| 1 | Inverted internal limiting membrane flap technique for very large macular hole显示文摘AIM: To assess the anatomical and visual outcome of idiopathic macular holes greater than 1000 μm using the inverted internal limiting membrane flap technique.METHODS: This retrospective case series included 5 eyes of 5 patients with idiopathic macular hole with base diameter greater than 1000 μm who underwent inverted internal limiting membrane flap technique along with standard 23 G pars plans vitrectomy with posterior hyaloid detachment and fluid gas exchange with 12%-14% perfluoropropane(C3F8). Preoperative and postoperative visual acuity and spectral domain optical coherence tomography images were evaluated. The main outcome measures were visual outcome and macular hole closure.RESULTS: Mean age was 63.2±8.4y with all 5 subjects being females. Mean duration of symptoms was 11±14 mo with a mean postoperative follow up of 13.2 ±13 mo. The mean base diameter of the macular holes was 1420±84.8 μm(1280-1480 μm). Type 1 closure was achieved in four out of five patients, while one patient had type 2 closure using the inverted internal limiting membrane(ILM) flap technique. Median baseline BCVA was 0.79 log MAR(Snellen's equivalent 20/120) and median final BCVA0.6 log MAR(Snellen's equivalent 20/80) with mean visual improvement of approximately three lines improvement. | 2 | 1α,25-dihydroxyvitamin D_(3) prevents DNA damage and restores antioxidant enzymes in rat hepatocarcinogenesis induced by diethylnitrosamine and promoted by phenobarbital显示文摘AIM:To investigate the chemopreventive iffects of 1α,25-dihydroxyvitamin D3 in diethylnitrosamine induced,phenobarbital promoted rat hepatocarcinogenesis.METHODS:The rats were randomly divided into 6 different groups(A-F).Group A,C and E rats received a single intraperitoneal (i.p) injection of diethylnitrosamine (DEN) at a dose of 200 mg/kg body mass in 9 g/L NaCl solution at 4 wk of age ,while group B served as normal vehicle control received normal saline once.After a brief recovery of 2 WK, all the DEN treated rats were given phenobarbital (PB) at 0.5g/L daily in the basal diet till WK 20.Group C was started 4 wk prior to DEN injection and continued thereafter till wk 20 at a dose (os) twice a week.Group E received the treatment of 1α,25-(OH)2D3 at the same dose mentioned as above for 15 wk.The rats in group D and F received 1α,25-(OH)2D3 alone as in group C without DEN injection.RESULTS:The comet assay showed statistically higher mean values for length to width ratios (L:W) of DNA mass and tailed cells (p<0.01;p<0.01 respectively) in DEN treated rats as compared to their normal controls.Continuous supplementation of 1α,25-dihydroxyvitaminD3 showed a significant (p<0.01) decrease in L:W ratio of DNA mass tailed cells.Furthermore,1α,25-(OH)2D3 supplementations elevated the super oxide dismutase (SOD) activety,hepatic malondialdehyde(MDA) levle,reduced glutathione (GSH) and glutathione S-transferase (GST) activety (p<0.01,p<0.01,p<0.05 and p<0.05 respectively).As an endpoint marker histological changes were observed to establish the chemopreventive effects of 1α,25-dihydroxyvitaminD3.CONCLUSION:Supplementations of 1α,25-(OH)2D3 has a marked protection against hepatic nodelogenesis,antioxidant enzymes and DNA damages in DEN induced rat hepatocarcinogenesis promoted by phenobarbital. | Mahendrakumar Chandrasekharappa Banakar Suresh Kanna Paramasivan Mitali Basu Chattopadhyay Subrata Datta Prabir Chakraborty Malay Chatterjee Kalaiselvi Kannan Elayaraja Thygarajan | 2004 | World Journal of Gastroenterology2004,10,9: | 5 |
| 3 | Catastrophic gastrointestinal complication of systemic immunosuppression显示文摘We present a case of acute upper gastrointestinal haemorrhage in a patient with systemic vasculitis immunosuppressed on cyclophosphamide and prednisolone.The patient presented with a diffuse haemorrhagic oesophagitis and a non-specific duodenitis.Biopsies taken from the oesophagus and duodenum demonstrated infection with herpes simplex virus(HSV)and cytomegalovirus(CMV)respectively.Viral infection of the upper gastrointestinal tract is a recognised complication of immunosuppression and HSV is one of the most common pathogens.CMV on the other hand most commonly causes a colitis or less commonly oesophagitis.CMV enteritis is rare as is the synchronous infection with two viral agents in an immunocompromised patient having being described in a few case series only.Viral infection of the gastrointestinal tract in immunocompromised patients should be treated with systemic anti-viral medication and consideration to withdrawal of the immunosuppressive therapy if possible and appropriate.The authors highlight the need for a high suspicion of viral infection in immunosuppressed patients presenting with upper gastrointestinal bleeding. | Lyn Alexandra Smith Mitali Gangopadhyay Daniel R Gaya | 2015 | World Journal of Gastroenterology2015,21,8: | 2 |

The influence of sodium borohydride, hydrazine hydrate and urea on the shape and size of the products were investigated by scanning electron microscopy(SEM) and X-ray diffraction(XRD). SEM showed the nano-sized spherical and rectangular shaped structures in case of sodium borohydride and hydrazine hydrate, whereas micro-sized hexagonal structures were formed in case of urea under the same irradiation power. The reducing agents played an important role in forming the various structures.
