| 1 | Severe acute pancreatitis: Pathogenetic aspects and prognostic factors显示文摘Approximately 20% of patients with acute pancreatitis develop a severe disease associated with complications and high risk of mortality. The purpose of this study is to review pathogenesis and prognostic factors of severe acute pancreatitis (SAP). An extensive medline search was undertaken with focusing on pathogenesis, complications and prognostic evaluation of SAP. Cytokines and other inflammatory markers play a major role in the pathogenesis and course of SAP and can be used as prognostic markers in its early phase. Other markers such as simple prognostic scores have been found to be as effective as multifactorial scoring systems (MFSS) at 48 h with the advantage of simplicity, efficacy, low cost, accuracy and early prediction of SAP. Recently, several laboratory markers including hematocrit, blood urea nitrogen (BUN), creatinine, matrix metalloproteinase-9 (MMP-9) and serum amyloid A (SAA) have been used as early predictors of severity within the first 24 h. The last few years have witnessed a tremendous progress in understanding the pathogenesis and predicting the outcome of SAP. | 2 | Aqueous suspension of anise “Pimpinella anisum” protects rats against chemically induced gastric ulcers显示文摘AIM: To substantiate the claims of Unani and Arabian traditional medicine practitioners on the gastroprotective potential effect of a popular spice anise, 'Pimpinella anisum L.' on experimentally-induced gastric ulceration and secretion in rats. METHODS: Acute gastric ulceration in rats was produced by various noxious chemicals including 80% ethanol, 0.2 mol/L NaOH, 25% NaCl and indomethacin. Anti-secretory studies were undertaken using pylorus- ligated Shay rat technique. Levels of gastric non-protein sulfhydryls (NP-SH) and wall mucus were estimated and gastric tissue was also examined histologically. Anise aqueous suspension was used in two doses (250 and 500 mg/kg body weight) in all experiments. RESULTS: Anise significantly inhibited gastric mu- cosal damage induced by necrotizing agents and indomethacin. The anti-ulcer effect was further confirmed histologically. In pylorus-ligated Shay rats, anise suspension significantly reduced the basal gastric acid secretion, acidity and completely inhibited the rumenal ulceration. On the other hand, the suspension significantly replenished ethanol-induced depleted levels of gastric mucosal NP-SH and gastric wall mucus concentration. CONCLUSION: Anise aqueous suspension possesses significant cytoprotective and anti-ulcer activities against experimentally-induced gastric lesions. | 3 | Spices, herbal xenobiotics and the stomach: Friends or foes?显示文摘Spices and herbal remedies have been used since ancient times to treat a variety of disorders. It has been experimentally demonstrated that spices, herbs, and their extracts possess antimicrobial, anti-inflammatory, antirheumatic, lipid-lowering, hepatoprotective, nephroprotective, antimutagenic and anticancer activities, besides their gastroprotective and anti-ulcer activities. Despite a number of reports on the toxicity of herbs and spices, they are generally accepted as safer alternatives to conventional therapy against gastric ulcers. To this end, it is also believed, that excessive consumption of spices may favor the pathogenesis of gastric and duodenal ulcer and some studies have substantiated this common perception. Based on various in vivo experiments and clinical studies, on the effects of spices and herbs on gastric ulcers, it has indeed been shown that certain spices do possess remarkable anti-ulcer properties mediated by antisecretory, cytoprotective, antioxidant, and anti-Helicobacter pylori effects and mechanisms regulated by nitric oxide, prostaglandins, non-protein sulfhydryl molecules and epidermal growth factor expression. Accordingly, their consumption may attenuate and help prevent peptic ulcer disease. | 4 | Esophageal ulceration complicating doxycycline therapy显示文摘AIM: To report present state of iatrogenic drug-induced esophageal injury (DIEI) induced by medications in a private clinic.METHODS: Iatrogenic drug-induced esophageal injury (DIEI) induced by medications has been more frequently reported. In a private clinic we encountered 36 cases of esophageal ulcerations complicating doxycycline therapy in a mainly younger Saudi population (median age 29 years).RESULTS: The most frequent presenting symptoms were oclynophagia, retrostemal burning pain and dysphagia (94 %,75 % and 56 %, respectively). The diagnosis was according to medical history and confirmed by endoscopy in all patients.Beside withdrawal of doxycycline, when feasible, all patients were treated with a proton-pump inhibitor (PPI) and a prokinetic. | 17 | Sparrow Search Optimization with Transfer Learning-Based Crowd Density Classification显示文摘Due to the rapid increase in urbanization and population,crowd gatherings are frequently observed in the form of concerts,political,and religious meetings.HAJJ is one of the well-known crowding events that takes place every year in Makkah,Saudi Arabia.Crowd density estimation and crowd monitoring are significant research areas in Artificial Intelligence(AI)applications.The current research study develops a new Sparrow Search Optimization with Deep Transfer Learning based Crowd Density Detection and Classification(SSODTL-CD2C)model.The presented SSODTL-CD2C technique majorly focuses on the identification and classification of crowd densities.To attain this,SSODTL-CD2C technique exploits Oppositional Salp Swarm Optimization Algorithm(OSSA)with EfficientNet model to derive the feature vectors.At the same time,Stacked Sparse Auto Encoder(SSAE)model is utilized for the classification of crowd densities.Finally,SSO algorithm is employed for optimal fine-tuning of the parameters involved in SSAE mechanism.The performance of the proposed SSODTL-CD2C technique was validated using a dataset with four different kinds of crowd densities.The obtained results demonstrated that the proposed SSODTLCD2C methodology accomplished an excellent crowd classification performance with a maximum accuracy of 93.25%.So,the proposed method will be highly helpful in managing HAJJ and other crowded events. | Mohammad Yamin Mishaal Mofleh Almutairi Saeed Badghish Saleh Bajaba | 2023 | Computers, Materials & Continua2023,,3: | 0 |
