| 1 | Further geological and palaeoanthropological investigations at the Maludong hominin site, Yunnan Province, Southwest China显示文摘Three-dimensional mapping and section work undertaken by us in 2008 have identified 11 stratigraphic units at Maludong site.AMS radiocarbon dating of charcoal established an accurate and internally consistent age profile for the sequence of 17.8±0.2 ka to 13.2±0.1 ka.Archaeomagnetic analysis showed changes in externally derived pedogenically enhanced material consistent with a warming in climate between the cold period of Henrich Event 1 and the B lling-Aller d interstadial.Human remains recovered during the 1989 excavation were derived from a deposit dating to this interstadial,or between 14.3±0.3 ka and 13.5±0.1 ka.Anthropogenic features,including burnt rocks,baked sediment and thick charcoal and ash layers,were identified and examined through archaeomagnetic analysis.Two monkey fossils are described here,one of them being reassigned from Macaca robustus to M.aff.M.assamensis.They confirm the young age of the site and also show signs of anthropogenic alteration in the form of burning.Additional human cranial remains are reported for the first time and new data are provided for some specimens described previously.A range of new features is identified that strengthen the affinities of the Maludong remains to archaic humans.The presence of this globally unique mosaic of archaic and modern features raises important questions about human evolutionary history in East Asia during the Late Upper Pleistocene. | JI XuePing CURNOE Darren BAO ZhenDe HERRIES Andy I R FINK David ZHU YunSheng HELLSTROM John LUO Yun TACON Paul S C | 2013 | Chinese Science Bulletin2013，58，35： | 3 |

| 9 | Effect of body mass index on functional outcome in primary total knee arthroplasty——a single institution analysis of 2180 primary total knee replacements显示文摘AIM To evaluate the effect of body mass index(BMI) on short-term functional outcome and complications in primary total knee arthroplasty. METHODS All patients undergoing primary total knee arthroplasty at a single institution between 2007 and 2013 were identified from a prospective arthroplasty database. 2180 patients were included in the study. Age, gender, BMI, pre- and post-operative functional scores [Western Ontario and Mc Master University Arthritis Index(WOMAC) and SF-36], complications and revision rate were recorded. Patients were grouped according to the WHO BMI classification. The functional outcome of the normal weight cohort(BMI < 25) was compared to the overweight and obese(BMI ≥ 25) cohort. A separate sub-group analysis was performed comparing all five WHO BMI groups; Normal weight, overweight, class 1 obese, class 2 obese and class 3 obese.RESULTS With a mean age of 67.89(28-92), 2180 primary total knee replacements were included. 64.36%(1403) were female. The mean BMI was 31.86(18-52). Ninty-three percent of patients were either overweight or obese. Mean follow-up 19.33 mo(6-60 mo). There was no significant difference in pre or post-operative WOMAC score in the normal weight(BMI < 25) cohort compared to patients with a BMI ≥ 25(P > 0.05). Sub-group analysis revealed significantly worse WOMAC scores in class 2 obese 30.80 compared to overweight 25.80(P < 0.01) and class 1 obese 25.50(P < 0.01). Similarly, there were significantly worse SF-36 scores in class 2 obese 58.16 compared to overweight 63.93(P < 0.01) and class 1 obese 63.65(P < 0.01) There were 32(1.47%) superficial infections, 9(0.41%) deep infections and 19(0.87%) revisions overall with no complications or revisions in the normal weight cohort(BMI < 25).CONCLUSION Post-operative functional outcome was not influenced by BMI comparing normal weight individuals with BMI > 25. | 10 | Outcomes following liver transplantation in intensive care unit patients显示文摘AIM:To determine feasibility of liver transplantation in patients from the intensive care unit (ICU) by estimating graft and patient survival.METHODS:This single center retrospective study included 39 patients who had their first liver transplant directly from the intensive care unit and 927 non-ICU patients who were transplanted from hospital ward or home between January 2005 and December 2010.RESULTS:In comparison to non-ICU patients,ICU patients had a higher model for end-stage liver disease (MELD) at transplant (median:37 vs 20,P < 0.001).Fourteen out of 39 patients (36%) required vasopressor support immediately prior to liver transplantation (LT) with 6 patients (15%) requiring both vasopressin and norepinephrine.Sixteen ICU patients (41%) were ventilator dependent immediately prior to LT with 9 patients undergoing percutaneous tracheostomy prior to transplantation.Twenty-five ICU patients (64%) required dialysis preoperatively.At 1,3 and 5 years after LT,graft survival was 76%,68% and 62% in ICU patients vs 90%,81% and 75% in non-ICU patients.Patient survival at 1,3 and 5 years after LT was 78%,70% and 65% in ICU patients vs 94%,85% and 79% in non-ICU patients.When formally comparing graft survival and patient survival between ICU and nonICU patients using Cox proportional hazards regression models,both graft survival [relative risk (RR):1.94,95%CI:1.09-3.48,P=0.026] and patient survival (RR:2.32,95%CI:1.26-4.27,P=0.007) were lower in ICU patients vs non-ICU patients in single variable analysis.These findings were consistent in multivariable analysis.Although not statistically significant,graft survival was worse in both patients with cryptogenic cirrhosis (RR:3.29,P=0.056) and patients who received donor after cardiac death (DCD) grafts (RR:3.38,P=0.060).These findings reached statistical significance when considering patient survival,which was worse for patients with cryptogenic cirrhosis (RR:3.97,P=0.031) and patients who were transplanted with DCD livers (RR:4.19,P=0.033).Graft survival and patient survival were not significantly worse for patients on mechanical ventilation (RR:0.91,P=0.88 in graft loss;RR:0.69,P=0.56 in death) or patients on vasopressors (RR:1.06,P=0.93 in graft loss;RR:1.24,P=0.74 in death) immediately prior to LT.Trends toward lower graft survival and patient survival were observed for patients on dialysis immediately before LT,however these findings did not approach statistical significance (RR:1.70,P=0.43 in graft loss;RR:1.46,P=0.58 in death).CONCLUSION:Although ICU patients when compared to non-ICU patients have lower survivals,outcomes are still acceptable.Pre-transplant ventilation,hemodialysis,and vasopressors were not associated with adverse outcomes. | Lena Sibulesky Michael G Heckman C Burcin Taner Juan M Canabal Nancy N Diehl Dana K Perry Darren L Willingham Surakit Pungpapong Barry G Rosser David J Kramer Justin H Nguyen | 2013 | World Journal of Hepatology2013，5，1： | 1 |
