HBeAg seroconversion in 14.4%patients.Among patients with stage F3 or F4 fibrosis at baseline by LSM,38.3%patients regressed to stage F0/1,and 22.0%of patients regressed to stage F2 at Week 144.Overall,67.7%patients experienced≥1 adverse events,13.8%patients experienced TDF-related adverse events,and 16.4%patients experienced serious(none were TDF-related).Conclusions:At Week 144 of TDF treatment,low incidence of HCC and disease progression were reported.Virological suppression was observed in 94.1%patients,which was associated with fibrosis regression.No new safety events were identified. | Huiying Rao Jia Shang Qing Xie Jianqi Lian Pujun Gao Junping Shi Xinyue Chen Jiefei Wang Min Xu Liaoyun Zhang Yingren Zhao QingMao Maorong Wang Wei Zhao Zong Zhang Jidong Jia Hong Tang Jiming Zhang Xin Zheng Chang Liu Lai Wei | 2022 | iLIVER2022,1,3: | 1 |

| 7 | Efficacy and complications of transarterial chemoembolization alone or in combination with different protocols for hepatocellular carcinoma: A Bayesian network meta-analysis of randomized controlled trials显示文摘Background and aims:Transarterial chemoembolization(TACE)is the standard treatment for most intermediate-toadvanced stage hepatocellular carcinoma.With the ongoing development of interventional therapies,TACE in combination with different protocols is being explored.A network meta-analysis to collect recent evidence in this field is required.Methods:We searched PubMed,Web of Science,Cochrane Central Register of Controlled Trials,Embase,Chinese National Knowledge Infrastructure database,Cochrane Library,and Wanfang database from their inception to May 2022 for randomized controlled trials(RCTs)that reported TACE in combination with different protocols for hepatocellular carcinoma.Results:In total,67 RCTs assessing eight combination treatments were included.The quality of primary outcomes was from moderate to low.The Bayesian network meta-analysis confirmed that TACE combined with iodine-125 seed implantation had the highest probability of 1-year survival rate and TACE combined with sorafenib had the highest probability of 2-year survival rate.TACE alone had the highest probability of incidence of fever,TACE combined with sorafenib had the highest probability of incidence of pain and TACE combined with percutaneous ethanol injection had the highest probability of incidence of nausea and vomiting.Conclusion:This Bayesian network meta-analysis found that TACE combined with iodine-125 seed and TACE combined with sorafenib may benefit patients in terms of 1-year survival and 2-year survival,respectively.To confirm this conclusion,high-quality and well-designed RCTs with a larger sample size are needed. | Jingxin Yan Manjun Deng Ting Li Changhao Dong Minglan Wang Shunyu Kong Yingxing Guo Haining Fan | 2023 | iLIVER2023,2,2: | 1 |
| 8 | Establishment of a risk assessment system for complications of hepatectomy based on preoperative variables显示文摘Background and aims:To reduce the incidence of postoperative complications,it is important to predict them and intervene before surgery if necessary.However,there is no ideal system to evaluate the overall risk of post-operative complications of liver surgery on the basis of preoperative variables.Therefore,this study aimed to design and validate a risk assessment system to predict postoperative complications of hepatectomy on the basis of preoperative variables.Methods:Binomial logistic regression was used to derive the“hepatectomy overall risk formula”(HORF)for predicting postoperative complications on the basis of preoperative variables.Results:Multivariate analysis revealed that Child–Pugh grade B–C(odds ratio[OR]=1.984,p=0.002),medical diseases requiring drug treatment(OR=1.883,p=0.003),major hepatectomy(OR=1.947,p<0.001),adjacent organ invasion(OR¼3.616,p¼0.023),and preoperative hospital stay>7 days(OR=1.565,p=0.004)were independent risk factors for postoperative complications of hepatectomy.The area under the curve for the HORF was 0.736.The optimal cut-off value for predicting complications was 0.32(32%).The area under the curve for the HORF in the validation dataset was 0.727.Conclusion:The HORF can accurately predict postoperative complications of hepatectomy on the basis of pre-operative variables,and thus enables the determination of the necessity for intervention before surgery. | Lining Xu Guiping Li Bo Yang | 2022 | iLIVER2022,1,4: | 1 |

| 10 | Glycemic control predicts the risk of hepatic fibrosis in biopsy-proven NAFLD:a possible mediating role for leukemia inhibitory factor?显示文摘Background:Despite the clear link between nonalcoholic fatty liver disease(NAFLD)and type 2 diabetes mellitus,little is understood about how glycemic control impacts histological severity.We aimed to investigate the deeper association between hemoglobin A1c(HbA1c)and the histologic severity of liver fibrosis.Methods:A total of 568 adults with biopsy-proven NAFLD from the PERSONS cohort in Wenzhou were enrolled.The association between mean HbA1c and hepatic histological features was investigated with ordinal logistic regression.Generalized additive models were used to identify the non-linear relationship between HbA1c and increased fibrosis stage(stage F3).Causal mediation analysis was performed to calculate the indirect effect of the relationship between HbA1c and hepatic fibrosis mediated by cytokines.Results:Every 1%increase in mean HbA1c was associated with 16%higher odds of increased fibrosis stage(odds ratio 1.16,95%CI 1.04–1.30),even after adjustment for confounding factors.There was a non-linear association between HbA1c and increased fibrosis stage(stage F3),and the HbA1c inflection point was 9.2%.In particular,the ORs on the left and right sides of this inflection point were 2.1(95%CI 1.4–3.3)and 0.1(95%CI 0–4.8),respectively.7%of the association(OR 1.07,95%CI 1.01–1.12)between HbA1c and liver fibrosis was mediated by leukemia inhibitory factor.Conclusions:Glycemic control predicts severity of hepatic fibrosis and leukemia inhibitory factor plays an intermediary role between them,and thus optimizing glycemic control may be a means of modifying the risk of fibrosis progression in NAFLD. | Feng Gong Kenneth I.Zheng Liang-Jie Tang Gang Li Rafael S.Rios Ou-Yang Huang Yang-Yang Li Christopher D.Byrne Giovanni Targher Man Mi Na He Hong-Lei Ma Ming-Hua Zheng | 2022 | iLIVER2022,1,1: | 1 |
| 11 | Anxiety and depression in older adult patients undergoing elective liver surgery in allopatry medical treatment显示文摘Background:Allopatry medical treatment is common in China.However,allopatry medical therapy can result in many problems,including a negative psychological impact on patients.Patients undergoing liver surgery often experience anxiety and depression.To understand the psychological status of older adult patients undergoing surgery better,this study was designed to investigate the incidence of anxiety and depression in older adult patients undergoing elective liver surgery and to analyze factors associated with it in allopatry medical therapy.Methods:A total of 173 older adult patients undergoing elective liver surgery were included in the study.Patients were evaluated using the hospital anxiety and depression scale(HADS).The contributing factors affecting the psychological state of older adult patients undergoing elective surgery were analyzed using a linear regression method.Results:The HADS-A(hospital anxiety and depression scale-anxiety).The correlation(scale)score of the older adult patients undergoing elective liver surgery was(8.692.38),including 53 asymptomatic patients,86 suspicious patients,and 34 symptomatic patients.The HADS-D(hospital anxiety and depression scale-depression)score was(8.312.90),including 83 asymptomatic patients,56 suspicious patients,and 34 symptomatic patients.Multivariate analysis showed that residence and complication grade(Clavien–Dindo Classification of Surgical Complications or Accordion Severity Grading System)significantly correlated with the anxiety level of patients undergoing elective surgery.Residence,a requirement of blood transfusion,total transfusion volume,and Accordion complication grade showed a significant correlation with depression in patients undergoing elective surgery.Conclusions:Anxiety and depression in older adult patients undergoing elective liver surgery were common.Regional differences(local patients vs non local patients)and the severity of complications were the risk factors for anxiety and depression in older adult patients undergoing elective liver surgery.Reducing both regional differences and the severity of complications would be beneficial to alleviate the risk of anxiety and depression in older adult patients undergoing elective liver surgery and thus promote their physical and mental health. | Lining Xu Yingying Xu Guiping Li Bo Yang | 2022 | iLIVER2022,1,2: | 0 |
| 12 | Clinical analysis of the characteristics of patients diagnosed with hemolytic anemia induced by hepatitis E viral infection显示文摘Background:The extrahepatic manifestations of hepatitis E virus(HEV)infection include hemolytic anemia.However,the clinical characteristics of HEV-induced hemolytic anemia have been only sporadically reported in case reports,and there is a scarcity of relevant systematic reviews.The aim of the current study was to analyze and summarize the clinical characteristics of patients diagnosed with HEV-induced hemolytic anemia.Methods:This study included 1 patient diagnosed with HEV-induced hemolytic anemia who was admitted to our hospital and 19 additional patients identified through the available literature.Results:Of the 20 patients included in this study,the majorities were from China and India(12 and 6 cases,respectively);furthermore,13 of them were men and 7 were women,with an average age of 35(16–56)years.The highest total bilirubin level was 646(452–913)μmol/L,highest alanine aminotransferase level was 1335(708–2798)IU/L,and lowest hemoglobin level was 6.02.0 g/dL.Additionally,10 patients(50.0%,10/20)underwent the glucose-6-phosphate dehydrogenase(G6PD)activity test,and all of them were diagnosed with G6PD deficiency(100.0%,10/10).Finally,18 patients(90.0%)recovered and 2 died(10.0%)after treatment.Conclusions:Most of the patients with HEV-induced hemolytic anemia recovered with treatment.Acute HEV infection that causes hemolytic anemia is strongly associated with the G6PD status of the patients. | Haohui Deng Min Xu Huiyuan Liu Hongbo Gao | 2022 | iLIVER2022,1,2: | 0 |

| 14 | Identification of publication characteristics and research trends in the management of gallbladder cancer显示文摘Background:To date,no comprehensive analysis of gallbladder cancer(GBC)management has been reported.We aimed to identify the publication characteristics and research trends in managing GBC over the past three decades.Methods:We selected the 100 most cited articles and performed a bibliometric analysis to summarize the publication characteristics,explore research hotspots,and identify research trends in the management of GBC.Results:The total citations of the included articles ranged from 123 to 1822.Period II(2001–2010)yielded the highest number of included articles,whereas the lowest was in Period III(2011–2020).The United States and Japan published the most papers,in which the Memorial Sloan–Kettering Cancer Center and Nagoya University were the leading institutions,respectively.The most influential authors were Blumgart LH and Fong YM from the United States.Cooperation among countries,institutions,and authors was weak.The Annals of Surgery contributed the most articles with the highest number of total citations.The most researched topic was surgery,followed by systemic therapy and adjuvant therapy.Since Period I,the percentage of surgery-related publications continuously decreased(Periods II and III versus Period I,both p<0.001),with a concomitant increase in those of adjuvant therapy(Period III versus Period I,p=0.004)and systemic therapy(Period II versus Period I,p=0.004;Period III versus Period I,p=0.002).Conclusions:Surgery remains the preferred treatment,while there is a tendency toward adjuvant and systemic therapy in GBC management.An increase in local and international collaboration for managing GBC is required. | Jiasheng Cao Jiahao Hu Jiliang Shen Bin Zhang Win Topatana Shijie Li Tianen Chen Sarun Jeungpanich Yitong Tian Ziyi Lu Shuyou Peng Xiujun Cai Mingyu Chen | 2022 | iLIVER2022,1,2: | 0 |

| 17 | Single-cell transcriptome analysis reveals a cancer-associated fibroblast marker gene signature in hepatocellular carcinoma that predicts prognosis显示文摘Background and aims:Hepatocellular carcinoma(HCC)is one of the leading causes of cancer death.Multi-pathway combination therapy is used to treat HCC,and immunotherapy is also a routine part of treatment.As a major component of the tumor microenvironment(TME),cancer-associated fibroblasts(CAFs)actively participate in cancer progression through complex functions.However,because CAFs dynamically change during cancer development,most of the current treatment strategies targeting CAFs fail.We created a prognostic CAF marker gene signature(CAFMGS)to investigate the utility of CAFs as a prognostic factor and therapeutic target.Methods:Gene Expression Omnibus(GEO)single-cell RNA sequencing(Sc-RNA-seq)data were analyzed to identify CAF marker genes in HCC.The Cancer Genome Atlas(TCGA)database was used as a training cohort to construct the CAFMGS model and the International Cancer Genome Consortium(ICGC)dataset was used to validate the CAFMGS.Results:Marker genes in the CAFMGS model were(0.0001-SPP1),(0.0084-VCX3A),(0.0015-HMGA1),(0.0082-PLOD2),and(0.0075-CACYBP).The CAFMGS_score was separated into high-risk and low-risk groups based on the median of the patients’OS.Univariate and multivariate analyses confirmed that CAFMGS_score was an independent prognostic factor in the training group.CAFMGS_score was a more accurate prognostic indicator compared with clinicopathological score and tumor mutational burden score.Conclusion:CAFMGS offers a fresh perspective on stromal cell marker genes in HCC prognosis and expands our knowledge of CAF heterogeneity and functional diversity,perhaps paving the way for CAF-targeted immunotherapy in HCC patients. | Hao Chi Dapeng Chen Yuliang Zhang Zilin Cui Yi Bai Yamin Zhang | 2023 | iLIVER2023,2,1: | 0 |
| 18 | A high level of secreted phosphoprotein 1 is associated with macrophage infiltration and poor prognosis in hepatocellular carcinoma显示文摘Background and aims:Secreted phosphoprotein 1(SPP1)functions in several physiological processes.The role of SPP1 expression in the prognosis and tumor immunity of hepatocellular carcinoma(HCC)is unknown.The aim of this study was to investigate the expression pattern of SPP1 in HCC and its correlation with prognosis and tumor immunity.Methods:Clinical and gene expression data of The Cancer Genome Atlas-liver hepatocellular carcinoma(LIHC)cohort and 11 other HCC datasets were collected.The Kaplan–Meier method and Cox regression analysis were used to analyze the prognostic value of SPP1.The DESeq2 package in R was used to analyze SPP1-related genes.Gene Ontology analysis and gene set enrichment analysis were used to determine the biological function of SPP1 in HCC.The single sample Gene Set Enrichment Analysis(ssGSEA)method was used to analyze the immune infiltrates of HCC.Illumina human methylation 450 data and level 3 HTSeq-FPKM data from The Cancer Genome Atlas-LIHC were used to analyze the effects of DNA methylation level on SPP1 expression.Results:SPP1 was overexpressed in HCC and correlated with T stage,histological grade,adjacent hepatic tissue inflammation,and vascular invasion in HCC.The analysis of survival rates indicated that high SPP1 levels were associated with poor overall survival in HCC.Functional analysis showed that SPP1 is related to tumor immunity,especially macrophage infiltration.Aberrant demethylation of the promoter region is one of the mechanisms underlying the increase of SPP1 in HCC.Conclusion:Our results indicate that SPP1 is an independent prognostic factor for HCC and is correlated with the clinical features and macrophage infiltration in HCC. | Jianping Song Jingxian Sun Shuhong Jing Tingxiao Zhang Jianlei Wang Yanfeng Liu | 2023 | iLIVER2023,2,1: | 0 |
