| 1 | Comparison of Snellen and Early Treatment Diabetic Retinopathy Study charts using a computer simulation显示文摘AIM:To compare accuracy,reproducibility and test duration for the Snellen and the Early Treatment Diabetic Retinopathy Study(ETDRS)charts,two main tools used to measure visual acuity(VA).·METHODS:A computer simulation was programmed to run multiple virtual patients,each with a unique set of assigned parameters,including VA,false-positive and false-negative error values.For each virtual patient,assigned VA was randomly chosen along a continuous scale spanning the range between 1.0 to 0.0 log MAR units(equivalent to 20/200 to 20/20).Each of 30 000virtual patients were run ten times on each of the two VA charts.·RESULTS:Average test duration(expressed as the total number of characters presented during the test±SD)was12.6±11.1 and 31.2±14.7 characters,for the Snellen and ETDRS,respectively.Accuracy,defined as the absolute difference(±SD)between the assigned VA and the measured VA,expressed in log MAR units,was superior in the ETDRS charts:0.12±0.14 and 0.08±0.08,for the Snellen and ETDRS charts,respectively.Reproducibility,expressed as test-retest variability,was superior in the ETDRS charts:0.23±0.17 and 0.11±0.09 log MAR units,for the Snellen and ETDRS charts,respectively.·CONCLUSION:A comparison of true(assigned)VA to measured VA,demonstrated,on average,better accuracy and reproducibility of the ETDRS chart,but at the penalty of significantly longer test duration.These differences were most pronounced in the low VA range.The reproducibility using a simulation approach is in line with reproducibility values found in several clinical studies. | Reuben R.Shamir Yael Friedman Leo Joskowicz Michael Mimouni Eytan Z.Blumenthal | 2016 | International Journal of Ophthalmology(English edition)2016,9,1: | 5 |
| 2 | Gut microbiome in primary sclerosing cholangitis:A review显示文摘Primary sclerosing cholangitis(PSC)is a chronic cholestatic liver disease characterized by biliary inflammation and stricturing.Exploration of the pathogenesis of PSC in light of its association with inflammatory bowel disease(IBD)and the“gut-liver”axis is an emerging area of interest.A growing number of studies have begun to elucidate the role of the gut microbiota,its metabolites and its influence on host immune responses in the development of PSC and PSCIBD.Studies of the fecal microbiota have highlighted enriched levels of certain species,including Veillonella,Streptococcus and Enterococcus,among others.A heightened immune response to enteric dysbiosis and bacterial translocation have also been implicated.For example,Klebsiella pneumoniae strains derived from gnotobiotic mice transplanted with PSC-IBD microbiota were found to induce pore formation in human intestinal epithelial cells and enhanced Th17 responses.Gut microbes have additionally been hypothesized to be implicated in PSC pathogenesis through their role in the synthesis of various metabolites,including bile acids(BAs),which function as signaling molecules with important gut and hepatic effects.An expanded knowledge of the gut microbiome as it relates to PSC offers critical insight into the development of microbe-altering therapeutic interventions,such as antibiotics,nutritional interventions and fecal microbial transplantation.Some of these have already shown some preliminary evidence of benefit.Despite exciting progress in the field,much work remains to be done;areas that are particularly lacking include functional characterization of the microbiome and examination of pediatric populations.In this review,we summarize studies that have investigated the microbiome in PSC and PSC-IBD as well as putative mechanisms,including the potential role of metabolites,such as BAs.We then briefly review the evidence for interventions with microbe-altering properties for treating PSC. | Rebecca Little Eytan Wine Binita M Kamath Anne M Griffiths Amanda Ricciuto | 2020 | World Journal of Gastroenterology2020,26,21: | 5 |
| 3 | 食管扩张——一种胃食管反流触发机制显示文摘目的 :明确食管扩张时下食管括约肌发生松弛状态下胃食管反流的发生率。方法 :12例健康志愿者在餐前和餐后同时检测食管压力和 pH ,食管扩张是通过快速和缓慢的气体注射诱发。 结果 :引起完全的下食管扩约肌松弛的气体阈值量平均为 (11 0± 1 0 )ml。只有极少的松弛伴有继发蠕动。快速注气下食管括约肌的松弛时间为(13.0± 1.5 )s;慢速注射时食管括约肌的松弛时间是 (16 0± 2 0 )s。由快速阈值量气体注射引起LES松弛并伴有胃食管反流率 ,在餐后为 (2 2± 6 ) % ;缓慢注射时为 (2 3± 10 ) %。对于快速食管扩张 ,单独的下食管括约肌松弛伴随反流的百分比显著高于伴有继发蠕动的下食管括约肌松弛时出现的胃食管反流情况。结论 :(1)胃食管反流可以发生在由食管扩张引起的下食管括约肌松弛期内 ;(2 )食管扩张可以引起不伴随继发蠕动的单纯下食管括约肌松弛 ;(3)单纯下食管括约肌松弛时的胃食管反流发生率显著高于伴有继发蠕动的情况。 | 谢鹏雁 Eytan Bardan Junlong Ren Walter J Hogan Reza Shaker | 2000 | 北京医科大学学报2000,32,2: | 5 |
