3Transplantation of mesenchymal stem cells from human umbilical cord versus human umbilical cord blood for peripheral nerve regeneration显示文摘BACKGROUND: Mesenchymal stem cells (MSCs) appear to be a good alternative to Schwann cells in the treatment of peripheral nerve injury. Fetal stem cells, like umbilical cord blood (UCB) and umbilical cord (UC) stem cells, have several advantages over adult stem cells. OBJECTIVE: To assess the effects of UC-derived MSCs (UCMSCs) and UCB-derived MSCs (UCBMSCs) in repair of sciatic nerve defects. DESIGN, TIME AND SETTING: A randomized controlled animal experiment was performed at the laboratory of Department of Oral and Maxillofacial Surgery, Seoul National University Dental Hospital, from July to December 2009. MATERIALS: UCMSCs were provided by the Research Institute of Biotechnology, Dongguk University. UCBMSCs were provided by the Laboratory of Stem Cells and Tumor Biology, College of Veterinary Medicine, Seoul National University. Dulbecco's modified Eagle's medium (DMEM) was purchased from Gibco-BRL, USA. METHODS: Seven-week-old Sprague-Dawley rats were randomly and evenly divided into three groups: DMEM, UCBMSCs, and UCMSCs. A 10-mm defect in the left sciatic nerve was constructed in all rats. DMEM (15 μL) containing 1 × 106 UCBMSCs or UCMSCs was injected into the gap between nerve stumps, with the surrounding epineurium as a natural conduit. For the DMEM group, simple DMEM was injected. MAIN OUTCOME MEASURES: At 7 weeks after sciatic nerve dissection, dorsal root ganglia neurons were labeled by fluorogold retrograde labeling. At 8 weeks, electrophysiology and histomorphometry were performed. At 2, 4, 6, and 8 weeks after surgery, sciatic nerve function was evaluated using gait analysis. RESULTS: The UCBMSCs group and the UCMSCs group exhibited similar sciatic nerve function and electrophysiological indices, which were better than the DMEM group, as measured by gait analysis (P < 0.05). Fluorogold retrograde labeling of sciatic nerve revealed that the UCBMSCs group demonstrated a higher number of labeled neurons; however, the differences were not significant. Histomorphometric indices were similar in the UCBMSCs and UCMSCs groups, and total axon counts, particularly axon density (P < 0.05), were significantly greater in the UCBMSCs and UCMSCs groups than in the DMEM group. CONCLUSION: Transplanting either UCBMSCs or UCMSCs into axotomized sciatic nerves could accelerate and promote sciatic nerve regeneration over 8 weeks. 4Low-frequency pulsed electromagnetic field pretreated bone marrow-derived mesenchymal stem cells promote the regeneration of crush-injured rat mental nerve显示文摘Bone marrow-derived mesenchymal stem cells(BMSCs) have been shown to promote the regeneration of injured peripheral nerves. Pulsed electromagnetic field(PEMF) reportedly promotes the proliferation and neuronal differentiation of BMSCs. Low-frequency PEMF can induce the neuronal differentiation of BMSCs in the absence of nerve growth factors. This study was designed to investigate the effects of low-frequency PEMF pretreatment on the proliferation and function of BMSCs and the effects of low-frequency PEMF pre-treated BMSCs on the regeneration of injured peripheral nerve using in vitro and in vivo experiments. In in vitro experiments, quantitative DNA analysis was performed to determine the proliferation of BMSCs, and reverse transcription-polymerase chain reaction was performed to detect S100(Schwann cell marker), glial fibrillary acidic protein(astrocyte marker), and brain-derived neurotrophic factor and nerve growth factor(neurotrophic factors) m RNA expression. In the in vivo experiments, rat models of crush-injured mental nerve established using clamp method were randomly injected with low-frequency PEMF pretreated BMSCs, unpretreated BMSCs or PBS at the injury site(1 × 106 cells). Di I-labeled BMSCs injected at the injury site were counted under the fluorescence microscope to determine cell survival. One or two weeks after cell injection, functional recovery of the injured nerve was assessed using the sensory test with von Frey filaments. Two weeks after cell injection, axonal regeneration was evaluated using histomorphometric analysis and retrograde labeling of trigeminal ganglion neurons. In vitro experiment results revealed that low-frequency PEMF pretreated BMSCs proliferated faster and had greater m RNA expression of growth factors than unpretreated BMSCs. In vivo experiment results revealed that compared with injection of unpretreated BMSCs, injection of low-frequency PEMF pretreated BMSCs led to higher myelinated axon count and axon density and more Di I-labeled neurons in the trigeminal ganglia, contributing to rapider functional recovery of injured mental nerve. 5Endoscopic and clinicopathologic characteristics of early gastric cancer with high microsatellite instability显示文摘AIM:To investigate endoscopic and clinicopathologic characteristics of early gastric cancer (EGC) according to microsatellite instability phenotype.METHODS:Data were retrospectively collected from a single tertiary referral center.Of 981 EGC patients surgically treated between December 2003 and October 2007,73 consecutive EGC patients with two or more microsatellite instability (MSI) mutation [high MSI (MSI-H)] and 146 consecutive EGC patients with one or no MSI mutation (non-MSI-H) were selected.The endoscopic and clinicopathologic features were compared between the MSI-H and non-MSI-H EGC groups.RESULTS:In terms of endoscopic characteristics,MSI-H EGCs more frequently presented with elevated pattern (OR 4.38,95% CI:2.40-8.01,P < 0.001),moderateto-severe atrophy in the surrounding mucosa (OR 1.91,95% CI:1.05-3.47,P = 0.033),antral location (OR 3.99,95% CI:2.12-7.52,P < 0.001) and synchronous lesions,compared to non-MSI-H EGCs (OR 2.65,95% CI:1.16-6.07,P = 0.021).Other significant clinicopathologic characteristics of MSI-H EGC included predominance of female sex (OR 2.77,95% CI:1.53-4.99,P < 0.001),older age (> 70 years) (OR 3.30,95% CI:1.57-6.92,P = 0.002),better histologic differentiation (OR 2.35,95% CI:1.27-4.34,P = 0.007),intestinal type by Lauren classification (OR 2.34,95% CI:1.15-4.76,P = 0.019),absence of a signet ring cell component (OR 2.44,95% CI:1.02-5.86,P = 0.046),presence of mucinous component (OR 5.06,95% CI:1.27-20.17,P = 0.022),moderate-to-severe lymphoid stromal reaction (OR 3.95,95% CI:1.59-9.80,P = 0.003),and co-existing underlying adenoma (OR 2.66,95% CI:1.43-4.95,P = 0.002).CONCLUSION:MSI-H EGC is associated with unique endoscopic and clinicopathologic characteristics including frequent presentation in protruded type,co-existing underlying adenoma,and synchronous lesions.Jaehoon Jahng Young Hoon Youn Kwang Hyun Kim Junghwan Yu Yong Chan Lee Woo Jin Hyung Sung Hoon Noh Hyunki Kim Hoguen Kim Hyojin Park Sang In Lee 2012World Journal of Gastroenterology2012,18,27:6
6Human umbilical cord blood-derived mesenchymal stem cells promote regeneration of crush-injured rat sciatic nerves显示文摘Several studies have demonstrated that human umbilical cord blood-derived mesenchymal stem cells can promote neural regeneration following brain injury. However, the therapeutic effects of human umbilical cord blood-derived mesenchymal stem cells in guiding peripheral nerve regeneration remain poorly understood. This study was designed to investigate the effects of human umbilical cord blood-derived mesenchymal stem cells on neural regeneration using a rat sciatic nerve crush injury model. Human umbilical cord blood-derived mesenchymal stem cells (1 × 10 6 ) or a PBS control were injected into the crush-injured segment of the sciatic nerve. Four weeks after cell injection, brain-derived neurotrophic factor and tyrosine kinase receptor B mRNA expression at the lesion site was increased in comparison to control. Furthermore, sciatic function index, Fluoro Gold-labeled neuron counts and axon density were also significantly increased when compared with control. 7A comparative study on the alternating mesophilic and thermophilic two-stage anaerobic digestion of food waste显示文摘An alternating mesophilic and thermophilic two stage anaerobic digestion(AD) process was conducted. The temperature of the acidogenic(A) and methanogenic(M) reactors was controlled as follows: System 1(S1) mesophilic A-mesophilic M;(S2) mesophilic A-thermophilic M; and(S3)thermophilic A-mesophilic M. Initially, the AD reactor was acclimatized and inoculated with digester sludge. Food waste was added with the soluble chemical oxygen demand(SCOD) concentrations of41.4–47.0 g/L and volatile fatty acids of 2.0–3.2 g/L. Based on the results, the highest total chemical oxygen demand removal(86.6%) was recorded in S2 while S3 exhibited the highest SCOD removal(96.6%). Comparing S1 with S2, total solids removal increased by 0.5%; S3 on the other hand decreased by 0.1 % as compared to S1. However, volatile solids(VS) removal in S1, S2, and S3was 78.5%, 81.7%, and 79.2%, respectively. S2 also exhibited the highest CH4 content, yield, and production rate of 70.7%, 0.44 L CH4/g VSadded, and 1.23 L CH4/(L·day), respectively. Bacterial community structure revealed that the richness, diversity, evenness, and dominance of S2 were high except for the archaeal community. The terminal restriction fragments dendrogram also revealed that the microbial community of the acidogenic and methanogenic reactors in S2 was distinct. 8Staining for p53 and Ki-67 increases the sensitivity of EUS-FNA to detect pancreatic malignancy显示文摘AIM:To investigate whether tumor marker staining can improve the sensitivity of endoscopic ultrasound-guided fine needle aspiration(EUS-FNA)to diagnose pancreatic malignancy. METHODS:Patients who underwent EUS-FNA were retrospectively identified.Each EUS-FNA specimen was evaluated by routine cytology and stained for tumor markers p53,Ki-67,carcinoembryonic antigen(CEA) and CA19-9.Sensitivity,specificity,positive and negative predictive values(PPV and NPV),and positive and negative likelihood ratios(PLR and NLR)were calculated in order to evaluate the performance of each test to detect malignancy. RESULTS:Sixty-one specimens had complete sets of stains,yielding 49 and 12 specimens from pancreatic adenocarcinomas and benign pancreatic lesions due to pancreatitis,respectively.Cytology alone had sensitivity and specificity of 41%and 100%to detect malignancy, respectively.In 46%of the specimens,routine cytology alone was deemed indeterminate.The addition of either p53 or Ki-67 increased the sensitivity to 51%and 53%,respectively,with perfect specificity,PPV and PLR (100%,100%and infinite).Both stains in combination increased the sensitivity to 57%.While additional staining with CEA and CA19-9 further increased the sensitivity to 86%,the specificity,PPV and PLR were significantly reduced(at minimum 42%,84%and 1,respectively).Markers in all combinations performed poorly as a negative test(NPV 26%to 47%,and NLR 0.27 and 0.70).CONCLUSION:Immunohistochemical staining for p53 and Ki-67 can improve the sensitivity of EUS-FNA to diagnose pancreatic adenocarcinoma.Alexander W Jahng Sonya Reicher David Chung Donna Varela Rahul Chhablani Anil Dev Binh Pham Jose Nieto Rose J Venegas Samuel W French Bruce E Stabile Viktor E Eysselein 2010World Journal of Gastrointestinal Endoscopy2010,2,11:3
