| 2 | Anemia and long-term outcome in adjuvant and neoadjuvant radiochemotherapy of stage Ⅱ and Ⅲ rectal adenocarcinoma:The Freiburg experience(1989-2002)显示文摘AIM:To evaluate the long-term outcome of standard5-FU based adjuvant or neoadjuvant radiochemotherapyand to identify the predictive factors,especially anemiabefore and after radiotherapy as well as hemoglobinincrease or decrease during radiotherapy.METHODS:Two hundred and eighty-six patientswith Union International Contre Cancer(UICC)stageⅡ and Ⅲ rectal adenocarcinomas,who underwentresection by conventional surgical techniques(lowanterior or abdominoperineal resection),receivedeither postoperative(n=233)or preoperative(n=53)radiochemotherapy from January 1989 until July 2002.Overall survival(OAS),cancer-specific survival(CSS),disease-free survival(DFS),local-relapse-free(LRS)and distant-relapse-free survival(DRS)were evaluatedusing Kaplan-Meier,Log-rank test and Cox's proportionalhazards as statistical methods.Multivariate analysis wasused to identify prognostic factors.Median follow-uptime was 8 years.RESULTS:Anemia before radiochemotherapy was anindependent prognostic factor for improved DFS(riskratio 0.76,P=0.04)as well as stage,grading,R status(free radial margins),type of surgery,carcinoembryonicantigen(CEA)levels,and gender.The univariate analysisrevealed that anemia was associated with impaired LRS (better local control)but with improved DFS.In contrast,hemoglobin decrease during radiotherapy was anindependent risk factor for DFS(risk ratio 1.97,P=0.04).During radiotherapy,only 30.8% of RO-resected patientssuffered from hemoglobin decrease compared to 55.6%if R1/2 resection was performed(P=0.04).The 5-yearOAS,CSS,DFS,LRS and DRS were 47.0%,60.0%,41.4%,67.2%,and 84.3%,respectively.Significantdifferences between preoperative and postoperativeradiochemotherapy were not found.CONCLUSION:Anemia before radiochemotherapyand hemoglobin decrease during radiotherapy have nopredictive value for the outcome of rectal cancer.Stage,grading,R status(free radial margins),type of surgery,CEA levels,and gender have predictive value for theoutcome of rectal cancer. | Christian Weissenberger Geissler Michael Otto Florian Barke Annette Henne Karl von Plehn Georg Rein Alex Müller Christine Bartelt Susanne Henke Michael | 2006 | World Journal of Gastroenterology2006,12,12: | 4 |
| 3 | Poly(3-hydroxybutyrate-co-3-hydroxyhexanoate) supports adhesion and migration of mesenchymal stem cells and tenocytes显示文摘AIM: To establish the potential of poly(3-hydroxybutyrate-co-3-hydroxyhexanoate) (PHBHHx) as a material for tendon repair. METHODS: The biocompatibility of PHBHHx with both rat tenocytes (rT) and human mesenchymal stem cells (hMSC) was explored by monitoring adhesive characteristics on films of varying weight/volume ratios coupled to a culture atmosphere of either 21% O2 (air) or 2% O2 (physiological normoxia). The diameter and stiffness of PHBHHx films was established using optical coherence tomography and mechanical testing, respectively. RESULTS: Film thickness correlated directly with weight/volume PHBHHx (r2 = 0.9473) ranging from 0.1 mm (0.8% weight/volume) to 0.19 mm (2.4% weight/volume). Film stiffness on the other hand displayed a biphasic response which increased rapidly at values > 1.6% weight/volume. Optimal cell attachment of rT required films of ≥ 1.6% and ≥ 2.0% weight/volume PHBHHx in 2% O2 and 21% O2 respectively. A qualitative adhesion increase was noted for hMSC in films ≥ 1.2% weight/volume, becoming significant at 2% weight/volume in 2% O2. An increase in cell adhesion was also noted with ≥ 2% weight/volume PHBHHx in 21% O2. Cell migration into films was not observed. | 11 | Non-small cell lung carcinoma(NSCLC):Implications on molecular pathology and advances in early diagnostics and therapeutics显示文摘Continuous revision of the histologic and stage-wise classification of lung cancer by the World Health Organization (WHO) provides the foundation for therapeutic advances by promoting molecular targeted and immunotherapies and ensuring accurate diagnosis. Cancer epidemiologic data provide helpful information for cancer prevention, diagnosis, and management, supporting health-care interventions. Global cancer mortality projections from 2016 to 2060 show that cancer will overtake ischemic heart diseases (IHD) as the leading cause of death (18.9 million) immediately after 2030, surpassing non-small cell lung cancer (NSCLC), which accounts for 85 percent of lung cancers. The clinical stage at the diagnosis is the main prognostic factor in NSCLC therapies. Advanced early diagnostic methods are essential as the initial stages of cancer show reduced mortality compared to the advanced stages. Sophisticated approaches to proper histological classification and NSCLC management have improved clinical efficiency. Although immune checkpoint inhibitors (ICIs) and targeted molecular therapies have refined the therapeutic management of late-stage NSCLC, the specificity and sensitivity of cancer biomarkers should be improved by focusing on prospective studies, followed by their use as therapeutic tools.
