1Clinical applications of free medial tibial flap with posterior tibial artery for head and neck reconstruction after tumor resection显示文摘Tumor resection causes damage in the head and neck which creates problems in swallowing, chewing, articulation,and vision, all of which seriously affect patients' quality of life. In this work, we evaluated the application of a free medial tibial flap in reconstruction of head and neck defects after tumor resection. We discussed the anatomy,surgical technique, and the advantages and disadvantages of the flap. We found several benefits for the flap, such as,it is especially effective for the defects that require thin-layer epithelium to cover or the separated soft tissue defect;a two-team approach can be used because the donor site is far away from the head and neck; and the flap is easy to integrate because of the subcutaneous fat layer of the free medial tibial flap is thin and the flap is soft. 2Factors contributing to lymph node occult metastasis in supraglottic laryngeal carcinoma cT2-T4 N0M0 and metastasis predictive equation显示文摘Objective: To investigate factors that contribute to lymph node metastasis(LNM) from clinical cT2-T4 N0M0(cN0) supraglottic laryngeal carcinoma(SLC), and to predict the risk of occult metastasis before surgery.Methods: A total of 121 patients who received surgery were retrospectively analyzed. Relevant factors regarding cervical LNM were analyzed. Multivariate analyses were conducted to predict the region where the metastasis occurred and prognosis. Results: The overall metastatic rate of c N0 SLC was 28.1%. Metastatic rates were 15.4%, 32.5% and 35.7% for T2, T3 and T4, respectively. Metastatic rates for SLC levels II, III and IV were 19.6%, 17.2% and 3.6%, respectively. A regression equation was formulated to predict the probability of metastasis in cN0 SLC as follows: Pn=e(–3.874+0.749T3+1.154T4+1.935P1+1.750P2)/[1+e(–3.874+0.749T3+1.154T4+1.935P1+1.750P2)]. Approximately 0.2% of patients experienced LNM with no recurrence of laryngeal cancer. Comparison of the intergroup survival curves between patients with and without LNM indicated a statistically significant difference(P=0.029).Conclusions: Cervical lymph node metastatic rates tended to increase in tandem with T stage in patients with LNM in cN0 SLC, and neck dissection is advised for these patients. 3The Characteristic of an Anti-Human DR5 Antibody A6显示文摘The efficacy of many cancer treatments is due to their ability to induce apoptosis. DR5 can activate apoptosis pathway after binding with its natural ligand, tumour necrosis factor-related apoptosis-inducing ligand (TRAIL/ Apo2L). Both TRAIL and agonistic anti-DR5 monoclonal antibody are currently being explored for cancer therapy. The mechanisms of cytotoxicity of our previously prepared monoclonal antibody A6 against DR5 were investigated here. A6 could cause viability loss of Jurkat cells in both time-and dose-dependent manner which could be attributed to the activation of apoptosis pathway. Caspases 3, 8 and 9 were activated in Jurkat cells and the caspase specific inhibitors, such as broad caspases inhibitor Z-VAD-FMK, caspase 8 specific inhibitor Z-IETD-FMK and caspase 9 specific inhibitor Z-LEHD-FMK could recover the viability loss caused by A6. The function and molecular mechanism of TRAIL-mediated apoptosis were also investigated and compared with those of A6. 4Inhibitory activity of Bifidobacterium adolescent combined with cisplatin on melanoma in mice and its mechanism显示文摘The aim of this study is to explore inhibitory activity of Bifidobacterium adolescent combined with cis-platin on the growth of melanoma(B16)in mice and the underlying mechanism.C57 mice were inoculated with B16 cancer cells to construct mouse model of melanoma and treated with bifidobacterium adolescent combined with cisplatin.Ratios of inhibitory activity on the growth of melanoma(B16)were calculated.Pathology changes of the tumor were observed by HE staining.B16 cell cycles were examined on a flow cytometer.Lymphocyte prolif-eration was measured with MTT assay and the T-cell sub-set was measured by double marked fluorescence.When bifidobacterium of 1010 cfu/L was injected,the ratio of inhibitory activity on the growth of melanoma(B16)reached 54%,which was similar to that of cisplatin group.The ratio of inhibitory activity reached 74.45%when the mice were treated by bifidobacterium combined with cis-platin.HE staining shows that bifidobacterium inhibited B16 cell proliferation and enhanced the cisplatin`s killing activity on B16 cells.The results of flow cytometry demonstrated that B16 cell proliferation was arrested at G1 stage after treatment with bifidobacterium.The B16 cell proliferation was arrested at S stage after treatment with cisplatin.The CD4+percentage increased and the difference was significant compared with the normal group after treatment with bifidobacterium,indicating that T-cell immune activity was enhanced.Treatment with bifidobacterium combined with cisplatin can enhance the inhibitory activity on the growth of melanoma(B16)of cisplatin.The mechanism of the inhibitory activity on B16 cell proliferation is correlated with the enhanced immune activity in mice.Hongying HUANG Guangchao LIU Yijun QI Yaowu DU Jugao CHEN Yuanfang MA 2008Frontiers of Medicine2008,2,2:0
