| 1 | Diagnosis of focal liver lesions with deep learning-based multichannel analysis of hepatocyte-specific contrast-enhanced magnetic resonance imaging显示文摘BACKGROUND The nature of input data is an essential factor when training neural networks.Research concerning magnetic resonance imaging(MRI)-based diagnosis of liver tumors using deep learning has been rapidly advancing.Still,evidence to support the utilization of multi-dimensional and multi-parametric image data is lacking.Due to higher information content,three-dimensional input should presumably result in higher classification precision.Also,the differentiation between focal liver lesions(FLLs)can only be plausible with simultaneous analysis of multisequence MRI images.AIM To compare diagnostic efficiency of two-dimensional(2D)and three-dimensional(3D)-densely connected convolutional neural networks(DenseNet)for FLLs on multi-sequence MRI.METHODS We retrospectively collected T2-weighted,gadoxetate disodium-enhanced arterial phase,portal venous phase,and hepatobiliary phase MRI scans from patients with focal nodular hyperplasia(FNH),hepatocellular carcinomas(HCC)or liver metastases(MET).Our search identified 71 FNH,69 HCC and 76 MET.After volume registration,the same three most representative axial slices from all sequences were combined into four-channel images to train the 2D-DenseNet264 network.Identical bounding boxes were selected on all scans and stacked into 4D volumes to train the 3D-DenseNet264 model.The test set consisted of 10-10-10 tumors.The performance of the models was compared using area under the receiver operating characteristic curve(AUROC),specificity,sensitivity,positive predictive values(PPV),negative predictive values(NPV),and f1 scores.RESULTS The average AUC value of the 2D model(0.98)was slightly higher than that of the 3D model(0.94).Mean PPV,sensitivity,NPV,specificity and f1 scores(0.94,0.93,0.97,0.97,and 0.93)of the 2D model were also superior to metrics of the 3D model(0.84,0.83,0.92,0.92,and 0.83).The classification metrics of FNH were 0.91,1.00,1.00,0.95,and 0.95 using the 2D and 0.90,0.90,0.95,0.95,and 0.90 using the 3D models.The 2D and 3D networks'performance in the diagnosis of HCC were 1.00,0.80,0.91,1.00,and 0.89 and 0.88,0.70,0.86,0.95,and 0.78,respectively;while the evaluation of MET lesions resulted in 0.91,1.00,1.00,0.95,and 0.95 and 0.75,0.90,0.94,0.85,and 0.82 using the 2D and 3D networks,respectively.CONCLUSION Both 2D and 3D-DenseNets can differentiate FNH,HCC and MET with good accuracy when trained on hepatocyte-specific contrast-enhanced multi-sequence MRI volumes. | Róbert Stollmayer Bettina K Budai Ambrus Tóth IldikóKalina Erika Hartmann Péter Szoldán Viktor Bérczi Pál Maurovich-Horvat Pál N Kaposi | 2021 | World Journal of Gastroenterology2021，27，35： | 6 |
| 2 | Compared efficacy of preservation solutions on the outcome of liver transplantation:Meta-analysis显示文摘AIM To compare the effects of the four most commonly used preservation solutions on the outcome of liver transplantations.METHODS A systematic literature search was performed using MEDLINE, Scopus, EMBASE and the Cochrane Library databases up to January 31^(st), 2017. The inclusion criteria were comparative, randomized controlled trials(RCTs) for deceased donor liver(DDL) allografts with adult and pediatric donors using the gold standard University of Wisconsin(UW) solution or histidinetryptophan-ketoglutarate(HTK), Celsior(CS) and Institut Georges Lopez(IGL-1) solutions. Fifteen RCTs(1830 livers) were included; the primary outcomes were primary non-function(PNF) and one-year posttransplant graft survival(OGS-1). RESULTS All trials were homogenous with respect to donor and recipient characteristics. There was no statistical difference in the incidence of PNF with the use of UW, HTK, CS and IGL-1(RR = 0.02, 95%CI: 0.01-0.03, P = 0.356). Comparing OGS-1 also failed to reveal any difference between UW, HTK, CS and IGL-1(RR = 0.80, 95%CI: 0.80-0.80, P = 0.369). Two trials demonstrated higher PNF levels for UW in comparison with the HTK group, and individual studies described higher rates of biliary complications where HTK and CS were used compared to the UW and IGL-1 solutions. | 3 | Current debate in the oncologic management of rectal cancer显示文摘Despite the considerable amount of research in the field,the management of locally advanced rectal cancer remains a subject to debate.To date,effective treatment centers on surgical resection with the standard approach of total mesorectal resection.Radiation therapy and chemotherapy have been incorporated in order to decrease local and systemic recurrence.While it is accepted that a multimodality treatment regimen is indicated,there remains significant debate for how best to accomplish this in regards to order,dosing,and choice of agents.Preoperative radiation is the standard of care,yet remains debated with the option for chemoradiation,short course radiation,and even ongoing studies looking at the possibility of leaving radiation out altogether.Chemotherapy was traditionally incorporated in the adjuvant setting,but recent reports suggest the possibility of improved efficacy and tolerance when given upfront.In this review,the major studies in the management of locally advanced rectal cancer will be discussed.In addition,future directions will be considered such as the role of immunotherapy and ongoing trials looking at timing of chemotherapy,inclusion of radiation,and nonoperative management. | Trish Millard Paul R Kunk Erika Ramsdale Osama E Rahma | 2016 | World Journal of Gastrointestinal Oncology2016，8，10： | 3 |
| 4 | Infiltrating adenocarcinoma arising in a villous adenoma of the anal canal显示文摘Primary neoplasms arising in the anal canal are relatively unusual.In particular, adenomas and adenocarcinomas are distinctly rare entities in this region.We describe an infiltrating, well-differentiated adenocarcinoma arising in a villous adenoma from the distal anal canal, in an otherwise healthy patient at low risk for gastrointestinal malignancy.This is the case of an octogenarian man with a several year history of hemorrhoids and intermittent rectal bleeding, more recently complaining of continuous hematochezia.Examination revealed a blood-covered pedunculated mass with a long stalk protruding from the anus.The lesion was amputated at the bedside.Microscopic evaluation revealed an infiltrating well-differentiated adenocarcinoma, arising from a villous adenoma.This was further evaluated under anesthesia and complete excision of distal anal tissue was performed.Our report is the f irst describing the possible malignant degeneration of a villous adenoma in the anal canal. | Marni Colvin Aris Delis Erika Bracamonte Hugo Villar Luis R Leon Jr | 2009 | World Journal of Gastroenterology2009，15，28： | 2 |
