1结合硬斑块特征的心血管内超声图像中-外膜边缘检测显示文摘针对心血管内超声(IVUS)图像中-外膜(media-adventitia)边缘检测中,硬斑块和声影等造成中-外膜边缘难以准确检测的问题,提出一种结合硬斑块特征的中-外膜边缘检测方法。首先,采用k均值聚类分割IVUS图像,根据图像中不同类型斑块的成像特点检测硬斑块位置;然后,计算IVUS图像方向梯度,结合斑块位置和图像灰度,获得代价矩阵;最后,利用启发式图搜索方法,实现IVUS图像的中-外膜边缘检测。对临床图像的实验结果表明,本方法能克服图像中声影和斑块干扰等问题,使检测正确率达到95.57%,提高了IVUS图像中-外膜边缘检测的准确性。邢栋 杨丰 黄靖 涂圣贤 DIJKSTRA Jouke 2012中国生物医学工程学报2012,31,1:7
2社区卫生服务守门人好不好——欧洲18个国家病人满意度与转诊制度的关系显示文摘目的探讨守门人制度与人群对全科医生服务满意度的关系。方法研究涉及了18个国家,包括欧盟成员国及挪威、冰岛和瑞士。本研究有两个数据来源:(1)请各国专家选择17种不同类型的卫生服务,然后计算各种服务的“直接可及性”,即病人可以自由选择的服务占总服务的比例;(2)采用EUROPEP工具,测量病人对全科医生服务的满意度,该测量涉及了14个欧洲国家。结果直接获得卫生服务越多的国家的病人对全科医生服务的满意度越高于需要转诊的国家(r=0.54,P=0.05)。对全科医学服务组织方面(即服务的安排,主要考虑到等待时间和预约可能)的满意度与服务“直接可及性”呈正相关(r=0.67,P=0.01)。对医患沟通的满意度以及对医学服务技术的满意度与“直接可及性”无关(r=0.46、0.41,P>0.05)。结论病人认为“直接可及性”是很重要的。如果病人可以自由地选择卫生服务提供者的类型,那么就会给全科医生服务更积极的评价。不过这种满意主要表现在全科医学服务的组织方面。是否能直接可及各种服务并不影响病人对全科医生服务质量的评价。杨辉 Madelon W.Kroneman Hans Maarse Jouke Vander Zee 2007中国全科医学2007,10,1:4

17Optimized Three-Dimensional Cardiovascular Magnetic Resonance Whole Heart Imaging Utilizing Non-Selective Excitation and Compressed Sensing in Children and Adults with Congenital Heart Disease显示文摘Background:In congenital heart disease(CHD)patients,detailed three-dimensional anatomy depiction plays a pivotal role for diagnosis and therapeutical decision making.Hence,the present study investigated the applicability of an advanced cardiovascular magnetic resonance(CMR)whole heart imaging approach utilizing nonselective excitation and compressed sensing for anatomical assessment and interventional guidance of CHD patients in comparison to conventional dynamic CMR angiography.Methods:86 consecutive pediatric patients and adults with congenital heart disease(age,1 to 74 years;mean,35 years)underwent CMR imaging including a freebreathing,ECG-triggered 3D nonselective SSFP whole heart acquisition using compressed SENSE(nsWHcs).Anatomical assessability and signal intensity ratio(SIR)measurements were compared with conventional dynamic 3D-/4D-MR angiography.Results:The most frequent diagnoses were partial anomalous pulmonary venous drainage(17/86,20%),transposition of the great arteries(15/86,17%),tetralogy of Fallot(12/86,14%),and a single ventricle(7/86,8%).Image quality of nsWHcs was rated as excellent/good in 98%of patients.nsWHcs resulted in a reliable depiction of all large thoracic vessels(anatomic assessability,99%–100%)and the proximal segments of coronary arteries and coronary sinus(>90%).nsWHcs achieved a homogenously distributed SIR in all cardiac cavities and thoracic vessels without a significant difference between pulmonary and systemic circulation(10.9±3.5 and 10.6±3.4;p=0.15),while 3D angiography showed significantly increased SIR for targeted vs.non-targeted circulation(PA-angiography,15.2±8.1 vs.5.8±3.6,p<0.001;PV-angiography,7.0±3.9 vs.17.3±6.8,p<0.001).Conclusions:The proposed nsWHcs imaging approach provided a consistently high image quality and a homogeneous signal intensity distribution within the pulmonary and systemic circulation in pediatric patients and adults with a wide spectrum of congenital heart diseases.nsWHcs enabled detailed anatomical assessment and three-dimensional reconstruction of all cardiac cavities and large thoracic vessels and can be regarded particularly useful for preprocedural planning and interventional guidance in CHD patients.Ingo Paetsch Roman Gebauer Christian Paech Frank-Thomas Riede Sabrina Oebel Andreas Bollmann Christian Stehning Jouke Smink Ingo Daehnert Cosima Jahnke 2023Congenital Heart Disease2023,18,3:0
18INTENSIFICATION OF GRASSLAND-BASED DAIRY PRODUCTION AND ITS IMPACTS ON LAND,NITROGEN AND PHOSPHORUS USE EFFICIENCIES显示文摘Many grassland-based dairy farms are intensifying production,i.e.,produce more milk per ha of land in response to the increasing demand for milk(by about 2%per year)in a globalized market.However,intensive dairy farming has been implicated for its resources use,ammonia and greenhouse gas emissions,and eutrophication impacts.This paper addresses the question of how the intensity of dairy production relates to N and P surpluses and use efficiencies on farms subjected to agri-environmental regulations.Detailed monitoring data were analyzed from 2858 grassland-based dairy farms in The Netherlands for the year 2015.The farms produced on average 925 Mg·yr^(-1)milk.Milk production per ha ranged from<10 to>30 Mg·ha^(-1)·yr^(-1).Purchased feed and manure export strongly increased with the level of intensification.Surpluses of N and P at farm level remained constant and ammonia emissions per kg milk decreased with the level of intensification.In conclusion,N and P surpluses did not differ much among dairy farms greatly differing in intensity due to legal N and P application limits and obligatory export of manure surpluses to other farms.Further,N and P use efficiencies also did not differ among dairy farms differing in intensity provided the externalization of feed production was accounted for.This paper provides lessons for proper monitoring and control of N and P cycling in dairy farming.Jouke OENEMA Oene OENEMA 2021Frontiers of Agricultural Science and Engineering2021,8,1:0
19Intravascular Ultrasound Image Hard Plaque Recognition and Media-adventitia Border Detection显示文摘Intravascular ultrasound (IVUS) is a new technology for the diagnosis of coronary artery disease, and for the support of coronary intervention. IVUS image segmentation often encounters difficulties when plaque and acoustic shadow are present A novel approach for hard plaque recognition and media-adventitia border detection of IVUS images is presented in this paper. The IVUS images were first enhanced by a spatial-frequency domain filter that was constructed by the directional filter and histogram equalization. Then, the hard plaque was recognized based on the intensity variation within different regions that were obtained using the k-means algorithm. In the next step, a cost matrix representing the probability of the media-adventitia border was generated by combining image gradient, plaque location and image intensity. A heuristic graph-searching was applied to find the media-adventitia border from the cost matrix.Experiment results showed that the accuracy of hard plaque recognition and media-adventitia border detection was 89.94% and 95.57%, respectively.
