| 1 | A rapid advice guideline for the diagnosis and treatment of 2019 novel coronavirus(2019-nCoV) infected pneumonia(standard version)显示文摘In December 2019, a new type viral pneumonia cases occurred in Wuhan, Hubei Province;and then named '2019 novel coronavirus(2019-nCoV)' by the World Health Organization(WHO) on 12 January 2020. For it is a never been experienced respiratory disease before and with infection ability widely and quickly, it attracted the world’s attention but without treatment and control manual. For the request from frontline clinicians and public health professionals of 2019-nCoV infected pneumonia management, an evidence-based guideline urgently needs to be developed. Therefore, we drafted this guideline according to the rapid advice guidelines methodology and general rules of WHO guideline development;we also added the first-hand management data of Zhongnan Hospital of Wuhan University. This guideline includes the guideline methodology, epidemiological characteristics, disease screening and population prevention, diagnosis, treatment and control(including traditional Chinese Medicine), nosocomial infection prevention and control, and disease nursing of the 2019-nCoV. Moreover, we also provide a whole process of a successful treatment case of the severe 2019-nCoV infected pneumonia and experience and lessons of hospital rescue for 2019-nCoV infections. | 2 | Deep Structural Framework and Genetic Analysis of Gold Concentration Areas in the Northwestern Jiaodong Peninsula, China: A New Understanding based on High-Resolution Reflective Seismic Survey显示文摘The gold concentration areas in the northwestern Jiaodong Peninsula constituted an important gold metallogenetic region in Eastern China during the Mesozoic. The deep geological bodies' texture characteristic is important for exploring the resources thoroughly and understanding the metallogenic process. The detailed textures were revealed using high-resolution seismic profiles through the three major ore-controlling structures-Sanshandao fault zone, Jiaojia fault zone and Zhaoping fault zone. This study aims to establish a deep structural framework of this area. Based on their formation mechanism, the fault structures developed in the area can be divided into regional and local fault structures. The structural styles are characterised by superimposing their compressional, strike-slip and extensional multi-stage activities. The crust is cut by vertical structures corresponding to a left-lateral strike-slip fault system on the surface. Nearby these structures are the arc-shape structures formed by multi-stage magma intrusions into the upper crust. Bounded by the Tancheng–Lujiang and Muping–Jimo fault zones, the current Jiaodong block, developed a series of NE-trending strike-slip fault systems, was probably formed by the assemblage of several obliquely aligned blocks. The intensive magmatism and hydrothermal activity between the blocks induced large-scale mineralisation. | 6 | Laparoscopic vs open total gastrectomy for gastric cancer:A meta-analysis显示文摘AIM:To conduct a meta-analysis comparing laparoscopic total gastrectomy(LTG)with open total gastrectomy(OTG)for the treatment of gastric cancer.METHODS:Major databases such as Medline(PubMed),Embase,Academic Search Premier(EBSCO),Science Citation Index Expanded and the Cochrane Central Register of Controlled Trials(CENTRAL)in The Cochrane Library were searched for studies comparing LTG and OTG from January 1994 to May 2013.Evaluated endpoints were operative,postoperative and oncological outcomes.Operative outcomes included operative time and intraoperative blood loss.Postoperative recovery included time to first fatus,time to first oral intake,hospital stay and analgesics use.Postoperative complications comprised morbidity,anastomotic leakage,anastomotic stenosis,ileus,bleeding,abdominal abscess,wound problems and mortality.Oncological outcomes included positive resection margins,number of retrieved lymph nodes,and proximal and distal resection margins.The pooled effect was calculated using either a fixed effects or a random effects model.RESULTS:Fifteen non-randomized comparative studies with 2022 patients were included(LTG-811,OTG-1211).Both groups had similar short-term oncological outcomes,analgesic use(WMD-0.09;95%CI:-2.39-2.20;P=0.94)and mortality(OR=0.74;95%CI:0.24-2.31;P=0.61).However,LTG was associated with a lower intraoperative blood loss(WMD-201.19 mL;95%CI:-296.50--105.87 mL;P<0.0001)and overall complication rate(OR=0.73;95%CI:0.57-0.92;P=0.009);fewer wound-related complications(OR=0.39;95%CI:0.21-0.72;P=0.002);a quicker recovery of gastrointestinal motility with shorter time to frst fatus(WMD-0.82;95%CI:-1.18--0.45;P<0.0001)and oral intake(WMD-1.30;95%CI:-1.84--0.75;P<0.00001);and a shorter hospital stay(WMD-3.55;95%CI:-5.13--1.96;P<0.0001),albeit with a longer operation time(WMD 48.25 min;95%CI:31.15-65.35;P<0.00001),as compared with OTG.CONCLUSION:LTG is safe and effective,and may offer some advantages over OTG in the treatment of gastric cancer. | Jun-Jie Xiong Quentin M Nunes Wei Huang Chun-Lu Tan Neng-Wen Ke Si-Ming Xie Xun Ran Hao Zhang Yong-Hua Chen Xu-Bao Liu | 2013 | World Journal of Gastroenterology2013,19,44: | 11 |

| 8 | Development and validation of an endoscopic images-based deep learning model for detection with nasopharyngeal malignancies显示文摘Background:Due to the occult anatomic location of the nasopharynx and frequent presence of adenoid hyperpla-sia,the positive rate for malignancy identification during biopsy is low,thus leading to delayed or missed diagnosis for nasopharyngeal malignancies upon initial attempt.Here,we aimed to develop an artificial intelligence tool to detect nasopharyngeal malignancies under endoscopic examination based on deep learning.Methods:An endoscopic images-based nasopharyngeal malignancy detection model(eNPM-DM)consisting of a fully convolutional network based on the inception architecture was developed and fine-tuned using separate training and validation sets for both classification and segmentation.Briefly,a total of 28,966 qualified images were collected.Among these images,27,536 biopsy-proven images from 7951 individuals obtained from January 1st,2008,to December 31st,2016,were split into the training,validation and test sets at a ratio of 7:1:2 using simple randomiza-tion.Additionally,1430 images obtained from January 1st,2017,to March 31st,2017,were used as a prospective test set to compare the performance of the established model against oncologist evaluation.The dice similarity coef-ficient(DSC)was used to evaluate the efficiency of eNPM-DM in automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images,by comparing automatic segmentation with manual segmenta-tion performed by the experts.Results:All images were histopathologically confirmed,and included 5713(19.7%)normal control,19,107(66.0%)nasopharyngeal carcinoma(NPC),335(1.2%)NPC and 3811(13.2%)benign diseases.The eNPM-DM attained an overall accuracy of 88.7%(95%confidence interval(CI)87.8%-89.5%)in detecting malignancies in the test set.In the prospective comparison phase,eNPM-DM outperformed the experts:the overall accuracy was 88.0%(95%CI 86.1%-89.6%)vs.80.5%(95%CI 77.0%-84.0%).The eNPM-DM required less time(40 s vs.110.0±5.8 min)and exhibited encouraging performance in automatic segmentation of nasopharyngeal malignant area from the background,with an average DSC of 0.78±0.24 and 0.75±0.26 in the test and prospective test sets,respectively.Conclusions:The eNPM-DM outperformed oncologist evaluation in diagnostic classification of nasopharyngeal mass into benign versus malignant,and realized automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images. | Chaofeng Li Bingzhong Jing Liangru Ke Bin Li Weixiong Xia Caisheng He Chaonan Qian Chong Zhao Haiqiang Mai Mingyuan Chen Kajia Cao Haoyuan Mo Ling Guo Qiuyan Chen Linquan Tang Wenze Qiu Yahui Yu Hu Liang Xinjun Huang Guoying Liu Wangzhong Li Lin Wang Rui Sun Xiong Zou Shanshan Guo Peiyu Huang Donghua Luo Fang Qiu Yishan Wu Yijun Hua Kuiyuan Liu Shuhui Lv Jingjing Miao Yanqun Xiang Ying Sun Xiang Guo Xing Lv | 2018 | Cancer Communications2018,38,1: | 9 |

Methods The BP and arsenic species of 560 participants were determined. Logistic regression analysis was applied to estimate the odds ratios of BP associated with arsenic metabolites and arsenic methylation capability. Results BP was positively associated with cumulative arsenic exposure(CAE). Subjects with abnormal diastolic blood pressure(DBP), systolic blood pressure(SBP), and pulse pressure(PP) usually had higher urinary i As(inorganic arsenic), MMA(monomethylated arsenic), DMA(dimethylated arsenic), and TAs(total arsenic) than subjects with normal DBP, SBP, and PP. The i As%, MMA%, and DMA% differed slightly between subjects with abnormal BP and those with normal BP. The PMI and SMI were slightly higher in subjects with abnormal PP than in those with normal PP. Conclusion Our findings suggest that higher CAE may elevate BP. Males may have a higher risk of abnormal DBP, whereas females have a higher risk of abnormal SBP and PP. | 10 | Dynamic evolution of collective emotions in social networks: a case study of Sina weibo显示文摘Emotion is a fundamental object of human existence and determined by a complex set of factors. With the rapid development of online social networks (OSNs), more and more people would like to express their emotion in OSNs, which provides wonderful opportunities to gain insight into how and why individual emotion is evolved in social network. In this paper, we focus on emotion dynamics in OSNs, and try to recognize the evolving process of collective emotions. As a basis of this research, we first construct a corpus and build an emotion classifier based on Bayes theory, and some effective strategies (entropy and salience) are introduced to improve the performance of our classifier, with which we can classify any Chinese tweet into a particular emotion with an accuracy as high as 82%. By analyzing the collective emotions in our sample networks in detail, we get some interesting findings, including a phenomenon of emotion synchronization between friends in OSNs, which offers good evidence for that human emotion can be spread from one person to another. Furthermore, we find that the number of friends has strong correlation with individual emotion. Based on those useful findings, we present a dynamic evolution model of collective emotions, in which both self-evolving process and mutual-evolving process are considered. To this end, extensive simulations on both real and artificial networks have been done to estimate the parameters of our emotion dynamic model, and we find that mutual-evolution plays a more important role than self-evolution in the distribution of collective emotions. | 11 | Metformin promotes microglial cells to facilitate myelin debris clearance and accelerate nerve repairment after spinal cord injury显示文摘Spinal cord injury(SCI)is one kind of severe trauma for central nervous system.Myelin debris clearance and axon regeneration are essential for nerve regeneration after SCI.Metformin,a glucose-lowering drug,has been demonstrated to promote the locomotor functional recovery after SCI.In this study,we investigated the role and molecular mechanism of metformin on myelin preservation in a rat SCI model.SCI was induced in rats by compression at T9 level using a vascular clip.We showed that administration of metformin(50 mg·kg^(-1)·d^(-1),ip)for 28 days significantly improved locomotor function in SCI rats.Metformin also ameliorated SCI-induced neuronal apoptosis and promoted axon regeneration in the spinal cord.Using co-immunofluorescence of IBa-1 and MBP,and luxol fasting blue(LFB)staining,we demonstrated that metformin promoted the transformation of M1 to M2 phenotype polarization of microglial cells,then greatly facilitated myelin debris clearance and protected the myelin in SCI rats.Furthermore,metformin ameliorated SCI-induced blockade of autophagic flux in the spinal cord,and enhanced the fusion of autophagosome and lysosome by inhibiting the AMPK-mTOR signaling pathway.Moreover,metformin significantly attenuated inflammatory responses in the spinal cord.In LPS-treated BV2 cells,pretreatment with metformin(2 mM)significantly enhanced autophagy level,suppressed inflammation and cell apoptosis.The protective effects were blocked in the presence of an autophagy inhibitor 3-methyladenine(3-MA,5 mM),suggesting that the effect of metformin on autophagy in microglial cells is essential for the myelin preservation during nerve recovery.This study reveals a novel therapeutic effect of metformin in SCI recovery by regulating the activation of microglial cells and enhancing its autophagy level. | Yan-qing Wu Jun Xiong Zi-li He Yuan Yuan Bei-ni Wang Jing-yu Xu Man Wu Su-su Zhang Shu-fang Cai Jia-xin Zhao Ke Xu Hong-yu Zhang Jian Xiao | 2022 | Acta Pharmacologica Sinica2022,43,6: | 8 |

| 15 | Tau-Induced Ca^(2+)/Calmodulin-Dependent Protein Kinase-IV Activation Aggravates Nuclear Tau Hyperphosphorylation显示文摘Hyperphosphorylated tau is the major protein component of neurofibrillary tangles in the brains of patients with Alzheimer's disease(AD). However, the mechanism underlying tau hyperphosphorylation is not fully understood. Here, we demonstrated that exogenously expressed wild-type human tau40 was detectable in the phosphorylated form at multiple AD-associated sites in cytoplasmic and nuclear fractions from HEK293 cells.Among these sites, tau phosphorylated at Thr205 and Ser214 was almost exclusively found in the nuclear fraction at the conditions used in the present study. With the intracellular tau accumulation, the Ca^(2+)concentration was significantly increased in both cytoplasmic and nuclear fractions. Further studies using site-specific mutagenesis and pharmacological treatment demonstrated that phosphorylation of tau at Thr205 increased nuclear Ca^(2+)concentration with a simultaneous increase in the phosphorylation of Ca^(2+)/calmodulin-dependent protein kinase IV(Ca MKIV) at Ser196. On the other hand, phosphorylation of tau at Ser214 did not significantly change the nuclear Ca^(2+)/Ca MKIV signaling. Finally, expressing calmodulin-binding protein-4 that disrupts formation ofthe Ca^(2+)/calmodulin complex abolished the okadaic acidinduced tau hyperphosphorylation in the nuclear fraction.We conclude that the intracellular accumulation of phosphorylated tau, as detected in the brains of AD patients, can trigger nuclear Ca^(2+)/Ca MKIV signaling, which in turn aggravates tau hyperphosphorylation. | 18 | Prevention of common bile duct injury during laparoscopic cholecystectomy显示文摘BACKGROUND:Since the widespread adoption of laparoscopic cholecystectomy(LC)in the late 1980s,a rise in common bile duct(CBD)injury has been reported.We analyzed the factors contributing to a record of zero CBD injuries in 10 000 consecutive LCs. METHODS:The retrospective investigation included 10 000 patients who underwent LC from July 1992 to June 2007. LC was performed by 4 teams of surgeons.The chief main surgeon of each team has had over 10 years of experience in hepatobiliary surgery.Calot's triangle was carefully dissected,and the relationship of the cystic duct to the CBD and common hepatic duct was clearly identified.A clip was applied to the cystic duct at the neck of the gallbladder and the duct was incised with scissors proximal to the clip.The cystic artery was dissected by the same method. Then,the gallbladder was dissected from its liver bed.A drain was routinely left at the gallbladder bed for 1-2 days postoperatively. RESULTS:No CBD injuries occurred in 10 000 consecutive LCs,and there were 16 duct leaks(0.16%).Among these, there were 10 Luschka duct leaks(0.1%)and 6 cystic duct leaks(0.06%).Four hundred thirty cases were converted to open cholecystectomy(OC),giving a conversion rate of 4.3%.After a mean follow-up of 17.5 months(range 6-24 months),no postoperative death due to LC occurred,and good results were observed in 95%of the patients. | 19 | Stepwise sedation for elderly patients with mild/moderate COPD during upper gastrointestinal endoscopy显示文摘AIM: To investigate stepwise sedation for elderly patients with mild/moderate chronic obstructive pulmonary disease (COPD) during upper gastrointestinal (GI) endoscopy. METHODS: Eighty-six elderly patients with mild/moderate COPD and 82 elderly patients without COPD scheduled for upper GI endoscopy were randomly assigned to receive one of the following two sedation methods: stepwise sedation involving three-stage administration of propofol combined with midazolam [COPD with stepwise sedation (group Cs), and nonCOPD with stepwise sedation (group Ns)] or continuous sedation involving continuous administration of propofol combined with midazolam [COPD with continuous sedation (group Cc), and non-COPD with continuous sedation (group Nc)]. Saturation of peripheral oxygen (SpO 2 ), blood pressure, and pulse rate were monitored, and patient discomfort, adverse events, drugs dosage, and recovery time were recorded. RESULTS: All endoscopies were completed successfully. The occurrences of hypoxemia in groups Cs, Cc, Ns, and Nc were 4 (9.3%), 12 (27.9%), 3 (7.3%), and 5 (12.2%), respectively. The occurrence of hypoxemia in group Cs was significantly lower than that in group Cc (P < 0.05). The average decreases in value of SpO 2 , systolic blood pressure, and diastolic blood pressure in group Cs were significantly lower than those in group Cc. Additionally, propofol dosage and overall rate of adverse events in group Cs were lower than those in group Cc. Finally, the recovery time in group Cs was significantly shorter than that in group Cc, and that in group Ns was significantly shorter than that in group Nc (P < 0.001). | 20 | Clinical Study on Suspension Pancreatic-Duct-Jejunum End-to-Side Continuous Suture Anastomosis in Pancreaticoduodenectomy显示文摘Objective To study the influence of Suspension Pancreatic-Duct-Jejunum End-to-Side Continuous Suture Anastomosis(SPDJCS) on the incidence of pancreatic fistula after pancreaticoduodenectomy,and to analyze its applicability,safety,and efficacies.Methods A prospective controlled trial was conducted with 165 cases receiving pancreaticoduodenectomy in the Department of Hepatopancreatobiliary Surgery from January 2010 to May 2012.The patients were divided into Group A(end-to-end/end-to-side invaginated anastomosis,n=52),Group B(end-to-side mucosal anastomosis,n=48),and Group C(SPDJCS,n=65).The preoperative data,intraoperative data,and operative outcomes(incidence of pancreatic fistula,operation time,intraoperative blood loss,peritoneal drainage,peritoneal hemorrhage,peritoneal abscess,delayed gastric emptying,pulmonary infection,postoperative infection,blood transfusion,and perioperative mortality) were compared among the 3 groups.Results The total incidence of pancreatic fistula was 13.9%(23/165) in all the 165 patients.The incidence in Group A and Group B was 23.1%(12/52) and 18.8%(9/48),both higher than that in Group C [3.1%(2/65),both P<0.05].Group C showed significantly better outcomes than group A and B in terms of the operation time(5.5±1.2 hours vs.6.1±1.1 hours,5.5±1.2 hours vs.6.3±1.5 hours),volume of blood loss(412.0±205.0 mL vs.525.0±217.0 mL,412.0±205.0 mL vs.514.0±217.0 mL),and postoperative drainage amount of plasma tubes(175.0±65.0 mL vs.275.0±80.0 mL,175.0±65.0 mL vs.255.0±75.0 mL)(all P<0.05),while Group A and Group B displayed no difference in these aspects(P>0.05).As complications other than pancreatic fistula were concerned,the three groups were not different from each other(P>0.05).Conclusions SPDJCS may have the effect of reducing the incidence of pancreatic fistula after pancreaticoduodenectomy.It could be safe,practical and convenient technique of anastomosis for pancreaticojejunostomy. | Ke Dong Wei Xiong Xiao-jiong Yu Chun Gu | 2013 | Chinese Medical Sciences Journal2013,28,1: | 5 |
