| 5 | Obstructive Sleep Apnea Syndrome Phenotyping by Cluster Analysis: Typical Sleepy, Obese Middle-aged Men with Desaturating Events are A Minority of Patients in A Multi-ethnic Cohort of 33% Women显示文摘Objective Several clinical obstructive sleep apnea syndrome(OSAS)phenotypes associated with heterogeneous cardiovascular risk profiles have been recently identified.The purpose of this study was to identify clusters amongst these profiles that allow for the differentiation of patients.Methods This retrospective study included all moderate-to-severe OSAS patients referred to the sleep unit over a 5-year period.Demographic,symptom,comorbidity,polysomnographic,and continuous positive airway pressure(CPAP)adherence data were collected.Statistical analyses were performed to identify clusters of patients.Results A total of 567 patients were included(67%men,54±13 years,body mass index:32±7 kg/m2,65%Caucasian,32%European African).Five clusters were identified:less severe OSAS(n=172);healthier severe OSAS(n=160);poorly sleeping OSAS patients with cardiometabolic comorbidities(n=87);younger obese men with sleepiness at the wheel(n=94);sleepy obese men with very severe desaturating OSAS and cardiometabolic comorbidities(n=54).Patients in clusters 3 and 5 were older than those in clusters 2 and 4(P=0.034).Patients in clusters 4 and 5 were significantly more obese than those in the other clusters(P=0.04).No significant differences were detected in terms of symptoms and comorbidities.Polysomnographic profiles were very discriminating between clusters.CPAP adherence was similar in all clusters but,among adherent patients,daily usage was more important in cluster 1(less severe patients)than in cluster 5.Conclusion This study highlights that the typical sleepy obese middle-aged men with desaturating events represent only a minority of patients in our multi-ethnic moderate-to-severe OSAS cohort of 33%females. | Chloe Van Overstraeten Fabio Andreozzi Sidali Ben Youssef Ionela Bold Sarah Carlier Alexia Gruwez Anne-Violette Bruyneel Marie Bruyneel | 2021 | Current Medical Science2021，41，4： | 1 |

| 15 | Oxaliplatin-induced neuropathy and colo-rectal cancer patient's quality of life:Practical lessons from a prospective cross-sectional,real-world study显示文摘BACKGROUND Colon cancer is one the most common forms of cancer in both sexes.Due to important progress in the field of early detection and effective treatment,colon and rectal cancer survivors currently account for 10%of cancer survivors worldwide.However,the effects of anti-cancer treatments,especially oxaliplatinbased chemotherapy,on the quality of life(QoL)have been less evaluated.Although the incidence of severe chemotherapy-induced neuropathy(CIPN)in clinical studies is below 20%,data from real-world studies is scarce,and CIPN is probably under-reported due to patient selection and the patients’fear that reporting side-effects might lead to treatment cessation.AIM To determine the impact of CIPN on QoL in colorectal cancer patients with a recent history of oxaliplatin-based chemotherapy.METHODS We performed a prospective cross-sectional study in two major Romanian oncology tertiary hospitals—the Regional Institute of Oncology Ia?i(Iasi,Romania)and the Fundeni Clinical Oncology Institute(Bucharest,Romania).All consecutive patients with colon or rectal cancer,undergoing Oxaliplatin-based chemotherapy that consented to enroll in the study,were assessed by means of two questionnaires—the EORTC QQ-CR29(quality of life in colon and rectal cancer patients)and the QLQ-CIPN20(assessment of neuropathy).Several demographical,social,clinical and treatment data were also collected.Statistical analysis was performed by means of SPSS v20.The student t test was used to assess the relationship between the QLQ-CIPN20 and QLQ-CR29 results.Kaplan Meyer-curves were used to report 3-year progression-free survival(PFS)in patients that discontinued chemotherapy vs those that completed the recommended course.RESULTS Of the 267 patients that fulfilled the inclusion criteria in the pre-specified time frame,101(37.8%)agreed to participate in the clinical study.At the time of the enrolment in the study,over 50%of the patients had recently interrupted their oxaliplatin-based chemotherapy,most often due to neuropathy.Almost 85%of the responders reported having tingling or numbness in their fingers or hands,symptoms that were associated with pain in over 20%of the cases.When comparing the scores in the two questionnaires,a statistically significant relationship(P<0.001)was found between the presence of neuropathic symptoms and a decreased quality of life.This correlation was consistent when the patients were stratified by sex,disease stage,comorbidities and the presence of stoma or treatment type,suggesting that neuropathy in itself may be a reason for a decreased quality of life.At the 3 year final assessment,median recurrence-free survival in stageⅢpatients was 26.88 mo.When stratified by completion of chemotherapy,median recurrence freesurvival of stageⅢpatients that completed chemotherapy was 28.27 mo vs 24.33 mo in patients that discontinued chemotherapy due to toxicity,a difference that did not reach statistical significance.CONCLUSION CIPN significantly impacts QoL in colorectal cancer patients.CIPN is also the most frequent reason for treatment discontinuation.Physicians should actively assess for CIPN in order to prevent chronic neuropathy. | Iulian Prutianu Teodora Alexa-Stratulat Elena Octaviana Cristea Andrei Nicolau Diana Cornelia Moisuc Alina Alexandra Covrig Karina Ivanov Adina Emilia Croitoru Monica Ionela Miron Mihaela Ioana Dinu Anca Viorica Ivanov Mihai Vasile Marinca Iulian Radu Bogdan Gafton | 2022 | World Journal of Clinical Cases2022，10，10： | 0 |

| 17 | Management of choroidal metastasis using external beam radiotherapy: a retrospective study and review of the literature显示文摘Aim:Choroidal metastases are rare in the evolution of solid cancers and constitute exceptional metastatic sites involving functional visual prognosis.The authors conducted a retrospective study to determine the interest of external radiotherapy for the treatment of choroidal metastases.Methods:The authors reviewed the records of 28 patients with choroidal metastases who had breast(n=15),lung(n=9),ovarian(n=1),kidney(n=1),prostate(n=1)cancer or carcinoma with unknown primitive at the moment of the diagnosis(n=1).The median age was 58 years(extremes:34-71 years).Tumor stage before the discovery of metastatic choroidal metastasis was 50%of patients.Ocular involvement was unilateral(n=22)or bilateral(n=6).The delivered doses ranged from 20 to 50 Gy fractionated with 3-5 Gy in 2D technique(n=5),conformational(n=21),intensity modulation(n=2).The most widely used prescription scheme delivered 30 Gy in 10 fractions(64%)using two 6 MV photons beams.Results:At the end of irradiation,13 patients(46%)showed an improvement of eye symptoms.For the others,a stabilization in symptoms was noted(n=15).No patient had visual degradation.No acute or late grade 2-3 toxicities were objectified.The histological type did not influence the response(P=0.5).There was no dose relationship-response in our series.Conclusion:External radiation therapy is a useful technique in the palliative treatment of choroidal metastases.Acute and late toxicities are acceptable. | Zakaria Ahmed Youbi Enachescu Ciprian Caraivan Ionela Bennani Zineb Khounigere Majdouline Kotzki Léa Pialat Pierre-Marie Sesques Pierre Yossi Séna | 2017 | Journal of Cancer Metastasis and Treatment2017，3，1： | 0 |
