| 1 | Diagnostic approach to patients with acute idiopathic and recurrent pancreatitis,what should be done?显示文摘Acute recurrent pancreatitis (ARP) is a common clinical condition that may be difficult to diagnose. Endoscopic ultrasound (EUS) is proposed to be a safe first line test of choice in the majority of patients. When interventions are needed to remove biliary stones, evaluate sphincter of Oddi or pancreas divisum, endoscopic retrograde cholangiopancreatography (ERCP) is recommended. Magnetic resonance cholangiopancreatography (MRCP) can be a suitable alternative from a diagnostic standpoint although may not be widely available. | 4 | Software for automated classification of probe-based confocal laser endomicroscopy videos of colorectal polyps显示文摘AIM:To support probe-based confocal laser endomi-croscopy (pCLE) diagnosis by designing software for the automated classification of colonic polyps. METHODS:Intravenous fluorescein pCLE imaging of colorectal lesions was performed on patients under-going screening and surveillance colonoscopies, followed by polypectomies. All resected specimens were reviewed by a reference gastrointestinal pathologist blinded to pCLE information. Histopathology was used as the criterion standard for the differentiation between neoplastic and non-neoplastic lesions. The pCLE video sequences, recorded for each polyp, were analyzed off-line by 2 expert endoscopists who were blinded to the endoscopic characteristics and histopathology. These pCLE videos, along with their histopathology diagnosis, were used to train the automated classification software which is a content-based image retrieval technique followed by k-nearest neighbor classification. The performance of the off-line diagnosis of pCLE videos established by the 2 expert endoscopists was compared with that of automated pCLE software classification. All evaluations were performed using leave-one-patient- out cross-validation to avoid bias. RESULTS:Colorectal lesions (135) were imaged in 71 patients. Based on histopathology, 93 of these 135 lesions were neoplastic and 42 were non-neoplastic. The study found no statistical significance for the difference between the performance of automated pCLE software classification (accuracy 89.6%, sensitivity 92.5%, specificity 83.3%, using leave-one-patient-out cross-validation) and the performance of the off-line diagnosis of pCLE videos established by the 2 expert endoscopists (accuracy 89.6%, sensitivity 91.4%, specificity 85.7%). There was very low power (< 6%) to detect the observed differences. The 95% confidence intervals for equivalence testing were:-0.073 to 0.073 for accuracy, -0.068 to 0.089 for sensitivity and -0.18 to 0.13 for specificity. The classification software proposed in this study is not a 'black box' but an informative tool based on the query by example model that produces, as intermediate results, visually similar annotated videos that are directly interpretable by the endoscopist. | 6 | Total pancreatectomy: Short- and long-term outcomes at a high-volume pancreas center显示文摘AIM To identify the current indications and outcomes of total pancreatectomy at a high-volume center. METHODS A single institutional retrospective study of patients undergoing total pancreatectomy from 1995 to 2014 was performed.RESULTS One hundred and three patients underwent totalpancreatectomy for indications including: Pancreatic ductal adenocarcinoma(n = 42, 40.8%), intraductal papillary mucinous neoplasms(n = 40, 38.8%), chronic pancreatitis(n = 8, 7.8%), pancreatic neuroendocrine tumors(n = 7, 6.8%), and miscellaneous(n = 6, 5.8%). The mean age was 66.2 years, and 59(57.3%) were female. Twenty-four patients(23.3%) underwent a laparoscopic total pancreatectomy. Splenic preservation and portal vein resection and reconstruction were performed in 24(23.3%) and 18 patients(17.5%), respectively. The 90 d major complications, readmission, and mortality rates were 32%, 17.5%, and 6.8% respectively. | 12 | Improving sorghum digestion in broilers by targeting fermentation of xylan显示文摘This study was to examine if it is possible to accelerate sorghum digestion in broiler chickens by targeting fermentation of the xylan.Cobb 500 broilers(n=960,80 birds per treatment)were fed 12 sorghumsoybean meal-based dietary treatments fed as 3 phases(starter d 0 to 12,grower d 13 to 23,finisher d 24 to 35),with 8 replicate pens of 10 birds per treatment.For half of the treatments(n=6),10%of the sorghum in the diet was directly replaced with 10%wheat bran,as a source of fermentable fibre.The diets were supplemented with either 0,50 or 2,000 mg/kg xylo-oligosaccharides(XOS),with or without xylanase application.Body weight gain(BWG),feed intake(FI)and feed conversion corrected for mortality(cFCR)was determined at d 0 to 35,and male and female body weight were measured on d 35.On d 35,ileum and caeca samples were collected from 2 birds per pen,for determination of caecal cellulase and xylanase activity,microbiota composition and short chain fatty acid(SCFA)concentration,and ileal XOS concentration.Supplementation with 2,000 mg/kg XOS caused increased BWG at d 0 to 35(P=0.007)and enhanced caecal propionic,valeric and succinic acid concentration(P<0.05).Wheat bran increased FI(P=0.018)and BWG(P=0.016),as well as caecal Bifidobacteria concentration(P<0.001).Ileal XOS concentration was greatest when feeding combined wheat bran,2,000 mg/kg XOS,and xylanase,resulting in increased caecal total SCFA,acetic acid and butyric acid concentration,and xylanase and cellulase activity(P<0.05).Results from this study present that feed efficiency in birds fed sorghum-based diets is improved as a consequence of supplementing with fermentable fibre,xylanase and XOS. | Natalie K.Morgan Andrew Wallace Michael R.Bedford | 2022 | Animal Nutrition2022，，3： | 2 |
