| 1 | Simultaneous intraductal papillary neoplasms of the bile duct and pancreas treated with chemoradiotherapy显示文摘Some authors have suggested that intraductal papillary mucinous neoplasms of the bile duct(IPMN-B) could be the the biliary counterpart of IPMN of the pancreas(IPMN-P) since they share several clinical-pathological features.These include prominent intraductal papil-lary proliferation pattern,a gastrointestinal phenotype,frequent mucin hyper-secretion and progression to mu-cinous carcinoma.To date there are just four reported cases of patients with synchronous IPMN-B and IPMN-P all of which were treated surgically.We hereby report the case of a 76-year-old woman who was incidentally diagnosed with both an asymptomatic 3 cm bulky uid lesion obstructing the bile duct lumen,diagnosed as a malignant IPMN-B,and synchronous multiple pancreatic cystic lesions(10-13 mm) communicating with an irreg-ular Wirsung,diagnosed as branch duct IPMN-P.Since surgery was ruled-out because of the patient's age and preferences,she underwent a conservative manage-ment regimen comprising both chemotherapy and radio-therapy.This was effective in decreasing the mass size and in resolving subsequent jaundice.This is also the f irst reported case of IPMN-B successfully treated with chemoradiotherapy.Clinicians should consider medical treatment as an option in this clinical scenario,in pa-tients who may be unf it for surgery. | Roberto Valente Gabriele Capurso Paola Pierantognetti Elsa Iannicelli Matteo Piciucchi Adriana Romiti Paolo Mercantini Alberto Larghi Giulia Francesca Federici Viola Barucca Maria Falchetto Osti Emilio Di Giulio Vincenzo Ziparo Gianfranco Delle Fave | 2012 | World Journal of Gastrointestinal Oncology2012，4，2： | 13 |
| 2 | Impact of number of run-off vessels on interwoven nitinol mesh stents patency in the femoropopliteal segment显示文摘Objective To evaluate the impact of run-off vessels number on the outcomes of Supera stent(Abbott Vascular,Santa Clara,Calif,USA)for treatment of femoropopliteal occlusive disease.Methods We retrospectively evaluated the medical records of 188 consecutive patients(mean age 68.2±9.6 years,100 males)undergone angiography and woven mesh stent implantation in femoral or popliteal arteries or both arterial segments,in our institution between January 12014 and January 12018.Target lesion revascularization and major adverse limb events at 12-month were evaluated comparing patients with 1-,2-or 3-run-off vessels in the foot.Results Interventional success was achieved in 100%.Stent implantation involved in the femoral site in 56 patients(30.3%),the femoropopliteal in 92 patients(48.9%)and the popliteal site in 40 patients(21.3%).A significant improvement of ankle-brachial index(0.29±0.6 vs.0.88±0.3,P<0.001)and Rutherford class(5.3±0.8 vs.0.7±1.9,P<0.01)were observed before discharge.The median follow-up duration was 12.3 months(inter quartile range:11.0 to 13.9).During the follow-up period,52 patients(27.6%)had clinical events.Primary patency at 12 months was 72.4%.The primary patency significantly increased when the runoff status.Comparing the number of events among patients with different number of run-off vessels,a significant difference(P<0.001)was observed for patients having one(24.0%)and two run-off vessels(15.0%).Conclusions The outcomes of Supera stent in femoropopliteal occlusive disease depend strictly on the number of run-off vessels. | Gianluca Rigatelli Marco Zuin Fabio Dell’Avvocata Dobrin Vassilev Stefano Barison Sabrina Osti Alberto Mazza Alberto Sacco | 2020 | Journal of Geriatric Cardiology2020，17，9： | 2 |

| 9 | Migration of the Western Marsh Harrier to the African wintering quarters along the Central Mediterranean flyway:a 5-year study显示文摘Background: The Western Marsh Harrier(Circus aeruginosus) is a partial migrant with the populations from Eastern and Northern Europe migrating south to sub-Saharan Africa. During the autumn migration, that is peaking in Septem ber, harriers move on a broad front heading SW and undertake long sea-crossings en route to their wintering quarters, passing in substantial numbers through Italy and Malta with the highest concentrations recorded at the Strait of Messina. Most of the individuals migrating across the Strait are heading for the wintering quarters in Africa, while fewer spend the winter in Sicily.Methods: In a 5-year study(2011-2015), between 26 August and 30 September, we determined age and sex of autumn migrating harriers through this flyway. In 2014 we determined, by marine radar and optical range finder, the flight altitude of migrating harriers.Results: A total of 10,261 Western Marsh Harriers were counted during the whole study, with an average of 2052 per autumn season. Adults outnumbered juveniles and males outnumbered females. Harriers flew at lower altitudes during the morning while large flocks flew lower than single birds or small flocks.Conclusions: Our observations are consistent with previous surveys and confirm that adult males have a tendency to migrate over a long distance, while substantial numbers of adult females and juveniles do not head for the wintering quarters in Africa. | 11 | Clinical spectrum and currently available treatment of typeⅠinterferonopathy Aicardi-Goutières syndrome显示文摘Background Aicardi-Goutières syndrome(AGS)is a genetically determined disorder with a variable phenotype.Since the original description of AGS,advances in gene sequencing techniques have resulted in a significant broadening of the phenotypic spectrum associated with AGS genes,and new clinical pictures have emerged beyond the classic presentation.The aim of this review is to provide a comprehensive analysis of the clinical spectrum of AGS and report currently available treatments and new immunosuppressive strategies.Data sources Literature reviews and original research articles were collected from databases,including PubMed and Clini-calTrials.gov.Relevant articles about AGS were included.Results The involvement of the nervous system certainly represents the major cause of mortality and morbidity in AGS patients.However,other clinical manifestations,such as chilblains,hepatosplenomegaly,and hematological disturbances,may lead to the diagnosis and considerably impact the prognosis and overall quality of life of these patients.Therapeutic approaches of AGS are limited to interventions aimed at specific symptoms and the management of multiple comorbidities.However,advances in understanding the pathogenesis of AGS could open new and more effective therapies.Conclusions The over-activation of innate immunity due to upregulated interferon production plays a critical role in AGS,leading to multi-organ damage with the main involvement of the central nervous system.To date,there is no specific and effective treatment for AGS.New drugs specifically targeting the interferon pathway may bring new hope to AGS patients. | Giovanni Battista Dell'Isola Gianluca Dini Kaleb Logan Culpepper Katherin Elizabeth Portwood Pietro Ferrara Giuseppe Di Cara Alberto Verrotti Mauro Lodolo | 2023 | World Journal of Pediatrics2023，19，7： | 0 |
