| 1 | Rheumatic manifestations of inflammatory bowel disease显示文摘This article reviews the literature concerning rheu-matic manifestations of inflammatory bowel disease (IBD),including common immune-mediated pathways,frequency,clinical course and therapy. Musculoskel-etal complications are frequent and well-recognized manifestations in IBD,and affect up to 33% of pa-tients with IBD. The strong link between the bowel and the osteo-articular system is suggested by many clinical and experimental observations,notably in HLA-B27 transgenic rats. The autoimmune pathogenic mechanisms shared by IBD and spondyloarthropathies include genetic susceptibility to abnormal antigen pre-sentation,aberrant recognition of self,the presence of autoantibodies against specific antigens shared by the colon and other extra-colonic tissues,and increased intestinal permeability. The response against microor-ganisms may have an important role through molecular mimicry and other mechanisms. Rheumatic mani-festations of IBD have been divided into peripheral arthritis,and axial involvement,including sacroiliitis,with or without spondylitis,similar to idiopathic anky-losing spondylitis. Other periarticular features can oc-cur,including enthesopathy,tendonitis,clubbing,peri-ostitis,and granulomatous lesions of joints and bones.Osteoporosis and osteomalacia secondary to IBD and iatrogenic complications can also occur. The manage-ment of the rheumatic manifestations of IBD consists of physical therapy in combination with local injection of corticosteroids and nonsteroidal anti-inflammatory drugs; caution is in order however,because of their possible harmful effects on intestinal integrity,perme-ability,and even on gut inflammation. Sulfasalazine,methotrexate,azathioprine,cyclosporine and lefluno-mide should be used for selected indications. | 6 | Treatment of chronic hepatitis C with direct-acting antivirals: The role of resistance显示文摘The use of direct-acting antivirals(DAAs) to treat chronic hepatitis C has resulted in a significant increase in rates of sustained viral response(around 90%-95%) as compared with the standard treatment of peginterferon/ribavirin. Despite this, however, the rates of therapeutic failure in daily clinical practice range from 10%-15%. Most of these cases are due to the presence of resistant viral variants, resulting from mutations produced by substitutions of amino acids in the viral target protein that reduce viral sensitivity to DAAs, thus limiting the efficacy of these drugs. The high genetic diversity of hepatitis C virus has resulted in the existence of resistance-associated variants(RAVs), sometimes even before starting treatment with DAAs, though generally at low levels. These preexisting RAVs do not appear to impact on the sustained viral response, whereas those that appear after DAA therapy could well be determinant in virological failure with future treatments. As well as the presence of RAVs, virological failure to treatment with DAAs is generally associated with other factors related with a poor response, such as the degree of fibrosis, the response to previous therapy, the viral load or the viral genotype. | 7 | Resection of the uncinate process of the pancreas due to a ganglioneuroma显示文摘A 33-year-old woman who presented with epigastric discomfort and diarrhea underwent an abdominal ultrasound(US).This investigation and subsequent contrastenhanced computed tomography,magnetic resonance imaging and endoscopic US with fine needle aspiration (FNA)revealed a 40 mm well-circumscribed mass in the uncinate process of the pancreas.Findings were suggestive of a mucinous or solid-cystic pseudopapillary tumor of the pancreas,although other lesions such as a nonfunctioning neuroendocrine tumor could not be ruled out.FNA samples were negative for malignant cells,but of limited value due to poor cellularity.It was decided to surgically remove the tumor because malignancy could not be discounted.Multiple intraoperative biopsies were suggestive of mesenchymal tumor and consequently a conservative resection(uncinatectomy)was performed. They are asymptomatic, unless they become complicated, with perforation being the rarest but most severe complication. Surgical treatment is the most frequently performed approach. We report the case of a patient with a perforated duodenal diverticulum, which was diagnosed early and treated conservatively with antibiotics and percutaneous drainage of secondary retroperitoneal abscesses. | 14 | A regionalized IO-model to value seasonal recreational ecosystem services in a mountain National Park in Spain显示文摘Recreational Ecosystem Services(RES)are among the most frequently evaluated ecosystem services.RES are seen as a major opportunity for sustainable development in areas of high ecological value resulting from the presence of emblematic species,habitats or scenery,often integrated in conservation areas,in particular in mountain areas affected by depopulation,rising environmental risks and poverty.Economically,the assessment of RES has been based on Contingent Valuation and Choice Experiments,methods with limitations related to their subjectivity.Alternatively,Input-Output(IO)models provide a very accurate and effective way of valuation of RES with regional information regarding interindustry transactions.Whilst data on nature tourist expenditure may be only available annually,tourism inflows have a strong seasonal behavior,which impacts the profitability of several local economic activities.In this paper,we firstly constructed a novel regionalized IO-model based on Cross-Industry Location Quotients using employment data and,secondly,based on Monte Carlo simulation,we estimated nature tourist expenditure monthly using data on nature tourism inflows.This method allowed for a more precise RES evaluation,estimating direct,indirect and induced monthly economic impacts of nature-based tourism.The method was applied to the Ordesa and Monte Perdido(OMP)National Park in the Spanish Pyrenees.To estimate the average tourist expenditure,we collected and analyzed spending on commerce,hospitality and restaurants,leisure,and transport of 385 visitors.Results suggest that using a regionalized IO model leads to a considerable reduction of over-estimation of the economic impact of tourist expenditure estimated by conventional methods.Taking into consideration the direct,indirect and induced effects of tourist expenditure in 2016-2018,the total annual output effect in the OMP National Park amounted to€208.8 million,while the total income effect amounted to€86.29 million.Also,a total of 2,429 jobs were created which indicates that for each thousand annual visitors to the National Park,3.8 jobs are created in the surrounding area.Overall,the proposed methodology can be easily applied to the remaining National Parks in Spain or other regions in the world,making it a valuable tool to estimate the value of RES in areas affects by strong seasonality,to set priorities and support regional policies for mountain sustainable development. | Víctor MARTIN BARROSO Mónica de CASTRO-PARDO Pascual FERNÁNDEZ MARTÍNEZ João C.AZEVEDO | 2022 | Journal of Mountain Science2022,19,1: | 2 |
