1China Stroke Statistics 2019:A Report From the National Center for Healthcare Quality Management in Neurological Diseases,China National Clinical Research Center for Neurological Diseases,the Chinese Stroke Association,National Center for Chronic and Non-communicable Disease Control and Prevention,Chinese Center for Disease Control and Prevention and Institute for Global Neuroscience and Stroke Collaborations显示文摘China faces the greatest challenge from stroke in the world.The death rate for cerebrovascular diseases in China was 149.49 per 100000,accounting for 1.57 million deaths in 2018.It ranked third among the leading causes of death behind malignant tumours and heart disease.The age-standardised prevalence and incidence of stroke in 2013 were 1114.8 per 100000 population and 246.8 per 100000 person-years,respectively.According to the Global Burden of Disease Study 2017,the years of life lost(YLLs)per 100000 population for stroke increased by 14.6%;YLLs due to stroke rose from third highest among all causes in 1990 to the highest in 2017.The absolute numbers and rates per 100000 population for all-age disability-adjusted life years(DALYs)for stroke increased substantially between 1990 and 2017,and stroke was the leading cause of all-age DALYs in 2017.The main contributors to cerebrovascular diseases include behavioural risk factors(smoking and alcohol use)and pre-existing conditions(hypertension,diabetes mellitus,dyslipidaemia and atrial fibrillation(AF)).The most prevalent risk factors among stroke survivors were hypertension(63.0%-84.2%)and smoking(31.7%-47.6%).The least prevalent was AF(2.7%-7.4%).The prevalences for major risk factors for stroke are high and most have increased over time.Based on the latest national epidemiological data,26.6%of adults aged≥15 years(307.6 million adults)smoked tobacco products.For those aged≥18 years,age-adjusted prevalence of hypertension was 25.2%;adjusted prevalence of hypercholesterolaemia was 5.8%;and the standardised prevalence of diabetes was 10.9%.For those aged≥40 years,the standardised prevalence of AF was 2.31%.Data from the Hospital Quality Monitoring System showed that 3010204 inpatients with stroke were admitted to 1853 tertiary care hospitals during 2018.Of those,2466785(81.9%)were ischaemic strokes(ISs);447609(14.9%)were intracerebral haemorrhages(ICHs);and 95810(3.2%)were subarachnoid haemorrhages(SAHs).The average age of patients admitted was 66 years old,and nearly 60%were male.A total of 1555(0.1%),2774(0.6%)and 1347(1.4%)paediatric strokes(age<18 years)were identified among IS,ICH and SAH,respectively.Over one-third(1063892(35.3%))of the patients were covered by urban resident basic medical insurance,followed by urban employee basic medical insurance(699513(23.2%))and new rural cooperative medical schema(489361(16.3%)).The leading risk factor was hypertension(67.4%for IS,77.2%for ICH and 49.1%for SAH),and the leading comorbidity was pneumonia or pulmonary infection(10.1%for IS,31.4%for ICH and 25.2%for SAH).In-hospital death/discharge against medical advice rate was 8.3%for stroke inpatients,ranging from 5.8%for IS to 19.5%for ICH.The median and IQR of length of stay was 10.0(7.0-14.0)days,ranging from 10.0(7.0-13.0)in IS to 14.0(8.0-22.0)in SAH.Data from the Chinese Stroke Center Alliance demonstrated that the composite scores of guideline-recommended key performance indicators for patients with IS,ICH and SAH were 0.77±0.21,0.72±0.28 and 0.59±0.32,respectively.Yong-Jun Wang Zi-Xiao Li Hong-Qiu Gu Yi Zhai Yong Jiang Xing-Quan Zhao Yi-Long Wang Xin Yang Chun-Juan Wang Xia Meng Hao Li Li-Ping Liu Jing Jing Jing Wu An-Ding Xu Qiang Dong David Wang Ji-Zong Zhao On behalf of China Stroke Statistics 2019 Writing Committee 2020Stroke & Vascular Neurology2020,5,3:181
2On the tuning of electrical and thermal transport in thermoelectrics: an integrated theory–experiment perspective显示文摘During the last two decades,we have witnessed great progress in research on thermoelectrics.There are two primary focuses.One is the fundamental understanding of electrical and thermal transport,enabled by the interplay of theory and experiment;the other is the substantial enhancement of the performance of various thermoelectric materials,through synergistic optimisation of those intercorrelated transport parameters.Here we review some of the successful strategies for tuning electrical and thermal transport.For electrical transport,we start from the classical but still very active strategy of tuning band degeneracy(or band convergence),then discuss the engineering of carrier scattering,and finally address the concept of conduction channels and conductive networks that emerge in complex thermoelectric materials.For thermal transport,we summarise the approaches for studying thermal transport based on phonon–phonon interactions valid for conventional solids,as well as some quantitative efforts for nanostructures.We also discuss the thermal transport in complex materials with chemical-bond hierarchy,in which a portion of the atoms(or subunits)are weakly bonded to the rest of the structure,leading to an intrinsic manifestation of part-crystalline part-liquid state at elevated temperatures.In this review,we provide a summary of achievements made in recent studies of thermoelectric transport properties,and demonstrate how they have led to improvements in thermoelectric performance by the integration of modern theory and experiment,and point out some challenges and possible directions.Jiong Yang Lili Xi Wujie Qiu Lihua Wu Xun Shi Lidong Chen Jihui Yang Wenqing Zhang Ctirad Uher David J Singh 2016npj Computational Materials2016,,1:24

6Efficacy of adjuvant XELOX and FOLFOX6 chemotherapy after D2 dissection for gastric cancer显示文摘AIM: To compare the efficacy of capecitabine and oxaliplatin (XELOX) with 5-fluorouracil, folinic acid and oxaliplatin (FOLFOX6) in gastric cancer patients after D2 dissection. METHODS: Between May 2004 and June 2010, patients in our gastric cancer database who underwent D2 dissection for gastric cancer at the First Affiliated Hospital of Sun Yat-Sen University were retrospectively analyzed. A total of 896 patients were enrolled into this study according to the established inclusion and exclusion criteria. Of these patients, 214 received the XELOX regimen, 48 received FOLFOX6 therapy and 634 patients underwent surgery only without chemotherapy. Overall survival was compared among the three groups using Cox regression and propensity score matchedpair analyses. RESULTS: Patients in the XELOX and FOLFOX6 groups were younger at the time of treatment (median age 55.2 years; 51.2 years vs 58.9 years), had more undifferentiated tumors (70.1%; 70.8% vs 61.4%), and more lymph node metastases (80.8%; 83.3% vs 57.7%), respectively. Overall 5-year survival was 57.3% in the XELOX group which was higher than that (47.5%) in the surgery only group (P = 0.062) and that (34.5%) in the FOLFOX6 group (P = 0.022). Multivariate analysis showed that XELOX therapy was an independent prognostic factor (hazard ratio = 0.564, P < 0.001). After propensity score adjustment, XELOX significantly increased overall 5-year survival compared to surgery only (58.2% vs 44.2%, P = 0.025) but not compared to FOLFOX6 therapy (48.5% vs 42.7%, P = 0.685). The incidence of grade 3/4 adverse reactions was similar between the XELOX and FOLFOX6 groups, and more patients suffered from hand-foot syndrome in the XELOX group (P = 0.018). 7The interaction of versican with its binding partners显示文摘Versican belongs to the family of the large aggregating chondroitin sulfate proteoglycans located primarily within theextracellular matrix (ECM). Versican, like other members of its family, has unique N- and C-terminal globular regions,each with multiple motifs. A large glycosaminoglycan-binding region lies between them. This review will begin byoutlining these structures, in the context of ECM proteoglycans. The diverse binding partners afforded to versican byvirtue of its modular design will then be examined. These include ECM components, such as hyaluronan, type Icollagen, tenascin-R, fibulin-1, and -2, fibrillin-1, fibronectin, P- and L-selectins, and chemokines. 8Management of granulomatous lobular mastitis: an international multidisciplinary consensus(2021 edition)显示文摘Granulomatous lobular mastitis(GLM) is a rare and chronic benign inflammatory disease of the breast. Difficulties exist in the management of GLM for many front-line surgeons and medical specialists who care for patients with inflammatory disorders of the breast. This consensus is summarized to establish evidence-based recommendations for the management of GLM. Literature was reviewed using PubMed from January 1, 1971 to July 31, 2020. Sixty-six international experienced multidisciplinary experts from 11 countries or regions were invited to review the evidence.Levels of evidence were determined using the American College of Physicians grading system, and recommendations were discussed until consensus. Experts discussed and concluded 30 recommendations on historical definitions,etiology and predisposing factors, diagnosis criteria, treatment, clinical stages, relapse and recurrence of GLM. GLM was recommended as a widely accepted definition. In addition, this consensus introduced a new clinical stages and management algorithm for GLM to provide individual treatment strategies. In conclusion, diagnosis of GLM depends on a combination of history, clinical manifestations, imaging examinations, laboratory examinations and pathology.The approach to treatment of GLM should be applied according to the different clinical stage of GLM. 9Black Locust Transpiration Responses to Soil Water Availability as Affected by Meteorological Factors and Soil Texture显示文摘On the Loess Plateau of China, a dry soil layer may form due to excess transpiration, leading to degradation of black locust(Robinia pseudoacacia) stands. In order to better manage projects involving black locust, this study was intended to investigate the response of black locust transpiration rate to soil water availability as affected by meteorological factors using two representative soils(loamy clay and sandy loam) on the Loess Plateau. Four soil water contents were maintained for black locust seedlings grown in pots initially outdoors and then in a climate-controlled chamber, by either drying or irrigating the pots. In both environments, daily transpiration rates were related by a power function to air temperature and by a logistic function to reference evapotranspiration(ET0). Transpiration rates were more susceptible to changes in the meteorological conditions in the sandy loam than in the loamy clay soil. The transpiration rate in the well-watered treatment was greater for black locust grown in the sandy loam than in the loamy clay soil. Normalized transpiration rates were unaffected by ET0 until a critical value of soil water content(θc) was attained; the θc value decreased significantly for the loamy clay soil but increased significantly for the sandy loam soil when ET0 increased. 10Submarine Landslides on the North Continental Slope of the South China Sea显示文摘Recent and paleo-submarine landslides are widely distributed within strata in deep-water areas along continental slopes, uplifts, and carbonate platforms on the north continental margin of the South China Sea(SCS). In this paper, high-resolution 3D seismic data and multibeam data based on seismic sedimentology and geomorphology are employed to assist in identifying submarine landslides. In addition, deposition models are proposed that are based on specific geological structures and features, and which illustrate the local stress field over entire submarine landslides in deep-water areas of the SCS. The SCS is one of the largest fluvial sediment sinks in enclosed or semi-enclosed marginal seas worldwide. It therefore provides a set of preconditions for the formation of submarine landslides, including rapid sediment accumulation, formation of gas hydrates, and fluid overpressure. A new concept involving temporal and spatial analyses is tested to construct a relationship between submarine landslides and different time scale trigger mechanisms, and three mechanisms are discussed in the context of spatial scale and temporal frequency: evolution of slope gradient and overpressure, global environmental changes, and tectonic events. Submarine landslides that are triggered by tectonic events are the largest but occur less frequently, while submarine landslides triggered by the combination of slope gradient and overpressure evolution are the smallest but most frequently occurring events. 11A large outbreak of acute gastroenteritis caused by the human norovirus GII.17 strain at a university in Henan Province,China显示文摘Background:Human noroviruses are a major cause of viral gastroenteritis and are the main etiological agents of acute gastroenteritis outbreaks.An increasing number of outbreaks and sporadic cases of norovirus have been reported in China in recent years.There was a large acute gastroenteritis outbreak at a university in Henan Province,China in the past five years.We want to identify the source,transmission routes of the outbreak by epidemiological investigation and laboratory testing in order to provide the effective control measures.Methods:The clinical cases were investigated,and analysed by descriptive epidemiological methods according to factors such as time,department,grade and so on.Samples were collected from clinical cases,healthy persons,the environment,water,and food at the university.These samples were tested for potential bacteria and viruses.The samples that tested positive for norovirus were selected for whole genome sequencing and the sequences were then analysed.Results:From 4 March to 3 April 2015,a total of 753 acute diarrhoea cases were reported at the university;the attack rate was 3.29%.The epidemic curve showed two peaks,with the main peak occurring between 10 and 20 March,accounting for 85.26%of reported cases.The rates of norovirus detection in samples from confirmed cases,people without symptoms,and environmental samples were 32.72%,17.39%,and 9.17%,respectively.The phylogenetic analysis showed that the norovirus belonged to the genotype GII.17.Conclusions:This is the largest and most severe outbreak caused by genotype GII.17 norovirus in recent years in China.The GII.17 viruses displayed high epidemic activity and have become a dominant strain in China since the winter of 2014,having replaced the previously dominant GII.4 Sydney 2012 strain.Xue-Yong Huang Jia Su Qian-Chao Lu Shi-Zheng Li Jia-Yong Zhao Meng-Lei Li Yi Li Xiao-Jing Shen Bai-Fan Zhang Hai-Feng Wang Yu-Jiao Mu Shu-Yu Wu Yan-Hua Du Li-Cheng Liu Wei-Jun Chen John David Klena Bian-Li Xu 2017Infectious Diseases of Poverty2017,6,1:10

13The policy-driven peak and reduction of China's carbon emissions显示文摘Pursuant to the Paris Agreement,China committed itself to peak its carbon emissions by around 2030 and to increase the non-fossil share ofprimary energy to 20%at the same time.The government has supported the international agreement by setting and strengthening the domesticpolicy targets for an earlier peak and faster reduction,aiming to contain the average global temperature increase to well below 2℃.We developa Kaya Inequality method to assess the time of peak and pace of reduction of China's energy-related CO2emissions based on the national energypolicy targets for 2030.We find that,despite the minor fluctuations,the current plateau essentially represents the peak emissions and should entera phase of steady decline by around 2025,given current trends in energy consumption and decarbonization.Such developments would beconsistent with the strengthened national policy target to achieve 50%of renewable power generation by 2030.However,the basic policy targetsea 20%share of non-fossil energy and 6 Gtce in total energy consumption by 2030ewould be insufficient to peak carbon emissions by around 2030.The synergy and interplay between domestic policy target setting and international climate commitments shed light on the need to elevatenational climate ambitions under the Paris Agreement and beyond.QI Ye Nicholas STERN HE Jian-Kun LU Jia-Qi LIU Tian-Le David KING WU Tong 2020Advances in Climate Change Research2020,11,2:10

15Inactivation of SACE_3446, a TetR family transcriptional regulator, stimulates erythromycin production in Saccharopolyspora erythraea显示文摘Erythromycin A is a widely used antibiotic produced by Saccharopolyspora erythraea;however,its biosynthetic cluster lacks a regulatory gene,limiting the yield enhancement via regulation engineering of S.erythraea.Herein,six TetR family transcriptional regulators(TFRs)belonging to three genomic context types were individually inactivated in S.erythraea A226,and one of them,SACE_3446,was proved to play a negative role in regulating erythromycin biosynthesis.EMSA and qRT-PCR analysis revealed that SACE_3446 covering intact N-terminal DNA binding domain specifically bound to the promoter regions of erythromycin biosynthetic gene eryAI,the resistant gene ermE and the adjacent gene SACE_3447(encoding a longchain fatty-acid CoA ligase),and repressed their transcription.Furthermore,we explored the interaction relationships of SACE_3446 and previously identified TFRs(SACE_3986 and SACE_7301)associated with erythromycin production.Given demonstrated relatively independent regulation mode of SACE_3446 and SACE_3986 in erythromycin biosynthesis,we individually and concomitantly inactivated them in an industrial S.erythraea WB.Compared with WB,the WBΔ3446 and WBΔ3446Δ3986 mutants respectively displayed 36%and 65%yield enhancement of erythromycin A,following significantly elevated transcription of eryAI and ermE.When cultured in a 5 L fermentor,erythromycin A ofWBΔ3446 and WBΔ3446Δ3986 successively reached 4095 mg/L and 4670 mg/L with 23%and 41%production improvement relative to WB.The strategy reported here will be useful to improve antibiotics production in other industrial actinomycete.Hang Wu Yansheng Wang Li Yuan Yongrong Mao Weiwei Wang Lin Zhu Panpan Wu Chengzhang Fu Rolf Muller David T.Weaver Lixin Zhang Buchang Zhang 2016Synthetic and Systems Biotechnology2016,1,1:7
16The origin, diversification and adaptation of a major mangrove clade (Rhizophoreae) revealed by whole-genome sequencing显示文摘Mangroves invade some very marginal habitats for woody plants—at the interface between land and sea.Since mangroves anchor tropical coastal communities globally, their origin, diversification and adaptation are of scientific significance, particularly at a time of global climate change. In this study, a combination of single-molecule long reads and the more conventional short reads are generated from Rhizophora apiculata for the de novo assembly of its genome to a near chromosome level. The longest scaffold, N50 and N90 for the R. apiculata genome, are 13.3 Mb, 5.4 Mb and 1.0 Mb, respectively. Short reads for the genomes and transcriptomes of eight related species are also generated. We find that the ancestor of Rhizophoreae experienced a whole-genome duplication ~70 Myrs ago, which is followed rather quickly by colonization and species diversification. Mangroves exhibit pan-exome modifications of amino acid(AA) usage as well as unusual AA substitutions among closely related species. The usage and substitution of AAs, unique among plants surveyed, is correlated with the rapid evolution of proteins in mangroves. A small subset of these substitutions is associated with mangroves' highly specialized traits(vivipary and red bark) thought to be adaptive in the intertidal habitats. Despite the many adaptive features, mangroves are among the least genetically diverse plants, likely the result of continual habitat turnovers caused by repeated rises and falls of sea level in the geologically recent past. 19Metal–Organic Framework-Assisted Synthesis of Compact Fe_2O_3 Nanotubes in Co_3O_4 Host with Enhanced Lithium Storage Properties显示文摘Transition metal oxides are promising candidates for the high-capacity anode material in lithium-ion batteries.The electrochemical performance of transition metal oxides can be improved by constructing suitable composite architectures. Herein, we demonstrate a metal–organic framework(MOF)-assisted strategy for the synthesis of a hierarchical hybrid nanostructure composed of Fe_2O_3 nanotubes assembled in Co_3O_4 host. Starting from MOF composite precursors(Fe-based MOF encapsulated in a Cobased host matrix), a complex structure of Co_3O_4 host and engulfed Fe_2O_3 nanotubes was prepared by a simple annealing treatment in air.
