| 2 | Over-the-scope-clip closure of long lasting gastrocutaneous fistula after percutaneous endoscopic gastrostomy tube removal in immunocompromised patients:A single center case series显示文摘Over-the-scope-clips(OTSC~?) have been shown to be an effective and safe endoscopic treatment option for the closure of gastrointestinal perforations, leakages and fistulae. Indications for endoscopic OTSC~? treatment have grown in number and also include gastro cutaneous fistula(GCF) after percutaneous endoscopic gastrostomy(PEG) tube removal. Non-healing GCF is a rare complication after removal of PEG tubes and may especially develop in immunosuppressed patients with multiple comorbidities. There is growing evidence in the literature that OTSC~? closure of GCF after PEG tube removal is emerging as an effective, simple and safe endoscopic treatment option. However current evidence is limited to the geriatric population and short standing GCF, while information on closure of long standing GCF after PEG tube removal in a younger population with significant comorbidities is lacking. In this retrospective single-center case-series we report on five patients undergoing OTSC~? closure of chronic GCF after PEG tube removal. Four out of five patients were afflicted with long lasting, symptomatic fistulae. All five patients suffered from chronic disease associated with a catabolic metabolism(cystic fibrosis, chemotherapy for neoplasia, liver cirrhosis). The mean patient age was 43 years. The mean dwell time of PEG tubes in all five patients was 808 d. PEG tube dwell time was shortest in patient 5(21 d). The mean duration from PEG tube removal to fistula closure in patients 1-4 was 360 d(range 144-850 d). The intervention was welltolerated by all patients and no adverse events occured. Successful immediate and long-term fistula closure was accomplished in all five patients. This single center case series is the first to show successful endoscopic OTSC~? closure of long lasting GCF in five consecutive middleaged patients with significant comorbidities. Endoscopic closure of chronic persistent GCF after PEG tube removal using an OTSC~? was achieved in all patients with no immediate or long-term complications. | 5 | Mucosal healing effect of mesalazine granules in naproxen-induced small bowel enteropathy显示文摘AIM:To investigate the effect of mesalazine granules on small intestinal injury induced by naproxen using capsule endoscopy (CE).METHODS:This was a single center,non-randomized,open-label,uncontrolled pilot study,using the PillCam SB CE system with RAPID 5 software.The Lewis Index Score (LIS) for small bowel injury was investigated to evaluate the severity of mucosal injury.Arthropathy patients with at least one month history of daily naproxen use of 1000 mg and proton pump inhibitor co-therapy were screened.Patients with a minimum LIS of 135 were eligible to enter the 4-wk treatment phase of the study.During this treatment period,3 × 1000 mg/d mesalazine granules were added to ongoing therapies of 1000 mg/d naproxen and 20 mg/d omeprazole.At the end of the 4-wk combined treatment period,a second small bowel CE was performed to re-evaluate the enteropathy according to the LIS results.The primary objective of this study was to assess the mucosal changes after 4 wk of mesalazine treatment.RESULTS:A total of 18 patients (16 females),ranging in age from 46 to 78 years (mean age 60.3 years) were screened,all had been taking 1000 mg/d naproxen for at least one month.Eight patients were excluded from the mesalazine therapeutic phase of the study for the following reasons:the screening CE showed normal small bowel mucosa or only insignificant damages (LIS < 135) in five patients,the screening esophagogastroduodenoscopy revealed gastric ulcer in one patient,capsule technical failure and incomplete CE due to poor small bowel cleanliness in two patients.Ten patients (9 female,mean age 56.2 years) whose initial LIS reached mild and moderate-to-severe enteropathy grades (between 135 and 790 and ≥ 790) entered the 4-wk therapeutic phase and a repeat CE was performed.When comparing the change in LIS from baseline to end of treatment in all patients,a marked decrease was seen (mean LIS:1236.4 ± 821.9 vs 925.2 ± 543.4,P=0.271).Moreover,a significant difference between pre-and post-treatment mean total LIS was detected in 7 patients who had moderate-tosevere enteropathy gradings at the inclusion CE (mean LIS:1615 ± 672vs 1064 ± 424,P=0.033).CONCLUSION:According to the small bowel CE evaluation mesalazine granules significantly attenuated mucosal injuries in patients with moderate-to-severe enteropathies induced by naproxen. | István Rácz Milán Szalai Valéria Kovács Henriett Regczi Gyngyi Kiss Zoltán Horváth | 2013 | World Journal of Gastroenterology2013，19，6： | 2 |

| 10 | Personalized treatment-which interaction ingredients should be focused to capture the unconscious显示文摘A recent meta-analysis revealed that mental health and baseline psychological impairment affect the quality of life and outcomes in different chronic conditions.Implementing mental health care in physical care services is still insufficient.Thus,interdisciplinary communication across treatment providers is essential.The standardized language provided by the diagnostic statistical manual favors a clear conceptualization.However,this approach might not focus on the individual,as thinking in categories might impede recognizing the continuum from healthy to diseased.Psychoanalytic theory is concerned with an individual’s unconscious conflictual wishes and motivations,manifested through enactments like psychic symptoms or(maladaptive)behavior with long-term consequences if not considered.Such modifiable internal and external factors often are inadequately treated.However,together with the physical chronic condition constraints,these factors determine degrees of freedom for a self-determined existence.The effect of therapeutic interventions,and especially therapy adherence,relies on a solid therapeutic relationship.Outcome and process research still investigates the mechanism of change in psychotherapeutic treatments with psychanalysis’s focus on attachment problems.This article examines existing knowledge about the mechanism of change in psychoanalysis under the consideration of current trends emerging from psychotherapy research.A clinical example is discussed.Additionally,further directions for research are given.The theoretical frame in psychoanalytic therapies is the affect-cognitive interface.Subliminal affect-perception is enabled via awareness of subjective meanings in oneself and the other;shaping this awareness is the main intervention point.The interactional ingredients,the patient’s inherent bioenvironmental history meeting the clinician,are relevant variables.Several intrinsic,subliminal parameters relevant for changing behavior are observed.Therapeutic interventions aim at supporting the internalization of the superego’s functions and at making this ability available in moments of self-reflection.By supporting mentalization abilities,a better understanding of oneself and higher selfregulation(including emotional regulation)can lead to better judgments(application of formal logic and abstract thinking).Thus,this facilitates enduring behavior change with presumably positive effects on mental and physical health. | Dagmar Steinmair Henriette Löffler-Stastka | 2022 | World Journal of Clinical Cases2022，10，7： | 1 |
