1Sleep disorders and chronic kidney disease显示文摘Sleep disorders have a profound and well-documented impact on overall health and quality of life in the general population. In patients with chronic disease, sleep disorders are more prevalent, with an additional morbidity and mortality burden. The complex and dynamic relationship between sleep disorders and chronic kidney disease(CKD) remain relatively little investigated. This article presents an overview of sleep disorders in patients with CKD, with emphasis on relevant pathophysiologic underpinnings and clinical presentations. Evidence-based interventions will be discussed, in the context of individual sleep disorders, namely sleep apnea, insomnia, restless leg syndrome and excessive daytime sleepiness. 2Relationship between hepatitis C virus infection and type 2 diabetes mellitus:Meta-analysis显示文摘AIM:To investigate the association between hepatitis C infection and type 2 diabetes mellitus.METHODS:Observational studies assessing the relationship between hepatitis C infection and type 2 diabetes mellitus were identified via electronic and hand searches.Studies published between 1988 to March 2011 were screened,according to the inclusion criteria set for the present analysis.Authors performed separate analyses for the comparisons between hepatitis C virus(HCV) infected and not infected,and HCV infected and hepatitis B virus infected.The included studies were further subgrouped according to the study design.Heterogenity was assessed using I2 statistics.The summary odds ratios with their corresponding 95% CIs were calculated based on a random-effects model.The included studies were subgrouped according to the study design.To assess any factor that could potentially affect the outcome,results were further stratified by age group(proportion of ≥ 40 years),gender(proportion of male gender),body mass index(BMI)(pro-portion of BMI ≥ 27),and family history of diabetes(i.e.,self reported).For stability of results,a sensitivity analysis was conducted including only prospective studies.RESULTS:Combining the electronic database and hand searches,a total of 35 observational studies(in 31 articles) were identified for the final analysis.Based on random-effects model,17 studies(n = 286 084) compared hepatitis C-infected patients with those who were uninfected [summary odds ratio(OR):1.68,95% CI:1.15-2.45].Of these 17 studies,7 were both a cross-sectional design(41.2%) and cohort design(41.2%),while 3 were case-control studies(17.6%).Nineteen studies(n = 51 156) compared hepatitis C-infected participants with hepatitis B-infected(summary OR:1.92,95% CI:1.41-2.62).Of these 19 studies,4(21.1%),6(31.6%) and 9(47.4%) were cross-sectional,cohort and case-control studies,respectively.A sensitivity analysis with 3 prospective studies indicated that hepatitis C-infected patients had a higher risk of developing type 2 diabetes compared with uninfected controls(summary odds ratio:1.41,95% CI:1.17-1.7;I2 = 0%).Among hepatitis C-infected patients,male patients(OR:1.26,95% CI:1.03-1.54) with age over 40 years(summary OR:7.39,95% CI:3.82-9.38) had an increased frequency of type 2 diabetes.Some caution must be taken in the interpretation of these results because there may be unmeasured confounding factors which may introduce bias.CONCLUSION:The findings support the association between hepatitis C infection and type 2 diabetes mellitus.The direction of association remains to be determined,however.Prospective studies with adequate sample sizes are recommended.Cho Naing Joon Wah Mak Syed Imran Ahmed Mala Maung 2012World Journal of Gastroenterology2012,18,14:12
3Issues and mitigations of wind energy penetrated network:Australian network case study显示文摘Longest geographically connected Australian power system is undergoing an unprecedented transition,under the effect of increased integration of renewable energy systems. This change in generation mix has implications for the whole interconnected system designs, its operational strategies and the regulatory framework. Frequency control policies about real-time balancing of demand and supply is one of the prominent and priority operational challenge requiring urgent attention. This paper reviews the Australian electricity market structure in presence of wind energy and its governance. Various issues related to increased wind generation systems integration are discussed in detail. Currently applied mitigations along with prospective mitigation methods requiring new or improved policies are also discussed. 4New contributions to the flora of Myanmar I显示文摘In this study, we describe several new taxa belonging to the flora of Myanmar. One new family, Polyosmaceae (Polyosma wallichii Benn.) is recorded. Over ten new genera are recorded for the first time, including Amentotaxus Pilger (Taxaceae), Hydrobryopsis Engler (Podostemaceae), Cyrtosia Blume and Biermannia King & Pantling (Orchidaceae), Eleutharrhena Forman and Haematocarpus Miers (Menispermaceae), Craigia W.W. Smith & W.E. Evans (Malvaceae), Amblyanthopsis Mez (Primulaceae), Huodendron Rehder and Rehderodendron Hu (Styracaceae), Platea Blume (Metteniusaceae), Achyrospermum Blume (Lamiaceae), Christisonia Gardner (Orobanchaceae). In addition, five new species are described and illustrated: Tupistra natmataungensis Y.H. Tan & H.B. Ding, Biermannia burmanica Y.H. Tan & Bin Yang, Impatiens megacalyx Y.H. Tan & H.B. Ding, Amblyanthopsis burmanica Y.H. Tan & H.B. Ding, Platea kachinensis Y.H. Tan & H.B. Ding. 5Hepatitis C virus therapy with peg-interferon and ribavirin in Myanmar: A resource-constrained country显示文摘AIM To investigate peg-interferon(peg-IFN) and ribavirin(RBV) therapy in Myanmar and to predict sustained virologic response(SVR).METHODS This single-center, open-label, study was conducted in Myanmar between 2009 and 2014. A total of 288 patients infected with HCV genotypes 1, 2, 3 and 6 were treated with peg-IFN alpha-2a(180 μg/wk) or alpha-2b(50 to 100 μg as a weight-based dose) and RBV as a weight-based dose(15 mg/kg/d). Treatment duration was 48 wk for genotypes 1 and 6, 24 wk for genotype 2, and 24 or 48 wk for genotype 3 based on rapid virologic response(RVR). Those co-infected with hepatitis B received 48 wk of therapy.RESULTS Overall, SVR was achieved for 82% of patients and the therapy was well tolerated. All patients achieved SVR at equivalent rates regardless of HCV genotype(P = 0.314). Low fibrosis scores(P < 0.001), high baseline albumin levels(P = 0.028) and low baseline viral loads(P = 0.029) all independently predicted SVR. On the other hand, IL-28 B TT and CC genotypes were not found to significantly predict SVR(P = 0.634; P = 0.618). Among those who completed treatment, the occurrence of RVR showed a > 96% positive predictive value for achieving SVR. Treatment duration did not significantly impact the likelihood of achieving SVR for patients infected with genotype 3 HCV(P = 0.371). The most common adverse events were fatigue(71%) and poor appetite(60%). Among patients with genotype 3 HCV, more patients in the 48-wk treatment group required erythropoietin than in the 24-wk treatment group(61.1% vs 49.2%).CONCLUSION SVR rates were high with peg-IFN and RBV therapy in Myanmar. 6Complete mitochondrial genome of the leaf muntjac(Muntiacus putaoensis) and phylogenetics of the genus Muntiacus显示文摘The leaf muntjac(Muntiacus putaoensis) is an endemic deer species found in the east transHimalayan region. In recent years, population numbers have decreased due to heavy hunting and habitat loss, and little genetic data exists for this species, thus our knowledge of distribution rangs and population sizes likewise remain limited. We obtained mt DNA genes and the complete mitochondrial genome sequence of M. putaoensis using PCR, followed by direct sequencing. The complete mitogenome sequence was determined as a circular 16 349 bp mitochondrial genome, containing 13 protein-coding genes, two r RNA genes, 22 t RNA genes, and one control region, the gene composition and order of which were similar to most other vertebrates so far reported. Most mitochondrial genes, except for ND6 and eight t RNAs, were encoded on the heavy strand. The overall base composition of the heavy strand was 33.1% A, 29.3% T, 24.2% C, and 13.4% G, with a strong AT bias of 62.4%. There were seven regions of gene overlap totaling 95 bp and 11 intergenic spacer regions totaling 74 bp. Phylogenetic analyses(ML and BI) among the Muntiacus genus based on the sequenced of mitogenome and ND4 L-ND4 supported M. putaoensis as a member of Muntiacus, most closely related to M. vuquangensis. However, when analyses based on cyt b included two more muntjacs, M. truongsonensis was most closely related to M. putaoensis rather than M. vuquangensis, and together with M. rooseveltorum, likely forming a M. rooseveltorum complex of the species. 7Embedding physics domain knowledge into a Bayesian network enables layer-by-layer process innovation for photovoltaics显示文摘Process optimization of photovoltaic devices is a time-intensive,trial-and-error endeavor,which lacks full transparency of the underlying physics and relies on user-imposed constraints that may or may not lead to a global optimum.Herein,we demonstrate that embedding physics domain knowledge into a Bayesian network enables an optimization approach for gallium arsenide(GaAs)solar cells that identifies the root cause(s)of underperformance with layer-by-layer resolution and reveals alternative optimal process windows beyond traditional black-box optimization.Our Bayesian network approach links a key GaAs process variable(growth temperature)to material descriptors(bulk and interface properties,e.g.,bulk lifetime,doping,and surface recombination)and device performance parameters(e.g.,cell efficiency).For this purpose,we combine a Bayesian inference framework with a neural network surrogate device-physics model that is 100×faster than numerical solvers.With the trained surrogate model and only a small number of experimental samples,our approach reduces significantly the time-consuming intervention and characterization required by the experimentalist.As a demonstration of our method,in only five metal organic chemical vapor depositions,we identify a superior growth temperature profile for the window,bulk,and back surface field layer of a GaAs solar cell,without any secondary measurements,and demonstrate a 6.5%relative AM1.5G efficiency improvement above traditional grid search methods.Zekun Ren Felipe Oviedo Maung Thway Siyu I.P.Tian Yue Wang Hansong Xue Jose Dario Perea Mariya Layurova Thomas Heumueller Erik Birgersson Armin G.Aberle Christoph J.Brabec Rolf Stangl Qianxiao Li Shijing Sun Fen Lin Ian Marius Peters Tonio Buonassisi 2020npj Computational Materials2020,,1:2
