| 3 | Mediastinal node staging by positron emission tomographycomputed tomography and selective endoscopic ultrasound with fine needle aspiration for patients with upper gastrointestinal cancer:Results from a regional centre显示文摘AIM To investigate the impact of endoscopic ultrasound-guided fine-needle aspiration(EUS-FNA) and positron emission tomography-computed tomography(PET-CT) in the nodal staging of upper gastrointestinal(GI) cancer in a tertiary referral centre.METHODS We performed a retrospective review of prospectively recorded data held on all patients with a diagnosis of upper GI cancer made between January 2009 and December 2015. Only those patients who had both a PET-CT and EUS with FNA sampling of a mediastinal node distant from the primary tumour were included. Using a positive EUS-FNA result as the gold standard for lymph node involvement, the sensitivity, specificity, positive and negative predictive values(PPV and NPV) and accuracy of PET-CT in the staging of mediastinal lymph nodes were calculated. The impact on therapeutic strategy of adding EUS-FNA to PET-CT was assessed.RESULTS One hundred and twenty one patients were included. Sixty nine patients had a diagnosis of oesophageal adenocarcinoma(Thirty one of whom were junctional), forty eight had oesophageal squamous cell carcinoma and four had gastric adenocarcinoma. The FNA results were inadequate in eleven cases and the PET-CT findings were indeterminate in two cases, therefore thirteen patients(10.7%) were excluded from further analysis. There was concordance between PET-CT and EUS-FNA findings in seventy one of the remaining one hundred and eight patients(65.7%). The sensitivity, specificity, PPV and NPV values of PET-CT were 92.5%, 50%, 52.1% and 91.9% respectively. There was discordance between PET-CT and EUS-FNA findings in thirty seven out of one hundred and eight patients(34.3%). MDT discussion led to a radical treatment pathway in twenty seven of these cases, after the final tumour stage was altered as a direct consequence of the EUS-FNA findings. Of these patients, fourteen(51.9%) experienced clinical remission of a median of nine months(range three to forty two months). | 5 | Development of a 100 J,10 Hz laser for compression experiments at the High Energy Density instrument at the European XFEL显示文摘In this paper we review the design and development of a 100 J, 10 Hz nanosecond pulsed laser, codenamed DiPOLE100 X,being built at the Central Laser Facility(CLF). This 1 kW average power diode-pumped solid-state laser(DPSSL) is based on a master oscillator power amplifier(MOPA) design, which includes two cryogenic gas cooled amplifier stages based on DiPOLE multi-slab ceramic Yb:YAG amplifier technology developed at the CLF. The laser will produce pulses between 2 and 15 ns in duration with precise, arbitrarily selectable shapes, at pulse repetition rates up to 10 Hz, allowing real-time shape optimization for compression experiments. | 6 | Seasonal Forecasts of the Summer 2016 Yangtze River Basin Rainfall显示文摘The Yangtze River has been subject to heavy flooding throughout history,and in recent times severe floods such as those in 1998 have resulted in heavy loss of life and livelihoods.Dams along the river help to manage flood waters,and are important sources of electricity for the region.Being able to forecast high-impact events at long lead times therefore has enormous potential benefit.Recent improvements in seasonal forecasting mean that dynamical climate models can start to be used directly for operational services.The teleconnection from El Ni ?no to Yangtze River basin rainfall meant that the strong El Ni ?no in winter 2015/16 provided a valuable opportunity to test the application of a dynamical forecast system.This paper therefore presents a case study of a real-time seasonal forecast for the Yangtze River basin,building on previous work demonstrating the retrospective skill of such a forecast.A simple forecasting methodology is presented,in which the forecast probabilities are derived from the historical relationship between hindcast and observations.Its performance for2016 is discussed.The heavy rainfall in the May–June–July period was correctly forecast well in advance.August saw anomalously low rainfall,and the forecasts for the June–July–August period correctly showed closer to average levels.The forecasts contributed to the confidence of decision-makers across the Yangtze River basin.Trials of climate services such as this help to promote appropriate use of seasonal forecasts,and highlight areas for future improvements. | Philip E.
