1Self-expandable metal stents for choledocholithiasis in Billroth II gastrectomy patients显示文摘Background: Efficient ampullary intervention is essential for endoscopic retrograde cholangiopancreatography(ERCP) in patients with a prior Billroth II gastrectomy. We retrospectively evaluated the safety and effectiveness of ampullary intervention using fully covered self-expandable metal stents(FCSEMSs) for the management of common bile duct(CBD) stones in a subset of patients with a history of Billroth II gastrectomy. Methods: This retrospective analysis involved patients with a prior Billroth II gastrectomy who underwent ampullary intervention with FCSEMSs for the management of CBD stones. The factors associated with FCSEMSs placement, treatment success, and procedural complications were analyzed. Results: A group of 15 patients(10 males; median age, 78 years) underwent biliary metal stent placement for high degree of CBD angulation(6), small or flat papilla with unclear margin(5), current use of double antiplatelet agents or an anticoagulant(2), unwanted instrumentation of the cystic duct(1), and insecure position of the scope(1). Ampullary intervention with FCSEMSs was successful in all patients. After dilating the ampulla of Vater and building a durable conduit with FCSEMSs immediately, CBD stones were removed successfully from all patients in a single session. A mild post-ERCP pancreatitis occurred in one patient, who recovered without complications. 3Potentially inappropriate prescribing of cardiovascular system and antiplatelet/anticoagulant drugs among elderly patients:a Korean population-based national study显示文摘OBJECTIVES To investigate the prevalence of potentially inappropriate prescribing(PIP)for cardiovascular system(CVS)and antiplatelet/anticoagulant(AP/AC)drugs among Korean elderly patients,using the Screening Tool of Older Persons’Prescriptions(STOPP)criteria version 2 and to identify the risk factors related to PIP.METHODS The 2016 National Aged Patient Sample data,comprising National Health Insurance claim records for a random sample of 20%of patients aged≥65 years,were used to calculate PIP prevalence of outpatient prescriptions.For criteria including drugdisease interactions,PIP prevalence per indication was estimated.RESULTS Among 1,274,148 elderly patients and 27,062,307 outpatient prescription claims,100,085 patients(7.85%)and 341,664 claims(1.27%)had one or more PIP.The most frequent PIP was“nonsteroidal antiinflammatory drug with concurrent antiplatelet agent(s)without protonpump inhibitor prophylaxis”in the claimlevel(0.97%)and patientlevel(6.33%)analyses.“Betablocker with bradycardia”(16.47%of claims)and“angiotensin receptor blockers in patients with hyperkalaemia”(23.89%of claims)showed the highest PIP prevalence per indication.Logistic regression analysis revealed that,among the patient and health care provider characteristics,female,older age,more severe comorbidities,polypharmacy,higher level of healthcare organization,and specialty of prescriber were significantly associated with a higher risk of PIP.CONCLUSIONS Our findings of a high prevalence of PIP for CVS and AP/AC drugs among the elderly suggest that an effective strategy is urgently needed to improve the prescription practices of these drugs.Jongyeon Kim Euna Han Hee-Jin Hwang Hyeonseok Cho Young-Sang Kim Hyejin Chun Jinkwon Kim Yon Chul Park Hye-Young Kang 2021Journal of Geriatric Cardiology2021,18,5:1
4Engineering of Materials for Respiratory Protection:Salt-Coated Antimicrobial Fabrics for Their Application in Respiratory Devices显示文摘CONSPECTUS:The development of a highly effective,low-cost method for protection against direct transmission through infectious media associated with respiratory diseases has been considered to be a major challenge in the management of the pandemic outbreak.Currently,filtration in respirators and masks depends on the mesh size,raising safety concerns about direct contact with the virus-laden layers of masks and respirators during use and disposal.Also,conventional medical face masks and respirators are recommended for single use only because long-term use and reuse of masks increase the risk of contact transmission of pathogens.As a result,mask reusability has gained much attention in addressing the global problem of respirator and mask shortages during the COVID-19 pandemic.Furthermore,coinfection by influenza,SARSCoV-2,and bacteria has been an ongoing issue in the current COVID-19 pandemic.In our response to the increasing demand for more convenient forms of disease control,our group has investigated antimicrobial technologies for respiratory devices against pandemic/epidemic diseases and developed two universal antimicrobial coating technologies:(1)nonwashable,reusable salt-coated fabrics and(2)washable,photopolymerizable polymer-coated fabrics which efficiently inactivate infectious viruses by contact with the filter.This is an opportune time to assess research efforts toward antimicrobial face mask technology for its expected impact on society and the economy.Sumin Han Euna Oh Erin Keltie Jong Sung Kim Hyo-Jick Choi 2022Accounts of Materials Research2022,3,3:0
