1Choriocapillaris changes in dry age-related macular degeneration and geographic atrophy: a review显示文摘Age-related macular degeneration(AMD)is a leading cause of central vision loss worldwide.The progression of dry AMD from early to intermediate stages is primarily characterized by increasing drusen formation and adverse impact on outer retinal cells.Late stage AMD consists of either geographic atrophy(GA),the non-exudative(dry)AMD subtype,or choroidal neovascularization,the exudative(wet)AMD subtype.GA is characterized by outer retinal and choroidal atrophy,specifically the photoreceptor layer,RPE,and choriocapillaris.Much remains to be discovered regarding the pathogenesis of AMD progression and subsequent development of GA.As the functionality of all three layers is closely linked,the temporal sequence of events that end up in atrophy is important in the understanding of the pathogenic pathway of the disease.The advent of OCTA,and particularly of swept-source technology,has allowed for depth-resolved imaging of retinal vasculature and the choriocapillaris.With the use of OCTA,recent studies demonstrate that choriocapillaris flow alterations are closely associated with the development and progression of AMD.Such changes may even possibly offer predictive value in determining progression of GA.This article reviews studies demonstrating choriocapillaris changes in dry AMD and summarizes the existing literature on the potential role of the choriocapillaris as a key factor in the pathogenesis of AMD.Malvika Arya Almyr S.Sabrosa Jay S.Duker Nadia K.Waheed 2018Eye and Vision2018,5,1:4
2Morphological changes in intraretinal microvascular abnormalities after anti-VEGF therapy visualized on optical coherence tomography angiography显示文摘Background:To examine the baseline morphological characteristics and alterations in intraretinal microvascular abnormalities(IRMAs)in response to anti-vascular endothelial growth factor(anti-VEGF)treatment,documented by optical coherence tomography angiography(OCTA)in diabetic eyes.Methods:In this retrospective study,IRMAs were evaluated with multimodal imaging(fundus photography,fluorescein angiography,OCTA)in treatment-naïve diabetic eyes before and after anti-VEGF treatment for diabetic macular edema(DME)and/or proliferative diabetic retinopathy(PDR)and compared to diabetic control eyes with similar diabetic retinopathy(DR)severity that did not receive anti-VEGF therapy.The morphological characteristics of IRMAs on enface OCTA imaging were graded by masked readers at baseline,then after anti-VEGF therapy in treated eyes or after observation in control eyes.Characterization of interval changes in an IRMA were based on the following parameters:branching,vessel caliber and area of adjacent capillary non-perfusion.Results:The treated group included 45 IRMA foci from 15 eyes of 11 patients,while the control group included 27 IRMA foci from 15 eyes of 14 patients.Following anti-VEGF treatment,enface OCTA demonstrated that 14 foci of IRMA(31%)demonstrated regression with normalization of appearance of the capillary bed,20 IRMAs(44%)remained unchanged,six IRMAs(13%)progressed with enlargement or development of new IRMAs and five IRMAs(11%)demonstrated complete obliteration defined as IRMA disappearance with advancing capillary drop-out.In the control group,17 IRMA(63%)remained stable,8 IRMAs(29.6%)progressed and 2 experienced total obliteration(7.4%).The difference in rank order between the two groups was statistically significant(p=0.022).Conclusions:In eyes with DR status post anti-VEGF therapy,foci of IRMAs have a variable course demonstrating one of four possible outcomes:regression,stabilit,progression or complete obliteration.In contrast,none of the untreated control diabetic eyes demonstrated regression of IRMAs,consistent with known progression of DR severity in high risk eyes.Morphologic evaluation of IRMAs with OCTA may help to monitor changes in retinal blood flow as well as the response to anti-VEGF treatment.Osama A.Sorour Nihaal Mehta Caroline R.Baumal Akihiro Ishibazawa Keke Liu Eleni K.Konstantinou Sarah Martin Phillip Braun A.Yasin Alibhai Malvika Arya Andre J.Witkin Jay S.Duker Nadia K.Waheed 2020Eye and Vision2020,7,1:4

4Exploring point-of-care transformation in diabetic care:A quality improvement approach显示文摘Objective:This quality improvement project evaluated the impact of a point-of-care(POC)HbA1c stat lab intervention and a nurse-assisted expanded visit implemented among patients with uncontrolled type 2 diabetes(T2D)at a community health center in Houston,TX.Methods:This was a before-and-after POC intervention among adult patients who received primary care services between 1 July 2014 and 31 December 2014(baseline visit)and who had at least one 3-month follow-up visit.Results:Three hundred eighty-seven patients were included in the study.The majority were<60 years of age(72.1%),female(60.5%),and Hispanic(63%),followed by black(16.5%)and Asian(11.1%).Almost 87%of the patients had uncontrolled T2D(HbA1c>9%)at baseline,with the highest average levels among Hispanic(10.9%)and black(10.7%)patients.There was a significant difference in the HbA1c level before(mean=10.65,SD=1.9291)and after(mean=9.25,SD=1.8187)intervention.The absolute reduction in the level of HbA1c was 1.4%(t=12.834,p<0.001),corresponding to a 13%overall percentage decrease from baseline.Conclusion:There is a distinct advantage in using a stat HbA1c lab when combined with shared POC visits to assist patients with uncontrolled T2D in lowering the HbA1c,improving self-management,and reducing long-term costs.Malvika Juneja Maria C.Mejia de Grubb Haijun Wang Kiara Spooner Roger J.Zoorob 2015Family Medicine and Community Health2015,3,2:2

6New findings and challenges in OCT angiography for diabetic retinopathy显示文摘Diabetic retinopathy(DR)is a leading cause of visual loss worldwide.Disease severity is graded from mild non-proliferative DR to proliferative DR.Optical coherence tomography angiography(OCTA)has become widely accepted as a useful noninvasive technique that provides detailed imaging of the ocular vessels.It is also becoming an increasingly essential tool for both qualitative and quantitative assessment of DR,especially with the advent of wider imaging capabilities.Various angiographic features of DR,such as microaneurysms,intraretinal microvascular abnormalities,neovascularization,and nonperfusion have been comprehensively studied and described using OCTA.Different quantitative OCTA metrics have been introduced,such as vessel density,foveal avascular zone(FAZ)area,and area of nonperfusion.Current research has been focusing on the application of quantitative OCTA for the diagnosis of DR and treatment monitoring.The primary purpose of this article is to review the use of OCTA,including its challenges,in the diagnosis and management of DR.Osama Sorour Malvika Arya Nadia Waheed 2018Annals of Eye Science2018,,1:2

12Effectiveness of simulation-based learning regarding management of post-COVID complications in terms of knowledge,clinical decision-making ability,and self-efficacy among nursing students:A quasi-experimental study显示文摘Objective:To assess the effectiveness of simulation-based learning regarding the management of post-COVID complications in terms of knowledge,clinical decision-making ability,and self-efficacy among nursing students.Methods:This was a quasi-experimental study conducted among 1152nd-year nursing students.The participants were selected by a simple random sampling technique.The participants were divided into an experimental(n=56)and a comparison group(n=59)by a random table method.Data were analyzed using descriptive and inferential statistics with SPSS version 20.Results:There were significant differences in mean post-test knowledge scores(P=0.03)and mean post-test self-efficacy scores(P=0.001)between the experimental and the comparison groups while the difference in mean post-test clinical decision-making ability scores between the two groups was non-significant(P=0.07).A positive correlation was found between knowledge and clinical decision-making ability in pre-test(P=0.03)and in post-test(P<0.001)and a non-significant correlation was found between pre-test knowledge and self-efficacy score(P=0.52)among the experimental group.Conclusions:Simulation-based learning regarding the management of post-COVID complications is effective among nursing students.Simulation labs should be established in health care settings where simulation training can be provided for updating the knowledge,clinical decision-making ability,and self-efficacy of nursing personnel during program installment and continuous nursing education.Thakur Malvika Eenu Kumar Yogesh Sarin Jyoti Nitesh Kumawat Shatrughan Pareek 2023Journal of Acute Disease2023,12,3:0
13Emerging trends in self-healable nanomaterials for triboelectric nanogenerators:A comprehensive review and roadmap显示文摘A thorough analysis of triboelectric nanogenerators (TENGs) that make use of self-healable nanomaterials is presented in this review. These TENGs have shown promise as independent energy sources that do not require an external power source to function. TENGs are developing into a viable choice for powering numerous applications as low-power electronics technology advances. Despite having less power than conventional energy sources, TENGs do not directly compete with these. TENGs, on the other hand, provide unique opportunities for future self-powered systems and might encourage advancements in energy and sensor technologies. Examining the many approaches used to improve nanogenerators by employing materials with shape memory and self-healable characteristics is the main goal of this review. The findings of this comprehensive review provide valuable information on the advancements and possibilities of TENGs, which opens the way for further research and advancement in this field. The discussion of life cycle evaluations of TENGs provides details on how well they perform in terms of the environment and identifies potential improvement areas.
