| 1 | Comparison between combination therapy of percutaneous ethanol injection and radiofrequency ablation and radiofrequency ablation alone for patients with hepatocellular carcinoma显示文摘AIM: In the present study, the characteristics of PEI-RFA treatment were further elucidated by analyzing the relationship between the volume of coagulated necrosis and the energy requirement for ablation or the amount of ethanol injected into HCC.METHODS: The volume of coagulated necrosis, total energy requirement and energy requirement for coagulation of per unit volume were examined in the groups of PEI-RFA and RFA alone using the Cool-tip RF system.RESULTS: The results showed that the volume of coagulated necrosis induced was significantly larger in PEI-RFA group than in routine RFA group, when the total energy administered was comparable in both groups.In PEI-RFA, enlargement of coagulated necrosis was admitted in 3 dimensions and the amount of energy requirement per unit volume of coagulated necrosis was negatively correlated with the amount of ethanol injected into HCC.CONCLUSION: These results suggest that, compared to RFA alone, PEI-RFA enables to induce comparable coagulated necrosis with smaller energy requirement, and that PEI-RFA is likely to be less invasive than RFA alone irrespective of inducing enhanced coagulated necrosis.Thus, simple prior injection of ethanol may make RFA treatment more effective and less invasive for the treatment of patients with HCC. | Kazutaka Kurokohchi Seishiro Watanabe Tsutomu Masaki Naoki Hosomi Yoshiaki Miyauchi Takashi Himoto Yasuhiko Kimura Seiji Nakai Akihiro Deguchi Hirohito Yoneyama Shuhei Yoshida Shigeki Kuriyama | 2005 | World Journal of Gastroenterology2005，11，10： | 29 |
| 2 | Short-Term Responses of Nitrous Oxide Emissions and Concentration Profiles to Fertilization and Irrigation in Greenhouse Vegetable Cultivation显示文摘In vegetable cultivation, the majority of N2O emissions occur after fertilization; it is therefore important to understand any factors contributing to this process. An experiment was conducted to investigate short-term N2O dynamics following topdressing in a greenhouse vegetable field in South China. During two topdressing processes, three different urea-N treatments with irrigation were conducted in May and June in a tomato (Lycopersicum esculentum) cultivation. The N2O fluxes, soil concentration profiles and soil environments at the 0-60 cm depths at 10 cm intervals were measured both immediately prior to and 5 days after topdressing. The N2O fluxes before topdressing ranged from 6.7±2.1 to 55.0±28.8 μ g N m 2 h 1 ; even higher numbers were recorded in highly fertilized plots. The NO3-N accumulation in the soil caused by vegetable cultivation during the 5 years prior to the start of the experiment, resulted in high background N2O fluxes. One day after topdressing (1 DAT) in May and June, N2O fluxes increased, which coincided with sharp increases in soil N2O concentrations at depths of 2.5 and 15 cm and in NO3-N and NH+4 -N contents at depths of 0-20 cm. From 1 to 5 DAT, fluctuations in the N2O fluxes did not harmonize with the N2O concentrations at a depth of 2.5 cm, which was attributed to different gas diffusion rates at depths of 0-10 cm. These results suggested that surface soil N and environmental conditions were crucial for determining the short-term N2O ebullitions during topdressing in greenhouse vegetable cultivation. | S. | 3 | Acupuncture Treatment for Idiopathic Trigeminal Neuralgia:A Longitudinal Case-Control Double Blinded Study显示文摘Objective: To evaluate the treatment effect of acupuncture on patients with idiopathic trigeminal neuralgia(ITN) by case-control longitudinal blinded study. Methods: Sixty ITN patients and 30 healthy subjects were included. The ITN patients were randomly assigned to acupuncture group(15 cases), sham-acupuncture group(15 cases) and carbamazepine group(30 cases), respectively. Clinical orofacial evaluation(including pain intensity and medication doses), research diagnostic criteria for temporomandibular disorders(RDC/TMD) and Helkimo indexes(for functional evaluation of the masticatory system), and quantitative sensory testing for sensory thresholds(gustative, olfactory, cold, warm, touch, vibration and superficial and deep pain) were evaluated before treatment, immediately after treatment, and 6 months after treatment. Results: The mean pain intensity by the Visual Analogue Scale only decreased in the acupuncture group at the last evaluation(P=0.012). Patients in the sham-acupuncture group had an increase in carbamazepine doses according to the prescriptions(P<0.01). There was a reduction in secondary myofascial pain and mandibular limitations at the acupuncture and sham-acupuncture groups, however only the acupuncture group kept the changes after 6 months(P<0.01, P=0.023). There was a decrease in mechanical thresholds in the acupuncture group(tactile, P<0.01; vibration, P=0.027) and an increase in deep pain thresholds in both acupuncture and sham-acupuncture groups(P=0.013). | 4 | Measurement of prostaglandin metabolites is useful in diagnosis of small bowel ulcerations显示文摘BACKGROUND We recently reported on a hereditary enteropathy associated with a gene encoding a prostaglandin transporter and referred to as chronic enteropathy associated with SLCO2 A1 gene(CEAS). Crohn's disease(CD) is a major differential diagnosis of CEAS, because these diseases share some clinical features. Therefore, there is a need to develop a convenient screening test to distinguish CEAS from CD.AIM To examine whether prostaglandin E major urinary metabolites(PGE-MUM) can serve as a biomarker to distinguish CEAS from CD.METHODS This was a transactional study of 20 patients with CEAS and 98 patients with CD.CEAS was diagnosed by the confirmation of homozygous or compound heterozygous mutation of SLCO2 A1. We measured the concentration of PGEMUM in spot urine by radioimmunoassay, and the concentration was compared between the two groups of patients. We also determined the optimal cut-off value of PGE-MUM to distinguish CEAS from CD by receiver operating characteristic(ROC) curve analysis.RESULTS Twenty Japanese patients with CEAS and 98 patients with CD were enrolled.PGE-MUM concentration in patients with CEAS was significantly higher than that in patients with CD(median 102.7 vs 27.9 μg/g × Cre, P < 0.0001). One log unit increase in PGE-MUM contributed to 7.3 increase in the likelihood for the diagnosis of CEAS [95% confidence interval(CI) 3.2-16.7]. A logistic regression analysis revealed that the association was significant even after adjusting confounding factors(adjusted odds ratio 29.6, 95%CI 4.7-185.7).
