| 1 | Developmental dysplasia of the hip in the newborn: A systematic review显示文摘Developmental dysplasia of the hip(DDH) denotes a wide spectrum of conditions ranging from subtle acetabular dysplasia to irreducible hip dislocations. Clinical diagnostic tests complement ultrasound imaging in allowing diagnosis, classification and monitoring of this condition. Classification systems relate to the alpha and beta angles in addition to the dynamic coverage index(DCI). Screening programmes for DDH show considerable geographic variation; certain risk factors have been identified which necessitate ultrasound assessment of the newborn. The treatment of DDH has undergone significant evolution, but the current gold standard is still the Pavlik harness. Duration of Pavlik harness treatment has been reported to range from 3 to 9.3 mo. The beta angle, DCI and the superior/lateral femoral head displacement can be assessed via ultrasound to estimate the likelihood of success. Success rates of between 7% and 99% have been reported when using the harness to treat DDH. Avascular necrosis remains the most devastating complication of harness usage with a reported rate of between 0% and 28%. Alternative non-surgical treatment methods used for DDH include devices proposed by LeD amany, Frejka, Lorenz and Ortolani. The Rosen splint and Wagner stocking have also been used for DDH treatment. Surgical treatment for DDH comprises open reduction alongside a combination of femoral or pelvic osteotomies. Femoral osteotomies are carried out in cases of excessive anteversion or valgus deformity of the femoral neck. The two principal pelvic osteotomies most commonly performed are the Salter osteotomy and Pemberton acetabuloplasty. | 2 | Antidiarrheal activity of Pterocarpus erinaceus methanol leaf extract in experimentally-induced diarrhea显示文摘Objective:To investigate the antidiarrheal activity of the methanol leaf extract of Pterocarpus erinaceus in vivo.Methods:The methanol leaf extract of Ptemcarpus erinaceus was evaluated using different doses(100,200 and 400 mg/kg body weight) orally for antidiarrheal activity using castor oil-induced diarrhea,charcoal meal transit lime and castor oil-induced enteropooling in different groups of albino Wistar mice.The activity of the extract at different doses were compared to diphenoxylate(3 mg/kg) and atropine sulphate(3 mg/kg) which were used as standard reference drugs and also to the distilled water administered negative control group of mice.Results:The extract at the doses used caused a significant(P< 0.01) reduction in the wet faeces passed by the mice in the castor oil-induced diarrhea,decreased the distance travelled by the charcoal meal by up to 54.8%and also caused a dose dependent and significant(P< 0.001) reduction in the intraluminal fluid accumulation in the castor oil-induced enteropooling. | 19 | Effectiveness of in-office blood pressure measurement by eye care practitioners in early detection and management of hypertension显示文摘AIM: To investigate the number of hypertensive patients, the optometrist is able to identify by routinely taking blood pressure(BP) measurements for patients in 'at-risk' groups, and to sample patients' opinions regarding in-office BP measurement. Many of the optometrists in Saudi Arabia practice in optical stores.These stores are wide spread, easily accessible and seldom need appointments. The expanding role of the optometrist as a primary health care provider(PHCP) and the increasing global prevalence of hypertension,highlight the need for an integrated approach towards detecting and monitoring hypertension.METHODS: Automated BP measurements were made twice(during the same session) at five selected optometry practices using a validated BP monitor(Omron M6) to assess the number of patients with high BP(HBP)- in at-risk groups-visiting the eye clinic routinely. Prior to data collection, practitioners underwent a two-day training workshop by a cardiologist on hypertension and how to obtain accurate BP readings. A protocol for BP measurement was distributed and retained in all participating clinics. The general,,.,attitude towards cardiovascular health of 480 patients aged 37.2(±12.4)y and their opinion towards in-office BP measurement was assessed using a self-administered questionnaire.RESULTS: A response rate of 83.6% was obtained for the survey. Ninety-three of the 443 patients(21.0%)tested for BP in this study had HBP. Of these,(62subjects) 67.7% were unaware of their HBP status. Thirty of the 105 subjects(28.6%) who had previously been diagnosed with HBP, still had HBP at the time of this study, and only 22(73.3%) of these patients were on medication. Also, only 25% of the diagnosed hypertensive patients owned a BP monitor.CONCLUSION: Taking BP measurements in optometry practices, we were able to identify one previously undiagnosed patient with HBP for every 8 adults tested.We also identified 30 of 105 previously diagnosed patients whose BP was poorly controlled, twenty-two of whom were on medication. | 20 | Motives and Commitment of Entrepreneurs in Hospitality/Tourism Firms in Port Harcourt显示文摘This aim of this study is to provide both theoretical and empirical insights on entrepreneurs’ motives and commitments in the hospitality and tourism industry in Nigeria, using simple descriptive statistics and complemented with interviews. The study adopted an exploratory research methodology to explore the phenomena of motives and commitment, using descriptive statistics, particularly mea, and standard deviation to present the extent of occurrences in the study subject. The study is based on a sample of 120 entrepreneurs who are owners of small restaurants in Port Harcourt, Rivers State. The results show that entrepreneurs in the hospitality industry are mostly motivated by family related factors such as family tradition, family situation, family line of business, and family prestige. Societal factors are more prominent in the list than both personal and income, financial considerations. The least considered factor that motivates a business start-up is finance or income.
