| 2 | Radiofrequency ablation for treatment of hypersplenism: A feasible therapeutic option显示文摘We present a case of a patient with hypersplenism secondary to portal hypertension due to hepato-splenic schistosomiasis, which was accompanied by severe and refractory thrombocytopenia. We performed spleen ablation and measured the total spleen and ablated volumes with contrast-enhanced computed tomography and volumetry. No major complications occurred, thrombocytopenia was resolved, and platelet levels remained stable, which allowed for early treatment of the patient's underlying disease. Previous work has shown that splenic radiofrequency ablation is an attractive alternative treatment for hypersplenism induced by liver cirrhosis. We aimed to contribute to the currently sparse literature evaluating the role of radiofrequency ablation(RFA) in the management of hypersplenism. | 3 | Comparison between endoscopic sphincterotomy vs endoscopic sphincterotomy associated with balloon dilation for removal of bile duct stones:A systematic review and meta-analysis based on randomized controlled trials显示文摘AIM To compare gallstones removal rate and incidence of bleeding, pancreatitis, use of mechanical lithotripsy, cholangitis and perforation between isolated sphincterotomy vs sphincterotomy associated with balloon dilation of papilla in choledocholithiasis through the meta-analysis of randomized clinical trials. METHODS We conducted a systematic review according to the PRISMA guidelines. Literature search was restricted to randomized controlled trials(RCTs) on Med Line, Cochrane Library, LILACS, and EMBASE database platforms in July 2017. The manual search included references of retrieved articles. We extracted data focusing on outcomes: The primary endpoint was the stones removal rate; Secondary endpoints were rates of pancreatitis, bleeding, use of mechanical lithotripsy(ML), perforation and cholangitis. RESULTS Eleven RCTs with 1824 patients were included. EST was associated with more post-endoscopic retrograde cholangiopancreatography(ERCP) bleeding [FE RD-0.02, CI(-0.03,-0.00), I2 = 33%, P = 0.05] and more need of mechanical lithotripsy in general [RE RD-0.16, CI(-0.25,-0.06), I2 = 90%, P = 0.002] and in subgroup analysis of stones greater than 15 mm [RE RD-0.20, CI(-0.38,-0.02), I2 = 82%, P = 0.003]. Incidence of pancreatitis [FE RD-0.01, CI(-0.03, 0.01), I2 = 0, P = 0.36], cholangitis [FE RD-0.00, CI(-0.01, 0.01), I2 =0, P = 0.97] and perforation [FE RD-0.01, CI(-0.01, 0.00), I2 = 0, P = 0.23] was similar between the groups as well as similar stone removal rates in general [FE RD-0.01, CI(-0.01, 0.04), I2 = 0, P = 0.23] and pooled analysis of stones greater than 15 mm [FE RD-0.02, CI(-0.02, 0.07), I2 = 11%, P = 0.31]. CONCLUSION Through meta-analysis of randomized clinical trials we found that isolated sphincterotomy was associated with more post-ERCP bleeding and more need for mechanical lithotripsy. | 6 | Explaining the influence of dopant concentration and excitation power density on the luminescence and brightness of β-NaYF4:Yb^3+,Er^3+ nanoparticles:Measurements and simulations显示文摘We assessed the influence of Yb^3+ and Er34 dopant concentration on the relative spectral distribution, quantum yield (Фuc), and decay kinetics of the upconversion luminescence (UCL) and particle brightness (BUC) for similarly sized (33 nm) oleate-capped p-NaYF4:Yb3t,Er^3+ upconversion (UC) nanoparticles (UCNPs) in toluene at broadly varied excitation power densities (F). This included an Yb^3+ series where the Yb^3+ concentration was varied between 11%—21% for a constant Er^3+ concentration of 3%, and an Er^3+ series, where the Er^3+ concentration was varied between 1%-4% for a constant Yb^3+ concentration of 14%. The results were fitted with a coupled rate equation model utilizing the UCL data and decay kinetics of the green and red Er34 emission and the Yb34 luminescence at 980 nm. An increasing Yb34 concentration favors a pronounced triphotonic population of 4F9/2 at high P by an enhanced back energy transfer (BET) from the 4G11/2 level. Simultaneously, the Yb^3+-controlled UCNPs absorption cross section overcompensates for the reduction in Фuc with increasing Yb^3+ concentration at high P, resulting in an increase in Sue. Additionally, our results show that an increase in Yb34 and a decrease in Er^3+ concentration enhance the color tuning range by P. | 9 | Erosion problem in tool steel using cold box core-making process显示文摘This paper presents the erosion results of the AISI H13 steel impinged by resin-bonded silica sand, using a testing rig that closely simulates the real blowing conditions during industrial core-making. Steel specimens were heat treated to obtain hardness of 294, 445 and 595 HV200 (29, 45 and 55 HRC). Erosion tests were carried out at impingement angles from 20° to 90° and air drag pressures of 1.38, 2.07 and 2.76 bar (20, 30 and 40 psi). The main results are summarized as follows:(i) The harder material, the lower erosion;(ii) the maximum erosion rate is at 30°;(iii) Little difference in erosion rate at impact angle of 60° and 90° for a constant pressure tested regardless of the hardness level;(iv) As the pressure increases, so does the erosion rate, being more sensitive for low impact angles. The material is self healing in such a sense that some external stimulus is needed in the form of heating by induction energy. Steel wool was added to porous asphalt concrete to enhance its electrical conductivity and induction heating was applied to increase the temperature to heal the micro-cracks and repair the bonding between aggregate and binder. The main purpose of this paper is to examine the mechanical properties of this sustainable porous asphalt concrete including indirect tensile strength,work of fracture,particle loss resistance and water sensitivity.
