1Different profile of cytokine synthesis according to the severity of acute pancreatitis显示文摘AIM: To evaluate the cellular synthetic ability of cytokines involved in pro- and anti-inflammatory reactions in patients with AP.METHODS: Sixty-seven patients with AP (16 severe,51 mild) and 10 controls were included in the study.Cultures of whole blood were performed in samples obtained within the first 72 h from the onset of pain.Serum levels of interleukins (IL) 6, 8, 10, and TNF-α were measured at baseline and in the supernatant of cultures with (functional reserve) or without stimulation with phytohemaglutinin.RESULTS: Basal levels of cytokines were significantly higher in patients with severe AP. A significant increase of all pro-inflammatory cytokines vs basal levels was observed in the supernatant after 24 h of whole blood cultures in patients, but not in controls. In contrast, IL-10 increased significantly in the supernatant of cultures only in patients with mild AP. Cells showed a statistically significant functional reserve for all IL in patients with mild, but only for pro-inflammatory cytokines in patients with severe AP.CONCLUSION: A marked activation of immune system may be observed in patients with AP, being balanced between pro- and anti-inflammatory cytokines in patients with mild but not severe AP. 4Management of nonalcoholic fatty liver disease:An evidence-based clinical practice review显示文摘AIM:To build a consensus among Chilean specialists on the appropriate management of patients with nonalcoholic fatty liver disease(NAFLD)in clinical practice.METHODS:NAFLD has now reached epidemic proportions worldwide.The optimal treatment for NAFLD has not been established due to a lack of evidence-based recommendations.An expert panel of members of the Chilean Gastroenterological Society and the Chilean Hepatology Association conducted a structured analysis of the current literature on NAFLD therapy.The quality of the evidence and the level of recommendations supporting each statement were assessed according to the recommendations of the United States Preventive Services Task Force.A modified three-round Delphi technique was used to reach a consensus among the experts.RESULTS:A group of thirteen experts was established.The survey included 17 open-ended questions that were distributed among the experts,who assessed the articles associated with each question.The levels of agreement achieved by the panel were 93.8%in the first round and 100%in the second and third rounds.The final recommendations support the indication of lifestyle changes,including diet and exercise,for all patients with NAFLD.Proven pharmacological therapies include only vitamin E and pioglitazone,which can be used in nondiabetic patients with biopsy-proven nonalcoholic steatohepatitis(the progressive form of NAFLD),although the long-term safety and efficacy of these therapies have not yet been established.CONCLUSION:Current NAFLD management is rapidly evolving,and new pathophysiology-based therapies are expected to be introduced in the near future.All NAFLD patients should be evaluated using a three-focused approach that considers the risks of liver disease,diabetes and cardiovascular events.Juan P Arab Roberto Candia Rodrigo Zapata Cristián Mu?oz Juan P Arancibia Jaime Poniachik Alejandro Soza Francisco Fuster Javier Brahm Edgar Sanhueza Jorge Contreras M Carolina Cuellar Marco Arrese Arnoldo Riquelme 2014World Journal of Gastroenterology2014,20,34:11

6miR-93-5p Transferred by Exosomes Promotes the Proliferation of Esophageal Cancer Cells via Intercellular Communication by Targeting PTEN显示文摘Objective To investigate the relationship between plasma miR-93-5p and the risk of esophageal cancer, as well as the influence of miR-93-5p on the biological function of esophageal cancer cells,exerted through exosomes.Methods The expression of plasma miR-93-5p in esophageal cancer patients and healthy controls was analysed by real-time quantitative PCR. The influence of miR-93-5p on the risk and prognosis of esophageal carcinoma was analyzed by conditional logistic regression and survival analysis. The effect of miR-93-5p on the biological function of recipient cells was investigated by establishing an in vitro donor cell co-culture model. 7Peritoneal metastases of colorectal origin treated by cytoreduction and HIPEC: An overview显示文摘Colorectal peritoneal carcinomatosis was considered a terminal condition with a merely palliative treatment that included only supportive care, palliative surgery and the best systemic chemotherapy. Since the birth of a new approach, cytoreductive surgery with peritonectomy procedures together with hyperthermic intraperitoneal chemotherapy and/or early postoperative intraperitoneal chemotherapy to treat peritoneal carcinomatosis, many research groups contributed with promising results using this procedure being up to date this strategy the only one that has shown curative benefits on colorectal peritoneal carcinomatosis achieving reported overall survival rates up to 64 mo and fiveyear survival rates up to 51%. 9Epidemiology of urticaria in China:a population-based study显示文摘Background:Urticaria is a common skin disease characterized by episodes of wheals,and it has a negative effect on patients’quality of life.Large-scale population-based epidemiological studies of urticaria are scarce in China.The aim of this survey was to determine the prevalence,clinical forms,and risk factors of urticaria in the Chinese population.Methods:This survey was conducted in 35 cities from 31 provinces,autonomous regions,and municipalities of China.Two to three communities in each city were selected in this investigation.Participants completed questionnaires and received dermatological examinations.We analyzed the prevalence,clinical forms,and risk factors of urticaria.Results:In total,44,875 questionnaires were distributed and 41,041 valid questionnaires were collected(17,563 male and 23,478 female participants).The lifetime prevalence of urticaria was 7.30%,with 8.26%in female and 6.34%in male individuals(P<0.05).The point prevalence of urticaria was 0.75%,with 0.79%in female and 0.71%in male individuals(P<0.05).Concomitant angioedema was found in 6.16%of patients.Adults had a higher prevalence of urticaria than adolescents and children.Living in urban areas,exposure to pollutants,an anxious or depressed psychological status,a personal and family history of allergy,thyroid diseases,and Helicobacter pylori infection were associated with a higher prevalence of urticaria.Smoking was correlated with a reduced risk of urticaria.Conclusion:This study demonstrated that the lifetime prevalence of urticaria was 7.30%and the point prevalence was 0.75%in the Chinese population;women had a higher prevalence of urticaria than men.Various factors were correlated with urticaria.Jiaqing Li Dandan Mao Shuoshuo Liu Ping Liu Jing Tian Chenhong Xue Xiaojing Liu Ruiqun Qi Bingxue Bai Jianjun Nie Siqi Ye Yu Wang Yuye Li Qing Sun Juan Tao Shuping Guo Hong Fang Jianqin Wang Qiri Mu Quanzhong Liu Yan Ding Jianzhong Zhang 2022Chinese Medical Journal2022,,11:6
10Alteration of the microRNA expression profile and identification of miRNA/mRNA negative regulation pairs in neural tube defects显示文摘MicroRNAs (miRNAs) are small, noncoding transcripts, and ~22 nucleotides long, which belong to a class of endogenous small regulatory RNA molecules that target mRNAs and trigger either translational suppression or mRNA degradation [1–3]. Several studies have shown that miRNAs play crucial roles in neurogenesis and development of nervous system [4,5] and have distinct expression patterns within the developing brain and central nervous system (CNS)[6]. Abnormal expression of miRNAs has also been reported in dysregulated neural tube (NT) development including NT defects (NTDs)[7,8]. To date, however, all NTD-related miRNA profiles were obtained from different microarray platforms, which always contain limited probes, and the data obtained are less precise in quantitative analysis. No quantitatively profiled global miRNA expression in mouse NTD fetuses has been characterized so far. To gain further insight of the possible roles of miRNAs in NTDs, we used deep sequencing to carry out a comprehensive survey of miRNA expression in all-trans retinoic acid (RA)-induced NTD mouse fetuses collected from three different embryonic time points (E8.5, E9.5, and E10.5). 11High levels of circulating GM-CSF^(+) CD4^(+) T cells are predictive of poor outcomes in sepsis patients: a prospective cohort study显示文摘Granulocyte colony-stimulating factor(GM-CSF),produced by CD4^(+)T cells,has recently been implicated in the pathogenesis of inflammatory diseases,such as multiple sclerosis and juvenile arthritis.However,the role of GM-CSF-producing CD4^(+)T cells in sepsis remains unknown.This study reports peripheral changes in GM-CSF-producing CD4^(+)T cells in septic patients and the possible underlying mechanism by which GM-CSF influences the outcome of sepsis.Forty-three septic patients,20 SIRS patients,and 20 healthy controls were enrolled in this study and followed for 28 days to assess mortality.We measured the peripheral frequency of GM-CSF^(+)CD4^(+)T cells and recorded their associated relationship with disease progression.Our data demonstrated that peripheral GM-CSF-producing CD4^(+)T cells were significantly higher in septic patients than in both SIRS patients and healthy controls.These cells exhibit a memory phenotype and impaired IFN-γ-secreting capacity in sepsis patients.Using a receiver operating curve analysis with 8.01%as a cut-off point,the percentage of GM-CSF^(+)CD4^(+)T cells could predict the outcome of septic patients.Combined with the increase in GM-CSF-producing CD4^(+)T cells,inflammatory cytokines IL-1βand IL-6 were also upregulated.Using an in vitro neutrophil model,we found that GM-CSF inhibited C3aR expression,while inducing IL-8 production.Furthermore,this effect was transferrable in plasma from sepsis patients and was attenuated by inhibition of GM-CSF using an anti-GM-CSF antibody.These results indicate that GM-CSF-producing CD4^(+)T cells may serve as a marker of sepsis severity.Thus,targeting GM-CSF overproduction may benefit sepsis patients.Huihuang Huang Siyu Wang Tianjun Jiang Rong Fan Zheng Zhang Jinsong Mu Ke Li Yonggang Wang Lei Jin Fang Lin Jie Xia Lijian Sun Biao Xu Chengcheng Ji Jing Chen Juan Chang Bo Tu Bing Song Chao Zhang Fu-Sheng Wang Ruonan Xu 2019Cellular & Molecular Immunology2019,16,6:6
12Effects of lifestyle interventions on rural patients with type 2 diabetes mellitus显示文摘BACKGROUND The prevalence of type 2 diabetes mellitus(T2DM)is rising rapidly in rural areas,and lifestyle interventions can effectively reduce the blood glucose levels of patients with T2DM.However,current dietary and exercise guidelines are still at experimental stages and are difficult for subjects to understand and implement.The Human Metabolism Analyzer provides real life interventions for the prevention and treatment of T2DM,and our pilot research has demonstrated its effectiveness and good compliance.AIM To investigate the effect of and compliance with lifestyle interventions in rural patients with T2DM.METHODS A total of ten rural villages were randomly selected in Chaoshui Township,Penglai City,Shandong Province,China,to conduct health screening among residents aged 50 years or older.Each rural village represented a group,and 12 patients with T2DM were randomly selected from each group(total:120)to participate in this study and receive real life lifestyle interventions and medication guidance.Lifestyle interventions included changing the meal order(A),postprandial activities(B),resistance exercise(C),and reverse abdominal breathing(D).Diabetes education was conducted at least once a month with a weekly phone follow-up to monitor exercise and diet.Waist circumference,blood pressure,body mass index(BMI),motor function,body composition,fasting blood glucose,and glycated hemoglobin(HbA1c)were analyzed before and 3 mo after the intervention.Moreover,patient compliance and adjustments of hypoglycemic drugs were evaluated.RESULTS A total of 109 subjects completed the study.The compliance rates for lifestyle interventions A,B,C,and D were 57.79%,60.55%,64.22%,and 75.23%,respectively.Among the subjects who received hypoglycemic drugs,the dose was reduced 2 to 3 times based on blood glucose in 54(67.50%)subjects and was tapered and discontinued in 5(6.25%)subjects within 3 mo,with no significant fluctuations in blood glucose after dose reduction and withdrawal.After lifestyle interventions,waist circumference,BMI,fasting blood glucose,and HbA1c significantly decreased(P<0.001);motor function and body composition also significantly improved(P<0.001).CONCLUSION For patients with T2DM,compliance to real-life lifestyle interventions is good,and the interventions significantly improve metabolic indicators such as waist circumference,BMI,blood pressure,HbA1c,body composition,and motor function.Some patients are able to taper or discontinue hypoglycemic drugs.Bo Wang Xiao-Li Mu Juan Zhao Hai-Ping Jiang Shan-Shan Li Ge Yan Ying-Ying Hua Xue-Yi Ren Li-Xia Xing Yan Liang Shu-Dong Zhang Yu-Chi Zhao 2020World Journal of Diabetes2020,11,6:5

15Betulinic acid hydroxamate prevents colonic inflammation and fibrosis in murine models of inflammatory bowel disease显示文摘Intestinal fibrosis is a common complication of inflammatory bowel disease (IBD) and is defined as an excessive accumulation of scar tissue in the intestinal wall. Intestinal fibrosis occurs in both forms of IBD: ulcerative colitis and Crohn’s disease. Small-molecule inhibitors targeting hypoxia-inducing factor (HIF) prolyl-hydroxylases are promising for the development of novel antifibrotic therapies in IBD. Herein, we evaluated the therapeutic efficacy of hydroxamate of betulinic acid (BHA), a hypoxia mimetic derivative of betulinic acid, against IBD in vitro and in vivo. We showed that BAH (5–20 μM) dose-dependently enhanced collagen gel contraction and activated the HIF pathway in NIH-3T3 fibroblasts;BAH treatment also prevented the loss of trans-epithelial electrical resistance induced by proinflammatory cytokines in Caco-2 cells. In two different murine models (TNBS- and DSS-induced IBD) that cause colon fibrosis, oral administration of BAH (20, 50 mg/kg·d, for 17 days) prevented colon inflammation and fibrosis, as detected using immunohistochemistry and qPCR assays. BAH-treated animals showed a significant reduction of fibrotic markers (Tnc, Col1a2, Col3a1, Timp-1, α-SMA) and inflammatory markers (F4/80+, CD3+, Il-1β, Ccl3) in colon tissue, as well as an improvement in epithelial barrier integrity and wound healing. BHA displayed promising oral bioavailability, no significant activity against a panel of 68 potential pharmacological targets and was devoid of genotoxicity and cardiotoxicity.
