| 1 | Prognostic signif icance of HIF-2α/EPAS1 expression in hepatocellular carcinoma显示文摘AIM: To evaluate the prognostic signif icance of HIF- 2α/EPAS1 expression in hepatocellular carcinoma (HCC). METHODS: Surgical specimens from 315 patients with HCC as well as 196 adjacent noncancerous lesions and 22 cases of normal liver tissue were investigated by immunohistochemistry (IHC) for HIF-2α/EPAS1 using a standard detection system. Correlations with clinicopathological factors, VEGF, microvessel density (MVD), and prognosis were analyzed. RESULTS: Immunoreactivity of HIF-2α/EPAS1 was positive in 69.5% of HCC, 55.6% of adjacent noncancerous tissue, and 0% of normal liver tissue. And it was significantly correlated with tumor grade, venous invasion, intrahepatic metastasis, necrosis, and capsule infiltration. Correlation analysis of HIF-2α/EPAS1 with angiogenic factor VEGF (P < 0.001), and MVD (P = 0.016) was also noted. HIF-2α/EPAS1 protein was less frequently expressed in low MVD cases, whereas a high rate of expression was noted in cases with both medium and high MVD (P = 0.042). By Kaplan-Meier analysis, strong HIF-2α/EPAS1 staining (> 50% of tumor cells) in HCC correlated with a shortened survival in patients (Cox's regression, P < 0.001, r = 3.699). CONCLUSION: We conclude that HIF-2α/EPAS1 expression may play an important role in tumor progression and prognosis of HCC. | 2 | Availability and Quality of Emergency Obstetric Care, an Alternative Strategy to Reduce Maternal Mortality: Experience of Tongji Hospital, Wuhan, China显示文摘The burden of maternal mortality (MM) and morbidity is especially high in Asia. However, China has made significant progress in reducing MM over the past two decades, and hence maternal death rate has declined considerably in last decade. To analyze availability and quality of emergency obstetric care (EmOC) received by women at Tongji Hospital, Wuhan, China, this study retrospectively analyzed various pregnancyrelated complications at the hospital from 2000 to 2009. Two baseline periods of equal length were used for the comparison of variables. A total of 11 223 obstetric complications leading to MM were identified on a total of 15 730 hospitalizations, either 71.35% of all activities. No maternal death was recorded. Mean age of women was 29.31 years with a wide range of 14-52 years. About 96.26% of women had higher levels of schooling, university degrees and above and received the education of secondary school or college. About 3.74% received primary education at period two (P2) from 2005 to 2009, which was significantly higher than that of period one (P1) from 2000 to 2004 (P<0.05) (OR: 0.586; 95% CI: 0.442 to 0.776). About 65.69% were employed as skilled or professional workers at P2, which was significantly higher than that of P1 (P<0.05). About 34.31% were unskilled workers at P2, which was significantly higher than that of P1 (P<0.05). Caesarean section was performed for 9,930 women (88.48%) and the percentage of the procedure increased significantly from 19.25% at P1 to 69.23% at P2 (P<0.05). We were led to conclude that, despite the progress, significant gaps in the performance of maternal health services between rural and urban areas remain. However, MM reduction can be achieved in China. | 3 | Anatomical variations of the cystic duct:Two case reports显示文摘Anatomical variations of the cystic duct often occur and may be encountered during cholecystectomy. Knowledge of the variable anatomy of the cystic duct and cysticohepatic junction is important to avoid significant ductal injury in biliary surgery. Here, we present two unusual cases with an anomalous cystic duct, namely, low lateral insertion and narrow-winding of the cystic duct. The first case was a 64-year-old man with cholelithiasis and chronic cholecystitis. During surgery, the entrance of the cystic duct was misidentified as being short and leading into the right hepatic duct. Further exploration showed multiple calculi in the right and common hepatic ducts. Cholecystectomy was completed, followed by T-tube drainage of the common and right hepatic ducts. Postoperative T-tube cholangiography demonstrated that the two T tubes were respectively located in the cystic and common hepatic duct. Six weeks later, the retained stones in the distal choledochus were extracted by cholangioscopy through the sinus tract of the T-tube. The second case was a 41-year-old woman, in which, preoperative endoscopic retrograde cholangiopancreatography (ERCP) revealed a long cystic duct, with a narrow and curved-in lumen. The patient underwent open cholecystectomy. Both patients were cured. Leaves and Roots of Securidaca longepedunculata Fresen on the Germination Rate of Cowpea Infected with Callosobruchus maculatus显示文摘Cowpea is a vegetable that contributes to food security in Guinea. Despite its importance, more than 30% of its production is lost between harvesting, storage and consumption. The main cowpea pest is Callosobruchus maculatus. The commonly used control strategy is essentially based on chemicals whose use is toxic, expensive and restrictive. In the search for alternatives to chemical control, this work was undertaken to evaluate the insecticidal potential of Ageratum conyzoid leaf powders and Securidaca longepedunculata roots. Powder toxicity and insecticidal efficacy tests were performed separately on groups of 20 Callosobruchus maculatus using 3 doses of Ageratum conyzoid leaf powders and Securidaca longepedunculata roots in jars each containing 100 g cowpea. The mortality of Callosobruchus maculatus and the impact of powders were noted respectively for 96 h and 4 months after infestation. The germination capacity of the treated seeds was assessed at the start of the study. Doses of 1 and 8 g of Ageratum conyzoides leaf powders and Securidaca longepedunculata roots were applied respectively to every 100 g of cowpea and led to 100% mortality of Callosobruchus maculatus. At these same doses, more than half of the treated seeds were attacked during the 4 months of storage. Germination tests have shown that powders of Ageratum conyzoides leaves and roots of Securidaca longepedunculata have no negative effect on the germination power of cowpea seeds. | 17 | Regulation of HIF-1 α to Expression of N-myc Downstream Regulated Gene 1 in Colorectal Carcinoma显示文摘Plasmid expressing small interfering RNA (siRNA) against HIF-1a (pSilence-2.1-U6-siRNA) was constructed and transfected into LS174T cells in hypoxia condition.After expression of siRNA against HIF-1α in LS174T cells,expressions of HIF-1α and N-myc downstream regulated gene 1 (NDRG1) gene were inhibited significantly. HIF-1α a transcripts were positive in 67.7% (42/62) and 44.4% (8/18) of colorectal adenocarcinoma and adenoma,respectively. The mean percentage of cells with positive hybridization of HIF-1α mRNA increases with the development from Duke stage A to stage C+D (p <0.05). The positive staining rate of NDRG1 protein was significant higher in colorectal adenocarcinoma group than that in colorectal adenoma group (p<0.05). The level of HIF-1 α transcripts was positively correlated with the level of NDRG1 protein (p <0.05) during colorectal tumor progression.
