| 1 | Cost-effectiveness analysis of treatments for metastatic castration resistant prostate cancer显示文摘Objective:Treatment options for metastatic castration resistant prostate cancer(mCRPC)have expanded rapidly in recent years.Given the significant economic burden,we sought perform a cost-effectiveness analysis(CEA)of the contemporary treatment paradigm for mCRPC.Methods:We devised a treatment protocol consisting of sipuleucel-T,enzalutamide,abiraterone,docetaxel,radium-223,and cabazitaxel.We estimated number and length of treatments for each therapy using dosing schedules or progression free survival data from published clinical trials.We estimated treatment cost using billing data and Medicare reimbursement values and performed a CEA.Our analysis assumed US$100,000 per life year saved(LYS)as the threshold societal willingness to pay.Results:Incremental cost-effectiveness ratios(ICER)for strategies incorporating sipuleucel-T that were not eliminated by extended dominance exceeded the societal threshold willingnessto-pay of US$100,000 per LYS,the lowest of which was sipuleucel-T+enzalutamide+abiraterone+docetaxel at US$207,714 per LYS.Enzalutamide+abiraterone+docetaxel exhibited the most favorable ICER among strategies without sipuleucel-T at US$165,460 per LYS.Conclusion:Based on the available survival data and current costs of treatment,all treatment strategies greatly exceed a commonly assumed societal willingness-to-pay threshold of US$100,000 per LYS.Improvements in this regard can only comewith a reduction in pricing,better tailoring of treatment or significant enhancements in survival with clinical use of treatment combinations or sequences. | Matthew E.Pollard Alan J.Moskowitz Michael A.Diefenbach Simon J.Hall | 2017 | Asian Journal of Urology2017，4，1： | 6 |

| 3 | An examination by GC×GC-TOFMS of organic molecules present inhighly degraded oils emerging from Caribbean terrestrial seeps of Cretaceous age显示文摘For our ancestors, oil seeps were both a fascination and a resource but as the planet's reserves of high quality low density oil becomes increasingly depleted, so there is now a renewed interest in heavier,biodegraded oils such as those encountered in terrestrial seeps. One such seep is Pitch Lake in the Caribbean island of Trinidad, which is the largest natural deposit of asphalt in the world. At the northern end of the Caribbean, oil emerges along a tectonic contact on the island on Cuba. The sources of the oils from these seeps are relatively recent and both are subject to intense weathering due to the tropical conditions. When analysed by gas chromatography(GC) both oils appear as unresolved complex mixtures(UCM) and show a very high degree of biodegradation thus presenting an analytical challenge. In this case study, these two Caribbean seep oils were analysed by comprehensive two dimensional GC with time of flight mass spectrometry(GC×GC-TOFMS) to expose many thousands of the individual compounds that comprise the UCM. The high chromatographic resolution of the GC×GC-TOFMS produced good quality mass spectra allowing many compounds including molecular fossil ‘biomarkers' to be identified. Compound classes included diamondoid hydrocarbons, demethylated hopanes and secohopanes, mono-and tri-aromatic steroids. D-ring aromatised structures of the 8,14-seco-hopanes,including demethylated forms were present in both oils but further demethylation, probably at position C-25 during biodegradation, was only observed in the Pitch Lake oil. Many polycyclic aromatic hydrocarbons(PAHs) were absent although the fungal-derived pentacyclic PAH perylene was present in both oils. The presence of the angiosperm biomarker lupane in the Pitch Lake oil constrained the age to the Late Cretaceous. | 7 | Tracking the Cretaceous transcontinental Ceduna River through Australia:The hafnium isotope record of detrital zircons from offshore southern Australia显示文摘The middle-upper Cretaceous Ceduna River system traversed continental Australia from the NE coast to the centre of the southern coast.At its mouth,it formed a vast delta system that is similar in scale to the Niger delta of West Africa.The delta system is composed of two main lobes that represent different phases of delta construction.A recent hypothesis has challenged the traditional idea that both lobes of the delta were derived from a transcontinental river system by suggesting that the upper lobe(Santonian- Maastrichtian) is instead derived from a restricted catchment within southern Australia.Hf isotopic data presented here fingerprint the original source of the upper delta lobe zircons to NE Australia,with data comparing well with similar U-Pb and Lu-Hf isotopic data from the Lachlan Orogen,the New England Orogen,the eastern Musgraves Province and the northern Flinders Ranges.These data do not preclude a model where the lobe is derived from recycled Eromanga Basin sediments during a phase of late Cretaceous inland Australian uplift,but when coupled with reconnaissance low-temperature thermochronometry from the region of the Ceduna River course indicating widespread Triassic-Jurassic exhumation,and comparisons with detrital zircon data from the Winton Formation upstream of any proposed uplift,we suggest that both lobes of the Ceduna Delta are likely to be derived from a transcontinental Ceduna River. | Jarred Lloyd Alan S.Collins Justin L.Payne Stijn Glorie Simon Holford Anthony J.Reid | 2016 | Geoscience Frontiers2016，7，2： | 2 |
