| 1 | Virtual reality simulators for gastrointestinal endoscopy training显示文摘The use of simulators as educational tools for medical procedures is spreading rapidly and many efforts have been made for their implementation in gastrointestinal endoscopy training. Endoscopy simulation training has been suggested for ascertaining patient safety while positively influencing the trainees' learning curve. Virtual simulators are the most promising tool among all available types of simulators. These integrated modalities offer a human-like endoscopy experience by combining virtual images of the gastrointestinal tract and haptic realism with using a customized endoscope. From their first steps in the 1980s until today, research involving virtual endoscopic simulators can be divided in two categories: investigation of the impact of virtual simulator training in acquiring endoscopy skills and measuring competence. Emphasis should also be given to the financial impact of their implementation in endoscopy, including the cost of these state-of-theart simulators and the potential economic benefits from their usage. | 3 | Management of erectile dysfunction in hypertension:Tips and tricks显示文摘Arterial hypertension is a major risk factor for cardiovascular disease and affects approximately one third of the adult population worldwide. The vascular origin of erectile dysfunction is now widely accepted in the vast majority of cases. Erectile dysfunction is frequently encountered in patients with arterial hypertension and greatly affects their quality of life of hypertensive patients and their sexual partners. Therefore, the management of erectile dysfunction in hypertensive patients is of paramount importance. Unfortunately, erectile dysfunction remains under-reported, under-recognized, and under-treated in hypertensive patients, mainly due to the lack of familiarity with this clinical entity by treating physicians. | 4 | Non-interventional management of resistant hypertension显示文摘Hypertension is one of the most popular fields of re-search in modern medicine due to its high prevalence and its major impact on cardiovascular risk and con-sequently on global health. Indeed, about one third of individuals worldwide has hypertension and is under increased long-term risk of myocardial infarction, stroke or cardiovascular death. On the other hand, resistant hypertension, the 'uncontrollable' part of arterial hy-pertension despite appropriate therapy, comprises a much greater menace since long-standing, high levels of blood pressure along with concomitant debilitating entities such as chronic kidney disease and diabetes mellitus create a prominent high cardiovascular risk milieu. However, despite the alarming consequences, resistant hypertension and its effective management still have not received proper scientific attention. As-pects like the exact prevalence and prognosis are yet tobe clarified. In an effort to manage patients with resis-tant hypertension appropriately, clinical doctors are still racking their brains in order to find the best therapeutic algorithm and surmount the substantial difficulties in controlling this clinical entity. | 5 | Improved bowel preparation increases polyp detection and unmasks significant polyp miss rate显示文摘AIM: To retrospectively compare previous-day vs splitdose preparation in terms of bowel cleanliness and polyp detection in patients referred for polypectomy. METHODS: Fifty patients underwent two colonoscopies: one diagnostic in a private clinic and a second for polypectomy in a University Hospital. The latter procedures were performed within 12 wk of the index ones. Examinations were accomplished by two experienced endoscopists, different in each facility. Twenty-seven patients underwent screening/surveillance colonoscopy, while the rest were symptomatic. Previous day bowel preparation was utilized initially and splitdose for polypectomy. Colon cleansing was evaluated using the Aronchick scale. We measured the number of detected polyps, and the polyp miss rates per-polyp.RESULTS: Excellent/good preparation was reported in 38 cases with previous-day preparation(76%) vs 46 with split-dose(92%), respectively(P = 0.03). One hundred and twenty-six polyps were detected initially and 169 subsequently(P < 0.0001); 88 vs 126 polyps were diminutive(P < 0.0001), 25 vs 29 small(P = 0.048) and 13 vs 14 equal or larger than 10 mm. The miss rates for total, diminutive, small and large polyps were 25.4%, 30.1%, 13.7% and 6.6%, respectively. Multivariate analysis revealed that split-dose preparation was significantly associated(OR, P) with increased number of polyps detected overall(0.869, P < 0.001), in the right(0.418, P = 0.008) and in the left colon(0.452, P = 0.02). | 8 | Resuspension of spherical particles due to surface vibration显示文摘In this study,particle resuspension due to surface vibration was investigated.A spherical particle was assumed to rest on top of a thin plate,and an external vibrating force was applied normal on the plate at t=0 and at distance R from the particle.The external driving force created a displacement field in both time and space domains,where deformations in the body of the plate were considered small elastic oscillations that are perpendicular to the propagation of the displacement.Free oscillations were introduced via the theory of elasticity and the creation of waves on thin plates,i.e.,bending waves.Particle motion in the vertical direction was initiated through plate displacement,provided that inertia due to particle mass is overcome.In particular,the particle was assumed to oscillate with a force(of oscillation)estimated via Newton’s second law of motion,comprising of two components:the acceleration due to the plate displacement and the particle mass.Subsequently,a simple force balance method was used to determine the conditions for resuspension.Accordingly,resuspension occurs when the oscillation force exceeds the couple of the adhesive and gravitational forces.The results suggest that the plate displacement depends on the characteristics of the applied force,material properties,and plate thickness.In addition,it was found that the oscillation force is substantially lower than the applied force and that it depends on plate displacement and particle mass.Additionally,the particle size significantly influencedthe outcome of resuspension.Thus,resuspension is favorable for large particles,strong applied forces,and high forcing frequencies. | Sofia Eirini Chatoutsidou Mihalis Lazaridis | 2021 | Particuology2021,19,1: | 2 |
