| 1 | Newer perspectives of coronary artery disease in young显示文摘Coronary artery disease(CAD) occurring in less than 45 years of age is termed as young CAD.Recent studies show a prevalence of 1.2% of CAD cases in this age group.Ethnic wise south Asians especially Indians are more vulnerable to have CAD in young age group with a prevalence of 5% to 10%.Conventional risk factors such as smoking,diabetes,hypertension,obesity and family history seems to be as important as in older CAD subjects.But the prevalence of these risk factors seems to vary in younger subjects.By far the most commonly associated risk factor is smoking in young CAD.Several genes associated with lipoprotein metabolism are now found to be associated with young CAD like cholesterol ester transfer protein(CETP) gene,hepatic lipase gene,lipoprotein lipase gene,apo A1 gene,apo E gene and apo B.Biomarkers such as lipoprotein(a),fibrinogen,D-dimer,serum Wnt,gamma glutamyl transferase,vitamin D2 and osteocalcin are seems to be associated with premature CAD in some newer studies.In general CAD in young has better prognosis than older subjects.In terms of prognosis two risk factors obesity and current smoking are associated with poorer outcomes.Angiographic studies shows predominance of single vessel disease in young CAD patients.Like CAD in older person primary and secondary prevention plays an important role in prevention of new and further coronary events. | Amitesh Aggarwal Saurabh Srivastava M Velmurugan | 2016 | World Journal of Cardiology2016,8,12: | 13 |
| 2 | Helicobacter pylori:Does it add to risk of coronary artery disease显示文摘Helicobacter pylori(H. pylori) is a known pathogen implicated in genesis of gastritis, peptic ulcer disease, gastric carcinoma and gastric lymphoma. Beyond the stomach, the organism has also been implicated in the causation of immune thrombocytopenia and iron deficiency anemia. Although an area of active clinical research, the role of this gram negative organism in causation of atherosclerosis and coronary artery disease(CAD) remains enigmatic. CAD is a multifactorial disease which results from the atherosclerosis involving coronaryarteries. The major risk factors include age, diabetes mellitus, smoking, hypertension and dyslipidemia. The risk of coronary artery disease is believed to increase with chronic inflammation. Various organisms like Chlamydia and Helicobacter have been suspected to have a role in genesis of atherosclerosis via causation of chronic inflammation. | 3 | Brain metastasis in advanced colorectal cancer: results from the South Australian metastatic colorectal cancer (SAmCRC) registry显示文摘Objective:Brain metastasis is considered rare in metastatic colorectal cancer(mCRC);thus,surveillance imaging does not routinely include the brain.The reported incidence of brain metastases ranges from 0.6% to 3.2%.Methods:The South Australian mCRC Registry(SAmCRC)was analyzed to assess the number of patients presenting with brain metastasis during their lifetime.Due to small numbers,a descriptive analysis is presented.Results:Only 59 patients of 4,100 on the registry at the time of analysis had developed brain metastasis(1.4%).The clinical characteristics of those with brain metastasis were as follows:the median age was 65.3 years and 51% were female.Where the V-Ki-ras2 Kirsten rat sarcoma viral oncogene homolog(KRAS)mutation status of the tumor was known,the majority harbored a KRAS mutation(55%);31(53%)underwent craniotomy and 55(93%)underwent whole-brain radiotherapy.The median survival time from diagnosis of brain metastasis was 4.2 months(95% confidence interval 2.9–5.5).Patients who underwent craniotomy and radiotherapy had superior survival compared to those who underwent whole-brain radiotherapy(8.5 months vs.2.2 months,respectively).Data from the SAmCRC(a population-based registry)confirm that brain metastases are rare and the median time to development is approximately 2 years.Conclusions:Brain metastasis is a rare outcome in advanced CRC.Patients within the registry tended to be female,young in age,and harbored with higher rates of KRAS mutations.Whether routine surveillance brain scanning should be considered remains controversial given the relative rarity of developing brain metastases in mCRC and ultimately,most patients with central nervous system involvement die from their extracranial disease. | Gonzalo Tapia Rico Timothy J. | 7 | Multi-criteria decision based geospatial mapping of flood susceptibility and temporal hydro-geomorphic changes in the Subarnarekha basin, India显示文摘The Subarnarekha River in east India experiences frequent high magnitude flooding in monsoon season.In this study,we present an in-depth analysis of flood hydrology and GIS-based flood susceptibility mapping of the entire catchment.About 40 years of annual peak discharge data,historical cross-sections of different gauging sites,and 12 flood conditioning factors were considered.Our flood susceptibility mapping followed an expert knowledge-based multi-parametric analytical hierarchy process(AHP)and optimized AHP-VIP methods.Peak hydrology data indicated more than 5 times higher discharge contrasted with the mean streamflow of the peak monsoon month in all hydro-monitoring stations that correspond to possible overbank flooding in the shallow semi-alluvial reaches of the Subarnarekha River.Widthdepth ratio revealed continuous changes on the channel cross-sections at decadal scale in all gauging sites.Predicted flood susceptibility map through optimized AHP-VIP method showed a great amount of areas(38%)have a high probability of flooding and demands earnest attention of administrative bodies.The AHP-VIP based flood susceptibility map was theoritically validated through AUC approach and it showed fairly high accuracy(AUC=0.93).Our study offers an exceptionally cost and time effective solution to the flooding issues in the Subarnarekha basin. | Sumit Das Amitesh Gupta | 2021 | Geoscience Frontiers2021,12,5: | 1 |

| 19 | Esophageal dilations in eosinophilic esophagitis: A single center experience显示文摘AIM:To diagnose the clinical and histologic features that may be associated with or predictive of the need for dilation and dilation related complications;examine the safety of dilation in patients with eosinophilic esophagitis(EoE).METHODS:The medical records of all patients diagnosed with EoE between January 2002 and July 2010were retrospectively reviewed.Esophageal biopsies were reexamined by an experienced pathologist to confirm the diagnosis(≥15 eos/hpf per current guidelines).Patients were divided into 2 groups:patients who did not receive dilation therapy and those who did.Demographics,clinical history,the use of pharmacologic therapy,endoscopic and pathology findings,and the number of biopsies and dilations carried out,if any,and their locations were recorded for each patient.The dilation group was further examined based on the interval between diagnosis and dilation,and whether or not a complication occurred.RESULTS:Sixty-one patients were identified with EoE and 22(36%)of them underwent esophageal dilations for stricture/narrowing.The peak eos/hpf was significantly higher in patients who received a dilation(P=0.04).Four(18%of pts.)minor complications occurred:deep mucosal tear 1,and small mucosal tears3.There were no cases of esophageal perforations.Higher peak eos/hpf counts were not associated with increased risk of complications.CONCLUSION:Esophageal dilation appears to be a safe procedure in EoE patients,carrying a low complication rate.No correlation was found between the peak of eosinophil count and complication rate.Complications can occur independently of the histologic features.The long-term outcome of EoE treatment,with or without dilation,needs to be determined. | Andrew Ukleja Jennifer Shiroky Amitesh Agarwal Daniela Allende | 2014 | World Journal of Gastroenterology2014,20,28: | 1 |
| 20 | Central obesity,hypertension and coronary artery disease: The seed and soil hypothesis显示文摘Coronary artery disease (CAD) is a multifactorial disease wherein hereditary and environmental factors play a major role. Our hypothesis is that an individual’s genetic profile functions as soil while various environmental factors such as physical inactivity, smoking, stress etc. act as seeds in the etiopathogenesis of CAD. Much of the information regarding genetic and environmental factors can be determined in a pedigree chart by taking a history of the index patient, including details of major risk factors such as age, sex, smoking, hypertension, diabetes, coronary artery disease and stroke in the family. Preparing such a chart is a cost-effective way of initiating primary preventive measures in patients in a developing economy. The advantage of a detailed pedigree chart is to provide a snapshot view of the evident and underlying risk factors in the family as a whole, and not to merely study conventional risk factors. It elucidates the hidden stressors and hereditary factors responsible for cardiovascular disease in the family.
