2Expenditure and financial burden for the diagnosis and treatment of colorectal cancer in China:a hospital.based,multicenter,cross-sectional survey显示文摘Background: The increasing prevalence of colorectal cancer(CRC) in China and the paucity of information about relevant expenditure highlight the necessity of better understanding the financial burden and effect of CRC diagnosis and treatment. We performed a survey to quantify the direct medical and non-medical expenditure as well as the resulting financial burden of CRC patients in China.Methods: We conducted a multicenter, cross-sectional survey in 37 tertiary hospitals in 13 provinces across China between 2012 and 2014. Each enrolled patient was interviewed using a structured questionnaire. All expenditure data were inflated to the 2014 Chinese Yuan(CNY; 1 CNY = 0.163 USD). We quantified the overall expenditure and financial burden and by subgroup(hospital type, age at diagnosis, sex, education, occupation, insurance type, household income, clinical stage, pathologic type, and therapeutic regimen). We then performed generalized linear modeling to determine the factors associated with overall expenditure.Results: A total of 2356 patients with a mean age of 57.4 years were included, 57.1 % of whom were men; 13.9% of patients had stage I cancer; and the average previous-year household income was 54,525 CNY.The overall average direct expenditure per patient was estimated to be 67,408 CNY, and the expenditures for stage Ⅰ, Ⅱ, Ⅲ, and Ⅳ disease were 56,099 CNY, 59,952 CNY, 67,292 CNY, and 82,729 CNY, respectively. Non-medical expenditure accounted for 8.3%of the overall expenditure. The 1-year out-of-pocket expenditure of a newly diagnosed patient was 32,649 CNY, which accounted for 59.9% of their previous-year household income and caused 75.0% of families to suffer an unmanageable financial burden. Univariate analysis showed that financial burden and overall expenditure differed in almost all subgroups(P < 0.05), except for sex. Multivariate analysis showed that patients who were treated in specialized hospitals and those who were diagnosed with adenocarcinoma or diagnosed at a later stage were likely to spend more,whereas those with a lower household income and those who underwent surgery spent less(all P < 0.05).Conclusions: For patients in China, direct expenditure for the diagnosis and treatment of CRC seemed catastrophic,and non-medical expenditure was non-ignorable. 6Distribution of polybrominated diphenyl ethers and decabromodiphenylethane in surface sediments from Fuhe River and Baiyangdian Lake,North China显示文摘Nineteen surface sediment samples collected from Baiyangdian Lake and its inflowing river (Fuhe River) in North China were analyzed for polybrominated diphenyl ethers (PBDEs) and decabromodiphenylethane (DBDPE).The concentrations of PBDEs and DBDPE in sediments ranged from 5.5 to 300.7 ng/g dry weight (dw) and 1.1 to 68.2 ng/g dw,respectively.Their levels in sediments in Fuhe River were significantly higher than those in Baiyandian Lake.Compared to data from other regions,the PBDE levels in surface sediments from Baiyangdian Lake and Fuhe River were in the medium to lower range.Among the PBDE congeners,BDE209 was predominant,with contributions to the total PBDEs ranging from 79.4% to 97.3% in sediment samples.For the lowly brominated congeners (tri-to hepta-BDE),BDE47 and BDE99 were the most abundant,which contributed 52.1% and 44.1% to the sum of tri-to hepta-BDEs in the sediments from Baiyangdian Lake and Fuhe River,respectively.The compositional patterns of PBDEs in Baiyangdian Lake sediments indicated that technical deca-BDE mixture was the major pollutant sources with a minor contribution of penta-BDE mixture.The present study suggested that the importance of Fuhe River as a possibly potential sources of PBDEs contamination in Baiyangdian Lake.Guocheng Hu Zhencheng Xu Jiayin Dai Bixian Mai Hong Cao Jianshe Wang Zhimin Shi Muqi Xu 2010Journal of Environmental Sciences2010,22,12:18

10A curative-intent endoscopic surgery for postradiation nasopharyngeal necrosis in patients with nasopharyngeal carcinoma显示文摘Background:Postradiation nasopharyngeal necrosis(PRNN)is a severe complication after radiotherapy in patients with nasopharyngeal carcinoma(NPC),which can severely affect the quality of life and threaten the patient’s life.Only 13.4%-28.6%of patients can be cured by traditional repeated endoscopic debridement.Here,we introduced an innovative curative-intent endoscopic surgery for PRNN patients and evaluated its clinical efficacy.Methods:Clinical data of 72 PRNN patients who underwent radical endoscopic necrectomy,followed by reconstruc-tion using a posterior pedicle nasal septum and floor mucoperiosteum flap were analyzed to determine the efficacy of this surgery.The endpoints were complete re-epithelialization of the nasopharyngeal defect,relief of headache,and overall survival(OS).Results:All surgeries were successfully performed without any severe postoperative complications or death.The median value of numeric rating scales of pain decreased from 8 before surgery to 0 after surgery(P<0.001).Fifty-one patients(70.8%)achieved complete re-epithelialization of the nasopharyngeal defect.The number of cycles of radiotherapy(odds ratio[OR],7.254;95%confidence interval[CI]1.035-50.821;P=0.046),postoperative pathological result(OR,34.087;95%CI 3.168-366.746;P=0.004),and survival status of flap(OR,261.179;95%CI 17.176-3971.599;P<0.001)were independent risk factors of re-epithelialization of the nasopharyngeal defects.Postoperative patho-logical result(hazard ratio[HR],5.018;95%CI 1.970-12.782;P=0.001)was an independent prognostic factor for OS.The 2-year OS rate of the entire cohort was 77.9%.Conclusion:Curative-intent endoscopic necrectomy followed by construction using the posterior pedicle nasal septum and floor mucoperiosteum flap is a novel,safe,and effective treatment of PRNN in patients with NPC.Xiong Zou Shun-Lan Wang You-Ping Liu Yan-Ling Liu Ru-Hai Zou Yi-Nuan Zhang Rui You Qi Yang Yu-Long Xie Mei Lin Pei-Yu Huang Rou Jiang Meng-Xia Zhang Chao-Nan Qian Hai-Qiang Mai Ling Guo Ming-Huang Hong Ming-Yuan Chen 2018Cancer Communications2018,38,1:11

13A Gain-of-Function Mutation in IAA7/AXR2 Confers Late Flowering under Short-day Light in Arabidopsis显示文摘Floral initiation is a major step in the life cycle of plants,which is influenced by photoperiod,temperature,and phytohormones,such as gibberellins(GAs).It is known that GAs promote floral initiation under short-day light conditions(SDs) by regulating the floral meristem-identity gene LEAFY(LFY) and the flowering-time gene SUPPRESSOR OF OVEREXPRESSION OF CO 1(SOC1).We have defined the role of the auxin signaling component INDOLE-3-ACETIC ACID 7(IAA7)/AUXIN RESISTANT 2(AXR2) in the regulation of flowering time in Arabidopsis thaliana.We demonstrate that the gain-of-function mutant of IAA7/AXR2,axr2-1,flowers late under SDs.The exogenous application of GAs rescued the late flowering phenotype of axr2-1 plants.The expression of the GA20 oxidase(GA20ox) genes,GA20ox1 and GA20ox2,was reduced in axr2-1 plants,and the levels of both LFY and SOC1 transcripts were reduced in axr2-1 mutants under SDs.Furthermore,the overexpression of SOC1 or LFY in axr2-1 mutants rescued the late flowering phenotype under SDs.Our results suggest that IAA7/AXR2 might act to inhibit the timing of floral transition under SDs,at least in part,by negatively regulating the expressions of the GA20ox1 and GA20ox2 genes.Yan-Xia Mai Long Wang Hong-Quan Yang 2011Journal of Integrative Plant Biology2011,53,6:10

16Development and validation of an endoscopic images-based deep learning model for detection with nasopharyngeal malignancies显示文摘Background:Due to the occult anatomic location of the nasopharynx and frequent presence of adenoid hyperpla-sia,the positive rate for malignancy identification during biopsy is low,thus leading to delayed or missed diagnosis for nasopharyngeal malignancies upon initial attempt.Here,we aimed to develop an artificial intelligence tool to detect nasopharyngeal malignancies under endoscopic examination based on deep learning.Methods:An endoscopic images-based nasopharyngeal malignancy detection model(eNPM-DM)consisting of a fully convolutional network based on the inception architecture was developed and fine-tuned using separate training and validation sets for both classification and segmentation.Briefly,a total of 28,966 qualified images were collected.Among these images,27,536 biopsy-proven images from 7951 individuals obtained from January 1st,2008,to December 31st,2016,were split into the training,validation and test sets at a ratio of 7:1:2 using simple randomiza-tion.Additionally,1430 images obtained from January 1st,2017,to March 31st,2017,were used as a prospective test set to compare the performance of the established model against oncologist evaluation.The dice similarity coef-ficient(DSC)was used to evaluate the efficiency of eNPM-DM in automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images,by comparing automatic segmentation with manual segmenta-tion performed by the experts.Results:All images were histopathologically confirmed,and included 5713(19.7%)normal control,19,107(66.0%)nasopharyngeal carcinoma(NPC),335(1.2%)NPC and 3811(13.2%)benign diseases.The eNPM-DM attained an overall accuracy of 88.7%(95%confidence interval(CI)87.8%-89.5%)in detecting malignancies in the test set.In the prospective comparison phase,eNPM-DM outperformed the experts:the overall accuracy was 88.0%(95%CI 86.1%-89.6%)vs.80.5%(95%CI 77.0%-84.0%).The eNPM-DM required less time(40 s vs.110.0±5.8 min)and exhibited encouraging performance in automatic segmentation of nasopharyngeal malignant area from the background,with an average DSC of 0.78±0.24 and 0.75±0.26 in the test and prospective test sets,respectively.Conclusions:The eNPM-DM outperformed oncologist evaluation in diagnostic classification of nasopharyngeal mass into benign versus malignant,and realized automatic segmentation of malignant area from the background of nasopharyngeal endoscopic images.Chaofeng Li Bingzhong Jing Liangru Ke Bin Li Weixiong Xia Caisheng He Chaonan Qian Chong Zhao Haiqiang Mai Mingyuan Chen Kajia Cao Haoyuan Mo Ling Guo Qiuyan Chen Linquan Tang Wenze Qiu Yahui Yu Hu Liang Xinjun Huang Guoying Liu Wangzhong Li Lin Wang Rui Sun Xiong Zou Shanshan Guo Peiyu Huang Donghua Luo Fang Qiu Yishan Wu Yijun Hua Kuiyuan Liu Shuhui Lv Jingjing Miao Yanqun Xiang Ying Sun Xiang Guo Xing Lv 2018Cancer Communications2018,38,1:9

18Speleothem Mg, Sr and Ba records during the MIS 5c-d, and implications for paleoclimate change in NE Sichuan, Central China显示文摘Trace metals of Mg, Sr and Ba in a stalagmite (SZ2) collected from Suozi Cave in NE Sichuan, Central China, were analyzed with ICP-AES. The stalagmite was dated to have developed between 120 and 103 ka BP. Results indicate that the Mg/Ca, Sr/Ca and Ba/Ca ratios of SZ2 varied between (9500-14700) 10 6 , (54-123) 10 6 and (31-82) 10 6 , respectively. The three records displayed significant millennial scale variations, which correlate with changes in past climate and environment. Lower values of Mg/Ca ratios and higher values of Sr/Ca and Ba/Ca ratios of SZ2 occurred during relatively cold-dry marine isotope stage (MIS) 5d, while the opposite trend was observed during relatively warm-humid MIS 5c. Trace metals in speleothems can be affected by distribution coefficients or trace metal concentrations in solutions from which speleothems precipitate. Temperature is suggested to be the dominant controller of Mg/Ca ratios in SZ2, whereas the Sr/Ca and Ba/Ca ratios may have been influenced by surface soil and atmospheric dust activities and prior calcite precipitation in Suozi Cave. Further investigations are warranted as to whether Sr/Ca and Ba/Ca ratios were affected by temperature and growth rate of the speleothem. The shifts of Mg/Ca, Sr/Ca and Ba/Ca records in SZ2 during the transition from MIS 5d to 5c apparently were earlier than the oxygen isotope record ( 18 O) of the same stalagmite. This possibly indicates (1) that atmospheric dust activity (the mechanism affecting SZ2 Sr/Ca and Ba/Ca ratios) is closely related to temperature, and (2) that temperature changes (dominated by solar insolation) precedes change in speleothem 18 O records which are controlled by both summer monsoons and winter temperatures.ZHOU HouYunt WANG Yue HUANG LiuYuan MAI ShangQuan 2011Chinese Science Bulletin2011,56,32:8
19Solid pseudopapillary tumor of the pancreas in male patients: Report of 16 cases显示文摘AIM:To investigate the clinical characteristics,surgical strategies and prognosis of solid pseudopapillary tumors(SPTs)of the pancreas in male patients.METHODS:From July 2003 to March 2013,116 patients were diagnosed with SPT of the pancreas in our institution.Of these patients,16 were male.The patients were divided into two groups based on gender:female(group 1)and male(group 2).The groups were compared with regard to demographic characteristics,clinical presentations,surgical strategies,complications and follow-up outcomes.RESULTS:Male patients were older than female patients(43.1±12.3 years vs 33.1±11.5 years,P=0.04).Tumor size,location,and symptoms were comparable between the two groups.All patients,with the exception of one,underwent complete surgical resection.The patients were regularly followed up.Themean follow-up period was 58 mo.Two female patients(1.7%)developed tumor recurrence or metastases and required a second resection,and two female patients(1.7%)died during the follow-up period.CONCLUSION:Male patients with SPT of the pancreas are older than female patients.There are no significant differences between male and female patients regarding surgical strategies and prognosis.Yun-Qiang Cai Si-Ming Xie Xun Ran Xing Wang Gang Mai Xu-Bao Liu 2014World Journal of Gastroenterology2014,20,22:8
20Novel MOF shell-derived surface modification of Li-rich layered oxide cathode for enhanced lithium storage显示文摘Li-rich layered oxide materials have attracted increasing attention because of their high specific capacity(>250 mAh g^(-1)). However, these materials typically suffer from poor cycling stability and low rate performance. Herein, we propose a facile and novel metal-organic-framework(MOF) shell-derived surface modification strategy to construct NiCo nanodots decorated(~5 nm in diameter) carbon-confined Li_(1.2)Mn_(0.54) Ni_(0.13)Co_(0.13)O_2 nanoparticles(LLO@C&NiCo). The MOF shell is firstly formed on the surface of as-prepared Li_(1.2)Mn_(0.54)Ni_(0.13)Co_(0.13)O_2 nanoparticles via low-pressure vapor superassembly and then is in situ converted to the NiCo nanodots decorated carbon shell after subsequent controlled pyrolysis.The obtained LLO@C&NiCo cathode exhibits enhanced cycling and rate capability with a capacity retention of 95% after 100 cycles at 0.4 C and a high capacity of 159 mAh g^(-1) at 5 C, respectively, compared with those of LLO(75% and 105 mAh g^(-1)). The electrochemical impedance spectroscopy and selected area electron diffraction analyses after cycling demonstrate that the thin C&NiCo shell can endow LLO with high electronic conductivity and structural stability, indicating the undesired formation of the spinel phase initiated from the particle surface is efficiently suppressed.
