| 2 | Proton nuclear magnetic resonance-based metabonomic models for non-invasive diagnosis of liver fibrosis in chronic hepatitis C:Optimizing the classification of intermediate fibrosis显示文摘AIM To develop metabonomic models(MMs), using 1 H nuclear magnetic resonance(NMR) spectra of serum, to predict significant liver fibrosis(SF: Metavir ≥ F2), advanced liver fibrosis(AF: METAVIR ≥ F3) and cirrhosis(C: METAVIR = F4 or clinical cirrhosis) in chronic hepatitis C(CHC) patients. Additionally, to compare the accuracy of the MMs with the aspartate aminotransferase to platelet ratio index(APRI) and fibrosis index based on four factors(FIB-4). METHODS Sixty-nine patients who had undergone biopsy in the previous 12 mo or had clinical cirrhosis were included. The presence of any other liver disease was a criterion for exclusion. The MMs, constructed using partial least squares discriminant analysis and linear discriminant analysis formalisms, were tested by cross-validation, considering SF, AF and C. RESULTS Results showed that forty-two patients(61%) presented SF, 28(40%) AF and 18(26%) C. The MMs showed sensitivity and specificity of 97.6% and 92.6% to predict SF; 96.4% and 95.1% to predict AF; and 100% and 98.0% to predict C. Besides that, the MMs correctly classified all 27(39.7%) and 25(38.8%) patients with intermediate values of APRI and FIB-4, respectively. | 3 | Initial floristic composition of rehabilitated gullies through bioengineering in the Mixteca Region, Sierra Madre del Sur, Mexico显示文摘Re-vegetation plays a fundamental role for erosion control and plant recovery in lands affected by gully erosion. Bioengineered practices facilitate the gullies rehabilitation. Objectives of the research were: 1) Identify taxonomically the pioneer vegetation on each gully section; 2) Characterize vegetation distribution preferences and 3) Assess structural/functional traits to recognize erosion control key species. Bioengineering was applied in a watershed belonging to Sierra Madre del Sur, at Oaxaca, Mexico, on eight gullies, with local support and minimal investment. 'La Mixteca' is a poor ecological and socio-economic region, comparable to other regions of the world. The Initial Floristic Composition(IFC) inventory is the baseline of the successional process. The transect method was used to determine the colonization of species. Cover abundance of registered species was estimated using the semi-quantitative scale of Braun-Blanquet. This procedure was repeated in five different positions(floor, hillslopes and tops), in the cross section of the gully. Throughcorrespondence analysis and clustering, the distribution of species was analyzed. Adequate responses were obtained in soil retention(quantity) and plant cover(existence and diversity); as measurable indicators of the bioengeneering works efficiency. Occupation of soil by native species from the Tropical Deciduous Forest was favored using live barriers. We detected species guilds with spatial distribution preferences in the gullies cross section. Plant cover characterization includes: native colonizer species, herbaceous, shrubby and trees of the forest community bordering the gully area, with cover abundance and structural/functional traits, useful to protect degraded areas. This spatial occupation process of plants responds to a secondary succession in gullies, where the proposed IFC model is correctly represented through bioengineering. | 4 | Convergent Agency:Encouraging Transdisciplinary Approaches for Effective Climate Change Adaptation and Disaster Risk Reduction显示文摘Three recent global agreements have been established to facilitate the implementation of global-level responsibilities to deal with disaster risk reduction(DRR),human development,and climate change adaptation(CCA)respectively.While these agreements have a common goal of reducing social,economic,and environmental vulnerability,they have been developed by largely independent communities of practice.This has limited cross-fertilization despite the inherent multidimensional nature of global challenges and the considerable thematic overlap.We argue that developing a transdisciplinary strategy that effectively integrates disciplines,approaches,and knowledge systems will lead to greater and more sustainable impacts,together with a more efficient use of financial resources.Hybrid approaches should be encouraged during planning of future development efforts so that risk reduction is conducted simultaneously with CCA.Transdisciplinary processes are central to generating contextsensitive knowledge to support decisions on CCA and DRR options that minimize trade-offs and maximize synergies and complementarities required to guide sustainable development trajectories.Finally,building codes together with climate and risk-smart research,education,and awareness raising,are identified as priority entry points to materialize the blending of DRR and CCA approaches and effectively reduce risk while mitigating and adapting to climate change. | América Bendito Edmundo Barrios | 2016 | International Journal of Disaster Risk Science2016,7,4: | 2 |
| 5 | Trans-brachial artery access for coronary artery procedures is feasible and safe: data from a single-center in Macao显示文摘Compared with trans-femoral artery access (TFA), trans-radial artery access (TRA) for percutaneous coronary interventions (PCI) has been shown to significantly reduce 30-day mortality, in-hospital major adverse cardiac and cardiovascular events (a composite of 30-day mortality and in-hospital myocardial re-infarction, target vessel revascularization, and cerebrovascular events), major bleeding, and access site complications.[1] TRA is more easily compressible, minimizing hematoma risk, and lower crossover rates to another access. Patients may ambulate immediately after procedure, that hospitalization can be shortened significantly. In some centers, patient with TRA can be discharged on the same day of procedure. | 8 | The aging burden of hospitalization for heart failure in Chinese populations:evidence from the Macao Heart Failure Study显示文摘Objective To assess the aging burden of hospitalization for heart failure in Chinese populations in Macao.Methods The Macao Heart Failure Study consists of patients hospitalized with a diagnosis of acute heart failure(AHF)at Centro Hospitalar Conde de São Januário(the only public hospital that provides medical care for the approximately 600,000 residents of Macao)from January 2014 to December 2016.First,we investigated the relationship between socioeconomic development and epidemiological characteristics of HF in Macao.Then we assessed the patients’clinical features and outcomes according to the age groups.Results A total of 967 patients were included in the final analysis.The median age at admission was 82 years old.The advanced age at the admission of HF in Macao was significantly associated with a high-income level and the aging population structure.Marked heterogeneity existed in the epidemiological characteristics,clinical features,utilization of evidence-based therapies,short-and long-term outcomes,and prognostic utility of clinical variables among the different age groups.Conclusion Rapid economic development and significantly aging populations have produced a profound impact on the epidemiological characteristics of HF in Chinese populations.Acute decompensated heart failure(ADHF)is predominantly a disease of the elderly in Macao,and a significant heterogeneity exists in the clinical features,managements,and outcomes among different age groups.Age-based risk stratification models and multidisciplinary HF teams are urgently needed to improve the management and outcomes of hospitalized heart failure(HHF)patients. | Zhi-Nan LU Mario Evora Edmundo Lao Man Ieng Pun Weng Hong Pun Monica Pon Kong Chu Pui I Ieong Toi-Meng Mok Yong-Jian WU | 2020 | Journal of Geriatric Cardiology2020,17,9: | 2 |
