| 1 | Elastic catenary cable element considering frictional slip effect显示文摘Conventional analysis methods of cable structures do not consider sliding of cables inside the joint,which may lead to inaccuracy of the theoretical behavior of the structure.In order to develop an effective method for cable sliding,a two-node cable element based on the analytical solution for an elastic catenary was studied.The cable sliding stiffness and the effect of friction were investigated.To validate the proposed numerical method,analyses of two examples given in the literature were conducted.The results demonstrated that the method given in this paper is accurate and effective,and can take into account cable sliding in cable structures.In addition,it was shown that the effect of cable sliding on the behavior of cable structures is significant.It was also shown that the friction at the support hampers the flow of the cable force,leading to unequal cable tensions on both sides of the support. | CAI JianGuo LIM James FENG Jian XU YiXiang WANG Kai | 2012 | Science China(Technological Sciences)2012,55,6: | 10 |
| 2 | Multicentre collaborative cohort study of the use of Kirschner wires for the management of supracondylar fractures in children显示文摘Purpose:Supracondylar fractures of the humerus cause significant morbidity in children.Nerve damage and loss of fracture reduction are common recognised complications in patients with this injury.Uncertainty surrounds the optimal Kirschner wire configuration and diameter for closed reduction and pinning of these fractures.This study describes current practice and examined the association between wire configuration or diameter and outcomes(clinical and radiological)in the operative management of paediatric supracondylar fractures.Methods:Children presenting w让h GartlandⅡorⅢsupracondylar fractures at five hospitals in southwest England were eligible for inclusion.Collaborators scrutinised paper and electronic case notes.Outcome measures were maintenance of reduction and iatrogenic nerve injury.Results:Altogether 209 patients were eligible for inclusion:15.7%had a documented neurological deficit at presentation;3.9%who were neurologically intact at presentation sustained a new deficit caused by treatment and 13.4%experienced a clinically significant loss of reduction following fixation.Maintenance of reduction was significantly better in patients treated specifically with crossed×3 Kirschner wire configuration compared to all other configurations.The incidence of iatrogenic nerve injury was not significantly different between groups treated with different wire configurations.Conclusion:We present a large multicentre cohort study showing that crossed×3 Kirschner wires are associated with better maintenance of reduction than crossed×2 or lateral entry wires.Greater numbers would be required to properly investigate nerve injury relating to operative management of supracondylar fractures.We found significant variations in practice and compliance w让h the British Orthopaedic Association Standard for Trauma(BOAST)11 guidelines. | Henry Claireaux Richard Goodall Joshua Hill Elizabeth Wilson Philippa Coull Sebastian Green James Schuster-Bruce Diana Lim Joanna Miles Payam Tarassoli | 2019 | Chinese Journal of Traumatology2019,22,5: | 7 |
| 3 | The Involvement of Lipids in Alzheimer's Disease显示文摘It has been estimated that Alzheimer's disease(AD), the most common form of dementia, will affect approximately 81 million individuals by 2040. To date, the actual cause and cascade of events in the progression of this disease have not been fully determined.Furthermore, there is currently no definitive blood test or simple diagnostic method for AD. Considerable efforts have been put into proteomic approaches to develop a diagnostic blood test, but to date these efforts have not been successful. More recently, there has been a stronger focus on lipidomic studies in the hope of increasing our understanding of the underlying mechanisms leading to AD and developing an AD blood test. It is well known that the strongest genetic risk factor for AD is the ε4 variant of apolipoprotein E(APOE).Evidence suggests that the ApoE protein, a major lipid transporter, plays a key role in the pathogenesis of AD, and its role in both normal and aberrant lipid metabolism warrants further extensive investigation. | 5 | Non-invasive splenic parameters of portal hypertension:Assessment and utility显示文摘BACKGROUND Portal hypertension is a major complication of cirrhosis that is associated with significant morbidity and mortality.The present gold-standard method to risk stratify and observe cirrhosis patients with portal hypertension is hepatic venous pressure gradient measurement or esophagogastroduodenoscopy.However,these methods are invasive,carry a risk of complications and are associated with significant patient discomfort.Therefore,non-invasive splenic parameters are of clinical interest as potential useful markers in determining the presence of portal hypertension.However,diagnostic accuracy and reproducibility remains unvalidated.AIM To assess the diagnostic accuracy of spleen stiffness,area and diameter in predicting the presence of portal hypertension.METHODS Of 50 patients with varying liver disease pathologies were prospectively recruited from the St.Mary’s Hospital Liver Unit in London;25 with evidence of portal hypertension and 25 with no evidence of portal hypertension.Liver stiffness,spleen stiffness,spleen diameter and spleen area were measured using the Philips Affiniti 70 elastography point quantification point shear wave elastography system.The aspartate aminotransferase-to-platelet-ratio-index(APRI)score was also calculated.Performance measures,univariate and multivariate logistic regression were used to evaluate demographic,clinical and elastography variables.Interclass correlation coefficient was used to determine the reproducibility of splenic area and diameter.RESULTS On univariate and individual performance,platelet count[area under the receiver operating characteristic(AUROC)0.846,P value<0.001],spleen area(AUROC 0.828,P value=0.002)and APRI score(AUROC 0.827,P value<0.001)were the most accurate variables in identifying the presence of portal hypertension.On multivariate logistic regression models constructed,the combination of spleen area greater than 57.90 cm2 and platelet count less than 126×10^9 had 63.2%sensitivity and 100%specificity,100%positive predictive value and 100%negative predictive value.An alternative combination of spleen stiffness greater than 29.99 kPa and platelet count less than 126×10^9 had 88%sensitivity,75%specificity,78.6%positive predictive value and 85.7%negative predictive value.An interclass correlation coefficient value of 0.98(95%CI:0.94-0.99,P value<0.001)and 0.96(95%CI:0.91-0.99,P value<0.001)were determined for inter-operator variability for spleen area and diameter respectively.CONCLUSION Spleen area,spleen stiffness and platelet count may be useful markers to assess the presence of portal hypertension in patients of various etiologies. | Ayesha Karim Ahmad Sebastiana Atzori James Maurice Simon D Taylor-Robinson Adrian KP Lim | 2020 | World Journal of Hepatology2020,12,11: | 2 |
