1Helicobacter pylori eradication therapy for functional dyspepsia: Systematic review and meta-analysis显示文摘AIM: To evaluate whether Helicobacter pylori(H. pylori) eradication therapy benefits patients with functional dyspepsia(FD).METHODS: Randomized controlled trials(RCTs) investigating the efficacy and safety of H. pylori eradication therapy for patients with functional dyspepsia published in English(up to May 2015) were identified by searching Pub Med, EMBASE, and The Cochrane Library. Pooled estimates were measured using the fixed or random effect model. Overall effect was expressed as a pooled risk ratio(RR) or a standard mean difference(SMD). All data were analyzed with Review Manager 5.3 and Stata 12.0.RESULTS: This systematic review included 25 RCTs with a total of 5555 patients with FD. Twenty-three of these studies were used to evaluate the benefits of H. pylori eradication therapy for symptom improvement; the pooled RR was 1.23(95%CI: 1.12-1.36, P < 0.0001). H. pylori eradication therapy demonstrated symptom improvement during long-term follow-up at ≥ 1 year(RR = 1.24; 95%CI: 1.12-1.37, P < 0.0001) but not during short-term follow-up at < 1 year(RR = 1.26; 95%CI: 0.83-1.92, P = 0.27). Seven studies showed no benefit of H. pylori eradication therapy on quality of life with an SMD of-0.01(95%CI:-0.11 to 0.08, P = 0.80). Six studies demonstrated that H. pylori eradication therapy reduced the development of peptic ulcer disease compared to no eradication therapy(RR = 0.35; 95%CI: 0.18-0.68, P = 0.002). Eight studies showed that H. pylori eradication therapy increased the likelihood of treatment-related side effects compared to no eradication therapy(RR = 2.02; 95%CI: 1.12-3.65, P = 0.02). 2Outcomes of furazolidone-and amoxicillin-based quadruple therapy for Helicobacter pylori infection and predictors of failed eradication显示文摘AIM To evaluate the outcomes of furazolidone-and amoxicillin-based quadruple therapy for treatment of Helicobacter pylori(H. pylori) infection and identify predictors of failed eradication.METHODS Patients with H. pylori infection treated with furazolidone, amoxicillin, bismuth, and proton pump inhibitor therapy(January 2015 to December 2015) who received the ^(13)C-urea breath test > 4 wk after treatment were evaluated. Demographic and clinical data including prior H. pylori treatment attempts, medication adherence, alcohol and cigarette consumption during therapy, and treatment-related adverse events were recorded by reviewing medical records and telephone surveys. H. pylori eradication rates for overall and subgroups were evaluated. Multivariate analysis was performed to identify independent predictors of failed H. pylori eradication.RESULTS Of the 992 patients treated and retested for H. pylori infection, the overall eradication rate was 94.5% [95% confidence interval(CI): 94.1%-95.9%]. H. pylori eradication rate of primary therapy was 95.0%(95%CI: 93.5%-96.5%), while that of rescue therapy was 91.3%(95%CI: 86.8%-95.8%). Among the 859 patients who completed the study protocol, 144(17%) reported treatment-related adverse events including 24(3%) leading to premature discontinuation. On multivariate analysis, poor medication adherence [adjusted odds ratio(AOR) = 6.7, 95%CI: 2.8-15.8], two or more previous H. pylori treatments(AOR = 7.4, 95%CI: 2.2-24.9), alcohol consumption during therapy(AOR = 4.4, 95%CI: 1.5-12.3), and possibly smoking during therapy(AOR = 1.9, 95%CI: 0.9-4.3) were associated with failed H. pylori eradication. CONCLUSION Furazolidone-and amoxicillin-based quadruple therapy for H. pylori infection in an area with a high prevalence of clarithromycin resistance demonstrated high eradication rates as primary and rescue therapies with a favorable safety profile. 4Paleoenvironmental records from the northern South China Sea since the Last Glacial Maximum显示文摘Core ZHS-176 contains the paleoenvironmental records from the northern South China Sea (NSCS) since the Last Glacial Maximum (LGM). A coupled approach based on clay mineral assemblages, planktonic foraminiferal oxygen and carbon isotopes, and calcium carbonate content is used to trace the sources of the fine-grained sediment and to investigate the paleoenviornmental evolution in this area. Clay mineral assemblages are dominated by illite (average about 39%) and chlorite (about 27%), which comes mainly from Taiwan and the East China Sea. Kaolinite, which accounts for about 13%, comes mainly from the Zhujiang (Pearl) River, and Luzon Island is the main source for smectite (about 21%). The planktonic foraminiferal oxygen isotopic oscillations during the last glacial period are coeval with climate variations recorded in the Greenland ice core and Western Pacific sediment. These variations include the LGM, Heinrich event 1, Bφlling-Allerφd (B/A), and Younger Dryas. For the Holocene, three periods of strong precipitation (S1-S3) and three periods of weak precipitation (W1-W3) are identified. The oxygen isotopic record exhibits corre-lation with climate records from distant regions, including the high-latitude Northern Hemisphere, providing evidence for global tele-connection among regional climate. A brief, negative planktonic foraminiferal carbon isotopic excursion during B/A reflects increased methane released from marine gas hydrate due to the rapid warming of the water. 6The enhanced X-ray Timing and Polarimetry mission—eXTP显示文摘In this paper we present the enhanced X-ray Timing and Polarimetry mission—eXTP. eXTP is a space science mission designed to study fundamental physics under extreme conditions of density, gravity and magnetism. The mission aims at determining the equation of state of matter at supra-nuclear density, measuring effects of QED, and understanding the dynamics of matter in strong-field gravity. In addition to investigating fundamental physics, eXTP will be a very powerful observatory for astrophysics that will provide observations of unprecedented quality on a variety of galactic and extragalactic objects. 9Report on Childhood Obesity in China(8):Effects and Sustainability of Physical Activity Intervention on Body Composition of Chinese Youth显示文摘Objectives To determine whether a large-scale physical activity intervention could affect body composition in primary school students in Beijing,China.Methods The study design was one-year cluster randomized controlled trial of physical activity intervention(20 min of daily exercise in the classroom) with an additional year of follow-up among 4 700 students aged 8-11 years at baseline.Results After the one-year intervention,BMI increased by 0.56 kg/m^2(SD 1.15) in the intervention group and by 0.72 kg/m^2(SD 1.20) in the control group,with a mean difference of-0.15 kg/m^2(95%CI:-0.28 to-0.02).BMI z score decreased by-0.05(SD 0.44) in the intervention group,but increased by 0.01(SD 0.46) in the control group,with a mean difference of-0.07(-0.13 to-0.01).After another year of follow up,compared to the control group,children in the intervention group had significantly lower BMI(-0.13,-0.25 to-0.01),BMI z score(-0.05,-0.10 to-0.01),fat mass(-0.27 kg,-0.53 to-0.02) and percent body fat(-0.53,-1.00 to-0.05).The intervention had a more pronounced effect on weight,height,BMI,BMI z score,and body composition among obese children than among normal weight or overweight children.Compared to the control group,the intervention group had a significantly higher percentage of children who maintained or reduced their BMI z score at year 1(P=0.008) and year 2(P=0.04).Conclusions These findings suggest that 20 min of daily moderate to vigorous physical activity during the school year is a feasible and effective way to prevent excessive gain of body weight,BMI,and body fatness in primary school students.YAN-PING LI XIAO=QI HU EVERT G. 11Fructo-oligosaccharide intensifies visceral hypersensitivity and intestinal inflammation in a stress-induced irritable bowel syndrome mouse model显示文摘AIM To determine whether fructo-oligosaccharide(FOS) affects visceral sensitivity, inflammation, and production of intestinal short-chain fatty acids(SCFA) in an irritable bowel syndrome(IBS) mouse model.METHODS Mice were randomly assigned to daily oral gavage of saline solution with or without FOS(8 g/kg body weight) for 14 d. Mice were further assigned to receive either daily one-hour water avoidance stress(WAS) or sham-WAS for the first 10 d. After 2 wk, visceral sensitivity was measured by abdominal withdrawal reflex in response to colorectal distension and mucosal inflammation was evaluated. Gas chromatography, real-time reverse transcription PCR, and immunohistochemistry assays were used to quantify cecal concentrations of SCFA, intestinal cytokine expression, and number of intestinal mast cells per high-power field(HPF), respectively.RESULTS Mice subjected to WAS exhibited visceral hypersensitivity and low-grade inflammation. Among mice subjected to WAS, FOS increased visceral hypersensitivity and led to higher cecal concentrations of acetic acid(2.49 ± 0.63 mmol/L vs 1.49 ± 0.72 mmol/L, P < 0.05), propionic acid(0.48 ± 0.09 mmol/L vs 0.36 ± 0.05 mmol/L, P < 0.01), butyric acid(0.28 ± 0.09 mmol/L vs 0.19 ± 0.003 mmol/L, P < 0.05), as well as total SCFA(3.62 ± 0.87 mmol/L vs 2.27 ± 0.75 mmol/L, P < 0.01) compared to saline administration. FOS also increased ileal interleukin(IL)-23 mR NA(4.71 ± 4.16 vs 1.00 ± 0.99, P < 0.05) and colonic IL-1β mR NA(2.15 ± 1.68 vs 0.88 ± 0.53, P < 0.05) expressions as well as increased mean mast cell counts in the ileum(12.3 ± 2.6 per HPF vs 8.3 ± 3.6 per HPF, P < 0.05) and colon(6.3 ± 3.2 per HPF vs 3.4 ± 1.2 per HPF, P < 0.05) compared to saline administration in mice subjected to WAS. 12MicroRNAs as emerging biomarkers and therapeutic targets for pancreatic cancer显示文摘Despite tremendous efforts from scientists and clinicians worldwide, pancreatic adenocarcinoma(PDAC) remains a deadly disease due to the lack of early diagnostic tools and reliable therapeutic approaches. Consequently, a majority of patients(80%) display an advanced disease that results in a low resection rate leading to an overall median survival of less than 6 months. Accordingly, robust markers for the early diagnosis and prognosis of pancreatic cancer, or markers indicative of survival and/or metastatic disease are des-perately needed to help alleviate the dismal prognosis of this cancer. In addition, the discovery of new therapeutic targets is mandatory to design effective treatments. 16Iron in the NEEM ice core relative to Asian loess records over the last glacial–interglacial cycle显示文摘Mineral dust can indirectly affect the climate by supplying bioavailable iron(Fe)to the ocean.Here,we present the records of dissolved Fe(DFe)and total Fe(TDFe)in North Greenland Eemian Ice Drilling(NEEM)ice core over the past 110 kyr BP.The Fe records are significantly negatively correlated with the carbon-dioxide(CO_(2))concentrations during cold periods.The results suggest that the changes in Fe fluxes over the past 110 kyr BP in the NEEM ice core are consistent with those in Chinese loess records because the mineral-dust distribution is controlled by the East Asian deserts.Furthermore,the variations in the dust input on a global scale are most likely driven by changes in solar radiation during the last glacial–interglacial cycle in response to Earth’s orbital cycles.In the last glacial–interglacial cycle,the DFe/TDFe ratios were higher during the warm periods(following the post-Industrial Revolution and during the Holocene and last interglacial period)than during the main cold period(i.e.the last glacial maximum(LGM)),indicating that the aeolian input of iron and the iron fertilization effect on the oceans have a non-linear relationship during different periods.Although the burning of biomass aerosols has released large amounts of DFe since the Industrial Revolution,no significant responses are observed in the DFe and TDFe variations during this period,indicating that severe anthropogenic contamination has no significant effect on the DFe(TDFe)release in the NEEM ice core.Cunde Xiao Zhiheng Du Mike J Handley Paul A Mayewski Junji Cao Simon Schüpbach Tong Zhang Jean-Robert Petit Chuanjin Li Yeongcheol Han Yuefang Li Jiawen Ren 2021National Science Review2021,8,7:2
