1Role of radiation therapy in neoadjuvant era in patients with locally advanced rectal cancer显示文摘Surgery remains the primary determinant of cure in patients with localized rectal cancer, and total mesorectal excision is now widely accepted as standard of care. The widespread implementation of neoadjuvant shortcourse radiotherapy (RT) or long-course chemoradiotherapy (CRT) has reduced local recurrence rates from 25% to 40% to less than 10%; Preoperative RT in resectable rectal cancer has a number of potential advantages, most importantly reducing local recurrence, and down-staging effect. In this article making a comprehensive literature review searching the reliable medical data bases of PubMed and Cochrane we present all available information on the role of radiation therapy alone or in combination with chemotherapy in preoperative setting of rectal cancer. Data reported show that in locally advanced rectal cancer the addition of radiation therapy or CRT pre surgically has significantly improved sphincter prevention surgery. Moreover, the addition of chemotherapy to radiation therapy in preoperative setting has significantly improved pathologic complete response rate and loco-regional control rate without improvement in sphincter preserving surgery. 2Transcriptome analyses of the Dof-like gene family in grapevine reveal its involvement in berry, flower and seed development显示文摘The Dof(DNA-binding with one finger)protein family spans a group of plant transcription factors involved in the regulation of several functions,such as plant responses to stress,hormones and light,phytochrome signaling and seed germination.Here we describe the Dof-like gene family in grapevine(Vitis vinifera L.),which consists of 25 genes coding for Dof.An extensive in silico characterization of the VviDofL gene family was performed.Additionally,the expression of the entire gene family was assessed in 54 grapevine tissues and organs using an integrated approach with microarray(cv Corvina)and real-time PCR(cv Pinot Noir)analyses.The phylogenetic analysis comparing grapevine sequences with those of Arabidopsis,tomato,poplar and already described Dof genes in other species allowed us to identify several duplicated genes.The diversification of grapevine DofL genes during evolution likely resulted in a broader range of biological roles.Furthermore,distinct expression patterns were identified between samples analyzed,corroborating such hypothesis.Our expression results indicate that several VviDofL genes perform their functional roles mainly during flower,berry and seed development,highlighting their importance for grapevine growth and production.The identification of similar expression profiles between both approaches strongly suggests that these genes have important regulatory roles that are evolutionally conserved between grapevine cvs Corvina and Pinot Noir.Danielle Costenaro da Silva Vítor da Silveira Falavigna Marianna Fasoli Vanessa Buffon Diogo Denardi Porto Georgios Joannis Pappas Jr Mario Pezzotti Giancarlo Pasquali Luís Fernando Revers 2016Horticulture Research2016,3,1:5
3Capecitabine for locally advanced and metastatic colorectal cancer:A review显示文摘Capecitabine (Xeloda) is an oral fluoropyrimidine which is produced as a pro-drug of fluorouracil, and shows improved tolerability and intratumor drug concentrations following its tumor-specif ic conversion to the active drug. We have searched the Pubmed and Cochrane databases from 1980 to 2009 with the purpose of reviewing all available information on Capecitabine, focusing on its clinical effectiveness against colorectal cancer. Special attention has been paid to trials that compared Capecitabine with standard folinic acid (leucovorin, LV)-modulated intravenous 5-fluorouracil (5-FU) bolus regimens in patients with metastatic colorectal cancer. Moreover the efficacy of Capecitabine on metastatic colorectal cancer, either alone or in various combinations with other active drugs such as Irinotecan and Oxaliplatin was also assessed. Finally, neoadjuvant therapy con- sisting of Capecitabine plus radiation therapy, for locally advanced rectal cancer was analysed. This combination of chemotherapy and radiotherapy has a special role in tumor down staging and in sphincter preservation for lower rectal tumors. Comparative trials have shown that Capecitabine is at least equivalent to the standard LV-5-FU combination in relation to progression-free and overall survival whilst showing a better tolerability prof ile with a much lower incidence of stomatitis. It is now known that Capecitabine can be combined with other active drugs such as Irinotecan and Oxaliplatin. The combination of Oxaliplatin with Capecitabine represents a new standard of care for metastatic colorectal cancer. Combinating the Capecitabine-Oxaliplatin regimen with promising new biological drugs such as Bevacizumab seems to give a realistic prospect of further improvement in time to progression of metastatic disease. Moreover, preoperative chemo-radiation using oral capecitabine is better tolerated than bolus 5-FU and is more effective in the promotion of both down-staging and sphincter preservation in patients with locally advanced rectal cancer. 4Patient dose considerations in computed tomography examinations显示文摘Ionizing radiation is extensively used in medicine and its contribution to both diagnosis and therapy is undisputable.However,the use of ionizing radiation also involves a certain risk since it may cause damage to tissues and organs and trigger carcinogenesis.Computed tomography(CT) is currently one of the major contributors to the collective population radiation dose both because it is a relatively high dose examination and an increasing number of people are subjected to CT examinations many times during their lifetime.The evolution of CT scanner technology has greatly increased the clinical applications of CT and its availability throughout the world and made it a routine rather than a specialized examination.With the modern multislice CT scanners,fast volume scanning of the whole human body within less than 1 min is now feasible.Two dimensional images of superb quality can be reconstructed in every possible plane with respect to the patient axis(e.g.axial,sagital and coronal).Furthermore,three-dimensional images of all anatomic structures and organs can be produced with only minimal additional effort(e.g.skeleton,tracheobronchial tree,gastrointestinal system and cardiovascular system).All these applications,which are diagnostically valuable,also involve a significant radiation risk.Therefore,all medical professionals involved with CT,either as referring or examining medical doctors must be aware of the risks involved before they decide to prescribe or perform CT examinations.Ultimately,the final decision concerning justification for a prescribed CT examination lies upon the radiologist.In this paper,we summarize the basic information concerning the detrimental effects of ionizing radiation,as well as the CT dosimetry background.Furthermore,after a brief summary of the evolution of CT scanning,the current CT scanner technology and its special features with respect to patient doses are given in detail.Some numerical data is also given in order to comprehend the magnitude of the potential radiation risk involved in comparison with risk from exposure to natural background radiation levels.Ioannis A Tsalafoutas Georgios V Koukourakis 2010World Journal of Radiology2010,2,7:4

6Association of liver cirrhosis related IgA nephropathy with portal hypertension显示文摘A high incidence of IgA nephropathy has been reported in patients with liver cirrhosis, though, clinically evident nephrotic syndrome is very uncommon. Impaired hepatic clearance of circulating IgA immune complexes and subsequent deposition in renal glomeruli has been considered principally in the pathogenesis of liver cirrhosis associated IgA nephropathy. Here we report on a patient with cryptogenic liver cirrhosis and splenic vein thrombosis, who presented with nephrotic syndrome. Renal biopsy showed findings consistent with IgA nephropathy. Lower endoscopy showed features of portal hypertensive colopathy. Following initiation of propranolol and anticoagulant treatment to reduce portal pressure, a gradual decrease of proteinuria and hematuria to normal range was noted. 7Fecal calprotectin measurement is a marker of shortterm clinical outcome and presence of mucosal healing in patients with inflammatory bowel disease显示文摘AIM To evaluate the utility of fecal calprotectin(FC) in predicting relapse and endoscopic activity during follow-up in an inflammatory bowel disease(IBD) cohort.METHODS All FC measurements that were obtained during a 3-year period from patients with inflammatory bowel disease in clinical remission were identified. Data regarding the short-term(6 mo) course of the disease were extracted from the medical files. Exclusion criteria were defined as:(1) An established flare of the disease at the time of FC measurement,(2) Loss to follow up within 6 mo from baseline FC measurement, and,(3) Insufficient data on file. Statistical analysis was performed to evaluate whether baseline FC measurement could predict the short term clinical relapse and/or the presence of mucosal healing.RESULTS We included 149 [Crohn's disease(CD) = 113, Ulcerative colitis(UC) = 36, male = 77] IBD patients in our study. Within the determined 6-month period post-FC measurement, 47(31.5%) had a disease flare. Among 76 patients who underwent endoscopy, 39(51.3%) had mucosal healing. Baseline FC concentrations were significantly higher in those who had clinical relapse compared to those who remained in remission during follow up(481.0 μg/g, 286.0-600.0 vs 89.0, 36.0-180.8, P < 0.001). The significant predictive value of baseline median with IQR FC for clinical relapse was confirmed by multivariate Cox analysis [HR for 100μg/g: 1.75(95%CI: 1.28-2.39), P = 0.001]. Furthermore, lower FC baseline values significantly correlated to the presence of mucosal healing in endoscopy(69.0 μg/g, 30.0-128.0 vs 481.0, 278.0-600.0, in those with mucosal inflammation, median with IQR, P < 0.001). We were able to extract cut-off values for FC concentration with a high sensitivity and specificity for predicting clinical relapse(261 μg/g with AUC = 0.901, sensitivity 87.2%, specificity 85.3%, P < 0.001) or mucosal healing(174 μg/g with AUC = 0.956, sensitivity 91.9%, specificity 87.2%, P < 0.001). FC was better than CRP in predicting either outcome; nevertheless, having a pathological CRP(> 5 mg/L) in addition to the cutoffs for FC, significantly enhanced the specificity for predicting clinical relapse(95.1% from 85.3%) or endoscopic activity(100% from 87.2%). 8Rifaximin for the prevention of spontaneous bacterial peritonitis显示文摘According to a review article by Biecker et al published in a previous issue of World Journal of Gastroenterology in March 2011,intestinal decontamination with norfloxacin remains the mainstay of primary prophylaxis of spontaneous bacterial peritonitis(SBP) at the expense of development of quinolone-resistant bacteria after long-term use.In our research,the administration of a 4-wk regimen with rifaximin 1200 mg/d reduced significantly the ascitic neutrophil count in cirrhotic patients with sterile ascites in line with a significant decrease in plasma endotoxin levels.Our observations concur with recent findings,showing a significantly reduced 5-year probability of SBP in cirrhotic patients taking rifaximin.Georgios N Kalambokis Athanasia Mouzaki Maria Rodi Epameinondas V Tsianos 2012World Journal of Gastroenterology2012,18,14:2

20Tissue-specific relaxin-2 is differentially associated with the presence/size of an arterial aneurysm and the severity of atherosclerotic disease in humans显示文摘Circulating or tissue-related biomarkers are of clinical value for risk stratification in patients with abdominal aortic aneurysms.Relaxin-2(RL2)has been linked to the presence and size of arterial aneurysms,and to the extent of atherosclerosis in human subjects.Here,we assessed the expression levels of RL2 in aneurysmal(AA,n=16)and atherosclerotic(ATH,n=22)arteries,and established the correlation between RL2 levels and the presence/size of AA and the clinical severity of atherosclerosis.The expression levels of metalloproteinases(MMPs)and endothelial nitric oxide synthetase(eNOS)were also detected for correlations with different phenotypes of atherosclerosis and AA.Temporal artery biopsy specimens(n=6)and abdominal aortic tissues harvested from accident victims during autopsy(n=10)were used as controls.Quantitative tissue biomarker analysis revealed that tissue-specific RL2 was increased in patients with larger or symptomatic AA compared to subjects with atherosclerotic disease and healthy controls.In situ RL2 levels were proportional to the size and the severity of aneurysmatic disease,and were substantially elevated in patients with symptomatic aneurysm of any diameter or asymptomatic aneurysm of a diameter>350%of that of the normal artery.In contrast,tissue RL2 was inversely associated with the clinical severity of atherosclerotic lesions.Correlation between RL2 and MMP2 was different between ATH1 and ATH2,depending on atherosclerosis grade.Overall,tissue RL2 is differentially associated with discrete phenotypes of arterial disease and might exert multipotent biological effects on vascular wall integrity and remodeling in human subjects.Konstantinos Papoutsis Alkistis Kapelouzou Georgios Georgiopoulos Christos Kontogiannis Christos Kourek Konstantinos S Mylonas Nikolaos Patelis Dennis V Cokkinos Ioannis Karavokyros Sotirios Georgopoulos 2020Acta Pharmacologica Sinica2020,41,6:1
