| 1 | Phase angle obtained by bioelectrical impedance analysis independently predicts mortality in patients with cirrhosis显示文摘AIM To evaluate the prognostic value of the phase angle(PA)obtained from bioelectrical impedance analysis(BIA) for mortality prediction in patients with cirrhosis. METHODS In total, 134 male cirrhotic patients prospectively completed clinical evaluations and nutritional assessment by BIA to obtain PAs during a 36-mo follow-up period. Mortality risk was analyzed by applying the PA cutoff point recently proposed as a malnutrition marker(PA ≤ 4.9°) in Kaplan-Meier curves and multivariate Cox regression models. RESULTS The patients were divided into two groups according to the PA cutoff value(PA > 4.9°, n = 73; PA ≤ 4.9°, n = 61). Weight, height, and body mass index were similar in both groups, but patients with PAs > 4.9° were younger and had higher mid-arm muscle circumference, albumin, and handgrip-strength values and lower severe ascites and encephalopathy incidences, interleukin(IL)-6/IL-10 ratios and C-reactive protein levels than did patients with PAs ≤ 4.9°(P ≤ 0.05). Forty-eight(35.80%) patients died due to cirrhosis, with a median of 18 mo(interquartile range, 3.3-25.6 mo) follow-up until death. Thirty-one(64.60%) of these patients were from the PA ≤ 4.9° group. PA ≤ 4.9° significantly and independently affected the mortality model adjusted for Model for End-Stage Liver Disease score and age(hazard ratio = 2.05, 95%CI: 1.11-3.77, P = 0.021). In addition, Kaplan-Meier curves showed that patients with PAs ≤ 4.9° were significantly more likely to die. CONCLUSION In male patients with cirrhosis, the PA ≤ 4.9° cutoff was associated independently with mortality and identified patients with worse metabolic, nutritional, and disease progression profiles. | 2 | Short-term complementary and alternative medicine on quality of life in women with fibromyalgia显示文摘BACKGROUND:Fibromyalgia(FMS)is a syndrome characterized by chronic widespread musculoskeletal pain,whose etiology is not completely understood.Different therapeutic approaches have been used with inconsistent results.This observation does not invalidate the continued search for alternative treatments aimed at improving quality of life(Qo L)in FMS.OBJECTIVE:This study compared three classical traditional Chinese medicine(TCM)therapies:acupuncture(AC),electroacupuncture(EAC)and moxibustion(MX)in the management of pain and promotion of Qo L in FMS patients.DESIGN,SETTING,PARTICIPANTS AND INTERVENTIONS:A preliminary,group-assigned,comparative study enrolled 30 women,mean age(46.90±9.24)years(range 20–60 years),who met the 1990 American College of Rheumatology criteria for FMS diagnosis and a pain-pressure threshold(PPT)< 4 kg/cm^2.The study was conducted in a teaching tertiary-care medical institution from May 2010 through April 2012.AC,EAC and MX were delivered for 30 min,once a week,for 8 weeks,bilaterally at Neiguan(PC6),Hegu(LI4),Yanglingquan(GB34),Sanyinjiao(SP6)and Taichong(LR3)acupoints.MAIN OUTCOME MEASURES:Each week,immediately before treatment and after treatment,subjects were tested for PPTs,Wong-Baker Faces Pain Scale(WBFPS;for pain intensity)and Medical Outcomes Study 36-item Short Form Health Survey(SF-36:for Qo L).RESULTS:There was no signifi cant improvement in pain or reduction of tender points in any of the groups studied,at the end of the 8th session.Signifi cant improvement of Qo L was perceived in vitality(after AC treatment)and in mental health(after EAC and MX treatments).CONCLUSION:TCM therapies(AC,EAC and MX)promoted an improvement in the Qo L in two areas(vitality and mental health)in FMS women.Further large-scale clinical trials are required to confi rm this effect. | Paulo Araujo Dias Andre Brito Bastos Guimaraes Andrea de Oliveira Albuquerque Karoline Lucas de Oliveira Maria Luzete Costa Cavalcante Sergio Botelho Guimaraes | 2016 | Journal of Integrative Medicine2016,14,1: | 7 |

| 4 | Beneficial effects of adenosine triphosphate-sensitive K^+ channel opener on liver ischemia/reperfusion injury显示文摘AIM:To investigate the effect of diazoxide administration on liver ischemia/reperfusion injury.METHODS:Wistar male rats underwent partial liver ischemia performed by clamping the pedicle from the medium and left anterior lateral segments for 1 h under mechanical ventilation.They were divided into 3 groups:Control Group,rats submitted to liver manipulation,Saline Group,rats received saline,and Diazoxide Group,rats received intravenous injection diazoxide(3.5 mg/kg) 15 min before liver reperfusion.4 h and 24 h after reperfusion,blood was collected for determination of aspartate transaminase(AST),alanine transaminase(ALT),tumor necrosis factor(TNF-α),interleukin-6(IL-6),interleukin-10(IL-10),nitrite/nitrate,creatinine and tumor growth factor-β1(TGF-β1).Liver tissues were assembled for mitochondrial oxidation and phosphorylation,malondialdehyde(MDA) content,and histologic analysis.Pulmonary vascular permeability and myeloperoxidase(MPO) were also determined.RESULTS:Four hours after reperfusion the diazoxide group presented with significant reduction of AST(2009 ± 257 U/L vs 3523 ± 424 U/L,P = 0.005); ALT(1794 ± 295 U/L vs 3316 ± 413 U/L,P = 0.005); TNF-α(17 ± 9 pg/mL vs 152 ± 43 pg/mL,P = 0.013; IL-6(62 ± 18 pg/mL vs 281 ± 92 pg/mL); IL-10(40 ± 9 pg/mL vs 78 ± 10 pg/mL P = 0.03),and nitrite/nitrate(3.8 ± 0.9 μmol/L vs 10.2 ± 2.4 μmol/L,P = 0.025) when compared to the saline group.A significant reduction in liver mitochondrial dysfunction was observed in the diazoxide group compared to the saline group(P < 0.05).No differences in liver MDA content,serum creatinine,pulmonary vascular permeability and MPO activity were observed between groups.Twenty four hours after reperfusion the diazoxide group showed a reduction of AST(495 ± 78 U/L vs 978 ± 192 U/L,P = 0.032); ALT(335 ± 59 U/L vs 742 ± 182 U/L,P = 0.048),and TGF-β1(11 ± 1 ng/mL vs 17 ± 0.5 ng/mL,P = 0.004) serum levels when compared to the saline group.The control group did not present alterations when compared to the diazoxide and saline groups.CONCLUSION:Diazoxide maintains liver mitochondrial function,increases liver tolerance to ischemia/reperfusion injury,and reduces the systemic inflammatory response.These effects require further evaluation for using in a clinical setting. | Mateus Antunes Nogueira Ana Maria Mendona Coelho Sandra Nassa Sampietre Rosely Antunes Patzina Fabiano Pinheiro da Silva Luiz Augusto Carneiro D'Albuquerque Marcel Cerqueira Cesar Machado | 2014 | World Journal of Gastroenterology2014,20,41: | 2 |
| 5 | Serum proinflammatory cytokines and nutritional status in pediatric chronic liver disease显示文摘AIM: To evaluate the nutritional status and its association with proinflammatory cytokines in children with chronic liver disease.METHODS: We performed a cross-sectional study with 43 children and adolescents, aged 0 to 17 years, diagnosed with chronic liver disease. All patients regularly attended the Pediatric Hepatology Unit and were under nutritional follow up. The exclusion criteria were fever from any etiology at the time of enrollment, inborn errors of the metabolism and any chronic illness. The severity of liver disease was assessed by Child-Pugh, Model for End-stage Liver Disease(MELD) and Pediatric End Stage Liver Disease(PELD) scores. Anthropometric parameters were height/age, body mass index/age and triceps skinfold/age according to World Health Organization standards. The cutoff points for nutritional status were risk of malnutrition(Z-score <-1.00) and malnutrition(Z-score <-2.00). Interleukin-1β(IL-1β), IL-6 and tumor necrosis factor-α levels were assessed by commercial ELISA kits. For multivariate analysis, linear regression was applied to assess the association between cytokine levels, disease severity and nutritional status. RESULTS: The median(25th-75 th centile) age of the study population was 60(17-116)-mo-old, and 53.5% were female. Biliary atresia was the main cause of chronic liver disease(72%). With respect to Child-Pugh score, cirrhotic patients were distributed as follows: 57.1% Child-Pugh A, a mild presentation of the disease, 34.3% Child-Pugh B, a moderate stage of cirrhosis and 8.6% Child-Pugh C, were considered severe cases. PELD and MELD scores were only above the cutoff point in 5 cases. IL-6 values were increased in patients at nutritional risk(34.9%) compared with those who were well-nourished [7.12(0.58-34.23) pg/m L vs 1.63(0.53-3.43) pg/m L; P = 0.02], correlating inversely with triceps skinfold-for-age z-score(rs =-0.61; P < 0.001). IL-6 levels were associated with liver disease severity assessed by Child-Pugh score(P = 0.001). This association remained significant after adjusting for nutritional status in a linear regression model. CONCLUSION: High IL-6 levels were found in children with chronic liver disease at nutritional risk. | 15 | Mortality Trends for Neglected Tropical Diseases in the State of Sergipe,Brazil,1980-2013显示文摘Background:Neglected Tropical Diseases are a set of communicable diseases that affect the population so low socioeconomic status,particularly 1.4 billion people who are living below the poverty level.This study has investigated the magnitude and mortality time trends for these diseases in the state of Sergipe,Northeast Region of Brazil.Methods:We conducted an ecological study of time series,based on secondary data derived from the Mortality Information System of the Ministry of Health.The mortality rates(crude,age-standardized rates and proportional ratio)were calculated from the deaths due to Neglected Tropical Diseases in the state of Sergipe,from 1980 to 2013.The time trends were obtained using the Joinpoint regression model.Results:Three hundred six thousand and eight hundred seventy-two deaths were certified in the state and Neglected Tropical Diseases were mentioned as the underlying cause in 1,203 certificates(0.39%).Mean number of deaths was 35.38 per year,and crude and age-standardized mortality rates were,respectively:2.16 per 100000 inhabitants(95%CI:1.45-2.87)and 2.87 per 100000 inhabitants(95%CI:1.93-3.82);the proportional mortality ratio was 0.41%(95%CI:0.27-0.54).In that period,Schistosomiasis caused 654 deaths(54.36%),followed by Chagas disease,with 211(17.54%),and by Leishmaniases,with 142(11.80%)deaths.The other diseases totalized 196 deaths(16.30%).There were increasing mortality trends for Neglected Tropical Diseases,Schistosomiasis and Chagas disease in the last 15 years,according to the age-standardized rates,and stability of the mortality trends for Leishmaniases.Conclusions:The Neglected Tropical Diseases show increasing trends and are a real public health problem in the state of Sergipe,since they are responsible for significant mortality rates.The following diseases call attention for showing greater number of deaths in the period of study:Schistosomiasis,Chagas disease and Leishmaniases.We finally suggest that public managers take appropriate actions to develop new strategies in epidemiological and therapeutic surveillance,and in the follow-up of these patients. | Marcos Antonio Costa de Albuquerque Danielle Menezes Dias Lucas Teixeira Vieira Carlos Anselmo Lima Angela Maria da Silva | 2017 | Infectious Diseases of Poverty2017,6,1: | 1 |
| 16 | Optimal agricultural spreading scheduling through surrogate-based optimization and MINLP models显示文摘The most commonly used definition of climate smart agriculture(CSA)is provided by the Food and Agricultural Organisation of the United Nations,which defines CSA as“agriculture that sustainably increases productivity,enhances resilience,reduces/removes greenhouse gas where possible,and enhances achievement of national food security and development goals”.In this definition,the principal goal of CSA is identified as food security and development,while productivity,adaptation,and mitigation are identified as the three interlinked pillars necessary for achieving this goal.In the context provided by the CSA,soils are seen as a lever to improve the carbon footprint of agriculture,namely through their role as carbon sinks.Improving soils and in particular agricultural soils’content in soil organic carbon(SOC)in one of the measures enabling to improve the environmental impact of agricultural practices.In this context,composts can be seen as an important feedstock for sustainable farming.To support the development of organic amendment strategies enabling to increase soils’SOC content,this work proposes a novel methodology to optimize the monthly scheduling of composts and mineral fertilizers amendments.The schedule proposed maximizes soil health-via improved SOC content-while ensuring optimal gross operating surplus from agriculture.This problem is subjected to certain operational,regulatory and soil-dynamics constraints which leads to a complex optimization problem and has to be solved in a relatively short time period for decision-making purposes.This is a nonlinear optimization problem(NLP)which is based on a soil-simulation model from which the analytic functions are not explicitly available for the optimization model.Operational and regulatory constraints are explicitly defined and integer and continuous variables are needed in the modeling.In order to effectively solve it in a deterministic way,a novel surrogate-modeling approach for the objective functions and constraints is proposed.Another novelty comes from the implementation of a continuous-variable-reduction procedure in order to build effective surrogates.The optimisation model results obtained from a real case study show that local optimal solutions can be identified with short computation times.The evaluated scenario shows that the optimized strategy could increase by 8%the carbon in soil after 13 years while increasing by 7%the estimated agricultural profit when compared to expert based application schedules. | Manuel Ramos-Castillo Marie Orvain Gabriela Naves-Maschietto Ana Barbara Bisinellade Faria Damien Chenu Maria Albuquerque | 2021 | Information Processing in Agriculture2021,8,1: | 0 |

| 18 | Dengue in renal transplant recipients: Clinical course and impact on renal function显示文摘AIM To present clinical characteristics from renal transplant recipients with dengue fever and its impact on graft function.METHODS We retrospectively evaluated 11 renal transplant recipients(RTR) with dengue infection confirmed by laboratory test, between January 2007 and July 2012, transplanted in the Renal Transplant Center of Walter Cantídio University Hospital from Federal University of Ceará. RESULTS Positive dengue serology(IgM) was found in all patients. The mean time between transplant and dengue infection was 43 mo. Fever was presented in all patients. Nine patients presented with classical dengue and two(18%) with dengue hemorrhagic fever. All cases had satisfactory evolution with complete recovery of the symptoms. The time for symptom resolution varied from 2 to 20 d, with an average of 9 d. An increase of creatinine after the infection was observed in three(27.2%) patients with no clinically impact on the kidney graft function. | 19 | A method for microbial decontamination of Acanthamoeba cultures using the peritoneal cavity of mice显示文摘Objective: To evaluate whether the inoculation of contaminated cultures in the peritoneal cavity of mice could implement decontamination of Acanthamoeba cultures.Methods: Suspensions of Acanthamoeba, Acanthamoeba polyphaga ATCC 30461, or Acanthamoeba spp. isolated from soil(Un B13 strain) were inoculated in the peritoneal cavity of Swiss mice(n = 24). After 1, 6, 12, or 24 h of exposure the peritoneal cavity was washed and assessed for the presence of bacteria, fungi, and Acanthamoeba.Results: After 1 h of intraperitoneal inoculation at least 97% of the bacteria and 96% of the fungi(P < 0.05) and 99% of the bacteria(P < 0.05) were successfully eliminated from the ATCC 30461 strain and from the soil isolate Un B13 strain, respectively.
