| 2 | 肝上腔静脉成形术联合肝后腔静脉扩展术在肝移植中的应用:首次115例经验报道显示文摘目的:本文介绍原位肝移植中腔静脉成形术的方法并评估其结果。方法:1994年11月至2000年9月,我们对连续115例原位肝移植(包括6例左外侧叶及2例右叶移植)病人采用了腔静脉成形术。 在起初的4年里,53例(66.3%)移植病人需行股-腋静脉体外转流,而以后的2年内该比例降至8例(22.9%)。无一例病人需中转为背驮式或经典术式。结果:中位手术时间4.5h,中位热缺血时间25min,中位输血量6U浓缩红细胞。上述结果在首次移植及再次移植间无差异。本组无与腔静脉成形技术相关的围手术期死亡。包括活体原位肝移植在内,均未观察到肝静脉流出道梗阻。没有病人因急性肾功能衰竭而需术后血透。病人留置于ICU的中位天数2d,中位住院天数为10d。结论:腔静脉成形术无需解剖肝后腔静脉、肝静脉或肝短静脉,肝下腔静脉也得以保留,并因此更有利于肝切除及维持稳定的血流动力学,在再次移植中优势更为明显。大开口的三角形腔静脉吻合容易操作,能在较短时间内完成供肝植入,且易与吻合血管口径的匹配,并可避免流出道阻塞。操作时间的缩短减少了静脉体外转流的可能。我们认为腔静脉成形技术能适用于所有病人,且相应手术的并发症少。 | You Min Wu Michael Voigt Stephen Rayhili Daniel Kartz Rou-Yee Chenhsu Warren Schmidt Rachel Miller Frank Mtros Douglas Labrecque 周光文 | 2002 | 外科理论与实践2002,7,2: | 7 |
| 3 | Circular smooth muscle contributes to esophageal shortening during peristalsis显示文摘AIM:To study the angle between the circular smooth muscle(CSM) and longitudinal smooth muscle(LSM) fibers in the distal esophagus.METHODS:In order to identify possible mechanisms for greater shortening in the distal compared to proximal esophagus during peristalsis,the angles between the LSM and CSM layers were measured in 9 cadavers.The outer longitudinal layer of the muscularis propria was exposed after stripping the outer serosa.The inner circular layer of the muscularis propria was then revealed after dissection of the esophageal mucosa and the underlying muscularis mucosa.Photographs of each specimen were taken with half of the open esophagus folded back showing both the outer longitudinal and inner circular muscle layers.Angles were measured every one cm for 10 cm proximal to the squamocolumnar junction(SCJ) by two independent investigators.Two human esophagi were obtained from organ transplant donors and the angles between the circular and longitudinal smooth muscle layers were measured using micro-computed tomography(micro CT) and Image J software.RESULTS:All data are presented as mean ± SE.The CSM to LSM angle at the SCJ and 1 cm proximal to SCJ on the autopsy specimens was 69.3 ± 4.62 degrees vs 74.9 ± 3.09 degrees,P = 0.32.The CSM to LSM angle at SCJ were statistically significantly lower than at 2,3,4 and 5 cm proximal to the SCJ,69.3 ± 4.62 degrees vs 82.58 ± 1.34 degrees,84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P = 0.013,P = 0.008,P = 0.004,P = 0.009 respectively.The CSM to LSM angle at SCJ was also statistically significantly lower than the angles at 6,7 and 8 cm proximal to the SCJ,69.3 ± 4.62 degrees vs 80.18 ± 2.09 degrees,81.81 ± 1.75 degrees and 80.96 ± 2.04 degrees,P = 0.05,P = 0.02,P = 0.03 respectively.The CSM to LSM angle at 1 cm proximal to SCJ was statistically significantly lower than at 3,4 and 5 cm proximal to the SCJ,74.94 ± 3.09 degrees vs 84.04 ± 1.64 degrees,84.87 ± 1.04 degrees and 83.72 ± 1.42 degrees,P = 0.019,P = 0.008,P = 0.02 respectively.At 10 cm above SCJ the angle was 80.06 ± 2.13 degrees which is close to being perpendicular but less than 90 degrees.The CSM to LSM angles measured on virtual dissection of the esophagus and the stomach on micro CT at the SCJ and 1 cm proximal to the SCJ were 48.39 ± 0.72 degrees and 50.81 ± 1.59 degrees.Rather than the angle of the CSM and LSM being perpendicular in the esophagus we found an acute angulation between these two muscle groups throughout the lower 10 cm of the esophagus.CONCLUSION:The oblique angulation of the CSM may contribute to the significantly greater shortening of distal esophagus when compared to the mid and proximal esophagus during peristalsis. | Anil K Vegesna Keng-Yu Chuang Ramashesai Besetty Steven J Phillips Alan S Braverman Mary F Barbe Michael R Ruggieri Larry S Miller | 2012 | World Journal of Gastroenterology2012,18,32: | 7 |
| 4 | Analysis of roof and pillar failure associated with weak floor at a limestone mine显示文摘A limestone mine in Ohio has had instability problems that have led to massive roof falls extending to the surface. This study focuses on the role that weak, moisture-sensitive floor has in the instability issues.Previous NIOSH research related to this subject did not include analysis for weak floor or weak bands and recommended that when such issues arise they should be investigated further using a more advanced analysis. Therefore, to further investigate the observed instability occurring on a large scale at the Ohio mine, FLAC3 D numerical models were employed to demonstrate the effect that a weak floor has on roof and pillar stability. This case study will provide important information to limestone mine operators regarding the impact of weak floor causing the potential for roof collapse, pillar failure, and subsequent subsidence of the ground surface. | Murphy Michael M. | 9 | Proteomic and genomic studies of non-alcoholic fatty liver disease-clues in the pathogenesis显示文摘Non-alcoholic fatty liver disease(NAFLD)is a widely prevalent hepatic disorder that covers wide spectrum of liver pathology.NAFLD is strongly associated with liver inflammation,metabolic hyperlipidaemia and insulin resistance.Frequently,NAFLD has been considered as the hepatic manifestation of metabolic syndrome.The pathophysiology of NAFLD has not been fully elucidated.Some patients can remain in the stage of simple steatosis,which generally is a benign condition;whereas others can develop liver inflammation and progress into non-alcoholic steatohepatitis,fibrosis,cirrhosis and hepatocellular carcinoma.The mechanism behind the progression is still not fully understood.Much ongoing proteomic researches have focused on discovering the unbiased circulating biochemical markers to allow early detection and treatment of NAFLD.Comprehensive genomic studies have also begun to provide new insights into the gene polymorphism to understand patientdisease variations.Therefore,NAFLD is considered a complex and mutifactorial disease phenotype resulting from environmental exposures acting on a susceptible polygenic background.This paper reviewed the current status of proteomic and genomic studies that have contributed to the understanding of NAFLD pathogenesis.For proteomics section,this review highlighted functional proteins that involved in:(1)transportation;(2)metabolic pathway;(3)acute phase reaction;(4)antiinflammatory;(5)extracellular matrix;and(6)immune system.In the genomic studies,this review will discuss genes which involved in:(1)lipolysis;(2)adipokines;and(3)cytokines production. | Jun Wei Lim John Dillon Michael Miller | 2014 | World Journal of Gastroenterology2014,20,26: | 3 |

| 16 | Anti-hypertensive drugs in children and adolescents显示文摘Worldwide the prevalence of essential hypertension inchildren and adolescents continues to increase. Tradi-tionally providers have used 'off-label' drugs to treatpediatric hypertension, meaning that rigorous clinicaltrials of these drugs have not been specifically per-formed in pediatric patient populations. Consequentlyproviders have extrapolated dosing, safety and efficacyfrom trials in adults. This practice is sub-optimal as chil-dren demonstrate unique differences in drug metabo-lism and response. Use of unstudied or understudieddrugs increases risk of adverse events and/or can leadto sub-optimal efficacy. Recognizing these concerns,regulatory agencies have created financial incentivesfor industry to conduct pediatric clinical trials. Theseincentives, coupled with the emerging pediatric hyper-tension epidemic, have spurred over 30 clinical trialsof anti-hypertensive drugs over the past 15 years andhave resulted in labeling of 10 new drugs by the UnitedStates Food and Drug Administration for treatment ofhypertension in children and adolescents. Unfortunatelythe financial incentive structures focus on newer drugsand drug classes. Consequently there is now a relativedearth of trial data for older but sometimes commonlyprescribed pediatric antihypertensive drugs. This article reviews recent pediatric antihypertensive drug trials with a focus on trial design and endpoints, drug dosing, safety, efficacy and specific drug indications.
