| 1 | Epirubicin,Cisplatin,5-FU combination chemotherapy in sorafenib-refractory metastatic hepatocellular carcinoma显示文摘AIM:To evaluate the clinical efficacy and safety of epirubicin,cisplatin,and 5-FU combination chemotherapy for the sorafenib-refractory metastatic hepatocellular carcinoma(HCC).METHODS:From April 2009 to June 2012,31 patients who were diagnosed with metastatic and progressive HCC after sorafenib treatment were retrospectively reviewed.Patients were treated with the combination of epirubicin(50 mg/m2Ⅳ;day 1),cisplatin(60 mg/m2Ⅳ;day 1),and 5-FU(1000 mg/m2Ⅳ;day 1-3)[Epirubicin,cisplatin,5-FU combination(ECF)],repeated every 4 wk.RESULTS:The overall response rate was 12.9%.Patients who responded to ECF chemotherapy showed a longer overall survival(OS)and time to progression(TTP)relative to those in the non-responder group(OS:20.4 mo vs 4.9 mo,P<0.001,TTP:9.4 mo vs 2.2 mo,P<0.001).Patients with a stable primary liver mass also exhibited a longer OS and TTP relative to those with progressive disease(OS:13.4 mo vs 5.3 mo,P=0.003;TTP:9.4 mo vs 2.3 mo,P=0.003).The most common hematologic toxicity was thrombocytopenia(87.2%),and the incidence of grade 3-4 neutropenia was 53.9%.Age older than 60,a stable primary mass,and a good response to chemotherapy were prognostic factors for OS and TTP.CONCLUSION:This combination cytotoxic chemotherapy can serve as another treatment option after sorafenib failure for the subset of patients with advanced metastatic HCC. | Ji Eun Lee Si Hyun Bae Jong Young Choi Seung Kew Yoon Young Kyoung You Myung Ah Lee | 2014 | World Journal of Gastroenterology2014，20，1： | 7 |
| 2 | Perceived risk as a barrier to appropriate diagnosis of irritable bowel syndrome显示文摘AIM:To evaluate perceived risk,diagnostic testing,and acceptance of a diagnosis of irritable bowel syndrome(IBS)among the Korean laypersons.METHODS:We designed a conceptual framework to evaluate the health-seeking behavior of subjects based on a knowledge,attitude,and practice model.We developed a vignette-based questionnaire about IBS based on a literature review and focused group interviews.The vignette described a 40-year-old woman who meets the RomeⅢcriteria for IBS without red-flag signs.It was followed by questions about demographic characteristics,health behaviors,IBS symptoms,risk perception,perceived need for diagnostic tests,and acceptance of a positive diagnosis of IBS.Weplanned a nationwide survey targeting laypersons without IBS and between the ages of 20 and 69 years.Survey participants were selected by quota sampling stratified by gender,age,and nationwide location.A multivariate logistic model was constructed based on literature reviews,univariate analysis,and a stepwise selection method to investigate correlations between the perceived risk,need for diagnostic tests,and acceptance of a positive diagnosis.RESULTS:Of 2354 eligible households,1000 subjects completed the survey and 983 subjects were analyzed,excluding those who met symptom criteria for IBS.After reading the IBS vignette,the majority of subjects(86.8%)responded that the patient was at increased risk of severe disease.The most frequent concern was colon cancer(59.8%),followed by surgical condition(51.5%).Most subjects responded the patient needs diagnostic tests(97.2%).Colonoscopy was the most commonly required test(79.5%).Less than half of the respondents requested a stool examination(45.0%),blood test(40.7%),abdominal ultrasound(36.0%),or computed tomography(20.2%).The subjects who felt increased risk were more likely to see a need for colonoscopy[adjusted odds ratio(a OR)=2.10,95%CI:1.38-3.18].When asked about the positive diagnosis,the most frequent response was that'the patient would not be reassured'(65.7%).The increased risk perception group was less likely to be reassured by a positive diagnosis of IBS,compared to the other respondents(a OR=0.52,95%CI:0.34-0.78).CONCLUSION:For IBS diagnosis,increased risk perception is a possible barrier to the appropriate use of diagnostic tests and to the patient’s acceptance of a positive diagnosis. | Eunmi Ahn Ki Young Son Dong Wook Shin Min Kyu Han Hyejin Lee Ah Reum An Eun Ho Kim Be Long Cho | 2014 | World Journal of Gastroenterology2014，20，48： | 2 |

| 4 | Lens culinaris agglutinin-reactive fraction of alpha-fetoprotein improves diagnostic accuracy for hepatocellular carcinoma显示文摘BACKGROUND Diagnostic accuracy of various tumor markers and their combinations for hepatocellular carcinoma(HCC)was not fully investigated.AIM To evaluate the diagnostic accuracy of alpha-fetoprotein(AFP),the Lens culinaris agglutinin-reactive fraction of AFP(AFP-L3),and protein induced by vitamin K absence or antagonist-II(PIVKA-II)and their combination for HCC diagnosis.METHODS Patients with newly detected liver mass or elevated serum AFP levels were considered eligible.Serum AFP level,AFP-L3 fraction,and PIVKA-II level were measured at the first visit.RESULTS In total,622 patients were included;355 patients(57.1%)had chronic liver disease,and 208(33.4%)had liver cirrhosis.HCC was diagnosed in 160 patients(25.7%).The area under the receiver operating characteristics curves(AUROCs)of the serum AFP,AFP-L3 fraction,AFP-L3,and PIVKA-II levels for the diagnosis of HCC were 0.775,0.792,0.814,and 0.834,respectively.A novel diagnostic model was developed by classifying patients in a 1:1 ratio into training and validation sets.Using the binary regression analysis of the training cohort,the AFP,AFP-L3 fraction,and PIVKA-II(ALPs)score was calculated as follows:ALPs score=3.8×[serum AFP level(ng/mL)×AFP-L3 fraction(%)×0.01]+0.2×PIVKA-II level(mAU/mL).The AUROC of the ALPs score for diagnosis of HCC was 0.878,significantly higher than that of serum AFP level(P<0.001),AFP-L3 fraction(P<0.001),PIVKA-II level(P=0.036),and AFP-L3 level(P=0.006).The optimal ALPs score cut-off was 5.3(sensitivity,85.0%,specificity 80.1%).The validation cohort showed similar results.CONCLUSION The ALPs score calculated using serum AFP level,AFP-L3 fraction,and PIVKA-II level showed improved accuracy in HCC diagnosis. | Han Ah Lee Yoo Ra Lee Young-Sun Lee Young Kul Jung Ji Hoon Kim Hyunggin An Hyung Joon Yim Yoon Tae Jeen Jong Eun Yeon Kwan Soo Byun Yeon Seok Seo | 2021 | World Journal of Gastroenterology2021，27，28： | 2 |
